Project description:BackgroundMisinformation about Lyme disease and other tick-transmitted pathogens circulates frequently on the internet and can compete with, or even overshadow, science-based guidance on tick-borne disease (TBD) prevention.ObjectiveWe surveyed internet users connected to academic tick-related resources to identify trusted sources of Lyme disease prevention information, explore confidence in tick bite prevention information, and examine associations of these responses with answers to commonly disputed issues.MethodsThe survey was conducted through social media and website pages for Western Connecticut State University Tickborne Disease Prevention Laboratory and the University of Rhode Island TickEncounter Resource Center.ResultsRespondents (N=1190) were predominantly female (903/1190, 76.3%), middle-aged (574/1182, 48.6%), and resided in New England states (663/1190, 55.7%). In total 984 of 1186 (83%) respondents identified conventional experts (eg, the Centers for Disease Control [CDC] or other government health agencies, physicians who follow Infectious Diseases Society of America guidelines for Lyme disease treatment guidelines, and academics) as trustworthy TBD prevention resources. However, nearly one-fourth of respondents would first consult personal contacts and web-based communities regarding prevention information before consulting conventional expert sources. The opinions of public health experts and physicians were rated among the top motivators underlying personal prevention decisions; yet, more than 50% of participants revealed distrustful attitudes toward, or were uncertain about, CDC-supported statements related to time to transmission of Lyme disease (708/1190, 59.5%), the safety of diethyltoluamide-based repellents for children (604/1183, 51.1%), and recommended use of antibiotic prophylaxis (773/1181, 65.4%). Multimodal regression models revealed that participants from high-Lyme-disease-incidence states were more likely to first seek TBD prevention information from personal networks and nontraditional sources before approaching conventional sources of TBD prevention information. We found that those reporting high rates of social media usage were more than twice as likely to first seek traditional expert sources of prevention information but were overall more likely to reject CDC-promoted Lyme disease information, in particular the established time to transmission of Lyme disease bacteria. Models also predicted that those participants who disagreed with the conventional scientific view on the antibiotic prophylaxis prevention statement were less likely to be confident in their ability to protect themselves from a tick bite. Overall, uncertainty in one's ability to protect oneself against tick bites was strongly associated with uncertainty about beliefs in CDC-promoted TBD prevention information. Self-reported trust in experts and frequency of social media use suggest that these platforms may provide opportunities to engage directly with the public about TBD prevention practices.ConclusionsUsing strategies to improve public trust and provide information where the public engages on social media may improve prevention communication and adoption of best practices.

Project description:HypothesisSenior hospital clinicians are poorly engaged with clinical coding and hospital episode statistics (HES).Aims ▸ To understand the current level of clinical engagement with collection of national data and clinical coding. ▸ To gain the views of frontline staff on proposed improvements to hospital statistics. ▸ To gain an indication of likely clinical engagement in change. ▸ To understand the clinical priority for improvement.DesignInternet e-survey accessible from Academy of Royal Medical College Website.SettingNational Health Service (NHS) Trusts.Participants1081 NHS hospital consultants and two general practitioners who volunteered to take part.Results3.4% of the sample regularly access HES data; 21% are regularly involved in clinical coding and 6.2% meet coding staff at least monthly. 95% would like to access HES data and there was a strong support for using this data for appraisal, revalidation and improving the quality of patient care. In terms of improvements, 91.9% would be prepared to code diagnosis in outpatients given the right tools. The highest priority for improvement is clinical validation of diagnostic data.ConclusionsClinical engagement with coding and access to HES data is poor. However, there is professional support for improvement. Clinical requirements should be considered in all future developments of national data collection to provide the quality and scope of data that is required to deliver the information revolution.

Project description:BACKGROUND:Public health surveillance is based on the continuous and systematic collection, analysis, and interpretation of data. This informs the development of early warning systems to monitor epidemics and documents the impact of intervention measures. The introduction of digital data sources, and specifically sources available on the internet, has impacted the field of public health surveillance. New opportunities enabled by the underlying availability and scale of internet-based sources (IBSs) have paved the way for novel approaches for disease surveillance, exploration of health communities, and the study of epidemic dynamics. This field and approach is also known as infodemiology or infoveillance. OBJECTIVE:This review aimed to assess research findings regarding the application of IBSs for public health surveillance (infodemiology or infoveillance). To achieve this, we have presented a comprehensive systematic literature review with a focus on these sources and their limitations, the diseases targeted, and commonly applied methods. METHODS:A systematic literature review was conducted targeting publications between 2012 and 2018 that leveraged IBSs for public health surveillance, outbreak forecasting, disease characterization, diagnosis prediction, content analysis, and health-topic identification. The search results were filtered according to previously defined inclusion and exclusion criteria. RESULTS:Spanning a total of 162 publications, we determined infectious diseases to be the preferred case study (108/162, 66.7%). Of the eight categories of IBSs (search queries, social media, news, discussion forums, websites, web encyclopedia, and online obituaries), search queries and social media were applied in 95.1% (154/162) of the reviewed publications. We also identified limitations in representativeness and biased user age groups, as well as high susceptibility to media events by search queries, social media, and web encyclopedias. CONCLUSIONS:IBSs are a valuable proxy to study illnesses affecting the general population; however, it is important to characterize which diseases are best suited for the available sources; the literature shows that the level of engagement among online platforms can be a potential indicator. There is a necessity to understand the population's online behavior; in addition, the exploration of health information dissemination and its content is significantly unexplored. With this information, we can understand how the population communicates about illnesses online and, in the process, benefit public health.

Project description:Public databases provide a wealth of freely available information about chemicals, genes, proteins, biological networks, phenotypes, diseases, and exposure science that can be integrated to construct pathways for systems toxicology applications. Relating this disparate information from public repositories, however, can be challenging since databases use a variety of ways to represent, describe, and make available their content. The use of standard vocabularies to annotate key data concepts, however, allows the information to be more easily exchanged and combined for discovery of new findings. We explore some of the many public data sources currently available to support systems toxicology, and demonstrate the value of standardizing data to help construct chemical-induced outcome pathways.

Project description:BackgroundInternet interventions have been proposed to improve the accessibility and use of evidence-based psychological treatments. However, little is known about attitudes toward such treatments, which can be an important barrier to their use.ObjectiveThis study aimed to (1) determine attitudes toward guided internet interventions, (2) assess its acceptability compared with other internet-based formats, and (3) explore predictors of acceptance.MethodsA convenience-sample Web-based survey (N=646) assessed attitudes toward guided internet therapies (ie, perceived usefulness and helpfulness, and advantage relative to face-to-face therapy), preferences for delivery modes (ie, e-preference: guided internet interventions, unguided internet interventions, or videoconferencing psychotherapy), and potential predictors of attitudes and preferences: sociodemographics, help-seeking-related variables, attachment style, and perceived stress.ResultsAlthough most participants perceived internet interventions as useful or helpful (426/646, 65.9%), a few indicated their advantage relative to face-to-face therapy (56/646, 8.7%). Most participants preferred guided internet interventions (252/646, 39.0%) over videoconferencing psychotherapy (147/646, 22.8%), unguided internet interventions (124/646, 19.2%), and not using internet interventions (121/646, 18.8%; missing data: 1/646, 0.2%). Attachment avoidance and stress were related to e-preference (all P<.05). Moreover, preference for therapist-guided internet interventions was higher for individuals who were aware of internet-based treatment (χ26=12.8; P=.046).ConclusionsParticipants assessed therapist-guided internet interventions as helpful, but not equivalent to face-to-face therapies. The vast majority (523/646, 81.0%) of the participants were potentially willing to use internet-based approaches. In lieu of providing patients with only one specific low-intensity treatment, implementation concepts should offer several options, including guided internet interventions, but not limited to them. Conversely, our results also indicate that efforts should focus on increasing public knowledge about internet interventions, including information about their effectiveness, to promote acceptance and uptake.

Project description:ObjectivesSocial media has changed methods of communication in the medical profession. As part of a "doctor as communicator" strategy, Chinese medical professionals are actively embracing social media, which may have an impact on tense physician-patient relationships in China. This paper aims to examine a pattern of Internet access, Internet usage, and trust among Chinese medical professionals and further explores the reasons for these relationships on an individual level.DesignsA web-based questionnaire was designed, based on a media literacy model, and was divided into 3 dimensions: public information, general medical information, and specialty information. After a two-round pilot study, 1001 physicians were included. Additionally, 4 interviewees were chosen to participate in in-depth interviews and content analysis was performed. Data were analyzed using SPSS 25.0.ResultsFindings showed that new media has become a major approach for medical professionals in China to retrieve and get access to various information. However, they still trust traditional media (n = 1001, P < 0.01) and professional journals more (n = 1001, P < 0.01). Internet access, usage, and trust were positively correlated (r = 0.185-0.344, P < 0.01). Regarding usage habits, 47.66% of the participating physicians would practice science popularization through their new media accounts and 71.23% would forward approved health information within their professional realm. A validated instrument can be provided for further related studies.ConclusionsScience popularization is a specific usage of new media among Chinese medical professionals, through which they have achieved new authority and empowered communication because of self-media. To some extent, physician-patient relationships in China can be improved because this online interaction is conducive to building harmonious and lasting offline physician-patient relationships.

Project description:More and more, social scientists are using (big) digital behavioral data for their research. In this context, the social network and microblogging platform Twitter is one of the most widely used data sources. In particular, geospatial analyses of Twitter data are proving to be fruitful for examining regional differences in user behavior and attitudes. However, ready-to-use spatial information in the form of GPS coordinates is only available for a tiny fraction of Twitter data, limiting research potential and making it difficult to link with data from other sources (e.g., official statistics and survey data) for regional analyses. We address this problem by using the free text locations provided by Twitter users in their profiles to determine the corresponding real-world locations. Since users can enter any text as a profile location, automated identification of geographic locations based on this information is highly complicated. With our method, we are able to assign over a quarter of the more than 866 million German tweets collected to real locations in Germany. This represents a vast improvement over the 0.18% of tweets in our corpus to which Twitter assigns geographic coordinates. Based on the geocoding results, we are not only able to determine a corresponding place for users with valid profile locations, but also the administrative level to which the place belongs. Enriching Twitter data with this information ensures that they can be directly linked to external data sources at different levels of aggregation. We show possible use cases for the fine-grained spatial data generated by our method and how it can be used to answer previously inaccessible research questions in the social sciences. We also provide a companion R package, nutscoder, to facilitate reuse of the geocoding method in this paper.

Project description:One of the essential concerns of Internet of Things (IoT) is in industrial systems or data architecture to support the evolutions in transportation and logistics. Considering the Industrial IoT (IIoT) openness, the need for accessibility, availability, and searching of data has rapidly increased. The primary purpose of this research is to propose an Efficient Two-Dimensional Filter (ETDF) to store multimedia data of IIoT applications in a specific format to achieve faster response and dynamic updating. This filter consists of a two-dimensional array and a hash function integrated into a cuckoo filter for efficient use of memory. This study evaluates the scalability of the filter by increasing the number of requests from 10,000 to 100,000. To assess the performance of the proposed filter, we measure the parameters of access time and lookup message latency. The results show that the proposed filter improves the access time by 12%, compared to a Fast Two-Dimensional Filter (FTDF). Moreover, it improves memory usage by 20% compared to FTDF. Experiments indicate a better access time of the proposed filter compared to other filters (i.e., Bloom, quotient, cuckoo, and FTD filters). Insertion and deletion times are essential parameters in comparing filters, so they are also analyzed.

Project description:Polyols are approved for use as sweeteners in specific foods but they may be used for other technological purposes in a wider range of foods, all on a quantum satis basis. The European Polyols Association (EPA) has identified 24 categories of food where polyols are used and it has been able to establish the levels at which the polyols are used in each type of food and whether for sweetening or non-sweetening purposes. The UK National Dietary and Nutrition survey database was used to estimate potential exposures to total polyols based on reported use levels. It was possible to express potential polyol intake on the basis of exposure relating to a single eating occasion, a meal period, 1 day and the average over 4 days of the survey. Potential intakes of polyols were approximately twice that found on a per-item or a meal-period basis when estimated on a daily basis. Apparent intakes were lower when averaged over the 4 days of the survey. It was felt that intake expressed on a per-meal occasion basis was most relevant to the development of digestive discomfort. On the basis of maximum use levels of polyols in all food categories, adults had the highest intake of total polyols up to 5.6 g per meal period at the 95th percentile. However, when expressed on a bodyweight basis, children had higher intakes, up to 0.15 g kg(-1) bw per meal period. Distributions of potential polyol exposures were highly skewed towards lower values with higher levels of exposure relating to sweetener uses occurring relatively infrequently.

Project description:BackgroundEstimating multimorbidity (presence of two or more chronic conditions) using administrative data is becoming increasingly common. We investigated (1) the concordance of identification of chronic conditions and multimorbidity using self-report survey and administrative datasets; (2) characteristics of people with multimorbidity ascertained using different data sources; and (3) whether the same individuals are classified as multimorbid using different data sources.MethodsBaseline survey data for 90,352 participants of the 45 and Up Study-a cohort study of residents of New South Wales, Australia, aged 45 years and over-were linked to prior two-year pharmaceutical claims and hospital admission records. Concordance of eight self-report chronic conditions (reference) with claims and hospital data were examined using sensitivity (Sn), positive predictive value (PPV), and kappa (?).The characteristics of people classified as multimorbid were compared using logistic regression modelling.ResultsAgreement was found to be highest for diabetes in both hospital and claims data (? = 0.79, 0.78; Sn = 79%, 72%; PPV = 86%, 90%). The prevalence of multimorbidity was highest using self-report data (37.4%), followed by claims data (36.1%) and hospital data (19.3%). Combining all three datasets identified a total of 46 683 (52%) people with multimorbidity, with half of these identified using a single dataset only, and up to 20% identified on all three datasets. Characteristics of persons with and without multimorbidity were generally similar. However, the age gradient was more pronounced and people speaking a language other than English at home were more likely to be identified as multimorbid by administrative data.ConclusionsDifferent individuals, with different combinations of conditions, are identified as multimorbid when different data sources are used. As such, caution should be applied when ascertaining morbidity from a single data source as the agreement between self-report and administrative data is generally poor. Future multimorbidity research exploring specific disease combinations and clusters of diseases that commonly co-occur, rather than a simple disease count, is likely to provide more useful insights into the complex care needs of individuals with multiple chronic conditions.
