Raising Awareness of Acute Kidney Injury: A Latin American Experience.

ABSTRACT:

Introduction

Raising awareness of acute kidney injury (AKI) is an essential strategy for minimizing the burden of this lethal syndrome. The AKI Commission of the Latin American Society of Nephrology and Hypertension conducted an educational program based on networked learning.

Methods

Two online courses with similar methodologies were developed, 1 course for nephrologists and the other for primary care physicians (PCP). The courses were developed as a distance education, asynchronous online modality with multiple educational strategies: written lessons, videos, e-rounds, and clinical simulation. Knowledge gain was explored through a 10-question test before and after course completion.

Results

The course for nephrologists had 779 participants from 21 countries; 52% were male, and 46% were <35 years of age. Mean qualification increased from 5.87 to 8.01 (36% gain of knowledge). The course for PCPs had 2011 participants, 81% of whom were physicians. The time from graduation was <5 years in 52%. In both courses, clinical simulation was considered the best part and lack of time the main limitation for learning. Because 48% of the nephrologist course attendees were interested in AKI activities, a Latin American AKI Network site (RedIRA) composed of a brief review, a clinical forum, a self-assessment, and a bibliography on AKI was launched on a monthly basis in November 2016. To date there are 335 users from 18 countries.

Conclusions

Distance education techniques were effective for learning about AKI and are a potential tool for the development of a sustainable structure for communication, exchange, and integration of physicians involved in the care of patients with AKI.

Project description:INTRODUCTION:Dengue is an escalating public health concern in Latin American Countries with a dramatic increase of cases reported during the past decade. The objectives of this study were to identify and provide insights into current management and attitudes toward dengue and to understand attitudes to vaccination and current behaviors to prevent dengue in Mexico and Colombia. METHODS:This was a community-based, cross-sectional, descriptive study conducted in urban and rural areas in endemic and non-endemic regions. The interviews were conducted face-to-face using a structured questionnaire containing 58 questions. A quota sampling approach was used to obtain a nationally representative sample of the adult population. All data were weighted to correct for differences between the samples surveyed in each country relative to their general population. RESULTS:A total of 1978 participants completed the survey. Two percent and 10% of participants in Mexico and Colombia, respectively, had experienced dengue fever, with just under one-third of adults and almost two-thirds of their children hospitalized as a result of the illness. Awareness of dengue was similar in Colombia (76%) and Mexico (68%), with awareness higher in endemic regions than in non-endemic regions. Colombia had a higher proportion of participants (84%) who considered dengue to be a common disease in their country, compared with Mexico (56%). In Mexico and Colombia, 55% and 54% in endemic areas, and 28% and 46% in non-endemic areas believed that everyone was at risk of contracting dengue. In both countries, the most common action undertaken by participants to prevent dengue infection was removal of standing water. At least 70% of participants believe their government could do more to prevent dengue in their country. CONCLUSIONS:Dengue was identified as a severe and common disease in Mexico and Colombia. Most participants recognized the need to reduce the risk of dengue infection by removal of standing water. Awareness was similar in Colombia and Mexico.

Project description:BACKGROUND:This retrospective study compared the change in serum creatinine between African American and Caucasian total knee arthroplasty (TKA) patients. The authors hypothesized that African Americans would demonstrate significantly greater change, and that a significantly greater proportion would demonstrate creatinine changes consistent with acute kidney injury (AKI). METHODS:Primary TKAs performed at a single institution between July 2011 and June 2016 were identified: 1035 primary TKAs met inclusion and exclusion criteria (110 African American, 925 Caucasian, excluding Hispanic and Asian patients). None were excluded based on gender, age, body mass index, preoperative diagnosis, or comorbidities. All patients had preoperative and postoperative creatinine levels available in the electronic medical records. Each patient received the same preop and postop protocol for nonsteroidal anti-inflammatory drug use along with other drugs administered including anesthesia. All patients received 1 g of intravenous vancomycin with some patients additionally receiving 1 g of vancomycin powder administered locally at the end of surgery. All patients were controlled for fluid intake and blood loss, along with no patient receiving a transfusion or intravenous contrast. Patient demographics and preoperative/postoperative serum creatinine were recorded and then analyzed for presence of AKI (?0.3 mg/dL). Preoperative/postoperative serum creatinine concentrations were compared between African American and Caucasian patients using 2 × 2 repeated measures analysis of variance. Prevalence of patients in each group demonstrating AKI was calculated using Fisher's exact test. RESULTS:African American patients had significantly greater serum creatinine preoperatively (1.00 ± 0.26 vs 0.90 ± 0.22, P < .001) and a significantly greater increase postoperatively (0.10 vs 0.03, P < .001). A significantly greater number of African American patients demonstrated AKI (10.9% vs 5.1%, P = .03). Furthermore, a significantly greater number of African American patients stayed in the hospital an additional 2 or more days for renal issues (2.7% vs 0.4%, P = .03). CONCLUSION:Altered renal function was significantly more common in African American TKA patients. Future studies are necessary to determine if tailoring anti-inflammatories, perioperative medications, and preoperative comorbidities reduce the risk of renal injury and/or a longer hospital stay for this subset of patients.
