Project description:BackgroundAfar pastoralists live in the northeast of Ethiopia, confined to the most arid part of the country, where there is least access to educational, health and other social services. Tuberculosis (TB) is one of the major public health problems in Afar region. Lack of knowledge about TB could affect the health-seeking behaviour of patients and sustain the transmission of the disease within the community. In this study, we assessed the knowledge and perception of apparently healthy individuals about pulmonary tuberculosis (PTB) in pastoral communities of Afar.MethodsBetween March and May 2009, a community-based cross-sectional questionnaire survey involving 818 randomly selected healthy individuals was conducted in pastoral communities of Afar region. Moreover, two focus group discussions (FGDs), one with men and one with women, were conducted in each of the study area to supplement the quantitative study.ResultsThe majority (95.6%) of the interviewees reported that they have heard about PTB (known locally as "Labadore"). However, the participants associated the cause of PTB with exposure to cold air (45.9%), starvation (38%), dust (21.8%) or smoking/chewing Khat (Catha edulis) (16.4%). The discussants also suggested these same factors as the cause of PTB. All the discussants and the majority (74.3%) of the interviewees reported that persistent cough as the main symptom of PTB. About 87.7% of the interviewees and all the discussants suggested that PTB is treatable with modern drugs. All the discussants and the majority (95%) of the interviewees mentioned that the disease can be transmitted from a patient to another person. Socio-cultural practices, e.g. sharing cups (87.6%), and house type (59.8%) were suggested as risk factors for exposure to PTB in the study areas, while shortage of food (69.7%) and chewing khat (53.8%) were mentioned as factors favouring disease development. Almost all discussants and a considerable number (20.4%) of the interviewees thought that men were the highest risk group to get PTB as well as playing a major role in the epidemiology of the disease.ConclusionThe findings indicate that pastoral communities had basic awareness about the disease. Nevertheless, health education to transform their traditional beliefs and perceptions about the disease to biomedical knowledge is crucial.

Project description:Saving is a crucial tool for enhancing the livelihoods of pastoral and agro-pastoral communities, but due to a number of factors, its status and intensity are still in their infancy. Because of this, the current state of saving practices, their causes, and the size of pastoral and agro-pastoral communities are all examined in this study. A multi-stage sampling process was used to determine the 600 typical selected households. In order to assess the data, a double hurdle model was used. From the descriptive analysis result, only 35% of pastoral and agro-pastoral groups were savers. In comparison to their counterparts, households who have access to credit, are financially literate, engage in non-farm activities, cultivate crops in addition to livestock husbandry, use informal financial institutions, are educated, and wealthier are more likely to be savers and eager to save a larger amount of property. Households with more livestock and who live far away from formal financial institutions, on the other hand, are less likely to be savers and save only a small fraction of their income. Male-headed families are more likely to participate in saving decisions, whereas female-headed households must save more than their male counterparts once they have opted to save. Instead of relying on ineffective monetary policy (changing interest rates), any concerned bodies should emphasize mixed farming practices, establish financial institutions nearby to improve saving habits, provide non-farm training, and empower women in order to close the gap between savers and non-savers and mobilize resources to save and invest. Furthermore, raise awareness of financial institutions' products and services, as well as provide credit.

Project description:The tomato had nutritional, economic and health benefits to the societies, however, its production and productivity were low in developing countries and particularly in Ethiopia. This might be due to technical inefficiency caused by institutional, governmental, and farmers related factors. Therefore this study tried to investigate the factors that affecting technical efficiency and estimating the mean level of technical efficiency of tomato producers in Asaita district, Afar Regional State, Ethiopia. Both primary and secondary data sources were used; the primary data was collected from 267 tomato producers from the study area cross-sectional by using a multistage sampling technique. The single-stage stochastic frontier model and Cobb Douglas production function were applied and statistical significance was declared at 0.05. The maximum likelihood estimates of the stochastic frontier model showed that land, labor, tomato seed, and oxen have a significant effect on tomato output; and education, extension contact, training, and access to credit have a positive and significant effect on technical efficiency, whereas household size, off-farm income, livestock ownership, distance to market, and pesticides have a worthy and significant effect on technical efficiency; and also estimated mean technical efficiency of tomato producer in a study area was 80.9%. In a line with this, the responsible body should prioritize rural infrastructure development in areas such as education, marketplace, and farmer training centers; demonstrate access to credit and extension services; use the recommended amount of pesticides per hectare, and give more intension to mixed farming rather than animal husbandry exclusively.

Project description:BackgroundMortality from preventable pregnancy-related complications remains high in Ethiopia. Antenatal care remains a major public health intervention that prevents maternal and neonatal mortality. Thus, this study aimed to assess the magnitude and determinants of adequate antenatal care utilization in Southern Ethiopia.MethodsA community-based cross-sectional study was conducted between November and December 2019. A systematic random sampling technique was used to select 670 women. Data were collected using a pre-tested structured questionnaire administered with a digital survey tool (open data kit) and directly exported to STATA version 15 for analysis. Descriptive statistics followed by a multivariable logistic regression analysis were performed. Both crude and adjusted odds ratios (ORs) with 95% confidence intervals were reported.ResultsThe magnitude of adequate antenatal care utilization was 23.13%. Tertiary and above education (AOR,4.15;95%CI: 1.95, 8.83), having the best friend who used maternal care (AOR,2.01;95%CI: 1.18,3.41), husband support (AOR,3.84; 95%CI: 1.05, 14.08), high wealth index (AOR,3.61; 95%CI: 1.86, 6.99), follow-up in private health facilities (AOR, 2.27;95% CI:1.33, 3.88), having a history of risky pregnancy (AOR,2.59; 95%CI: 1.55, 4.35), and planned pregnancy (AOR,2.60;95% CI: 1.35, 4.99) were significant determinants of overall adequate ANC service utilization.ConclusionThe utilization of adequate antenatal care services is quite low. The study findings suggest that interventions should be in place to improve husband's support, social networks, and women's education. There is also a need to counsel women to utilize family planning.

Project description:BackgroundIn Ethiopia, children and mothers have been facing several health problems due to poor access to modern health care facilities and lack of effective demand to utilize the available ones. In response to this, the Ethiopian government initiated the health extension program in 2003 to improve equity in access to preventive, promotive and selected curative health interventions through health extension program. However, the level of health extension service utilization is not known. Therefore, this study presents the level of health extension service utilization and associated factors.MethodsA community based cross sectional study was carried out from February to March 2012. Data was collected through face-to-face interview by using pretested structured questionnaires adopted from review of different related literatures and entered in to EPI Info version 3.5.1. Bivariate analysis between dependent and independent variables was performed. Multivariate analysis was also done to control for possible confounding variable by selecting variable which show statistically significant association (P < 0.2) in bivariate analyses to identify independent predictor factors.ResultsThe proportion of community utilization of health extension service was 39%. Age (AOR = 2.52; 95% CI = 1.53-4.15), occupation (AOR = 3.79; 95% CI = 1.64-12.5), knowledge of community on health extension service (AOR = 0.25; 95% CI = 0.18-0.36), community participation in planning of health extension activities (AOR = 0.22; 95% CI = 0.15-0.33) and graduation of model family (AOR = 0.74; 95% CI = 0.47-0.76) have statistically significant association with community health extension services utilization.ConclusionsThe proportion of community utilization of health extension service was low. Age, occupation, knowledge of community on health extension service, community participation in planning of health extension activities and graduation of model family were identified as the independent factors affecting the community's utilization of health extension services.

Project description:Sero-epidemiological studies of brucellosis in the Afar Region showed that the disease is prevalent in livestock. However, there is little information regarding the pastoral community members' awareness about brucellosis as a cause of abortion in animals and its zoonotic importance. In this study, we assessed knowledge and perception of pastoral community members about brucellosis as a cause of abortion in animals and its zoonotic importance in Amibara district, Afar Region, Ethiopia. Between October and December 2016, a total of 475 study participants (age range 18-80 years, mean age 35.9 years) were interviewed about abortion in their animals, its causes, and diseases that can be transmitted to humans through consumption of raw milk. Almost all (97.7%) of the study participants reported that abortion in animals, especially in goats, is a major problem in the area, and they mentioned that disease (44.6%), drought (58.4%) and fly bites (29.5%) as the main causes of abortion. The study participantsalso thought that malaria (42.9%) and bovine tuberculosis (19.3%) can be transmitted to humans through consuming raw milk. Five respondents (4.2%) mentioned brucellosis (locally known as "hahayita") as a disease that can be transmitted through frequent consumption of raw milk. The majority (91.9%) mentioned malaria as a cause of febrile illness in humans and 16 (4.4%) participants mentioned brucellosis as a cause of febrile illness. Some participants also mentioned brucellosis as a cause of joint swelling (hygroma) in cattle. In conclusion, the pastoral community members in the present study area lack clear understanding about brucellosis as one of the diseases that cause abortion in their animals and its zoonotic importance. There is a need to create awareness about the zoonotic and animal health importance of brucellosis through various means such as community health extension/veterinary workers and community leaders.

Project description:BackgroundInfection with Mycobacterium bovis (Mb) predominantly causes cervical TB lymphadenitis (TBL). Raw milk is considered the main source of Mb infection and raw milk is a major food source for Afar pastoralists. The aim of this study was to assess Afar pastoralists' knowledge concerning cervical TBL and its treatment.MethodsA community-based cross-sectional survey involving 818 interviewees was conducted in two districts of the Afar Region, Ethiopia. In addition, two focus group discussions (FGDs) were conducted in each of the study areas, one with men and the other with women.ResultsOf the 818 interviewees [357 (43.6%) females and 461 (56.4%) males], 742 (90.7%) reported that they had knowledge of cervical TBL, mentioning that swelling(s) on the neck resulting in a lesion and scar are common symptoms. However, only 11 (1.5%) individuals mentioned that bacteria or germs are the causative agents of TBL. Three interviewees and a male discussant mentioned drinking raw milk as the cause of TBL. A considerable proportion (34.2%) of the interviewees and almost all the discussants suggested herbal medicine as an effective treatment. Male study participants were 1.82 times more likely to have overall knowledge of TBL than female study participants (adjusted OR, 1.82; 95% CI, 1.32 to 2.51, p < 0.001).ConclusionThe pastoral community members in the study areas had little biomedical knowledge of the cause, the source of infection and the transmission route of cervical TBL. Furthermore, most community members believed that herbal medicines are the most effective treatment for TBL. Therefore, TB control programs in the Afar Region require the incorporation of public health education introducing current biomedical knowledge of the disease. In addition, further studies are important to elucidate which medicinal plants are used by Afar pastoralists to treat TBL.

Project description:BackgroundThere is wide variation in the utilization of institutional delivery service in Ethiopia. Various socioeconomic and cultural factors affect the decision where to give birth. Although there has been a growing interest in the assessment of institutional delivery service utilization and its predictors, nationally representative evidence is scarce. This study was aimed to estimate the pooled national prevalence of institutional delivery service utilization and associated factors in Ethiopia.MethodsStudies were accessed through PubMed, Cochrane library, Web of Science, and Google Scholar. The funnel plot and Egger's regression test were used to see publication bias, and I-squared statistic was applied to check heterogeneity of studies. A weighted Dersimonian laired random effect model was applied to estimate the pooled national prevalence and the effect size of institutional delivery service utilization and associated factors.ResultTwenty four studies were included in this review. The pooled prevalence of institutional delivery service utilization was 31% (95% Confidence interval (CI): 30, 31.2%; I2 =?0.00%). Attitude towards institutional delivery (Adjusted Odd Ratio (AOR)?=?2.83; 95% CI 1.35,5.92) in 3 studies, maternal age at first pregnancy (AOR?=?3.59; 95% CI 2.27,5.69) in 4 studies, residence setting (AOR?=?3.84; 95% CI 1.31, 11.25) in 7 studies, educational status (AOR?=?2.91;95% 1.88,4.52) in 5 studies, availability of information source (AOR?=?1.80;95% CI 1.16,2.78) in 6 studies, ANC follow-up (AOR?=?2.57 95% CI 1.46,4.54) in 13 studies, frequency of ANC follow up (AOR?=?4.04;95% CI 1.21,13.46) in 4 studies, knowledge on danger signs during pregnancy and benefits of institutional delivery (AOR?=?3.04;95% CI 1.76,5.24) in 11 studies and place of birth of the elder child (AOR?=?8.44;95% CI 5.75,12.39) in 4 studies were the significant predictors of institutional delivery service utilization.ConclusionThis review found that there are several modifiable factors such as empowering women through education; promoting antenatal care to prevent home delivery; increasing awareness of women through mass media and making services more accessible would likely increase utilization of institutional delivery.

Project description:BackgroundThe populations in Ethiopia have developed their indigenous knowledge to use, manage and conserve wild edible plants (WEPs). In the eastern part of Ethiopia, wild edible plants are used as a means of survival during times of food shortage and as dietary supplements. Documenting the traditional and cultural use of wild food plants is a vital step in obtaining baseline data for investigating nutritional values and possible side effects, preserving indigenous knowledge, and ultimately interesting in wild edible plant conservation. However, their significance, management and utilization have not been documented in Mieso District. Therefore, this study aimed to provide documentation of wild edible plant use associated with ethnobotanical knowledge in Mieso District, eastern Ethiopia.MethodsAn ethnobotanical study of wild edible plant species was conducted from March 2021 to May 2022. A total of 120 (72 males and 48 females) informants were selected using the snowball method. Data collection methods, including semistructured interviews, direct observation and field walks were used. Data were analysed using descriptive statistics, including independent sample t test and analysis of variance (ANOVA). We calculated use values (UVs) to analyse the relative cultural importance of each plant species.ResultsA total of 41 wild edible plant species belonging to 33 genera in 21 families were documented to be used as food sources both during times of food shortage and as dietary supplements. Family Malvaceae was best-represented with 6 species, followed by Fabaceae and Rhamnaceae (4 species each). The dominant growth form (habit) was shrubs (30 species), followed by trees (11 species). The most widely used plant parts were fruits (covering 39 species, 95%). The largest number (23 species) was collected from forest habitats only, followed by both village and forest habitats (8 species). The majority of wild edible plants (28 species or 68%) were consumed only during famine or in the time of food shortage followed by supplementing staple foods (9 species or 22%). Wild edible plants in Mieso are used for multiple other uses, including for fodder, fuel, medicine, construction, cosmetics and bee keeping. Twenty-three species were mentioned for fodder use, followed by fuel purpose (21 species) and medicinal value (13 species). The species that had the highest use values were Flacourtia indica (Burm.f.) Merr. (1.4), Carissa spinarum L. (1.1), Ziziphus spina-christi (L.) Desf. (0.6), Grewia villosa Willd. (0.5), Cordia monoica Roxb. (0.3) and Opuntia ficus-indica (L.) Mill. (L.) (0.2). Most WEPs were collected from March to May (Badheysa) (33 species). The highest mentioned wild edible plant sold in the market was F. indica (Burm.f.) Merr. mentioned by 20 informants, followed by Z. spina-christi (L.) Desf. (14).ConclusionThe people in Mieso use wild plants as supplementary food to cultivated crops, during famine, and many could be utilized for day-to-day human consumption. Some plants in the district provide cash income for local people. However, deforestation (54%), drought (22%) and agricultural expansion (12%) were the highest threats to wild plants in Mieso District. Hence, on-site and off-site conservation would help protect wild plant resources in Mieso, eastern Ethiopia.

Project description:BackgroundEthiopia has been striving to promote institutional delivery through community wide programs. However, home is still the preferred place of delivery for most women encouraged by the community`s perception that delivery is a normal process and home is the ideal environment. The proportion of women using institutional delivery service is below the expected level. Therefore, we examined the impact of perception on institutional delivery service use by using the health belief model.MethodsA community-based cross-sectional study was conducted among 1,394 women who gave birth during the past 1 year from September to December 2019. A multistage sampling technique was used to select the study participants. Data were collected by using health belief model constructs, and structured and pretested questionnaire. Binary logistic regression was performed to identify factors associated with the outcome variable at 95% confidence level.ResultsInstitutional delivery service was used by 58.17% (95% CI: 55.57- 60.77%) of women. The study showed that high perceived susceptibility (AOR = 1.87; 95% CI 1.19-2.92), high cues to action (AOR = 1.57; 95% CI: 1.04-2.36), husbands with primary school education (AOR = 1.43; 95% CI 1.06-1.94), multiparty(5 or more) (AOR = 2.96; 95% CI 1.85-4.72), discussion on institutional delivery at home (AOR = 4.25; 95% CI 2.85-6.35), no close follow-up by health workers (AOR = 0.59;95% CI 0.39-0.88), regular antenatal care follow-up (AOR = 1.77;95% CI 1.23,2.58), health professionals lack of respect to clients (AOR = 2.32; 95% CI 1.45-3.79), and lack of health workers (AOR = 0.43;95% CI 0.29-0.61) were significantly associated with the utilization health behavior of institutional delivery service.ConclusionThe prevalence of institutional delivery in the study area was low. The current study revealed that among the health belief model construct perceived susceptibility and cues to action were significantly associated with the utilization behavior of institutional delivery service. On top of that strong follow-up of the community and home based discussion was a significant factor for the utilization behavior of institutional delivery service.
