Project description:Normal arm swing plays a role in decreasing the cost of transport during walking. However, whether excessive arm swing can reduce the cost of transport even further is unknown. Therefore, we tested the effects of normal and exaggerated arm swing on the cost of transport in the current study. Healthy participants (n=12) walked on a treadmill (1.25 m/s) in seven trials with different arm swing amplitudes (in-phase, passive restricted, active restricted, normal, three gradations of extra arm swing), while metabolic energy cost and the vertical angular momentum (VAM) and ground reaction moment (GRM) were measured. In general, VAM and GRM decreased as arm swing amplitude was increased, except for in the largest arm swing amplitude condition. The decreases in VAM and GRM were accompanied by a decrease in cost of transport from in-phase walking (negative amplitude) up to a slightly increased arm swing (non-significant difference compared to normal arm swing). The most excessive arm swings led to an increase in the cost of transport, most likely due to the cost of swinging the arms. In conclusion, increasing arm swing amplitude leads to a reduction in VAM and GRM, but it does not lead to a reduction in cost of transport for the most excessive arm swing amplitudes. Normal or slightly increased arm swing amplitude appear to be optimal in terms of cost of transport in young and healthy individuals.This article has an associated First Person interview with the first author of the paper.

Project description:Human arm swing looks and feels highly automated, yet it is increasingly apparent that higher centres, including the cortex, are involved in many aspects of locomotor control. The addition of a cognitive task increases arm swing asymmetry during walking, but the characteristics and mechanism of this asymmetry are unclear. We hypothesized that this effect is lateralized and a Stroop word-colour naming task-primarily involving left hemisphere structures-would reduce right arm swing only. We recorded gait in 83 healthy subjects aged 18-80 walking normally on a treadmill and while performing a congruent and incongruent Stroop task. The primary measure of arm swing asymmetry-an index based on both three-dimensional wrist trajectories in which positive values indicate proportionally smaller movements on the right-increased significantly under dual-task conditions in those aged 40-59 and further still in the over-60s, driven by reduced right arm flexion. Right arm swing attenuation appears to be the norm in humans performing a locomotor-cognitive dual-task, confirming a prominent role of the brain in locomotor behaviour. Women under 60 are surprisingly resistant to this effect, revealing unexpected gender differences atop the hierarchical chain of locomotor control.

Project description:Interlimb neural coupling might underlie human bipedal locomotion, which is reflected in the fact that people swing their arms synchronously with leg movement in normal gait. Therefore, arm swing should be included in gait training to provide coordinated interlimb performance. The present study aimed to develop a Rotational Orthosis for Walking with Arm Swing (ROWAS), and evaluate its feasibility from the perspectives of implementation, acceptability and responsiveness. We developed the mechanical structures of the ROWAS system in SolidWorks, and implemented the concept in a prototype. Normal gait data were used as the reference performance of the shoulder, hip, knee and ankle joints of the prototype. The ROWAS prototype was tested for function assessment and further evaluated using five able-bodied subjects for user feedback. The ROWAS prototype produced coordinated performance in the upper and lower limbs, with joint profiles similar to those occurring in normal gait. The subjects reported a stronger feeling of walking with arm swing than without. The ROWAS system was deemed feasible according to the formal assessment criteria.

Project description:Based on interlimb neural coupling, gait robotic systems should produce walking-like movement in both upper and lower limbs for effective walking restoration. Two orthoses were previously designed in our lab to provide passive walking with arm swing. However, an active system for walking with arm swing is desirable to serve as a testbed for investigation of interlimb neural coupling in response to voluntary input. Given the important function of the ankle joint during normal walking, this work aimed to develop an improved rotational orthosis for walking with arm swing, which is called ROWAS II, and especially to develop and evaluate the algorithms for active ankle control. After description of the mechanical structure and control schemes of the overall ROWAS II system, the closed-loop position control and adjustable admittance control algorithms were firstly deduced, then simulated in Matlab/Simulink and finally implemented in the ROWAS II system. Six able-bodied participants were recruited to use the ROWAS II system in passive mode, and then to estimate the active ankle mechanism. It was showed that the closed-loop position control algorithms enabled the ROWAS II system to track the target arm-leg walking movement patterns well in passive mode, with the tracking error of each joint <0.7°. The adjustable admittance control algorithms enabled the participants to voluntarily adjust the ankle movement by exerting various active force. Higher admittance gains enabled the participants to more easily adjust the movement trajectory of the ankle mechanism. The ROWAS II system is technically feasible to produce walking-like movement in the bilateral upper and lower limbs in passive mode, and the ankle mechanism has technical potential to provide various active ankle training during gait rehabilitation. This novel ROWAS II system can serve as a testbed for further investigation of interlimb neural coupling in response to voluntary ankle movement and is technically feasible to provide a new training paradigm of walking with arm swing and active ankle control.

Project description:Background: Interlimb neural coupling implies that arm swing should be included during gait training to improve rehabilitation outcomes. We previously developed several systems for production of walking with arm swing, but the reaction forces on the foot sole during usage of the systems were not satisfactory and there was potential to improve control system performance. This work aimed to design and technically evaluate a novel system for producing walking with synchronised arm and leg movement and with dynamic force loading on the foot soles. Methods: The robotic system included a passive curved treadmill and a trunk frame, upon which the rigs for the upper and lower limbs were mounted. Ten actuators and servocontrollers with EtherCAT communication protocol controlled the bilateral shoulder, elbow, hip, knee and ankle joints. Impedance control algorithms were developed and ran in an industrial PC. Flexible pressure sensors recorded the plantar forces on the foot soles. The criteria of implementation and responsiveness were used to formally evaluate the technical feasibility of the system. Results: Using impedance algorithms, the system produced synchronous walking with arm swing on the curved treadmill, with mean RMS angular tracking error <2° in the 10 joint profiles. The foot trajectories relative to the hip presented similar shapes to those during normal gait, with mean RMS displacement error <1.5 cm. A force pattern that started at the heel and finished at the forefoot was observed during walking using the system, which was similar to the pattern from overground walking. Conclusion: The robotic system produced walking-like kinematics in the 10 joints and in the foot trajectories. Integrated with the curved treadmill, the system also produced walking-like force patterns on the foot soles. The system is considered feasible as far as implementation and responsiveness are concerned. Future work will focus on improvement of the mechanical system for future clinical application.

Project description:Slopes are present in everyday environments and require specific postural strategies for successful navigation; different arm strategies may be used to manage external perturbations while walking. It has yet to be determined what impact arm swing has on postural strategies and gait stability during sloped walking. We investigated the potentially interacting effects of surface slope and arm motion on gait stability and postural strategies in healthy young adults. We tested 15 healthy adults, using the CAREN-Extended system to simulate a rolling-hills environment which imparted both incline (uphill) and decline (downhill) slopes (± 3°). This protocol was completed under three imposed arm swing conditions: held, normal, active. Spatiotemporal gait parameters, mediolateral margin of stability, and postural kinematics in anteroposterior (AP), mediolateral (ML), and vertical (VT) directions were assessed. Main effects of conditions and interactions were evaluated by 2-way repeated measures analysis of variance. Our results showed no interactions between arm swing and slope; however, we found main effects of arm swing and main effects of slope. As expected, uphill and downhill sections of the rolling-hills yielded opposite stepping and postural strategies compared to level walking, and active and held arm swings led to opposite postural strategies compared to normal arm swing. Arm swing effects were consistent across slope conditions. Walking with arms held decreased gait speed, indicating a level of caution, but maintained stability comparable to that of walking with normal arm swing. Active arm swing increased both step width variability and ML-MoS during downhill sections. Alternately, ML-MoS was larger with increased step width and double support time during uphill sections compared to level, which demonstrates that distinct base of support strategies are used to manage arm swing compared to slope. The variability of the rolling-hills also required proactive base of support changes despite the mild slopes to maintain balance.

Project description:BackgroundIndividuals with Parkinson's disease present arm swing alterations that can adversely affect their locomotion.ObjectiveTo identify differences in arm swing asymmetry (ASA) between individuals with Parkinson's disease (PD) and healthy individuals and to investigate the relationship between ASA, temporal-spatial gait parameters, and disease progression.MethodsA literature search was conducted in PubMed, Scopus, ProQuest, Web of Science, and EBSCOhost up to February 2023. Cross-sectional studies evaluating parameters of arm swing (AS) and ASA were included. Methodological quality was assessed using the Critical Appraisal Checklist, and the quality of the evidence was measured with a modified Grading of Recommendations Assessment, Development, and Evaluation.ResultsFourteen studies were included in the systematic review (1130 participants). Irrespective of the medication phase (ON or OFF) and the type of walk test employed, the meta-analysis showed moderate-quality evidence that individuals with PD have increased ASA amplitude (SMD = 0.84; 95% CI: 0.69, 0.99; I²= 0%).Very low-quality evidence suggests higher ASA velocity (SMD=0.64; 95% CI: 0.24, 1.05; I²=59%) and lower AS amplitude on both the most affected (ES = -1.99, 95% CI: -3.04, -0.94, I2: 91%) and the least affected sides (ES = -0.75, 95% CI: -1.05, -0.44; I²=66%). Meta-regression indicated that ASA is inversely related to disease duration (Z: -2.4892, P< 0.05) and motor symptoms progression (Z: -2.1336, P< 0.05).ConclusionsRegardless of the medication phase and the type of walk test employed, individuals with PD exhibited greater ASA and decreased AS amplitude than healthy individuals. ASA decreases as the disease progresses and symptoms worsen.

Project description:BackgroundOmnidirectional treadmills (ODTs) offer a promising solution to the virtual reality (VR) locomotion problem, which describes the mismatch between visual and somatosensory information and contributes to VR sickness. However, little is known about how walking on ODTs impacts the biomechanics of gait. This project aimed to compare overground and ODT walking and turning in healthy young adults.MethodsFifteen young adults completed forward walk, 180° turn, and 360° turn tasks under three conditions: (1) overground, (2) on the Infinadeck ODT in a virtual environment without a handrail, and (3) on the ODT with a handrail. Kinematic data for all walking trials were gathered using 3D optical motion capture.ResultsOverall, gait speed was slower during ODT walking than overground. When controlling for gait speed, ODT walking resulted in shorter steps and greater variability in step length. There were no significant differences in other spatiotemporal metrics between ODT and overground walking. Turning on the ODT required more steps and slower rotational speeds than overground turns. The addition of the stability handrail to the ODT resulted in decreased gait variability relative to the ODT gait without the handrail.ConclusionWalking on an ODT resembles natural gait patterns apart from slower gait speed and shorter step length. Slower walking and shorter step length are likely due to the novelty of physically navigating a virtual environment which may result in a more conservative approach to gait. Future work will evaluate how older adults and those with neurological disease respond to ODT walking.

Project description:Little information is currently available on interpersonal gait synchronization in overground walking. This is caused by difficulties in continuous gait monitoring over many steps while ensuring repeatability of experimental conditions. These challenges could be overcome by using immersive virtual reality (VR), assuming it offers ecological validity. To this end, this study provides some of the first evidence of gait coordination patterns for overground walking dyads in VR. Six subjects covered the total distance of 27 km while walking with a pacer. The pacer was either a real human subject or their anatomically and biomechanically representative VR avatar driven by an artificial intelligence algorithm. Side-by-side and front-to-back arrangements were tested without and with the instruction to synchronize steps. Little evidence of spontaneous gait coordination was found in both visual conditions, but persistent gait coordination patterns were found in the case of intentional synchronization. Front-to-back rather than side-by-side arrangement consistently yielded in the latter case higher mean synchronization strength index. Although the mean magnitude of synchronization strength index was overall comparable in both visual conditions when walking under the instruction to synchronize steps, quantitative and qualitative differences were found which might be associated with common limitations of VR solutions.

Project description:BackgroundSuccessful walking requires the execution of the pre-swing biomechanical tasks of body propulsion and leg swing initiation, which are often impaired post-stroke. While excess rectus femoris activity during swing is often associated with low knee flexion, previous work has suggested that deficits in propulsion and leg swing initiation may also contribute. The purpose of this study was to determine underlying causes of propulsion, leg swing initiation and knee flexion deficits in pre-swing and their link to stiff knee gait in individuals post-stroke.MethodsMusculoskeletal models and forward dynamic simulations were developed for individuals post-stroke (n = 15) and healthy participants (n = 5). Linear regressions were used to evaluate the relationships between peak knee flexion, braking and propulsion symmetry, and individual muscle contributions to braking, propulsion, knee flexion in pre-swing, and leg swing initiation.ResultsFour out of fifteen of individuals post-stroke had higher plantarflexor contributions to propulsion and seven out of fifteen had higher vasti contributions to braking on their paretic leg relative to their nonparetic leg. Higher gastrocnemius contributions to propulsion predicted paretic propulsion symmetry (p = 0.005) while soleus contributions did not. Higher vasti contributions to braking in pre-swing predicted lower knee flexion (p = 0.022). The rectus femoris had minimal contributions to lower knee flexion acceleration in pre-swing compared to contributions from the vasti. However, for some individuals with low knee flexion, during pre-swing the rectus femoris absorbed more power and the iliopsoas contributed less power to the paretic leg. Total musculotendon work done on the paretic leg in pre-swing did not predict knee flexion during swing.ConclusionsThese results emphasize the multiple causes of propulsion asymmetry in individuals post-stroke, including low plantarflexor contributions to propulsion, increased vasti contributions to braking and reliance on compensatory mechanisms. The results also show that the rectus femoris is not a major contributor to knee flexion in pre-swing, but absorbs more power from the paretic leg in pre-swing in some individuals with stiff knee gait. These results highlight the need to identify individual causes of propulsion and knee flexion deficits to design more effective rehabilitation strategies.
