Project description:This paper describes one large multi-annual research project-CaliBRANDO-about subjective wellbeing in a developing country. CaliBRANDO is a life satisfaction measurement system implemented in Cali, the third largest city in Colombia, South America. Data have been collected annually since 2014 and aim at collecting comprehensive temporal information about individual-level subjective wellbeing and its relationship with government performance. CaliBRANDO is the only study in Colombia that measures subjective wellbeing in a large city in this way. This paper presents the methodology followed in the study and discusses the relevance of the data collected.

Project description:BackgroundCoagulation abnormality is a significant complication and cause of mortality in children with uncorrected congenital heart defects (CHD). The aim of this study was to determine the prevalence of coagulation abnormalities and the associated factors in children with uncorrected CHD.MethodA cross sectional study conducted to determine the prevalence of coagulation abnormalities among 70 children with uncorrected CHD aged six months to 17 years and 70 age and sex matched apparently healthy controls. Coagulation abnormalities was determined using complete blood count, prothrombin time, activated partial thromboplastin time and D-dimer assay.ResultsThe prevalence of coagulation abnormalities among children with CHD and controls was 37.1% and 7.1% respectively. Children with Cyanotic CHD had a significantly higher prevalence of coagulation abnormalities compared to children with Acyanotic CHD (57.1% versus 17.1%). Haematocrit and oxygen saturation levels were significantly associated with coagulation abnormalities.ConclusionThis study affirms that coagulation abnormalities are frequent in children with uncorrected CHD. Oxygen saturation and haematocrit are risk factors of coagulation abnormalities. Routine coagulation screen is recommended especially in children with cyanotic congenital heart defects to improve their quality of life and reduce morbidity and mortality while awaiting definitive surgeries.

Project description:This paper presents an overview of the activities and outcomes of the Leadership Institute (LI), a short-term leadership development professional development course offered to physiotherapists in a low-resource country. Previous studies have provided examples of the benefits of such programs in medicine and nursing, but this has yet to be documented in the rehabilitation literature. The prototype of leadership development presented may provide guidance for similar trainings in other low-resource countries and offer the rehabilitation community an opportunity to build on the model to construct a research agenda around rehabilitation leadership development.The course used a constructivist approach to integrate participants' experiences, background, beliefs, and prior knowledge into the content. Transformational leadership development theory was emphasized with the generation of active learning projects, a key component of the training.Positive changes after the course included an increase in the number of community outreach activities completed by participants and increased involvement with their professional organization. Thirteen leadership projects were proposed and presented.The LI provided present and future leaders throughout Rwanda with exposure to transformative leadership concepts and offered them the opportunity to work together on projects that enhanced their profession and met the needs of underserved communities.Challenges included limited funding for physiotherapy positions allocated to hospitals in Rwanda, particularly in the rural areas. Participants experienced difficulties in carrying out leadership projects without additional funding to support them.While the emphasis on group projects to foster local advocacy and community education is highly recommended, the projects would benefit from a strong long-term mentorship program and further budgeting considerations.The LI can serve as a model to develop leadership skills and spur professional growth in low-resource settings. Leadership development is necessary to address worldwide inequities in health care. The LI model presents a method to cultivate transformational leadership and work toward improvements in health care and delivery of service.

Project description:This article presents data that examined the modeling of nexus between entrepreneurs? commitment and business performance in a developing country. 315 copies of questionnaire were retrieved from 400 copies that were administered to Nigerian Association of Small Scale Industrialists (NASSI) Southwest chapters which comprised of six states. In addition to descriptive analysis of part of the data, correlation and regression analysis were used to present the data through structural equation model (SEM). The SEM path analysis shows the estimates of the interconnectedness of the major constructs in the data.

Project description:BackgroundThis study has analyzed the impact of applying a decision training program, in which video-feedback and questioning were used, on the development of decision-making, skill execution and procedural knowledge in basketball players.MethodsParticipants were eleven male players aged between 12 and 13 years old (M age = 12.75, SD age = .65), who were assigned to an experimental or control group within a pre-test/intervention test/retention test quasi-experimental design. The decision training program was applied over 11 weeks. Throughout this intervention, players had to analyze the causes and reasons for the decision made, using video feedback and questioning to this end. Decision-making and skill execution variables were analyzed using the French & Thomas (1987) observation instrument, while a validated questionnaire was used to assess procedural knowledge in basketball.ResultsThe results reported that sport expertise improved in players from the experimental group, who had significantly higher intervention test scores for successful decisions and skill executions when compared to players in the control group. In the intra-group analysis, the experimental group significantly improved in the intervention test compared to the pre-test, in terms of some of the variables of decision-making, skill execution and procedural knowledge.DiscussionThese results reinforce the idea of including cognitive tools in training, such as video-feedback and questioning, to improve sport expertise in players' formative stages, and presumably to improve their performance whilst maintaining decision training throughout time.

Project description:Developing countries can generate effective solutions for today's global health challenges. This paper reviews relevant literature to construct the case for international cooperation, and in particular, developed-developing country partnerships. Standard database and web-based searches were conducted for publications in English between 1990 and 2010. Studies containing full or partial data relating to international cooperation between developed and developing countries were retained for further analysis. Of 227 articles retained through initial screening, 65 were included in the final analysis. The results were two-fold: some articles pointed to intangible benefits accrued by developed country partners, but the majority of information pointed to developing country innovations that can potentially inform health systems in developed countries. This information spanned all six WHO health system components. Ten key health areas where developed countries have the most to learn from the developing world were identified and include, rural health service delivery; skills substitution; decentralisation of management; creative problem-solving; education in communicable disease control; innovation in mobile phone use; low technology simulation training; local product manufacture; health financing; and social entrepreneurship. While there are no guarantees that innovations from developing country experiences can effectively transfer to developed countries, combined developed-developing country learning processes can potentially generate effective solutions for global health systems. However, the global pool of knowledge in this area is virgin and further work needs to be undertaken to advance understanding of health innovation diffusion. Even more urgently, a standardized method for reporting partnership benefits is needed--this is perhaps the single most immediate need in planning for, and realizing, the full potential of international cooperation between developed and developing countries.

Project description:A slew of participatory and community-demand-driven approaches have emerged in order to address the multi-dimensional nature of poverty in developing nations. The present study identifies critical factors responsible for poverty alleviation in India with the aid of fuzzy cognitive maps (FCMs) deployed for showcasing causal reasoning. It is through FCM-based simulations that the study evaluates the efficacy of existing poverty alleviation approaches, including community organisation based micro-financing, capability and social security, market-based and good governance. Our findings confirm, to some degree, the complementarity of various approaches to poverty alleviation that need to be implemented simultaneously for a comprehensive poverty alleviation drive. FCM-based simulations underscore the need for applying an integrated and multi-dimensional approach incorporating elements of various approaches for eradicating poverty, which happens to be a multi-dimensional phenomenon. Besides, the study offers policy implications for the design, management, and implementation of poverty eradication programmes. On the methodological front, the study enriches FCM literature in the areas of knowledge capture, sample adequacy, and robustness of the dynamic system model.

Project description:BackgroundMany countries are currently rethinking their global health research funding priorities. When resources are limited, it is important to understand and use information about existing research strengths to inform research strategies and investments and to drive impact. This study describes a method to rapidly assess a country's global health research expertise and applies this method in the Canadian context.MethodsWe developed a three-pronged rapid environmental scan to evaluate Canadian global health research expertise that focused on research funding inputs, research activities and research outputs. We assessed research funding inputs from Canada's national health research funding agency and identified the 30 Canadian universities that received the most global health research funding. We systematically searched university websites and secondary databases to identify research activities, including research centres, research chairs and research training programmes. To evaluate research outputs, we searched PubMed to identify global health research publications by Canadian university-affiliated researchers. We used these three perspectives to develop a more nuanced understanding of Canadian strengths in global health research from different perspectives.ResultsCanada's main global health research funder, the Canadian Institutes of Health Research, invested a total of $314 M from 2000 to 2016 on global health research grants. This investment has contributed to Canada's wealth of global health research expertise, including 12 training programmes, 27 Canada Research Chairs, 6 research centres and 30 WHO Collaborating Centres across 27 universities. Research activities were concentrated in Canada's biggest cities and most commonly focused on health equity and globalisation issues. Canadian-affiliated researchers have contributed to a research output of 822 unique publications on PubMed. There is an opportunity to build global health expertise in regions not already concentrated with research activity, focusing on transnational risks and neglected conditions research.ConclusionsOur three-pronged approach allowed us to rapidly identify clear geographic and substantive areas of strength in Canadian global health research, including urban regions and research focused on health equity and globalisation topics. This information can be used to support research policy directives, including to inform a Canadian global health research strategy, and to allow relevant academic institutions and funding organisations to make more strategic decisions regarding their future investments.

Project description:Obesity is the fastest growing health problem worldwide. Ethical issues linked to obesity are numerous and still under debate even in countries with a long history in obesity treatment.From 2007 to 2015 we performed several types of bariatric surgical approaches on 250 patients with an average body mass index (BMI) of 42. The age range was 12-64 years. No death was recorded. Direct or phone contact was possible with 90% of them during follow-up. Starting from a specific question based approach in ethics we present aspects regarding obesity surgery in Romania. Patients' safety, informed consent, cost cover, the role of bariatric surgery in children and bariatric surgeons' training are discussed.Co-morbidities improved or even disappeared in 90% of our patients. Informed consent is a major problem, due to the lack of public knowledge necessary. The private system in Romania offers bariatric surgery at lower prices than Western Europe but is still out of reach for a person with an average income. Lack of maturity and disharmonic family relations raise a series of challenges in assessing the best interest of children and adolescents. Ethics committees, which operate according to well-defined processes, are more and more active in universities and research centers in Romania, checking that methods and performance of scientific studies meet adequate standards.A detailed informed consent, thorough preoperative patient assessment and method selection are mandatory for good results in obesity surgery. Insufficient financial resources combined with the long time necessary to acquire the expertise for laparoscopic bariatric surgery may represent an additional pressure on both physicians and patients.

Project description:Each year many dentists embark on mission trips to foreign countries. This article shares what one group learned in their journey over the course of 17 years to bring oral health to a rural community in Honduras. The group began by delivering acute dental care, but soon realized that this treatment would never change the status of oral health in the community. Year by year they learned what worked and what did not. A school-based dental prevention program was initiated using proven preventive techniques to demonstrate to the community that prevention of oral disease was possible. As of 2015, the school-based program had grown to over 10 schools and nearly 1,000 children had benefited from this program. Children in the program received all necessary treatments for the prevention and treatment of dental caries. As importantly, they and their families learned to understand how to be responsible for their own dental needs. In conclusion, it is possible to effect long-term change in a developing country by focusing on prevention of oral problems rather than focusing on the extraction hopeless teeth. The good intentions, time, and financial resources of volunteers can be put to best use by first learning about the needs and wants of a particular community. The authors recommend that volunteers partner with local health-care providers and research what other organizations are currently doing in their country of interest.
