Project description:ObjectiveThe objective of this report is to describe measles activity in Canada during 2013, in order to support the documentation and maintenance of measles elimination status.MethodsA descriptive analysis of measles counts and incidence by age group, immunization history, hospitalization and province/territory, as well as a summary of 2013 outbreaks, was conducted using enhanced measles data captured through the Canadian Measles and Rubella Surveillance System. Genotype information and phylogenetic analysis for 2013 were summarized.ResultsIn 2013, 83 confirmed measles cases were reported in seven provinces/territories for an incidence rate of 2.4 per 1,000,000 population. Incidence was highest in the youngest age groups (< 1 year, 1 to 4 years). Burden of disease was highest in the youngest age groups and children 10 to 14 years. Three-quarters of cases had been inadequately immunized, and 10% were hospitalized. There were nine measles outbreaks reported in 2013, one of which consisted of 42 cases in a non-immunizing community in Alberta.Discussion2013 saw the fifth highest number of reported measles cases since 1998. While we continue to face challenges related to importation and heterogeneous immunization coverage, in 2013 Canada met or partially met all four criteria outlined by the Pan American Health Organization for measles elimination.

The Public Health Agency of Canada compiles evidence for the Pan American Health Organization to confirm that criteria for the verification of measles elimination continue to be met.ObjectiveTo describe measles activity in Canada for 2014 in order to support Canada's ongoing measles elimination status.MethodsUsing data captured by the Canadian Measles and Rubella Surveillance System and the Measles and Rubella Surveillance pilot project during 2014, the distribution of measles cases by demographics, immunization status and hospitalization were assessed, outbreak characteristics were summarized and genotypic and phylogenetic analyses were conducted and described.ResultsDuring 2014, 418 measles cases were reported by five provinces and territories for an overall incidence rate of 11.8 cases per 1,000,000 population. Case counts and incidence rates were highest among those five to 14 years of age and the majority of cases were not immunized. Overall, five percent of cases were hospitalized, most frequently the youngest and oldest age groups. Eighteen outbreaks were reported, the largest of which occurred in a non-immunizing religious community in British Columbia. Genotype information was available for 98% of measles events (18/18 outbreaks and 31/32 sporadic cases). Canada continued to meet or partially meet all four of PAHO's criteria for verification of measles elimination.ConclusionDespite significant measles activity in 2014, Canada continues to provide strong evidence that measles elimination status is being maintained.

Methods:Measles surveillance data were captured by the Canadian Measles and Rubella Surveillance System (CMRSS) and the Measles and Rubella Surveillance (MARS) pilot project and assessed for distribution by demographics and risk factors. Outbreak characteristics were summarized and genotypic and phylogenetic analyses were conducted and described. Surveillance data for 2015 were evaluated against PAHO's essential criteria for measles elimination status. Results:In 2015, the incidence of measles in Canada was 5.5 cases per 1,000,000 population, with 196 cases across four provinces. The majority of cases (87.2%, n=171) were not immunized and both age-specific incidence rates and case counts were highest among those aged 10 to 14 years (29.5 cases per 1,000,000 population, n=55). This was due in large part to a sizeable outbreak in a non-immunizing religious community. Overall, 10.7% (n=21) of cases were hospitalized. Genotype information was available for 100% of measles events (4/4 outbreaks and 6/6 sporadic cases). Canada met or partially met most of PAHO's criteria for verification of measles elimination. Conclusion:Although importations and areas of low immunization coverage continue to challenge Canada's elimination status, surveillance data for 2015 provides strong evidence that measles elimination has been maintained.

Outbreak characteristics were summarized and genotypic analyses conducted. Surveillance data for 2018 were evaluated against PAHO's essential criteria for measles elimination status.ResultsIn 2018, 29 measles cases were reported across five provinces in Canada, an incidence rate of 0.8 cases per 1,000,000 population. Of these 29 cases, 16 were imported and five resulted in further transmission within Canada. The age-specific incidence rate was highest among those aged younger than one year (10.2 cases per 1,000,000 population, n=4). Only nine cases were considered up-to-date for measles vaccination, and 11 cases were hospitalized. Genotype information was available for most of the measles cases (n=27); they were all found to be genotypes that circulated globally in 2018. Canada met or partially met three out of four of PAHO's criteria for verification of measles elimination.ConclusionAlthough importations and areas of low vaccination coverage continue to challenge Canada's elimination status, there is no evidence that endemic transmission of the measles virus has been re-established. Canada maintains its measles elimination status.

Project description:BackgroundThe Public Health Agency of Canada (PHAC) has conducted enhanced measles surveillance since 1998, the year endemic measles transmission was eliminated in Canada. The objective of this annual national measles surveillance report is to provide an epidemiologic summary of measles activity reported in Canada for 2019 in order to provide evidence to support the continued verification of Canada's measles elimination status.MethodsMeasles surveillance data are housed in the Canadian Measles and Rubella Surveillance System (CMRSS) database. Descriptive analyses of demographics and risk factors were performed. Outbreak characteristics were summarized and genotypic analyses conducted. Surveillance, laboratory and vaccine coverage data for 2019 were used to assess Canada's status against the Pan American Health Organization (PAHO) essential criteria for the verification of measles elimination.ResultsIn 2019, 113 measles cases were reported in Canada (crude incidence rate of 3.0 cases per 1,000,000 population). Of these cases, 42 (37%) were imported into Canada, and of the imported cases, 12 (29%) resulted in further transmission. Infants younger than one year had the highest age-specific incidence rate at 13.1 cases per 1,000,000 population. Only 29% of cases had one or more documented doses of measles-containing vaccine. One-fifth (19%) of cases were hospitalized; no deaths were reported. Genotype information was available for 100% of outbreaks reported in 2019 and 90% of non-outbreak-related measles cases; of cases with genotype information available, 27% were B3 and 73% were D8.ConclusionDespite meeting/partially meeting only three out of four of PAHO's essential criteria for measles elimination status, there is no evidence that endemic measles transmission has been reestablished in Canada.

Project description:Measles elimination relies on vaccination programmes. In Japan, a major outbreak started in 2007. In response, 5-year two-dose catch-up vaccination programme was initiated in April 2008 for children 13-16-years-old. In this study, we analysed the epidemic curves, incidence rates for each age group, virus genotype, vaccination coverage and ratio of measles gelatin particle agglutination (PA) antibody using surveillance data for 2008-2015. Monthly case counts markedly decreased as vaccination coverage increased. D5, which is the endemic virus type, disappeared after 2011, with the following epidemic caused by imported viruses. Most cases were confirmed to have a no-dose or single-dose vaccination status. Although the incidence rate among all age groups ⩾5-years-old decreased during the study period, for children <5-years-old, the incidence rate remained relatively high and increased in 2014. The ratio of PA antibody (⩾1:128 titres) increased for the majority of age groups, but with a decrease for specific age groups: the 0-5 months and the 2-4, 14, 19 and most of the 26-55- and the 60-year-old groups (-1 to -9%). This seems to be the result of higher vaccination coverage, which would result in decreasing natural immunity booster along with decreasing passive immunity in infants whose mothers did not have the natural immunity booster. The 20-29- and 30-39-year-old age groups had higher number of cases, suggesting that vaccination within these age groups might be important for eliminating imported viruses.

Project description:There is a paucity of data on the reason for and the quantity of antimicrobials used in broiler chickens in Canada. To address this, the Canadian Integrated Program for Antimicrobial Resistance Surveillance (CIPARS) implemented surveillance of antimicrobial use (AMU) and antimicrobial resistance (AMR) in broiler chicken flocks in 2013. Shortly after this (2014), the poultry industry banned the preventive use of ceftiofur in broiler chickens. The objectives of this analysis were to describe antimicrobial use (AMU) in Canadian broiler chickens between 2013 and 2015 (n = 378 flocks), compare these results to other animal species in Canada, to highlight the utility of farm surveillance data to evaluate the impact of a policy change, and to explore how different antimicrobial use metrics might affect data interpretation and communication. The surveillance data indicated that the poultry industry policy resulted in lower antimicrobial use and resistance, and they successfully captured information on when, where, why, and how much antimicrobials were being used. The majority of antimicrobials were administered via the feed (95%). The relative frequency of antimicrobial classes used in broiler chickens differed from those used in swine or in food animal production in general. Coccidiostats were the most frequently used antimicrobial classes (53% of total kg). Excluding coccidiostats, the top three most frequently used antimicrobial classes were bacitracin (53% of flocks), virginiamycin (25%) and avilamycin (21%), mainly used for the prevention of necrotic enteritis. Depending on the AMU metric utilized, the relative rankings of the top antimicrobials changed; hence the choice of the AMU metric is an important consideration for any AMU reporting. When using milligrams/Population Correction Unit (mg/PCU) the top three antimicrobial classes used were bacitracins (76 mg/PCU), trimethoprim-sulfonamides (24 mg/PCU), and penicillins (15 mg/PCU), whereas when using a number of Defined Daily Doses in animals using Canadian standards /1,000 chicken-days at risk (nDDDvetCA/1,000 CD) the ranking was bacitracins (223 nDDDvetCA/1,000 CD), streptogramins (118 nDDDvetCA/1,000 CD), and trimethoprim-sulfonamides (87 nDDDvetCA/1,000 CD). The median animal treatment days in feed for one cycle (ATD/cycle) during the three-year study were 34 ATD/cycle; this was equal to the mean age of the flocks at pre-harvest sampling day (days at risk), indicating that the studied flocks except those that were raised without antibiotics and organic, were fed with medicated rations throughout the observation period. Overall, more than half (59%) of antimicrobials used in broiler chickens were in classes not used in human medicine, such as ionophores and chemical coccidiostats aimed to prevent coccidiosis. Compared to grower-finisher pigs and in production animal species (national sales data), the mg/PCU of antimicrobials used in broiler chickens was relatively lower. The findings of this paper highlighted the importance of farm-level AMU surveillance in measuring the impact of interventions to reduce antimicrobials in poultry.

Project description:BACKGROUND:Historical changes in the nature of sedentary activities have been observed in other countries, but it is not clear if similar trends exist in Canada. It is also unclear how changes in the measurement of sedentary behaviour affects national estimates. Our objective is to document all sources and measures of sedentary behaviour from Canadian, nationally representative surveys, and report on selected estimates of time spent in sedentary activities. Lessons learned can benefit the wider international surveillance community. METHODS:We describe and document all data sources of sedentary behaviour at the national level in Canada, and report on selected prevalence data from repeated cross-sectional surveys. We summarize amounts of total device-assessed sedentary time and self-reported sedentary activities (e.g., passive travel, leisure television, computer, video games, screen, and reading) by age group over time. RESULTS:Nineteen national surveys were identified. Changes in questions and/or response categories precluded direct assessment of trends over time for some measures; however, certain trends were observed. Accelerometer-measured sedentary time, leisure reading (among those <?50?years) and television/video viewing in younger age groups have remained relatively stable (with a possible slight decline in television/video viewing). Time spent in passive travel and leisure computer and electronic device use appears to have increased. Television and video viewing appears to have increased in older adults while their leisure reading appears to have fallen. CONCLUSIONS:Changes in measurement of sedentary behaviour can affect estimates and reduce comparability over time. Total leisure screen use appears to have increased over time, reflecting the ways in which Canadians spend their free time and technological advances. The main public health message is the need for continued efforts to reduce leisure screen use, especially among youth and older adults.

Project description:Heavy drinking is a major factor in morbidity and mortality worldwide. Little information is available on trends in Canada regarding alcohol abuse. We sought to estimate abstinence, binge drinking and alcohol intake exceeding low-risk drinking guidelines in the Canadian population from 1996 to 2013.The data sources for this analysis were a series of cross-sectional national health surveys of the Canadian population carried out by Statistics Canada between 1996 and 2013. These were cross-sectional files from the National Population Health Surveys of 1996 and 1998, plus the Canadian Community Health Surveys from 2000 to 2013. Respondents were aged 18 years and older.The proportion of binge drinkers increased steadily from 13.7% (95% confidence interval [CI] 13.2%-14.2%) in 1996 to 19.7% (95% CI 19.1%-20.3%) in 2013. The corresponding proportions for men were 20.8% (95% CI 19.9%-21.7%) in 1996, and 25.7% (95% CI 24.7%-26.6%) in 2013; for women, these proportions were 6.9% (95% CI 6.4%-7.5%) in 1996, and 13.8% (95% CI 13.1%-14.5%) in 2013. No significant increases were seen in the proportion of people who exceeded low-risk drinking guidelines or of abstainers during the same period.The rate of self-reported binge drinking in Canada has increased from 1996 to 2013, relatively more so among women than among men. No evidence of an increase in the proportion of people exceeding low-risk drinking guidelines or of abstainers was seen during the same period. These results suggest that binge drinking is of particular concern regarding intervention strategies aimed at improvement of public health.

Project description:BackgroundRoutine surveillance of congenital anomalies has shown recent increases in ankyloglossia (tongue-tie) in British Columbia, Canada. We examined the temporal trends in ankyloglossia and its surgical treatment (frenotomy).MethodsWe conducted a population-based cohort study involving all live births in British Columbia from Apr. 1, 2004, to Mar. 31, 2014, with data obtained from the province's Perinatal Data Registry. Spatiotemporal trends in ankyloglossia and frenotomy, and associations with maternal and infant characteristics, were quantified using logistic regression analysis.ResultsThere were 459 445 live births and 3022 cases of ankyloglossia between 2004 and 2013. The population incidence of ankyloglossia increased by 70% (rate ratio 1.70, 95% confidence interval [CI] 1.44-2.01), from 5.0 per 1000 live births in 2004 to 8.4 per 1000 in 2013. During the same period, the population rate of frenotomy increased by 89% (95% CI 52%-134%), from 2.8 per 1000 live births in 2004 to 5.3 per 1000 in 2013. The 2 regional health authorities with the lowest population rates of frenotomy (1.5 and 1.8 per 1000 live births) had the lowest rates of ankyloglossia and the lowest rates of frenotomy among cases with ankyloglossia, whereas the 2 regional health authorities with the highest population rates of frenotomy (5.2 and 5.3 per 1000 live births) had high rates of ankyloglossia and the highest rates of frenotomy among cases of ankyloglossia. Nulliparity, multiple birth, male infant sex, birth weight and year were independently associated with ankyloglossia.InterpretationLarge temporal increases and substantial spatial variations in ankyloglossia and frenotomy rates were observed that may indicate a diagnostic suspicion bias and increasing use of a potentially unnecessary surgical procedure among infants.
