Endoscopic Extrabursal Excision of Olecranon Spur.

ABSTRACT: Olecranon spur is a common underlying clinical problem in patients with inflammatory conditions such as triceps tendonitis, olecranon bursitis, and gout. It is often managed conservatively. Conventionally, symptomatic olecranon spurs that have failed conservative management are excised along with the overlying bursa using an open technique. We describe an endoscopic extrabursal technique, where the spur is dissected out and excised in its entirety under endoscopic vision. Using this technique, a satisfactory view is achieved with less morbidity compared with the open excision; it also avoids a wound over the sensitive skin over the olecranon.

Project description:The humeral supracondylar process and Struthers ligament comprise a relatively rare but well-known anatomic variant. They are usually asymptomatic but may produce clinical symptoms related to compression of the median nerve or brachial artery below the ligament. Previously, surgery has been performed with an open ligament release and supracondylar process excision. This article reports on the use of endoscopic findings and the method of ligament release and process excision. Endoscopy is a minimally invasive technique that provides excellent visualization and enables the surgeon to perform dissection with magnification and precision. It allows the surgeon to introduce open surgical techniques into the depths of the wound in a controlled manner. Because of the dead space created, there is a risk of hematoma formation. Many of the concepts used in open surgery are now being used for endoscopic surgery, and vice versa. The barriers and differences among endoscopic, arthroscopic, and open procedures are being broken down. We report another endoscopic technique, which is part of the ongoing evolution of musculoskeletal surgery.

Endoscopic Decompression of Olecranon Tophus.

Project description:The olecranon bursa is very commonly involved in tophaceous gout because of the tendency of monosodium urate crystals to deposit in superficial structures with low temperatures. Surgery is indicated if the olecranon tophus is recalcitrant to medical treatment. Open surgery requires a long incision over the tophus and may lead to wound complications. Endoscopic debridement of the tophus can reduce the risk of wound complications. In this Technical Note, the technical details of endoscopic decompression of an olecranon tophus are described. This endoscopic technique also allows debridement of tophus infiltration of the triceps tendon.

Cuff meshoma post-laparoscopic sacrocolpopexy: vaginal-endoscopic mesh excision.

Project description:Introduction and hypothesisThe objective was to present endoscopic images of a meshoma and describe the complete excision of a complicated mesh after sacrocolpopexy (SCP) using a combined vaginal-endoscopic technique.MethodsWe present a video documentation of an innovative technique. A 58-year-old woman was referred with painless, foul-smelling vaginal discharge and recurrent vaginal mesh erosions. She had undergone a laparoscopic SCP 12 years ago and her symptoms had begun 5 years ago. A pre-operative MRI scan revealed a cuff meshoma and an inflammatory sinus around the mesh extending from the cuff to the sacral promontory. Under general anesthesia, a 30° hysteroscope was inserted transvaginally into the sinus, where the retained mesh was seen in the form of a shrunken meshoma, and then the mesh arms were recognized extending cephalad into a sinus tract. Under direct endoscopic visualization, the mesh was carefully mobilized at its highest point with the use of laparoscopic grasping forceps. Then, the mesh was dissected with hysteroscopic scissors in close proximity to the bone. No peri-operative complications were recognized.ResultsA combined vaginal-endoscopic approach was successfully used to remove an eroded mesh and cuff meshoma after SCP.ConclusionThis procedure offers a minimally invasive, low-morbidity, and rapid-recovery approach.

Surgical excision of trigeminal (V3) schwannoma through endoscopic transpterygoid approach

Project description:Background: Endoscopic endonasal transpterygoid (EET) approach is well suited for trigeminal schwannomas. Case Description: A 25-year-old female presented with the right hemifacial pain for 1 month. Examination revealed mild sensory loss to both touch and pain in the right V2 and V3 dermatomal distribution. On magnetic resonance imaging, a well-defined extra-axial lesion was seen in the right infratemporal fossa extending intracranially, widening the foramen ovale. Erosion of the lateral pterygoid plate was evident on computed tomography. Diagnosis of a trigeminal schwannoma was made and excision through endoscopic transnasal transpterygoid approach was planned. After adequate nasal decongestion, lateralization of the inferior turbinate followed by medialization of middle turbinate done to expose the posterior part of the lateral nasal wall. Sphenopalatine foramen was visualized and the branches of the sphenopalatine artery to turbinate were coagulated followed by partial inferior and middle turbinectomy. Next, antegrade uncinectomy, bullectomy, and middle meatal antrostomy performed. Anterior and posterior ethmoidectomy was done to gain access to the sphenoid sinus. Medial maxillectomy done, posterior maxillary sinus wall exposed and drilled to reach infratemporal fossa harboring the tumor. The lateral recess was further exposed to visualize the vidian and maxillary nerves. Palatine bone and pterygoid body along with the medial pterygoid plate were completely drilled to expose the tumor capsule, which was gently dissected off. Complete tumor excision was performed uneventfully. Conclusion: EET provides a good oncological and functional outcome. With a lesser risk of surgical morbidities, it is an excellent alternative to conventional skull base approaches to trigeminal schwannomas in the modern era.

Exophiala oligosperma causing olecranon bursitis.

Project description:A 62-year-old male with a history of Wegener's granulomatosis and immunosuppressive therapy presented with chronic olecranon bursitis. A black velvety mould with brown septate hyphae and tapered annellides was isolated from a left elbow bursa aspirate and was identified as an Exophiala species. Internal transcribed sequence rRNA sequencing showed the isolate to be identical to Exophiala oligosperma. The patient was successfully treated with aspiration and intrabursal amphotericin B.

Endoscopic Excision of Intraventricular Neurocysticercosis Cyst with an Angiocatheter: A Retrospective Analysis.

Project description:IntroductionAn intraventricular cysticercus cyst is observed in 7%-30% patients of neurocysticercosis (NCC). Apart from causing arachnoiditis, intraventricular NCC (IVNCC) can cause sudden death due to acute episodes of hydrocephalus. Various treatment modalities available are external cerebrospinal fluid (CSF) diversion, microsurgical removal, and endoscopic management. There is no consensus regarding the optimal surgical treatment strategy. We are presenting our experience by doing a retrospective analysis of 26 patients having endoscopic removal of IVNCC with a rigid endoscope and angiocatheter.AimThe aim of this study is to evaluate the results of neuroendoscopy in restoring the CSF pathway and removal of the cyst in patients of IVNCC.Materials and methodsRetrospective analysis of clinical record and follow-up of 26 patients of IVNCC who were treated endoscopically between 2010 and 2018 was done. The diagnosis of IVNCC was made based on contrast enhanced magnetic resonance imaging of the brain. Transcranial endoscopy with 0° endoscope was performed through the frontal burr hole. Third ventriculostomy and removal of the cysticercus cyst with angiocatheter were the procedures done.ResultsThe success rate of intraventricular cyst excision, whether complete or partial, was 100% (14/14) in the third ventricular cyst, 62.5% (5/8) in the fourth ventricular cyst and 100% (4/4) in the lateral ventricular cyst. The overall success rate of cyst excision by endoscopy was 88.46%. The overall successful CSF flow pathway was established in 88.46% cases. The mean duration of follow-up was 44 months, and all the patients were found symptom free in the follow-up period.ConclusionNeuroendoscopy, being a single burr hole technique successfully providing internal CSF diversion and cyst removal, is the treatment modality of choice for IVNCC.

Forces Applied at the Skull Base during Transnasal Endoscopic Transsphenoidal Pituitary Tumor Excision.

Project description:Objectives Our laboratory is developing a surgical robotic system to further improve dexterity and visualization that will allow for broader application of transnasal skull base surgery. To optimize this system, intraoperative force data are required. Using a modified curette, force data were recorded and analyzed during pituitary tumor excision. Design A neurosurgical curette was modified by the addition of a force sensor. The instrument was validated in an in vitro model to measure forces during simulated pituitary tumor excision. Following this, intraoperative force data from three patients during transnasal endoscopic excision of pituitary tumors was obtained. Setting Academic medical center. Main Outcome Measures Forces applied at the skull base during surgical excision of pituitary tumors. Results Average forces applied during in vitro testing ranged from 0.1 to 0.15 N. Average forces recorded during in vivo testing ranged from 0.1 to 0.5 N. Maximal forces occurred with collisions of the bony sella. The average maximal force was 1.61 N. There were no complications related to the use of the modified curette. Conclusions Forces to remove pituitary tumor are small and are similar between patients. The in vitro model presented here is adequate for further testing of a robotic skull base surgery system.

New 'ZISIS' (Zigzag Incision-Suction-Irrigation System) Technique for Endoscopic-Assisted Forehead Tumour Excision.

Project description:Background:Endoscopic-assisted excision of forehead tumours like osteomas and lipomas is well established, but the conventional techniques suffer from many limitations like inadequate access, fogging of the endoscope and unclear vision due to collection of blood and debris. Method:Three simple modifications of the conventional endoscopic forehead technique for benign tumour excisions are described by the acronym 'ZISIS'. ZI Zigzag scalp incision increases the surface area of the opening permitting easy insertion of multiple instruments along with the endoscope.S Suction tubing made from a disposable suction catheter tube is taped along the endoscope sheath for continuous suctioning and good vision.IS Irrigation system of warm saline made with an infant feeding tube is also taped along the endoscope just opening in the front of the suction tube. Results:A total of 12 consecutive patients underwent endoscopic excision of forehead benign tumours in 2 years. This included 2 lipomas and 10 osteomas cases. All patients achieved excellent hidden scars in the scalp. All patients rated their results as excellent with respect to the hidden scar and aesthetic result. No early or late complications were reported with follow-up ranging from 6 months to 2 years. Conclusion:A new simple modification called 'ZISIS' endoscopic forehead excision technique is described for benign forehead tumours excision making it easier, efficient and ergonomic. Level of Evidence IV:Evidence obtained from multiple time series with or without the intervention, such as case studies.

Blindness Following Endoscopic Excision of Acinic Cell Carcinoma of Sphenoid Sinus: A Case Report.

Project description:Acinic cell carcinoma (A.C.C.) is a low-grade malignancy involving salivary glands. A.C.C. accounts for only 1-4% of all sinonasal malignancies. We report the case of a 45-year-old female who presented with A.C.C. of paranasal sinus who developed loss of vision after Endoscopic sinus surgery (E.S.S.). Blindness, though rare, is a devastating complication of E.S.S. In this report, the rare occurrence of a papillary cystic variant of A.C.C. in the sphenoid sinus is reported. The causes that may lead to blindness during E.S.S. are analyzed in the absence of direct neural trauma.Supplementary informationThe online version contains supplementary material available at 10.1007/s12070-022-03190-2.
