Little Finger Flexor Tendoscopy, Endoscopy of the Ulnar Bursa and Midpalmar Space.

ABSTRACT: Inflammatory, infectious, and even neoplastic processes can involve the deep subfascial space of the hand. Suppurative deep-space infection is treated primarily with surgical drainage. Open approaches to these deep spaces carry the risk of exposed flexor tendons and nerves without adequate soft tissue coverage. The purpose of this Technical Note is to describe the technical details of little finger flexor tendoscopy, endoscopy of the ulnar bursa and midpalmar space, which will reduce the risk of exposed tendons or nerves associated with open approaches and allow aggressive hand therapy in the early postoperative period because of the minimal surgical trauma and small surgical wounds.

Project description:The deep palmar spaces of the hand include the midpalmar space, the hypothenar space, the thenar space, space of Parona, and the interdigital web spaces. There are various communications between different spaces. These communications are of clinical importance in explaining the spread of inflammatory, infectious, and even neoplasic processes that involve this region. Surgical incision and drainage of all potentially communicating spaces and compartments is mandatory in deep hand infections. The purpose of this technical note is to describe the minimally invasive approach of endoscopic drainage and debridement of the flexor pollicis longus tendon sheath, radial bursa, and thenar space.

Endoscopic Ganglionectomy of Palmar Ganglion via Flexor Carpi Radialis Tendoscopy.

Project description:Palmar ganglion is rare. Deep palmar ganglion usually arises from the wrist joint and may cause compression neuropathy. Superficial palmar ganglion may arise from the flexor tendons of the wrist. The chance of compression neuropathy is expected to be less than deep palmar ganglion. However, there is a higher chance of perforation through the skin and the ganglion may affect hand grip. Open resection of the ganglion does not require extensive deep soft tissue dissection. However, the skin incision can still be extensile to trace the stalk to the tendon of origin. The purpose of this Technical Note is to report the details of endoscopic ganglionectomy for superficial palmar ganglion arising from the flexor carpi radialis tendon.

Flexor Hallucis Longus Tenosynovitis and First Metatarsophalangeal Synovitis After Penetrating Nail Prick Injury: Treated by Zone 3 Flexor Hallucis Longus Tendoscopy and Metatarsosesamoid Arthroscopy.

Project description:Pedal penetrating nail prick injury around the first metatarsal head can result in persistent synovitis of the first metatarsophalangeal joint and tenosynovitis of the flexor hallucis longus tendon. Exploration and debridement is indicated if the condition does not improve with antibiotics. Open surgery requires extensive dorsal and plantar incisions. The purpose of this Technical Note is to report the combined arthroscopic and tendoscopic approaches to address the first metatarsophalangeal joint and flexor hallucis longus tendon pathologies. Because it is a result of a pedal injury, the layer-by-layer exploration and debridement is from plantar dorsally. It starts with zone 3 flexor hallucis longus tendoscopy, followed by arthroscopy of the metatarsosesamoid compartment, and finally arthroscopy of the metatarsophalangeal compartment.

Endoscopic Synovectomy of the Ulnar Bursa of the Palm of the Hand.

Project description:Synovitis of the ulnar bursa is associated with tenosynovitis of the flexor tendons. Reported causes include suppurative or tuberculous infection, rheumatoid arthritis, and pigmented villonodular synovitis. Open synovectomy requires extensive soft-tissue dissection and may result in extensive scarring. The purpose of this Technical Note is to describe the technical details of endoscopic synovectomy of the ulnar bursa of the palm of the hand. This minimally invasive approach with small incisions allows aggressive postoperative hand therapy and avoids the complications of tendon adhesions, joint contractures, and hand stiffness.

ACTA1-myopathy with prominent finger flexor weakness and rimmed vacuoles.

Project description:Actinopathy is a group of clinically and pathologically heterogeneous myopathies due to mutations in the skeletal muscle sarcomeric α-actin 1-encoding gene (ACTA1). Disease-onset spans from prenatal life to adulthood and weakness can preferentially affect proximal or distal muscles. Myopathological findings include a spectrum of structural abnormalities with nemaline rods being the most common. We report a daughter and father with prominent finger flexors and/or quadriceps involvement. Muscle biopsies revealed rimmed vacuoles in both patients, associated with type 1 fiber atrophy in the daughter, and nemaline rods in the father. Next generation sequencing identified a novel dominant ACTA1 variant, c.149G>A (p.Gly50Asp) in both individuals and no abnormal variants in vacuolar myopathy-associated genes. Our findings expand the clinico-pathological spectrum of actinopathy.

Brachioradialis to flexor digitorum profundus tendon transfer to restore finger flexion.

Project description:IntroductionThe main deformity following an adult lower brachial plexus injury is the loss of finger flexion. Distal nerve transfers have been used to restore finger and thumb flexion followed by tendon transfers for intrinsic replacement for opening of the fingers. When patients present beyond 6 months, only tendon transfers are applicable. Since the brachioradialis (BR) is always spared in such injuries, it is the ideal muscle to provide finger flexion. Wrist extensor power may not be normal for the use of the radial wrist extensor to serve as donor. BR to FDP transfer provides reasonable flexion range and an acceptable hand function to permit activities of daily living, when associated with ancillary procedures like opponensplasty, PIPJ arthrodesis.Materials and methodsEleven patients underwent a BR to FDP tendon transfer between January 2013 and January 2017 of which eight patients came for follow-up.ResultsFour of the eight patients got a functionally useful hand to carry out activities of daily living with hook grip, span grasp, key pinch, chuck grip and pulp pinch. These patients also underwent simultaneous or secondary ancillary procedures. Four of the patients need secondary procedures to further improve functionality of the hand inspite of having a flexion range.ConclusionThe BR is an effective donor in providing adequate range and power of finger flexion in lower plexus injuries.

Peroneal tendoscopy.

Project description:Peroneal tendoscopy is an innovative technique that allows visualization of the tendons from the myotendinous junction to the peroneal tubercle, together with adjacent anatomic structures such as the recently unveiled vincula. Through a minimally invasive approach, it is possible to diagnose and treat several disorders, such as common tenosynovitis, accessory muscles, hypertrophic bony prominences, and thickened vincula, that can cause pain and tendon catching. Surgical morbidity and postoperative pain are significantly reduced when compared with open procedures. In this paper, the main indications for peroneal tendoscopy are discussed, the available literature is reviewed, and the surgical technique is described. Advantages of this procedure and current limitations are also presented. Anatomic and histological studies were also performed in order to verify: 1) the feasibility of peroneal tendoscopy for evaluation of peroneal tendons, using cadaver specimens; 2) the presence of nervous tissue in cadaver peroneal vincula as well as in tendoscopic vincula biopsies from patients undergoing surgery for chronic lateral ankle pain.

Carbon dioxide gas endoscopy of the deep gluteal space.

Project description:The treatment of hip and pelvic pain associated with abnormalities of the deep gluteal space has evolved and increasingly involves endoscopic techniques with a saline expansion medium. This investigation presents a surgical technique utilizing carbon dioxide as the insufflation medium for deep gluteal space endoscopy in 17 cadaveric hips. This technique was successful in 94% (16/17) of the hips, allowing for visualization of the sciatic nerve, posterior femoral cutaneous nerve, pudendal nerve, branch of the inferior gluteal artery crossing the sciatic nerve, piriformis muscle, hamstring tendon origin, and lesser trochanter. Our experience suggests that gas expansion presents several advantages over fluid expansion.

TF is characterized by catching, snapping or locking of the involved finger flexor tendon, associated with pain. Extensor Tendoscopy of the Wrist.

Project description:Extensor tenosynovitis of the wrist can lead to rupture of the extensor tendons. Extensor tenosynovectomy is indicated if the tenosynovitis cannot be controlled by conservative measures. Open tenosynovectomy requires extensive dissection of soft tissue, including the extensor retinaculum. This article describes extensor tendoscopy of the wrist. This technique allows approach to the extensor compartments with small incisions and minimal soft tissue dissection. It is indicated when extensor tenosynovitis persists despite adequate medical treatments including medications, immobilization, and steroid injection. Synovectomy of the extensor compartments can be performed with preservation of the extensor retinaculum. This technique is contraindicated if the wrist cannot be flexed or there is severe tendinopathy of the extensor tendon that may need open reconstruction. The purpose of this technical note was to describe a minimally invasive approach of complete synovectomy of the extensor compartments with preservation of the extensor compartment.
