Psychometric properties of the Ndetei-Othieno-Kathuku (NOK) Scale: A mental health assessment tool for an African setting.

ABSTRACT: Research suggests that psychiatric conditions in children and adolescents are highly debilitating, with sparse resources for assessment and treatment in low- and middle-income countries (LMICs).The primary aim of this study was to evaluate the reliability, validity, and latent factor structure of an ethnographically-grounded assessment instrument for detecting common mental health complaints among rural Kenyan children and adolescents.The Ndetei-Othieno-Kathuku Scale (NOK) was delivered to 2 282 children aged 10 to 18 years old. Exploratory factor analysis identified four latent factors. This structure was confirmed in subsequent confirmatory factor analyses. External validity was explored by investigating associations among NOK factors and Youth Self-Report DSM-oriented scales.Findings suggest the NOK possesses good internal reliability and a four-factor latent structure corresponding to depression, anxiety, somatic complaints, and a mixed factor. Significant associations ranging from small to medium effect sizes were noted between NOK factors and YSR DSM-oriented scales.Exploratory findings suggest that the NOK possesses adequate psychometric properties among this population. This ethnographically-grounded instrument may be uniquely suited to screening for mental health complaints among Kenyan children and adolescents.

Project description:Positive mental health is crucial to individuals' total well-being and especially to their emotional, psychological, and social functioning. To assess the positive aspects of mental health, the Positive Mental Health Scale (PMH-scale) is being used as one of the most significant and practical short unidimensional psychological tools. However, the PMH-scale has not yet been validated for the Bangladeshi population nor has it been translated into Bangla. Therefore, the purpose of this study was to investigate the psychometric properties of the Bangla version of PMH-scale and validate it with the Brief Aggression Questionnaire (BAQ) and Brunel Mood Scale (BRUMS). The sample of the study consisted of 3145 university students (61.8% men) aged from 17 to 27 (M = 22.07, SD = 1.74) and 298 general population (53.4% men) aged from 30 to 65 (M = 41.05, SD = 7.88) of Bangladesh. The factor structure of the PMH-scale as well as measurement invariance for sex and age (age ≤30 years; age >30 years) were tested, using the confirmatory factor analysis (CFA). The CFA revealed that the originally proposed unidimensional model of PMH-scale had a good fit in the current sample which confirms factorial validity of the Bangla version of PMH-scale. The value of Cronbach's alpha (for both groups combined α = 0.85; for the student sample α = .85; and for the general sample α = .73) ensured the high internal consistency of the items. Concurrent validity of the PMH-scale was confirmed through the expected correlation with aggression (BAQ) and mood (BRUMS). The PMH-scale was also partially invariant over groups (student, general, men and women) indicating that the PMH-scale is equally applicable to student, general, men and women population. Therefore, this study tells us that the Bangla version of PMH-scale is a quick and easy-to-administer tool for assessing positive mental health in different groups of people in Bangladeshi culture. This work will also be useful for mental health studies in Bangladesh.

Project description:BackgroundAlthough community pharmacists in the United Kingdom are expected to assess elderly patients' needs for additional support in managing their medicines, there is limited data on potentially useful assessment tools. We sought to evaluate a 13-item assessment instrument among community dwelling elderly patients, 65 years and above. The instrument is composed of a cognitive risk sub-scale of 6 items and a physical risk sub-scale of 7 items.FindingsThe instrument was administered to elderly patients in a survey performed in a community to the west of Glasgow, Scotland. The survey recruited 37 participants, 31 from 4 community pharmacies and 6 patients whose medication management tasks were managed by the West Glasgow Community Health and Care Partnership (managed patients). Community pharmacists independently rated 29 of the 37 participants' comprehension of, and dexterity in handling their medicines. We assessed scale reliability, convergent validity and criterion validity. In sub-analyses, we assessed differences in scores between the managed patients and those recruited from the community pharmacies, and between multi-compartment compliance aid users and non-users. The instrument showed satisfactory internal consistency (Cronbach's alpha of 0.792 for 13-item scale). There was significant strong negative correlation between the cognitive risk sub-scores and community pharmacists' assessment of comprehension (? = -0.546, p = 0.0038); and physical risk sub-scores and community pharmacists' assessment of dexterity (? = -0.491, p = 0.0093). The Area Under the Receiver Operator Characteristic Curve (AUC ± SE; 95%CI) showed that the instrument had good discriminatory capacity (0.86 ± 0.07; 0.68, 0.96). The best cut-off (sensitivity, specificity) was ?4 (65%, 100%). In the sub-analyses, managed patients had significantly higher cognitive risk sub-scores (6.5 versus 4.0, p = 0.0461) compared to non-managed patients. There was a significant difference in total risk score (4 versus 2, p = 0.0135) and cognitive risk sub-score (4 versus 1.5, p = 0.0029) between users and non-users of multi-compartment compliance aids.ConclusionsThis instrument shows potential for use in identifying elderly patients who may have problems managing their own medicines in the community setting. However, more robust validity and reliability assessments are needed prior to introduction of the tool into routine practice.

Project description:BackgroundAlzheimer's disease (AD) is the most frequent cause of cognitive impairment. Community knowledge of the disease has proven to be a very important aspect of the development of interventions and the evaluation of their effectiveness. However, it is necessary to have standardized and recognized tools in different languages. The aim of the current study was to develop a cross-cultural adaptation of the Spanish Dementia Knowledge Assessment Scale (DKAS-S) and to assess their psychometric properties with cohorts of health students and professional and non-professional caregivers of AD patients from several regions of Spain.MethodsWe developed and translated the DKAS into Spanish following the forward-back-forward translation procedure. Then, we performed a cross-sectional study to assess the validity, reliability and feasibility of the DKAS-S. We also performed an analysis to obtain test-retest reliability measures. The study was performed in four medical centres across three regions in Spain. From May to September 2019, we administered the scale to students, professional and non-professional caregivers; including a subgroup of non-professional caregivers of patients with early-onset AD (< 65 years).ResultsEight hundred forty-six volunteer participants completed the DKAS-S: 233 students (mean age 26.3 ± 9.2 years), 270 professional caregivers (mean age 42.5 ± 11.7 years) and 343 non-professional caregivers of AD patients. (mean age was 56.4 ± 13.16). The DKAS-S showed good internal consistency (Cronbach's α = 0.819) and good test-retest reliability (time 1: 28.1 ± 8.09 vs time 2: 28.8 ± 7.96; t = - 1.379; p = 0.173). Sensitivity to change was also significant in a subgroup of 31 students who received education related to AD and dementias between each administration (time 1: 25.6 ± 6.03) to (time 2: 32.5 ± 7.12; t = - 5.252, p = 0.000). The validity of the construct was verified by confirmatory factor analysis, although there were challenges in the inclusion of some items in the original 4 factors.ConclusionsThe 25-item DKAS-S showed good psychometric properties for validity and reliability and the factorial analysis when it was administered to a population of students and professional and non-professional caregivers. It was a useful instrument for measuring levels of knowledge about dementia in Spanish population.
