Adult aortic coarctation presenting with refractory heart failure and pulsation below the bilateral clavicle.

ABSTRACT: Adult aortic coarctation is often asymptomatic and this condition can be detected because of a murmur or unexplained hypertension. Here, we report an adult case of aortic coarctaion with heart failure and a characteristic finding of pulsation below the bilateral clavicle.? A 58-year-old man with refractory heart failure due to unknown reasons was referred to our hospital. Auscultation presented no murmur and high blood pressure had been treated with medicine. Interestingly, precise physical examination revealed the bilateral pulsation at the midclavicular line from the 2nd to the 5th intercostal areas. Echographic examination revealed the dilated vessel and arterial blood flow 1-2 cm in depth from the body surface at the midclavicular 2nd intercostal areas. Contrast-enhanced computed tomography showed thoracic aortic coarctation and a well-developed collateral circulation via the bilateral internal thoracic arteries and epigastric arteries. The cause of heart failure was diagnosed as aortic coarctation. Palliative revascularization was performed and his blood pressure was lowered. When we see the patients with refractory heart failure due to unknown reasons, pulsation below the bilateral clavicle may give us a clue to diagnose the "hidden" aortic coarctation. <Learning objective: We must consider adult aortic coarctation when we see patients with refractory heart failure. Although the present case did not show a cardiac murmur characteristic of aortic coarctation, we found out the very unique sign of pulsation below the bilateral clavicle. When we see patients with refractory heart failure due to unknown reasons, pulsation below the bilateral clavicle may give us an important clue to diagnose the "hidden" aortic coarctation.>.

Project description:Background The long-term burden of cardiovascular disease after repair of coarctation of the aorta (CoA) has not been elucidated. We aimed to determine the incidence of and risk factors for cardiovascular events in adult patients with repaired CoA. Additionally, mortality rates were compared between adults with repaired CoA and the general population. Methods and Results Using the Dutch Congenital Corvitia (CONCOR) registry, patients aged ≥16 years with previous surgical or transcatheter CoA repair from 5 tertiary referral centers were included. Cardiovascular events were recorded, comprising coronary artery disease, stroke/transient ischemic attack, aortic complications, arrhythmias, heart failure hospitalizations, endocarditis, and cardiovascular death. In total, 920 patients (median age, 24 years [range 16-74 years]) were included. After a mean follow-up of 9.3±5.1 years, 191 patients (21%) experienced at least 1 cardiovascular event. A total of 270 cardiovascular events occurred, of which aortic complications and arrhythmias were most frequent. Older age at initial CoA repair (hazard ratio [HR], 1.017; 95% CI, 1.000-1.033 [P=0.048]) and elevated left ventricular mass index (HR, 1.009; 95% CI, 1.005-1.013 [P<0.001]) were independently associated with an increased risk of cardiovascular events. The mortality rate was 3.3 times higher than expected based on an age- and sex-matched cohort from the Dutch general population (standardized mortality ratio, 3.3; 95% CI, 2.3-4.4 [P<0.001]). Conclusions This large, prospective cohort of adults with repaired CoA showed a high burden of cardiovascular events, particularly aortic complications and arrhythmias, during long-term follow-up. Older age at initial CoA repair and elevated left ventricular mass index were independent risk factors for the occurrence of cardiovascular events. Mortality was 3.3-fold higher compared with the general population. These results advocate stringent follow-up after CoA repair and emphasize the need for improved preventive strategies.

Project description:ObjectivesThis study aimed to explore aortic morphology and the associations between morphological features and cardiovascular function in a population of patients with bicuspid aortic valve, while further assessing differences between patients with repaired coarctation, patients with unrepaired coarctation and patients without coarctation.MethodsThis is a single-centre retrospective study that included patients with available cardiovascular magnetic resonance imaging data and native bicuspid aortic valve diagnosis (n = 525). A statistical shape analysis was performed on patients with a 3-dimensional magnetic imaging resonance (MRI) dataset (n = 108), deriving 3-dimensional aortic reconstructions and computing a mean aortic shape (template) for the whole population as well as for the 3 subgroups of interest (no coarctation, repaired coarctation and unrepaired coarctation). Shape deformations (modes) were computed and correlated with demographic variables, 2-dimensional MRI measurements and volumetric and functional data.ResultsOverall, the results showed that patients with coarctation tended towards a more Gothic arch architecture, with decreased ascending and increased descending aorta diameters, with the unrepaired-aortic coarctation subgroup exhibiting more ascending aorta dilation. Careful assessment of patients with repaired coarctation only revealed that a more Gothic arch, increased descending aorta dimensions and ascending aorta dilation were associated with reduced ejection fraction (P ≤ 0.04), increased end-diastolic volume (P ≤ 0.04) and increased ventricular mass (P ≤ 0.02), with arch morphology distinguishing patients with and without recoarctation (P = 0.05).ConclusionsA statistical shape modelling framework was applied to a bicuspid aortic valve population revealing nuanced differences in arch morphology and demonstrating that morphological features, not immediately described by conventional measurements, can indicate those shape phenotypes associated with compromised function and thus possibly warranting closer follow-up.
