Embolization of hemorrhagic renal angiomyolipoma complicated by arteriovenous shunting: A case report.

ABSTRACT: Arteriovenous shunting associated with angiomyolipoma is an unusual entity, which carries important implications to embolization approach. We present a distinctive case involving a 41-year-old woman who presented with retroperitoneal hemorrhage relating to renal angiomyolipoma. During angiography for urgent embolization, a complex vascular supply with arteriovenous shunting was encountered. Superselective embolization using alcohol or small particles is the standard approach to definitive treatment of symptomatic angiomyolipoma; however; their use is precluded in the setting of arteriovenous shunt hemodynamics. In this case, a 2-step approach was employed by which the initial hemorrhage was treated with proximal embolization using large gelatin foam and metallic coils. This resulted in decreased flow through the arteriovenous shunt, allowing the use high viscosity ethylene vinyl alcohol copolymer dissolved in dimethyl sulfoxide for definitive treatment.

Giant renal angiomyolipoma: A case report.

Project description:Renal angiomyolipoma (AML), also referred to as renal hamartoma, is a rare solid tumor without malignant characteristics. The inheritance pattern of renal AML is autosomal dominant. If the lesion grows to a large size, a series of clinical manifestations and serious complications may occur. We herein present a case of giant renal AML in a 34-year-old female patient, who presented with left-sided abdominal bloating for ~6 months. Following abdominal ultrasound and computed tomography examination, the patient underwent total left nephrectomy. The resected mass was sized 29×20x10 cm. Postoperative histopathological examination confirmed the lesion as a giant renal AML. There are only few cases of giant renal AML (>20 cm) reported in the literature to date. Due to the large size of the tumor, it is crucial to report similar cases, their diagnosis and treatment.

Renal arteriovenous fistula simulating hydronephrosis: A case report.

Project description:INTRODUCTION: Acquired renal arteriovenous fistulas (AVFs) include those that occur as a complication of renal biopsy. CASE REPORT: The authors report the case of a woman with recent-onset grade I hypertension, who was referred to our staff for sonographic studies of the kidneys and urinary tract. Laboratory data revealed microhematuria and proteinuria <0.5 g/24 h, and renal function was borderline (MDRD GFR 58 mL/min). Renal sonography of the left kidney revealed an anechoic, arboriform area at the level of the pelvis, which was suggestive of hydronephrosis. The color Doppler examination showed turbulent flow within the anechoic area, with high-velocity arterial flow and arterialization of the venous waveform at spectral analysis. Selective renal angiography later confirmed the presence of a middle renal AVF with pseudoaneurysm, which had been provoked by a renal biopsy performed over 10 years earlier in another center. Since the patient was currently in good health, the prescribed management consisted solely of close clinical and US follow-up. DISCUSSION: AV fistulas are among the most commonly diagnosed renovascular malformations. The case reported here underlines the importance of using color Doppler ultrasound when obstructive uropathy is suspected, especially in patients who have undergone renal biopsy.

High-output heart failure associated with primary plasma cell leukaemia due to arteriovenous shunting: a case report.

Project description:BackgroundPrimary plasma cell leukaemia is rarely associated with high-output heart failure, and the underlying mechanism is not well understood. We encountered a rare case of high-output heart failure caused by primary plasma cell leukaemia. Its underlying mechanism was clarified through imaging studies.Case summaryA 49-year-old man with no specific medical history was admitted to our hospital because of heart failure that did not improve with diuretic therapy. His condition was diagnosed as high-output heart failure and primary plasma cell leukaemia after admission. Extensive bone involvement in primary plasma cell leukaemia and arteriovenous shunts in the same lesion were suspected after various imaging studies. The first cycle of chemotherapy with bortezomib, adriamycin, and dexamethasone led to remission of primary plasma cell leukaemia and improved heart failure symptoms. The patient received further chemotherapy in addition to autologous peripheral blood stem cell transplantation and maintenance therapy and had no recurrence of pPCL or heart failure for 1 year to date.DiscussionPrimary plasma cell leukaemia can be associated with high-output heart failure, which is caused by arteriovenous shunting at the lesion site with diffuse bone involvement. Imaging studies may lead to the early diagnosis of aetiology and treatment of patients with high-output heart failure associated with primary plasma cell leukaemia.

Transvenous retrograde embolization of ruptured brain arteriovenous malformations: A case report and review of the literature.

Project description:Hemorrhagic stroke due to ruptured brain arteriovenous malformations (AVMs) is a common cause in young stroke patients. When the ruptured AVMs are in deep location, the choice of endovascular intervention with the arterial approach to AVM embolization is routine but in many cases, it is not feasible due to the inability to access because of the small and tortuous arterial branch, however, the intravenous approach also results in high complete obliteration rates but also carries a higher risk of stroke than the intra-arterial route. We describe a 36-year-old female patient diagnosed with intracranial and intraventricular hemorrhage who underwent complete transvenous embolization of the ruptured AVMs, and achieved near-complete clinical recovery after 1 month with the modified Rankin scale 1.

Multiple chromoanasynthesis in a rare case of sporadic renal leiomyosarcoma: case report

Project description:Renal leiomyosarcomas (LMS) are extremely rare neoplasms with aggressive behaviour and poor survival prognosis. The most frequent somatic events in leiomyosarcomas are mutations in TP53, RB1, ATRX and PTEN genes, chromosomal instability and chromoanagenesis. By using chromosomal microarray analysis we identified monosomy of chromosomes 3 and 11, gain of Xp (ATRX) arm and three chromoanasynthesis regions (6q21-q27, 7p22.3-p12.1 and 12q13.11-q21.2), with MDM2 and CDK4 oncogenes copy number gains, whereas no CNVs or tumor specific SNVs in TP53, RB1 and PTEN genes were observed.

Retroperitoneal extrarenal angiomyolipoma at the surgical bed 8 years after a renal angiomyolipoma nephrectomy: A case report and review of literature.

Project description:Retroperitoneal extrarenal angiomyolipoma (RERAML) are rare and close mimickers of retroperitoneal liposarcoma on both imaging and histopathology. However, imaging findings including heterogeneity, hyperdensity on unenhanced computed tomography, intralesional hemorrhage, absence of calcifications, low signal intensity on T2-weighted magnetic resonance imaging, and dilated intratumoral vessels can lead to the diagnosis of RERAML. Diagnosis of RERAML can avoid unnecessary surgery since conservative medical management with continued surveillance has been proven to be effective for RERAML whereas surgical resection is the treatment for liposarcoma. Imaging and laboratory follow-up for at least 5 years has been recommended in patients who underwent surgical resection of angiomyolipoma (AML). We present a case of RERAML in an asymptomatic patient whose AML recurred in the surgical bed 8 years after an ipsilateral nephrectomy for renal AML.

Conservative treatment of monolateral giant renal angiomyolipoma in a patient with Tuberous Sclerosis Complex (TSC): A case report.

Project description:Tuberous sclerosis complex has several renal manifestations like angiomyolipomas. We report a case of a giant AML and discuss its diagnosis and treatment. A 42-year-old woman was admitted to emergency department due to flank pain and hematuria. The patient had history of mental retardation and epilepsy. Abdominal CT without contrast medium revealed a large mass with a fat/blood content inside. On those findings, we diagnosed the patient a bleeding giant AML. We performed selective embolization of the bleeding source with subsequent conservative management. TSC-associated AMLs occur more frequently as multiple lesions and grow to larger size than idiopathic AML.

Diffusional arteriovenous shunting in the heart.

Project description:Previous indicator dilution experiments in isolated blood-perfused dog hearts suggested that there was intramyocardial diffusional shunting of water relative to a flow-limited solute, antipyrine. Two sets of studies have been done to assess the importance of this shunting, since it implies the possibility of a diffusional bypass for oxygen and other substances, which may be important in ischemia. Nonconsumed tracers were used to show the phenomenon. In the first set, bolus injections of 133Xe dissolved in saline were made into the coronary inflow and the tracer content of the organ recorded by an external gamma detector. The initial Xe washout was disproportionately rapid at low flows, and the late phase was also relatively retarded. In the second set, boluses of cool saline containing indocyanine green were injected into the coronary arterial inflow while coronary sinus outflow dilution curves were recorded via a thermistor and a dye densitometer over a wide range of flows. The thermal curves showed emergence of heat preceding the dye; the degree of precession was much greater at low flows, and, unlike the dye curves, the thermal dilution curves showed dramatic differences in shape at different flows. A model for diffusional countercurrent exchange shows similar changes in residue curves and outflow dilution curves. The conclusion is that there is diffusional shunting of small lipid-soluble molecules whose diffusion coefficients in tissue are high. While the shunting of heat is great, the shunting of soluble gases will not be large and that of normal substrates will be negligible.
