Project description:Many radiomics features were originally developed for non-medical imaging applications and therefore original assumptions may need to be reexamined. In this study, we investigated the impact of slice thickness and pixel spacing (or pixel size) on radiomics features extracted from Computed Tomography (CT) phantom images acquired with different scanners as well as different acquisition and reconstruction parameters. The dependence of CT texture features on gray-level discretization was also evaluated.A texture phantom composed of 10 different cartridges of different materials was scanned on eight different CT scanners from three different manufacturers. The images were reconstructed for various slice thicknesses. For each slice thickness, the reconstruction Field Of View (FOV) was varied to render pixel sizes ranging from 0.39 to 0.98 mm. A fixed spherical region of interest (ROI) was contoured on the images of the shredded rubber cartridge and the 3D printed, 20% fill, acrylonitrile butadiene styrene plastic cartridge (ABS20) for all phantom imaging sets. Radiomic features were extracted from the ROIs using an in-house program. Features categories were: shape (10), intensity (16), GLCM (24), GLZSM (11), GLRLM (11), and NGTDM (5), fractal dimensions (8) and first-order wavelets (128), for a total of 213 features. Voxel-size resampling was performed to investigate the usefulness of extracting features using a suitably chosen voxel size. Acquired phantom image sets were resampled to a voxel size of 1 × 1 × 2 mm3 using linear interpolation. Image features were therefore extracted from resampled and original datasets and the absolute value of the percent coefficient of variation (%COV) for each feature was calculated. Based on the %COV values, features were classified in 3 groups: (1) features with large variations before and after resampling (%COV >50); (2) features with diminished variation (%COV <30) after resampling; and (3) features that had originally moderate variation (%COV <50%) and were negligibly affected by resampling. Group 2 features were further studied by modifying feature definitions to include voxel size. Original and voxel-size normalized features were used for interscanner comparisons. A subsequent analysis investigated feature dependency on gray-level discretization by extracting 51 texture features from ROIs from each of the 10 different phantom cartridges using 16, 32, 64, 128, and 256 gray levels.Out of the 213 features extracted, 150 were reproducible across voxel sizes, 42 improved significantly (%COV <30, Group 2) after resampling, and 21 had large variations before and after resampling (Group 1). Ten features improved significantly after definition modification effectively removed their voxel-size dependency. Interscanner comparison indicated that feature variability among scanners nearly vanished for 8 of these 10 features. Furthermore, 17 out of 51 texture features were found to be dependent on the number of gray levels. These features were redefined to include the number of gray levels which greatly reduced this dependency.Voxel-size resampling is an appropriate pre-processing step for image datasets acquired with variable voxel sizes to obtain more reproducible CT features. We found that some of the radiomics features were voxel size and gray-level discretization-dependent. The introduction of normalizing factors in their definitions greatly reduced or removed these dependencies.

Project description:Purpose: To design and validate a preprocessing procedure dedicated to T2-weighted MR images of lung cancers so as to improve the ability of radiomic features to distinguish between adenocarcinoma and other histological types. Materials and Methods: A discovery set of 52 patients with advanced lung cancer who underwent T2-weighted MR imaging at 3 Tesla in a single center study from August 2017 to May 2019 was used. Findings were then validated using a validation set of 19 additional patients included from May to October 2019. Tumor type was obtained from the pathology report after trans-thoracic needle biopsy, metastatic lymph node or metastasis samples, or surgical excisions. MR images were preprocessed using N4ITK bias field correction and by normalizing voxel intensities with fat as a reference region. Segmentation and extraction of radiomic features were performed with LIFEx software on the raw images, on the N4ITK-corrected images and on the fully preprocessed images. Two analyses were conducted where radiomic features were extracted: (1) from the whole tumor volume (3D analysis); (2) from all slices encompassing the tumor (2D analysis). Receiver operating characteristic (ROC) analysis was used to identify features that could distinguish between adenocarcinoma and other histological types. Sham experiments were also designed to control the number of false positive findings. Results: There were 31 (12) adenocarcinomas and 21 (7) other histological types in the discovery (validation) set. In 2D, preprocessing increased the number of discriminant radiomic features from 8 without preprocessing to 22 with preprocessing. 2D analysis yielded more features able to identify adenocarcinoma than 3D analysis (12 discriminant radiomic features after preprocessing in 3D). Preprocessing did not increase false positive findings as no discriminant features were identified in any of the sham experiments. The greatest sensitivity of the 2D analysis applied to preprocessed data was confirmed in the validation set. Conclusion: Correction for magnetic field inhomogeneities and normalization of voxel values are essential to reveal the full potential of radiomic features to identify the tumor histological type from MR T2-weighted images, with classification performance similar to those reported in PET/CT and in multiphase CT in lung cancers.

Project description:PurposeTo evaluate the effect of the anatomic size on 3D radiomic imaging features of the breast cancer hepatic metastases.Materials and methodsCT scans of 81 liver metastases from 54 patients with breast cancer were evaluated. Ten most common 3D radiomic features from the histogram and gray level co-occurrence matrix (GLCM) categories were calculated for the hepatic metastases (HM) and compared to normal liver (NL). The effect of size was evaluated by using linear mixed-effects regression models. The effect of size on different radiomic features was analyzed for both liver lesions and background liver.ResultsThree-dimensional radiomic features from GLCM demonstrate an important size dependence. The texture-feature size dependence was found to be different among feature categories and between the HM and NL, thus demonstrating a discriminatory power for the tissue type. Significant difference in the slope was found for GLCM homogeneity (NL slope = 0.004, slope difference 95% confidence interval [CI] 0.06-0.1, p <0.001), contrast (NL slope = 45, slope difference 95% CI 205-305, p <0.001), correlation (NL slope = 0.04, slope difference 95% CI 0.11-0.21, p <0.001), and dissimilarity (NL slope = 0.7, slope difference 95% CI 3.6-5.4, p <0.001). The GLCM energy (NL slope = 0.002, slope difference 95% CI -0.0005 to -0.0003, p <0.007), and entropy (NL slope = 1.49, slope difference 95% CI 0.07-0.52, p <0.009) exhibited size-dependence for both NL and HM, although demonstrating a difference in the slope between themselves.ConclusionRadiomic features of breast cancer hepatic metastasis exhibited significant correlation with tumor size. This finding demonstrates the complex behavior of imaging features and the need to include feature-specific properties into radiomic models.

Project description:OBJECTIVE:To retrospectively assess the effect of CT slice thickness on the reproducibility of radiomic features (RFs) of lung cancer, and to investigate whether convolutional neural network (CNN)-based super-resolution (SR) algorithms can improve the reproducibility of RFs obtained from images with different slice thicknesses. MATERIALS AND METHODS:CT images with 1-, 3-, and 5-mm slice thicknesses obtained from 100 pathologically proven lung cancers between July 2017 and December 2017 were evaluated. CNN-based SR algorithms using residual learning were developed to convert thick-slice images into 1-mm slices. Lung cancers were semi-automatically segmented and a total of 702 RFs (tumor intensity, texture, and wavelet features) were extracted from 1-, 3-, and 5-mm slices, as well as the 1-mm slices generated from the 3- and 5-mm images. The stabilities of the RFs were evaluated using concordance correlation coefficients (CCCs). RESULTS:The mean CCCs for the comparisons of original 1 mm vs. 3 mm, 1 mm vs. 5 mm, and 3 mm vs. 5 mm images were 0.41, 0.27, and 0.65, respectively (p < 0.001 for all comparisons). Tumor intensity features showed the best reproducibility while wavelets showed the lowest reproducibility. The majority of RFs failed to achieve reproducibility (CCC ≥ 0.85; 3.6%, 1.0%, and 21.5%, respectively). After applying the CNN-based SR algorithms, the reproducibility significantly improved in all three pairings (mean CCCs: 0.58, 0.45, and 0.72; p < 0.001 for all comparisons). The reproducible RFs also increased (36.3%, 17.4%, and 36.9%, respectively). CONCLUSION:The reproducibility of RFs in lung cancer is significantly influenced by CT slice thickness, which can be improved by the CNN-based SR algorithms.

Project description:Purpose To evaluate ability of radiomic (computer-extracted imaging) features to distinguish non-small cell lung cancer adenocarcinomas from granulomas at noncontrast CT. Materials and Methods For this retrospective study, screening or standard diagnostic noncontrast CT images were collected for 290 patients (mean age, 68 years; range, 18-92 years; 125 men [mean age, 67 years; range, 18-90 years] and 165 women [mean age, 68 years; range, 33-92 years]) from two institutions between 2007 and 2013. Histopathologic analysis was available for one nodule per patient. Corresponding nodule of interest was identified on axial CT images by a radiologist with manual annotation. Nodule shape, wavelet (Gabor), and texture-based (Haralick and Laws energy) features were extracted from intra- and perinodular regions. Features were pruned to train machine learning classifiers with 145 patients. In a test set of 145 patients, classifier results were compared against a convolutional neural network (CNN) and diagnostic readings of two radiologists. Results Support vector machine classifier with intranodular radiomic features achieved an area under the receiver operating characteristic curve (AUC) of 0.75 on the test set. Combining radiomics of intranodular with perinodular regions improved the AUC to 0.80. On the same test set, CNN resulted in an AUC of 0.76. Radiologist readers achieved AUCs of 0.61 and 0.60, respectively. Conclusion Radiomic features from intranodular and perinodular regions of nodules can distinguish non-small cell lung cancer adenocarcinomas from benign granulomas at noncontrast CT. © RSNA, 2018 Online supplemental material is available for this article. Project description:The purpose of this study was to explore the effects of CT slice thickness, reconstruction algorithm, and radiation dose on quantification of CT features to characterize lung nodules using a chest phantom. Spherical lung nodule phantoms of known densities (-630 and + 100 HU) were inserted into an anthropomorphic thorax phantom. CT scan was performed ten times with relocations. CT data were reconstructed using 12 different imaging settings; three different slice thicknesses of 1.25, 2.5, and 5.0 mm, two reconstruction kernels of sharp and standard, and two radiation dose of 30 mAs and 12 mAs. Lesions were segmented using a semiautomated method. Twenty representative CT quantitative features representing CT density and texture were compared using multiple regression analysis. In 100 HU nodule phantoms, 18 and 19 among 20 computer features showed significant difference between different mAs and reconstruction algorithms, respectively (p ≤ 0.05). 20, 19, and 19 computer features showed difference between slice thickness of 5.0 vs 1.25, 5.0 vs 2.5, and 2.5 vs 1.25 mm, respectively (p ≤ 0.05). In -630 HU nodule phantoms, 18 and 19 showed significant difference between different mAs and reconstruction algorithms, respectively (p ≤ 0.05). 18, 11, and 17 computer features showed difference between slice thickness of 5.0 vs 1.25, 5.0 vs 2.5, and 2.5 vs 1.25 mm, respectively (p ≤ 0.05). When comparing the absolute value of regression coefficient, the effect of slice thickness in 100 HU nodule and reconstruction algorithm in -630 HU nodule was greater than the effect of remaining scan parameters. The slice thickness, mAs, and reconstruction algorithm had a significant impact on the quantitative image features. In clinical studies involving deep learning or radiomics, it should be noted that differences in values can occur when using computer features obtained from different CT scan parameters in combination. Therefore, when interpreting the statistical analysis results, it is necessary to reflect the difference in the computer features depending on the scan parameters.

Project description:Site-specific investigations of the role of radiomics in cancer diagnosis and therapy are emerging. We evaluated the reproducibility of radiomic features extracted from 18 Flourine-fluorodeoxyglucose (18 F-FDG) PET images for three parameters: manual versus computer-aided segmentation methods, gray-level discretization, and PET image reconstruction algorithms. Our cohort consisted of pretreatment PET/CT scans from 88 cervical cancer patients. Two board-certified radiation oncologists manually segmented the metabolic tumor volume (MTV1 and MTV2 ) for each patient. For comparison, we used a graphical-based method to generate semiautomated segmented volumes (GBSV). To address any perturbations in radiomic feature values, we down-sampled the tumor volumes into three gray-levels: 32, 64, and 128 from the original gray-level of 256. Finally, we analyzed the effect on radiomic features on PET images of eight patients due to four PET 3D-reconstruction algorithms: maximum likelihood-ordered subset expectation maximization (OSEM) iterative reconstruction (IR) method, fourier rebinning-ML-OSEM (FOREIR), FORE-filtered back projection (FOREFBP), and 3D-Reprojection (3DRP) analytical method. We extracted 79 features from all segmentation method, gray-levels of down-sampled volumes, and PET reconstruction algorithms. The features were extracted using gray-level co-occurrence matrices (GLCM), gray-level size zone matrices (GLSZM), gray-level run-length matrices (GLRLM), neighborhood gray-tone difference matrices (NGTDM), shape-based features (SF), and intensity histogram features (IHF). We computed the Dice coefficient between each MTV and GBSV to measure segmentation accuracy. Coefficient values close to one indicate high agreement, and values close to zero indicate low agreement. We evaluated the effect on radiomic features by calculating the mean percentage differences (d¯) between feature values measured from each pair of parameter elements (i.e. segmentation methods: MTV1 -MTV2 , MTV1 -GBSV, MTV2 -GBSV; gray-levels: 64-32, 64-128, and 64-256; reconstruction algorithms: OSEM-FORE-OSEM, OSEM-FOREFBP, and OSEM-3DRP). We used |d¯| as a measure of radiomic feature reproducibility level, where any feature scored |d¯| ±SD ? |25|% ± 35% was considered reproducible. We used Bland-Altman analysis to evaluate the mean, standard deviation (SD), and upper/lower reproducibility limits (U/LRL) for radiomic features in response to variation in each testing parameter. Furthermore, we proposed U/LRL as a method to classify the level of reproducibility: High- ±1% ? U/LRL ? ±30%; Intermediate- ±30% < U/LRL ? ±45%; Low- ±45 < U/LRL ? ±50%. We considered any feature below the low level as nonreproducible (NR). Finally, we calculated the interclass correlation coefficient (ICC) to evaluate the reliability of radiomic feature measurements for each parameter. The segmented volumes of 65 patients (81.3%) scored Dice coefficient >0.75 for all three volumes. The result outcomes revealed a tendency of higher radiomic feature reproducibility among segmentation pair MTV1 -GBSV than MTV2 -GBSV, gray-level pairs of 64-32 and 64-128 than 64-256, and reconstruction algorithm pairs of OSEM-FOREIR and OSEM-FOREFBP than OSEM-3DRP. Although the choice of cervical tumor segmentation method, gray-level value, and reconstruction algorithm may affect radiomic features, some features were characterized by high reproducibility through all testing parameters. The number of radiomic features that showed insensitivity to variations in segmentation methods, gray-level discretization, and reconstruction algorithms was 10 (13%), 4 (5%), and 1 (1%), respectively. These results suggest that a careful analysis of the effects of these parameters is essential prior to any radiomics clinical application.

Project description:IntroductionAccurate segmentation of tumors and quantification of tumor features are important for cancer detection, diagnosis, monitoring, and planning therapeutic intervention. Due to inherent noise components in multi-parametric imaging and inter-observer and intra-observer variations, it is common that various segmentation methods may produce large segmentation errors in tumor volumes and their associated radiomic features. The purpose of this study is to carry out the stability analysis for radiomic features with respect to segmentation variation in oropharyngeal cancer (OPC).MethodsIn this study, 436 contrast-enhanced computed tomography (CT) axial images were collected from patients with OPC. In order to derive various segmentations of tumor volumes, two additional segmentations were obtained via resizing the original segmented regions of interest (ROIs) based on their geometric information on the boundary. For three ROI image groups, we calculated 109 radiomic features. Then, a logistic regression model was built to investigate the correlation between the radiomic features extracted from GTVp and the response to chemotherapy and radiation in terms of overall survival (OS). Finally, in order to evaluate the stability of each feature with respect to segmentation results, based on the prediction probabilities, we assessed the inter-rater reliability and reproducibility by calculating the intra-class correlation coefficients (ICC) and concordance correlation coefficients (CCC).ResultsMost radiomic features in this study varied a lot when the ROIs were not well segmented. For both the representation agreement and predictive agreement, the ICC and CCC were below 0.5 for all the features. We still found some robust features with relatively high ICC and CCC compared to most features. For example, 25percentile (ICC = 0.38, CCC = 0.37 in representation agreement and ICC = CCC = 0.27 in predictive agreement) is a quantile based feature, which is robust to the extremely high or low values; and Hu_1_std (ICC = 0.31, CCC = 0.31 in representation agreement) is a feature calculated based on the first Hu moment, which is invariant to the transformation of ROIs.ConclusionIn OPC studies, the tumor segmentation variation affects the radiomic features from CT images in terms of both representation and prediction. Some features that are robust to the extreme values or invariant to the transformation of ROIs may be treated as radiomic markers to assist with OPC treatment monitoring and prognostic prediction.

Project description:PurposeTo assess (1) the repeatability and (2) the impact of reconstruction methods and delineation on the repeatability of 105 radiomic features in non-small-cell lung cancer (NSCLC) 2-deoxy-2-[(18)F]fluoro-D-glucose ([(18)F]FDG) positron emission tomorgraphy/computed tomography (PET/CT) studies.ProceduresEleven NSCLC patients received two baseline whole-body PET/CT scans. Each scan was reconstructed twice, once using the point spread function (PSF) and once complying with the European Association for Nuclear Medicine (EANM) guidelines for tumor PET imaging. Volumes of interest (n?=?19) were delineated twice, once on PET and once on CT images.ResultsSixty-three features showed an intraclass correlation coefficient???0.90 independent of delineation or reconstruction. More features were sensitive to a change in delineation than to a change in reconstruction (25 and 3 features, respectively).ConclusionsThe majority of features in NSCLC [(18)F]FDG-PET/CT studies show a high level of repeatability that is similar or better compared to simple standardized uptake value measures.
