Project description:This article introduces a dataset comprising a portfolio of six projects each having a baseline consisting of a budget derived from a standardized first-principles, bottom-up estimation technique. The projects utilize a homogeneous set of resources, both consumable and non-consumable, which are inter-related in a highly complex, multi-dimensional manner with appropriate correlation between quantity, productivity rates and cost rates. The dataset also includes detailed time-phased actual costs and progress over the life of each project as well as the time-phased values of revenue claimed for each project. The collection and attribution of 12,139 actual costs and the measurement of progress over a period of just under 109 consecutive weeks is consistent and standardized across all six projects providing a unique differentiator to other datasets. The data is valid for research in portfolio, program, or project (PPP) management or control of multiple resource-constrained projects, where projects are managed collectively, generally in a standardized manner to support a strategic business aim.

Project description:The present data was carried out based on macro policies of the municipality in order to optimize the waste collection and transportation system in the city of Bumehen. The data of this research, the average weights, time taken for collection and transportation of municipal solid waste (MSW) was measured and each of these criteria was calculated and evaluated from environmental and time perspectives under the current management system. Then, data collection, identification of the general characteristics of the region and the type of waste management were conducted using field studies and GIS software was later used to generate maps of classes, route type, per capita waste generation. In the next steps and based on information such as density, population, waste generation capacity, available routes and existing route types, number, type and capacity of tanks, the site of temporary transfer stations was determined and the appropriate routes were designed for the garbage trucks. The data showed that distance from urban space, as a physical criterion, and noise pollution, as an environmental criterion, in the selection of urban waste transfer station of a relative weight of 0.594 is the most important indicator for the construction of a waste transfer station. Also, the qualitative analysis of the dry wastes of the city of Bumehen showed that plastics, cardboard and paper were 8.6%, 8.6% and 8.3% respectively, of the highest amount of waste the city Bumehen formed.

Project description:This paper contains a dataset and spreadsheet with the inventory, calculations and results for the Life Cycle Assessment of PET (polyethylene terephthalate) bottle waste management options in Bauru, Brazil. Data for the Life Cycle Inventory (LCI) was collected in situ at sorting cooperatives, through interviews with municipal authorities, and through literature review. Data analysis was performed using the SimaPro v8.0 software for assessment of environmental impacts, using the ReCiPe midpoint hierarchical method. The data and results contained in the spreadsheet are divided as follows: worksheet 1 provides a title page with the reference to this article; worksheet 2 provides the impact values for the impact categories considered; worksheet 3 presents the impact values for all collection and transportation fleets; worksheets 4 to 7 provide the impacts for each process and each stage (operation, emissions, and construction) for the case where: waste PET is disposed of in a landfill and the treated sludge is composted (worksheet 4); waste PET is incinerated (worksheet 5); waste PET is sent to sorting cooperatives and the reject is landfilled (worksheet 6); waste PET is sent to sorting cooperatives and the reject is incinerated (worksheet 7); worksheets 8 to 16 contain the impacts for each stage and the total impact for scenarios 1 to 9, which are combinations of the processes presented in worksheets 4 to 7, and the collection and transportation calculations for each scenario; worksheet 17 presents a summary of the net impact and the impact for each stage for all scenarios and all impact categories considered; worksheet 18 contains the impacts for each process and each stage for the case where waste PET is disposed of in a landfill, and the treated sludge is landfilled. Project description:IntroductionSelf-management is an integral component of CKD treatment. Nevertheless, many patients with CKD do not adequately engage in self-management behaviors, and little is known on the underlying reasons. We aimed to identify and describe the factors that influence self-management behaviors from the perspective of adults with CKD.MethodsWe conducted 30 semistructured interviews with adults with CKD stage 3 or 4 from an academic nephrology clinic in the United States. Interviews were analyzed thematically.ResultsThe following are the 3 key phases of CKD self-management behavior engagement identified: (i) prioritization, (ii) performance, and (iii) maintenance. Prioritization was favorably influenced by optimism, stress management, and patient-provider communication and hampered by fatalism and competing priorities. Behavior performance was facilitated by motivating factors, self-efficacy, and support resources and impeded by comorbid conditions that caused treatment burden and adverse symptoms. Behavior maintenance relied on effective routines, influenced by similar factors as behavior performance, and reinforced by memory aids, goal setting, self-monitoring, and proactive preparation.ConclusionWe identified modifiable facilitators and barriers that influence the incorporation of CKD self-management into daily life. Our findings have important implications for the care of patients with CKD by providing a framework for providers to develop effective, tailored approaches to promote self-management engagement.

Project description:To explore changes in healthcare professionals' views about the diagnosis and management of heart failure since a study in 2003.Focus groups and a national online cross-sectional survey.Focus groups (n=8 with a total of 56 participants) were conducted in the North East of England using a phenomenological framework and purposive sampling, informing a UK online survey (n=514).4 categories were identified as contributing to variations in the diagnosis and management of heart failure. Three previously known categories included: uncertainty about clinical practice, the value of clinical guidelines and tensions between individual and organisational practice. A new category concerned uncertainty about end-of-life care. Survey responses found that confidence varied among professional groups in diagnosing left ventricular systolic dysfunction (LVSD): 95% of cardiologists, 93% of general physicians, 66% of general practitioners (GPs) and 32% of heart failure nurses. For heart failure with preserved ejection fraction (HFpEF), confidence levels were much lower: 58% of cardiologists, 43% of general physicians, 7% of GPs and 6% of heart failure nurses. Only 5-35% of respondents used natriuretic peptides for LVSD or HFpEF. Confidence in interpreting test findings was fundamental to the use of all diagnostic tests. Clinical guidelines were reported to be helpful when diagnosing LVSD by 33% of nurses and 50-56% of other groups, but fell to 5-28% for HFpEF. Some GPs did not routinely initiate diuretics (23%), ACE-inhibitors (22%) or ?-blockers (38%) for LVSD for reasons including historical teaching, perceived side effects and burden of monitoring. For end-of-life care, there was no consensus about responsibility for heart failure management.Reported differences in the way heart failure is diagnosed and managed have changed little in the past decade. Variable access to diagnostic tests, modes of care delivery and non-uniform management approaches persist. The current National Health Service (NHS) context may not be conducive to addressing these issues.

Project description:The data of this research was the investigation of knowledge, attitude and practices of biomedical waste management among healthcare staff and performed in some general hospitals in Tehran, Iran. In this descriptive data, 162 participants were chosen according to stratified sampling method and a self-made questionnaire was used for data collection. Also, Kruskal-wallis test, Mann -Whitney U tests and Spearman correlation coefficient were used to analyze the data in R software, version 3.4.4. The weighted mean of data showed that the knowledge level in staff is "Low" and their activity level is "Moderate". Also, the data of the statistical analysis revealed that there is no significant difference between male and female health care personnel in knowledge, attitude and practices. However, the results of Kruskal-Wallis test showed that there was no significant difference between the views of hospital staff in occupational and educational groups about knowledge and attitude and their relationship with the history of passing the health course, while the difference in practices level was significant (P < 0.0001). Furthermore, the relation between attitude and practices level of participants with different work experience were not significant.

Project description:BackgroundDiabetes mellitus (DM) is an important public health concern in Singapore and places a massive burden on health care spending. Tackling chronic diseases such as DM requires innovative strategies to integrate patients' data from diverse sources and use scientific discovery to inform clinical practice that can help better manage the disease. The Observational Medical Outcomes Partnership (OMOP) Common Data Model (CDM) was chosen as the framework for integrating data with disparate formats.ObjectiveThe study aimed to evaluate the feasibility of converting Singapore based data source, comprising of electronic health records (EHR), cognitive and depression assessment questionnaire data to OMOP CDM standard. Additionally, we also validate whether our OMOP CDM instance is fit for the purpose of research by executing a simple treatment pathways study using Atlas, a graphical user interface tool to conduct analysis on OMOP CDM data as a proof of concept.MethodsWe used de-identified EHR, cognitive, and depression assessment questionnaires data from a tertiary care hospital in Singapore to convert it to version 5.3.1 of OMOP CDM standard. We evaluate the OMOP CDM conversion by (1) assessing the mapping coverage (that is the percentage of source terms mapped to OMOP CDM standard); (2) local raw dataset versus CDM dataset analysis; and (3) Implementing Harmonized Intrinsic Data Quality Framework using an open-source R package called Data Quality Dashboard.ResultsThe content coverage of OMOP CDM vocabularies is more than 90% for clinical data, but only around 11% for questionnaire data. The comparison of characteristics between source and target data returned consistent results and our transformed data did not pass 38 (1.4%) out of 2,622 quality checks.ConclusionAdoption of OMOP CDM at our site demonstrated that EHR data are feasible for standardization with minimal information loss, whereas challenges remain for standardizing cognitive and depression assessment questionnaire data that requires further work.

Project description:BACKGROUND:Although the management of patients presenting with vertigo and dizziness in primary care has been reported to be inefficient, little is known about the primary care providers' (PCPs) perspectives, needs, and attitudes regarding vertigo management. The objective of this study was to understand which challenges and barriers PCPs see when diagnosing and treating patients presenting with vertigo or dizziness. Specifically, we wanted to identify facilitators and barriers of successful guideline implementation in order to inform the development of targeted interventions. METHODS:A theory-based interview structure was developed based on the implementation theory of capability, opportunity, and motivation for behaviour change (COM-B) using questions based on constructs from the Theoretical Domains Framework (TDF) and the Consolidated Framework for Implementation Research (CFIR). Transcripts of the semi-structured interviews were analysed using directed content analysis. The pathways through which guideline characteristics and supportive interventions affect the relationship between the PCPs' perceived capability, opportunity, and motivation as well as their practice of managing vertigo patients were graphically presented using the COM-B model structure. RESULTS:Twelve PCPs from Bavaria in Southern Germany participated in semi-structured interviews. Diagnostics posed the biggest challenge in vertigo management to the PCPs. Requirements for an acceptable guideline were stakeholder involvement in the development process, clarity of presentation, and high applicability. Guideline implementation might be effectively supported through educational meetings and sustained by organisational interventions. CONCLUSIONS:From the PCPs' perspective, both guideline characteristics and interventions supporting guideline implementation may help resolve challenges in vertigo management in primary care. These results should be used to guide future interventions in the primary care setting to ensure successful and targeted patient management.

Project description:ObjectiveTo understand the facilitators and barriers to the self-management of chronic obstructive pulmonary disease (COPD) in rural Nepal.SettingsCommunity and primary care centres in rural Nepal.ParticipantsA total of 14 participants (10 people with COPD and 4 health care providers) were interviewed.Primary and secondary outcome measuresPeople with COPD and healthcare provider's experience of COPD self-management in rural Nepal.ResultsFacilitators and barriers affecting COPD self-management in Nepal operated at the patient-family, community and service provider levels. People with COPD were found to have a limited understanding of COPD and medications. Some participants reported receiving inadequate family support and described poor emotional health. At the community level, widespread use of complementary and alternative treatment was found to be driven by social networks and was used instead of western medicine. There were limited quality controls in place to monitor the safe use of alternative treatment. While a number of service level factors were identified by all participants, the pertinent concerns were the levels of trust and respect between doctors and their patients. Service level factors included patients' demands for doctor time and attention, limited confidence of people with COPD in communicating confidently and openly with their doctor, limited skills and expertise of the doctors in promoting behavioural change, frustration with doctors prescribing too many medicines and the length of time to diagnose the disease. These service level factors were underpinned by resource constraints operating in rural areas. These included inadequate infrastructure and resources, limited skills of primary level providers and lack of educational materials for COPD.ConclusionsThe study findings suggest the need for a more integrated model of care with multiple strategies targeting all three levels in order to improve the self-management practices among people with COPD.

Project description:BackgroundDealing with cardiovascular disease is challenging, and people often struggle to follow rehabilitation and self-management programs. Several systematic reviews have explored quantitative evidence on the potential of digital interventions to support cardiac rehabilitation (CR) and self-management. However, although promising, evidence regarding the effectiveness and uptake of existing interventions is mixed. This paper takes a different but complementary approach, focusing on qualitative data related to people's experiences of technology in this space.ObjectiveThrough a qualitative approach, this review aims to engage more directly with people's experiences of technology that supports CR and self-management. The primary objective of this paper is to provide answers to the following research question: What are the primary barriers to and facilitators and trends of digital interventions to support CR and self-management? This question is addressed by synthesizing evidence from both medical and computer science literature. Given the strong evidence from the field of human-computer interaction that user-centered and iterative design methods increase the success of digital health interventions, we also assess the degree to which user-centered and iterative methods have been applied in previous work.MethodsA grounded theory literature review of articles from the following major electronic databases was conducted: ACM Digital Library, PsycINFO, Scopus, and PubMed. Papers published in the last 10 years, 2009 to 2019, were considered, and a systematic search with predefined keywords was conducted. Papers were screened against predefined inclusion and exclusion criteria. Comparative and in-depth analysis of the extracted qualitative data was carried out through 3 levels of iterative coding and concept development.ResultsA total of 4282 articles were identified in the initial search. After screening, 61 articles remained, which were both qualitative and quantitative studies and met our inclusion criteria for technology use and health condition. Of the 61 articles, 16 qualitative articles were included in the final analysis. Key factors that acted as barriers and facilitators were background knowledge and in-the-moment understanding, personal responsibility and social connectedness, and the need to support engagement while avoiding overburdening people. Although some studies applied user-centered methods, only 6 involved users throughout the design process. There was limited evidence of studies applying iterative approaches.ConclusionsThe use of technology is acceptable to many people undergoing CR and self-management. Although background knowledge is an important facilitator, technology should also support greater ongoing and in-the-moment understanding. Connectedness is valuable, but to avoid becoming a barrier, technology must also respect and enable individual responsibility. Personalization and gamification can also act as facilitators of engagement, but care must be taken to avoid overburdening people. Further application of user-centered and iterative methods represents a significant opportunity in this space.
