Surgical technique for management of isolated lenticular coloboma with high corneal astigmatism.

ABSTRACT: We describe a surgical technique for the correction of isolated congenital lenticular coloboma associated with high corneal astigmatism. Transscleral fixation of the capsular bag with a single eyelet Cionni capsular tension ring was followed by in-the-bag implantation of a toric intraocular lens (IOL). This lead to complete correction of the lenticular defect and perfect alignment of the toric lens. In this case, the child attained an unaided distance visual acuity of 20/30 following amblyopia therapy and a well-aligned toric IOL at 12 months of follow-up. This technique can be used in cases with concomitant lenticular coloboma and significant corneal astigmatism.

Contribution of Posterior Corneal Astigmatism to Total Corneal Astigmatism in a Sample of Egyptian Population.

Project description:Purpose:To determine the percentage of contribution of the magnitude of posterior corneal astigmatism to total corneal astigmatism using Scheimpflug imaging. Methods:This prospective cross-sectional study was conducted on 356 eyes of 356 patients, where the total corneal astigmatism was calculated by addition of anterior and posterior corneal astigmatism using vector analysis and then the percentage of posterior to total corneal astigmatism was calculated. Results:The percentage of contribution of posterior to total corneal astigmatism was about 30% in patients with With The Rule astigmatism and about 8% in patients with Against The Rule astigmatism. Conclusion:Posterior corneal astigmatism should not be neglected during calculation of total corneal astigmatism as neglecting posterior corneal astigmatism can result in errors during calculation and correction of astigmatism.

Ocular residual and corneal astigmatism in a clinical population of high school students.

Project description:Total refractive astigmatism is usually the first consideration that guides the selection of contact lens type (e.g., spherical or toric), while the ocular source of the astigmatism is a second, but more important consideration, for the final clinical decision. This study was conducted to provide detailed data on this topic by evaluating astigmatic components in Chinese adolescents.Participants were recruited from healthy high school students undergoing an annual ocular examination at a local hospital. Total astigmatism (TA), corneal astigmatism (CA), and ocular residual astigmatism (ORA) were determined by a Hartmann-Shack wavefront analyzer system (KR-1W, Topcon) with the natural pupil. The axis relationship between CA and ORA was placed into three categories: on-axis, defined as an axis with a difference of 0 ± 10°; opposite-axis, a difference of 90 ± 10°; and the rest defined as oblique-axis.The study consisted of 1,466 students (57.84% girls, age: 16.49 ± 1.05 years). ORA was present in 83.97%, 66.64%, and 45.23% of participants, according to the various criteria for astigmatism (≥ 0.50 D, ≥ 0.75 D, and ≥ 1.00 D, respectively). While with-the-rule was the most common axis orientation for both TA (76.28%) and CA (89.94%), against-the-rule predominated in ORA (93.82%; χ2 = 1688.544, p < 0.001). Opposite-axis was the major type of axis difference (90.96%) of clinical significance (i.e., ≥ 1.00 D) between CA and ORA, which also prevailed in all levels of TA (range: 56.25-82.26%).ORA is common in high school students and usually demonstrates a compensation relationship with CA, which should be taken into consideration when determining the design of contact lenses to correct refractive error.

Corneal Elevation, Power, and Astigmatism to Assess Toric Orthokeratology Lenses in Moderate-to-High Astigmats.

Project description:ObjectivesFitting philosophies for toric orthokeratology are based on elevation or corneal astigmatism, but it is unclear which is more effective. The purpose of this analysis was to further understand corneal shape and the relationship between peripheral elevation and central astigmatism in moderate-to-high astigmats.MethodsCorneal tomography was measured three times on the right eyes of 25 moderate-to-high refractive myopic astigmatic adults. Corneal astigmatism and elevation were calculated at 4-, 6-, and 8-mm chords. Subjects were fitted with toric orthokeratology lenses following the manufacturer's guidelines based on elevation. Twenty subjects completed 10 days of wear. A masked examiner assessed movement and centration via slitlamp videos and quantified treatment zone and decentration from tangential power difference tomography maps. Correlations between variables were assessed.ResultsAverage corneal astigmatism was 2.20±0.70 DC and peripheral elevation was 50.88±18.92 μm and they were strongly correlated (4 mm R2=0.96, 6 mm R2=0.92, 8 mm R2=0.86, all P<0.001). Each diopter of astigmatism equated to 25 μm of elevation at an 8-mm chord. Via slitlamp, average treatment zone area was 12.73±4.62 mm2 and 13 lenses decentered. From tomography, average treatment zone area was 7.16±2.56 mm2 and 17 were decentered. Tomography treatment zone area was negatively correlated with central corneal astigmatism (R2=0.60) and elevation at an 8-mm chord (R2=0.64, both P<0.001).ConclusionsFor tomography images, central corneal astigmatism was highly correlated with peripheral elevation and may be a more expedient measure for clinical use. Treatment area decreased as corneal astigmatism and elevation increased.

Interocular Symmetry of Fixation, Optic Disc, and Corneal Astigmatism in Bilateral High Myopia: The Shanghai High Myopia Study.

Project description:Purpose:We investigate the interocular symmetry of fixation, optic disc, and corneal astigmatism in bilateral high myopia, and evaluate the predictive relationships between them. Methods:We enrolled 202 cases with bilateral high myopia. Fixation, in terms of the bivariate contour ellipse area (BCEA), was evaluated with the Macular Integrity Assessment microperimetry. Optic disc features, including orientation, tilt, and rotation, were evaluated with ultrawide-field retinal photographs. Corneal topography was performed with Pentacam. Interocular symmetry of fixation, optic disc, and corneal astigmatism was assessed, and the predictive relationships between these parameters were investigated. Results:Axial length differences between the two eyes were: ?0 to ?1 mm, 67.8%; 1 to ?2 mm, 20.3%; 2 to ?3 mm, 9.4%; and >3 mm, 2.5%. Axial length, 95% BCEA, and magnitude of corneal astigmatism showed good interocular symmetry, whereas the optic disc tilt, rotation, and axis of corneal astigmatism (mirror axes) showed less symmetry (all P < 0.05). No interocular symmetry was observed in the direction of the fixation ellipse. In both eyes, the corneal steep meridian more often was consistent with the optic disc orientation than inconsistent (right eye [OD], P < 0.001; left eye [OS], P = 0.029). Conclusions:As different parameters presented different degrees of symmetry, cautions are needed when including both eyes or only one lateral eye in cases of bilateral high myopia for clinical investigations. The optic disc orientation, to some extent, may indicate the steep meridian of the cornea. Translational Relevance:Our study provided evidences for selection of eye laterality in clinical investigations of highly myopic eyes.

Contributions of Anterior Corneal and Ocular Residual Astigmatism to Autorefraction Astigmatism in a Myopic Adult Sample

Project description:Aim To evaluate the contributions of anterior corneal and ocular residual astigmatism to autorefraction astigmatism in adult myopic and myopic astigmatic subjects and how these compensate each other. Subjects and Methods This retrospective study was completed in private eye centre, Ismailia, Egypt, between September 2017 and November 2019. The study included eyes with myopia (0.5 to 10.0D) or myopic astigmatism (0 to 8.5D). The refractive errors, including autorefraction astigmatism, were measured after using 1% cyclopentolate with autorefractometer (Topcon, Tokyo Optical Co., Ltd., Japan). Corneal topography (Sirius; CSO, Florence, Italy) was used to measure anterior corneal astigmatism. Ocular residual astigmatism was measured by vectorial subtraction of the anterior corneal astigmatism from autorefraction astigmatism determined to the corneal plane. Results This study included 1158 eyes (right 582 and left 576) with myopia or myopic astigmatism of 582 participants (206 males with 406 eyes and 376 females with 752 eyes). The mean±SD age of the total participants was 26±5.7 years, range (21 to 50 years). The mean±SD of spherical error was ?3.2±1.9D, range (?0.5 to ?10.0D). The mean±SD of autorefraction astigmatism was 1.13±1.1D; range (0.5 to 8.5D). The mean±SD of anterior corneal astigmatism was 1.22±0.8D; range (0.03 to 5.6D). The mean±SD of ocular residual astigmatism was 0.6±0.5D; range (0 to 4.8D). Of the total eyes, 75.4% had significant autorefraction astigmatism, 82.5% had significant anterior corneal astigmatism, and 16.8% had significant ocular residual astigmatism. Conclusion The percentage of the significant autorefraction astigmatism (>0.5D) was 75.4% which is mainly anterior corneal. In 26.8% of participants, anterior corneal astigmatism is compensated by ocular residual astigmatism.

Hybrid clear corneal micro-incision surgical technique for stage 5 retinopathy of prematurity.

Project description:A safe technique for entry incisions and closure in stage 5 retinopathy of prematurity (ROP) surgery is being described. Three 23G clear corneal incisions are made which allow for safe and snug entry of 25G calibrated infusion and 25G instruments for performing lensectomy, membrane removal and vitrectomy. At the end of surgery, air is injected and corneal entries are hydrated for sutureless closure. The technique was performed in 50 eyes of 36 children with stage 5 ROP. The hybrid technique ensured safe entry and exit with stable anterior chamber during surgery. None of the cases developed retinal breaks during surgical entry nor had any complications such as hypotony, flat anterior chamber, hyphaema or corneal edema in post operative period. Clear corneal entry using 23G incisions for 25G instrument access is a safe and effective technique for performing lensectomy and vitrectomy with sutureless closure in cases with stage 5 ROP.

Surgical Technique for Management of Severe Calcaneofibular Impingement: Case Series.

Project description:BackgroundCalcaneofibular impingement is characterized by lateral hindfoot pain and is commonly resulting from calcaneal fracture malunion or severe flatfoot deformity. Lateral calcaneal wall decompression has been used successfully to relieve pain in patients who have calcaneofibular impingement. However, in cases of severe impingement and hindfoot valgus, lateral wall excision may leave only a small remnant of calcaneal bone for weightbearing and can lead to chronic heel pain. We describe a surgical technique using a medial displacement calcaneal osteotomy (MDCO) combined with a lateral wall exostectomy and report on the outcomes from our series of patients.MethodsRetrospective study of a single surgeon's patients was done from 2010 to 2020 who underwent medial slide calcaneal osteotomy and lateral wall exostectomy for calcaneofibular impingement. Descriptive statistics were used to summarize patient characteristics. Our study included 9 patients, 6 females and 3 males, with a mean age of 59 years (range: 19-77) and a mean follow-up of 62 weeks (range: 6-184).ResultsFive had an Achilles split approach, 2 had an oblique lateral approach, and 1 had an extensile lateral approach. Patients achieved radiographic relief of impingement and improvement in pain. Minor skin and soft tissue complications occurred in 3 patients, all of which were associated with laterally based incisions, and all resolved after a 10-day course of oral antibiotics. No major complications, emergency department visits, or readmissions occurred.ConclusionsMDCO and lateral wall exostectomy was a safe and effective treatment for severe calcaneofibular impingement.Level of evidenceLevel IV, retrospective case series.

A novel blunt dissection technique to treat modified deep anterior lamellar keratoplasty (DALK)-associated high astigmatism.

Project description:ObjectivesTo describe a new surgical technique to correct a high astigmatism following modified deep anterior lamellar keratoplasty (DALK) and to evaluate the associated clinical outcomes.MethodsRetrospective interventional case series included eyes with high astigmatism following modified DALK performed between July 2012 and June 2016. The prevalence of keratometric astigmatism (KA) higher than 4.5 diopters (D) was evaluated after complete suture removal. Surgical correction was obtained by opening the surgical wound at the steeper meridian and carrying out a blunt dissection at the graft-host surface of contact under keratoscopic control. KA, Refractive astigmatism (RA), and best spectacle-corrected visual acuity (BSCVA) were measured preoperatively and at 1, 12, and 24 months postoperatively. Vector analysis of 1 year change in KA was performed.ResultsHigh astigmatism was found in 47 of 511 eyes (9.2%). KA (mean ± standard deviation [SD]) decreased from 7.67 ± 2.78 to 3.10 ± 1.37 D at 1 month after astigmatic correction (p < 0.001) with no subsequent significant change. Vector analysis of 1 year change in KA: surgically induced astigmatism was 5.30 ± 3.29 D and mean absolute angle of error was 6.97 ± 4.25°. BSCVA improved from 0.21 ± 0.20 logMAR preoperatively to 0.11 ± 0.13 logMAR (p < 0.001); RA decreased from 6.32 ± 2.56 to 2.61 ± 1.05 D (p < 0.001). No intra- or post-operative complications occurred.ConclusionsHigh astigmatism is present in <10% of cases after modified DALK and can be effectively managed by means of blunt relaxing dissection at the graft-host junction.

Influence of peripheral corneal relaxing incisions during cataract surgery for corneal astigmatism up to 2.5 dioptres on corneal densitometry.

Project description:PurposeTo assess the effect of peripheral corneal relaxing incisions (PCRI) for astigmatism between 0.75 and 2.5 dioptres during cataract surgery on corneal densitometry (CD).MethodsIn this prospective, randomised study, 80 eyes (80 patients), received either tIOL or PCRI. Assessment at pre-operative and 1, 3, 6, 12 months post-operative visit included uncorrected (UCDVA) and best-corrected distance visual acuity (BCDVA), spherical equivalent (SEQ) (only post-operatively), mean anterior and posterior keratometric astigmatism (KA) and CD with Schiempflug system. CD was analysed in four concentric radial zones from centre to periphery (Zone 1 to 4) and in 3 layers (anterior, mid-stromal and posterior).ResultsComparing tIOLs vs. PCRIs, there was no significant difference in the UCDVA, BCDVA and SEQ. In PCRI group, anterior KA decreased at 1 month and remained stable thereafter. For zones 1, 2 and 3, CD significantly reduced after 3 months with tIOLs whereas with PCRIs, it reduced 1 month onwards. For zone 4, CD reduced only at 12 months with tIOLs compared to 3 months onwards with PCRIs. In both groups CD was higher in the zone 4 and anterior layer. Significant reduction in CD was found in all three layers of cornea after 3 months in tIOL and after first month in PCRI groups, respectively.ConclusionsCataract surgery alone reduces the CD. Reducing keratometric astigmatism with PCRIs shows significant differences in CD from early post-operative period for central and anterior corneal layer.

Surgical management of complete atrioventricular septal defect: associations with surgical technique, age, and trisomy 21.

Project description:ObjectivesWe sought to evaluate the contemporary results after repair of a complete atrioventricular septal defect and to determine the factors associated with suboptimal outcomes.MethodsThe demographic, procedural, and outcome data were obtained within 1 and 6 months after repair of a complete atrioventricular septal defect in 120 children in a multicenter observational study from June 2004 to 2006.ResultsThe median age at surgery was 3.7 months (range, 9 days to 1.1 years). The type of surgical repair was a single patch (18%), double patch (72%), and a single atrial septal defect patch with primary ventricular septal defect closure (10%). The incidence of residual septal defects and the degree of left atrioventricular valve regurgitation (LAVVR) did not differ by repair type. The median interval of intensive care stay were 4 days, ventilation use 2 days, and total hospitalization 8 days. All were independent of the presence of trisomy 21 (80% of the cohort). The in-hospital mortality rate was 2.5% (3/120). The overall 6-month mortality rate was 4% (5/120). The presence of associated anomalies and younger age at surgery were independently associated with a longer hospital stay. The age at repair was not associated with residual ventricular septal defect or moderate or greater LAVVR at 6 months. Moderate or greater LAVVR occurred in 22% at 6 months, and the strongest predictor for this was moderate or greater LAVVR at 1 month (odds ratio, 6.9; 95% confidence interval, 2.2-21.7; P < .001).ConclusionsThe outcomes after repair of complete atrioventricular septal defect did not differ by repair type or the presence of trisomy 21. An earlier age at surgery was associated with increased resource use but had no association with the incidence of residual ventricular septal defect or significant LAVVR.
