Online Survey of Medical and Psychological Professionals on Structured Instruments for the Assessment of Work Ability in Psychiatric Patients.

ABSTRACT: Objective: To evaluate perceived needs and difficulties related to instruments for assessing work ability in individuals with mental disorders. Method: We conducted an online survey of 104 German-speaking medico-legal experts (forensic psychiatric and psychology experts, insurance physicians) and therapists. Results: The large majority of respondents reported they would welcome a standardized, structured instrument for the assessment of work ability. High predictiveness, inter-rater agreement, comprehensibility for laymen, and symptom validity were desired in roughly equal measure as the main characteristic of such an instrument. More women than men, and more medico-legal experts than therapists, considered symptom validation as always necessary. Pain, personality, and affective disorders were perceived to be the most difficult disorders in the context of work ability assessments. Conclusion: Our survey documents professionals' wish for a structured assessment of work ability in both medico-legal and therapeutic settings.

Project description:Background: The SARS-CoV-2 pandemic was announced on March 11th, 2020, due to a surge of newly confirmed cases that significantly impacted populations worldwide, both directly and indirectly. Based on past epidemics research, the mental health implications of introduced restrictions should be expected and adequately addressed irrespective of the practiced profession. Objective: The study aimed to explore psychopathological responses, including post-traumatic stress disorder (PTSD), concerning coping strategy clusters during the COVID-19 pandemic among medical and non-medical workers. Methods: A cross-sectional web survey of the general population of internet users was performed from March 16th to April 26th, 2020, in Poland during the first peak of COVID-19 cases. A sample of 1,831 professionally active respondents, 64.0% of which pursuing a medical career, filled out General Health Questionnaire-28 (GHQ-28), The Impact of Event Scale-Revised (IES-R), and MiniCOPE, along with the socio-demographic questionnaire exploring personal as well as the work-related possibility of direct exposure to contagion and availability of proper protection, contact with the infected without accurate protective measures as well as the adequacy of workers when compared settings. Results: Individuals labeled with specific clusters had significantly different psychopathological manifestations. Irrespective of performed job maladaptive cluster was associated with significantly higher GHQ-28 and IES-R scores on total subscales and all subscales compared to those representing the non-specific and adaptive cluster. Similar findings were observed concerning the frequency of the GHQ-28 positive score. Moreover, the non-specific cluster was associated with significantly higher GHQ-28 total scores among medical professionals. However, GHQ-28 positive scores were significantly more frequent in medical workers using adaptive clusters when compared to non-specific. Such relations were not observed in the non-medical group. IES-R total and subscales' scores did not significantly vary within medical and non-medical groups when adaptive and non-specific clusters were compared. Pursuing a non-medical career was found to be a determinant of lower scores, while female sex was observed to be determinant of higher scores in both GHQ-28 and IES-R scales. Conclusions: Positive screening for psychopathological and PTSD symptoms was expected regardless of the analyzed groups' coping strategies. Given the dramatically developing situation of the COVID-19 pandemic, support initiatives grounded in research evidence may be essential for maintaining the mental well-being and resilience of both the medical and non-medical workforce.

Project description:Background: According to epidemiological data, over 450 million people worldwide suffer from mental disorders, presenting one of the major challenges of modern medicine. In their everyday lives, patients, in addition to fighting the disease itself, often struggle with stigmatization. This phenomenon negatively affects both the diagnostic and therapeutic processes, as well as the patients' everyday functioning. This study aimed to assess stigma attitudes toward psychiatry and psychiatric patients among undergraduate medical students. Methods: This study used a Computer-Assisted Web Interview (CAWI), which included the standardized items from the Mental Illness: Clinicians' Attitudes (MICA-2) scale to evaluate stigma. The study was disseminated via the internet to students from medical universities from 65 countries worldwide. Participation was voluntary and anonymous. The study involved 1,216 students from these 65 countries. Most of the sample were women, and most were medical faculty students and students living in cities with more than 500,000 residents. Taking into consideration Gross Domestic Product (GDP) per capita and Human Development Index (HDI) variables, it can be seen that there was a prevalence of medical students from highly developed countries. Results: For the whole sample, the mean MICA-2 score was 40.5 points. Women and medical and nursing students showed more positive attitudes toward psychiatric patients. Students from countries with the highest economic development levels also achieved statistically lower MICA-2 scores. Lower score means a more positive attitude. Conclusion: Stigma toward both psychiatry and psychiatric patients is common among undergraduate medical students. Female students and respondents with a history of mental disorders in countries with high HDI and GDP per capita indices show more favorable attitudes than other medical students. There is a need to further our understanding of the problem of stigmatization, both among the general population and among medical personnel, and to implement and maintain appropriate measures to reduce stigma toward psychiatry.

Project description:BackgroundThe prevalence of the group of workers that had a cancer diagnosis in the past is growing. These workers may still be confronted with late effects of cancer (treatment) possibly affecting their work ability. As little is known about the guidance of this group, the aim of this study was to explore the experiences and ideas of managers and professionals about the guidance of these workers in the case of late effects of cancer (treatment). Given the positive associations with work ability of the job resources autonomy, social support by colleagues and an open organisational culture found in several quantitative studies, these job resources were also discussed. Further ideas about the influences of other factors and points of attention in the guidance of this group of workers were explored.MethodsSemi-structured interviews were conducted with managers (n = 11) and professionals (n = 47). Data-collection was from November 2019 to June 2020. The data were coded and analysed using directed content analyses.ResultsThe late effects of cancer or cancer treatment discussed were physical problems, fatigue, cognitive problems, anxiety for cancer recurrence, and a different view of life. The self-employed have less options for guidance but may struggle with late effects affecting work ability in the same way as the salaried. Late effects may affect work ability and various approaches have been described. Autonomy, social support of colleagues and an open organisational culture were regarded as beneficial. It was indicated that interventions need to be tailor-made and created in dialogue with the worker.ConclusionsEspecially with respect to cognitive problems and fatigue, guidance sometimes turned out to be complicated. In general, the importance of psychological safety to be open about late effects that affect work ability was emphasized. Moreover, it is important to take the perspective of the worker as the starting point and explore the possibilities together with the worker. Autonomy is an important factor in general, and a factor that must always be monitored when adjustments in work are considered. There is a lot of experience, but there are still gaps in knowledge and opportunities for more knowledge sharing.
