Decline in Kidney Function among Apparently Healthy Young Adults at Risk of Mesoamerican Nephropathy.

ABSTRACT: Background Epidemic levels of CKD of undetermined cause, termed Mesoamerican nephropathy in Central America, have been found in low- and middle-income countries. We investigated the natural history of, and factors associated with, loss of kidney function in a population at high risk for this disease.Methods We conducted a 2-year prospective, longitudinal study with follow-up every 6 months in nine rural communities in northwestern Nicaragua and included all men (n=263) and a random sample of women (n=87) ages 18-30 years old without self-reported CKD, diabetes, or hypertension. We used growth mixture modeling to identify subgroups of eGFR trajectory and weighted multinomial logistic regression to examine associations with proposed risk factors.Results Among men, we identified three subpopulations of eGFR trajectory (mean baseline eGFR; mean eGFR change over follow-up): 81% remained stable (116 ml/min per 1.73 m²; -0.6 ml/min per 1.73 m² per year), 9.5% experienced rapid decline despite normal baseline function (112 ml/min per 1.73 m²; -18.2 ml/min per 1.73 m² per year), and 9.5% had baseline dysfunction (58 ml/min per 1.73 m²; -3.8 ml/min per 1.73 m² per year). Among women: 96.6% remained stable (121 ml/min per 1.73 m²; -0.6 ml/min per 1.73 m² per year), and 3.4% experienced rapid decline (132 ml/min per 1.73 m²; -14.6 ml/min per 1.73 m² per year; n=3 women). Among men, outdoor and agricultural work and lack of shade availability during work breaks, reported at baseline, were associated with rapid decline.Conclusions Although Mesoamerican nephropathy is associated with agricultural work, other factors may also contribute to this disease.

Project description:BackgroundIdentifying potentially modifiable risk factors is critically important for reducing the burden of chronic kidney disease. We sought to examine the association of body mass index (BMI) with kidney function decline in a cohort of young adults with preserved glomerular filtration at baseline.Study designLongitudinal cohort.Setting & participants2,839 black and white young adults with cystatin C-based estimated glomerular filtration rate (eGFRcys)>90mL/min/1.73m(2) taking part in the year-10 examination (in 1995-1996) of the Coronary Artery Risk Development in Young Adults (CARDIA) Study.PredictorBMI, categorized as 18.5-24.9 (reference), 25.0-29.9, 30.0-39.9, and ≥40.0kg/m(2).OutcomesTrajectory of kidney function decline, rapid decline (>3% per year), and incident eGFRcys <60mL/min/1.73m(2) over 10 years of follow-up.MeasurementsGFRcys estimated from the CKD-EPI (Chronic Kidney Disease Epidemiology Collaboration) equation for calibrated cystatin C at CARDIA years 10, 15, and 20.ResultsAt year 10, participants had a mean age of 35.1 years, median eGFRcys of 114mL/min/1.73m(2), and 24.5% had BMI≥30.0kg/m(2). After age 30 years, average eGFRcys was progressively lower with each increment in BMI after adjustment for baseline age, race, sex, hyperlipidemia, smoking status, and physical activity. Higher BMI category was associated with successively higher odds of rapid decline (for 25.0-29.9, 30.0-39.9, and ≥40.0kg/m(2), adjusted ORs were 1.50 [95% CI, 1.21-1.87], 2.01 [95% CI, 1.57-2.87], and 2.57 [95% CI, 1.67-3.94], respectively). 18 participants (0.6%) had incident eGFRcys<60mL/min/1.73m(2). In unadjusted analysis, higher BMI category was associated with incident eGFRcys<60mL/min/1.73m(2) (for 25.0-29.9, 30.0-39.9, and ≥40.0kg/m(2), ORs were 5.17 [95% CI, 1.10-25.38], 7.44 [95% CI, 1.54-35.95], and 5.55 [95% CI, 0.50-61.81], respectively); adjusted associations were no longer significant.LimitationsInability to describe kidney function before differences by BMI category were already evident. Absence of data for measured GFR or GFR estimated from serum creatinine level.ConclusionsHigher BMI categories are associated with greater declines in kidney function in a cohort of young adults with preserved GFR at baseline. Clinicians should vigilantly monitor overweight and obese patients for evidence of early kidney function decline.

Project description:BACKGROUND:Clinical laboratory reference intervals (RIs) are essential for clinical diagnosis, treatment and therapeutic monitoring. Locally established RIs are required to correctly interpret clinical laboratory results. In Ethiopia, clinical laboratory test results are interpreted based on RIs derived from a western population. METHODS:A multicenter cross-sectional study was conducted among blood donors in Amhara National Regional State, Ethiopia from March 2016 to May 2017. A total of 1,175 apparently healthy study participants were included in the study from four blood banks in the region. All clinical chemistry parameters were analyzed using Mindray BS-200E full automated clinical chemistry analyzer. The 95% RIs were estimated using reference limits at 2.5th percentile for the lower reference limit and 97.5th percentile for the upper reference limit. Kolmogorov-Sminorv and Wilcoxon rank-sum tests were used to check data distribution normality and whether partitions were needed between variables, respectively. RESULTS:RIs established include: ALT 5.13-42.88 U/L for males and 4.3-37 U/L for females; AST 12.13-46.88 for males and 10-43.8 U/L for females; ALP 77.2-475.8 U/L for males and 89-381 U/L for females; amylase 29-309.8 U/L for males and 29-287.9 U/L for females; GGT 7-69.8 U/L for males and 6-39.1 U/L for females; total bilirubin 0.11-1.18 mg/dl for males and 0.08-0.91 mg/dl for females; creatinine 0.48-1.13 mg/dl for males and 0.47-1.09 mg/dl for females; total cholesterol 78.13-211.75 mg/dl for males and 83.6-202.7 mg/dl for females; total protein 5.7-9.7 g/dl for males and 5.6-9.47 for females; triglycerides 36-221.9 mg/dl for males and 35.3-201.5 mg/dl for females; urea 12-43 mg/dl for males and 10-38.7 mg/dl for females; and uric acid 2.7-6.9 mg/dl for males and 2.1-5.9 mg/dl for females.
