Reliability of sagittal vertical axis measurement and association with measures of age-related hyperkyphosis.

ABSTRACT: [Purpose] Persons with age-related hyperkyphosis often have concomitant sagittal plane imbalance of the spine. This study investigated the reliability of sagittal vertical axis (SVA) measurement of sagittal balance, association between thoracic Cobb angle of kyphosis and SVA measure of sagittal balance, and compared the degree of SVA in males and females with age-related hyperkyphosis. [Participants and Methods] Measurements of SVA and Cobb angle of kyphosis were obtained from baseline radiographs of 112 community-dwelling males and females, mean age 70.0 (SD=5.7) years with kyphosis ?40 degrees, recruited for a randomized controlled trial. Spearman correlation coefficients were used to determine associations between SVA and kyphosis, and Wilcoxon nonparametric tests to compare SVA between genders. [Results] SVA was acquired with excellent intra-rater [0.95 (95% CI: 0.88, 0.98)] and inter-rater reliability [0.93 (95% CI: 0.83,0.97)]. There was no significant correlation between Cobb angle of thoracic kyphosis and SVA, (r=-0.05). More males than females had sagittal imbalance (SVA?5?cm). [Conclusion] In older adults with hyperkyphosis, SVA was a reliable measure of sagittal balance, and more extreme in males. SVA was not associated with Cobb angle of thoracic kyphosis, and could be considered an independent phenotype of age-related hyperkyphosis to be targeted in future intervention trials.

Project description:BackgroundRepeat power ability (RPA) assessments are a valuable evaluation of an athlete's ability to repeatedly perform high intensity movements. Establishing the most reliable and valid loaded jump RPA assessment and method to quantify RPA has yet to be determined. This study aimed to compare the reliability and validity of an RPA assessment performed with loaded squat jumps (SJ) or countermovement jumps (CMJ) using force-time derived mean and peak power output.Materials and methodsRPA was quantified using calculations of average power output, a fatigue index and a percent decrement score for all repetitions and with the first and last repetitions removed. Validity was established by comparing to a 30 second Bosco repeated jump test (30BJT). Eleven well-trained male field hockey players performed one set of 20 repetitions of both SJs (20SJ) and CMJs (20CMJ) on separate occasions using a 30% one repetition maximum half squat load. These assessments were repeated 7 days apart to establish inter-test reliability. On a separate occasion, each participant performed the 30BJT.ResultsThe reliability of average peak power for 20SJ and 20CMJ was acceptable (CV < 5%; ICC > 0.9), while average mean power reliability for 20CMJ (CV < 5%; ICC > 0.9) was better than 20SJ (CV > 5%; ICC > 0.8). Percent decrement of 20CMJ peak power, with the first and final jump removed from the percent decrement calculation (PD%CMJpeak18), was the most reliable measurement of power output decline (CV < 5 %; ICC > 0.8). Average mean and peak power for both RPA protocols had moderate to strong correlations with 30BJT average mean and peak power (r = 0.5-0.8; p< 0.05-0.01). No RPA measurements of power decline were significantly related to BJT measurements of power decline.ConclusionsThese findings indicate that PD%CMJpeak18 is the most reliable measure of RPA power decline. The lack of relationship between power decline in the loaded RPA and the 30BJT assessment suggest that each assessment may be measuring a different physical quality. These results provide sport science practitioners with additional methods to assess RPA and provide useful information on the reliability and validity of these outcome measures. Additional research needs to be performed to examine the reliability and validity of the novel RPA assessments in other athletic populations and to determine the sensitivity of these measurements to training and injury.

Project description:As we age, the acuity of our sensory organs declines, which may affect our lifestyle. Sensory deterioration in the vestibular system is typically bilateral and gradual, and could lead to problems with balance and spatial orientation. To compensate for the sensory deterioration, it has been suggested that the brain reweights the sensory information sources according to their relative noise characteristics. For rehabilitation and training programs, it is important to understand the consequences of this reweighting, preferably at the individual subject level. We psychometrically examined the age-dependent reweighting of visual and vestibular cues used in spatial orientation in a group of 32 subjects (age range: 19-76 yr). We asked subjects to indicate the orientation of a line (clockwise or counterclockwise relative to the gravitational vertical) presented within an oriented square visual frame when seated upright or with their head tilted 30° relative to the body. Results show that subjects' vertical perception is biased by the orientation of the visual frame. Both the magnitude of this bias and response variability become larger with increasing age. Deducing the underlying sensory noise characteristics, using Bayesian inference, suggests an age-dependent reweighting of sensory information, with an increasing weight of the visual contextual information. Further scrutiny of the model suggests that this shift in sensory weights is the result of an increase in the noise of the vestibular signal. Our approach quantifies how noise properties of visual and vestibular systems change over the life span, which helps to understand the aging process at the neurocomputational level. NEW & NOTEWORTHY Perception of visual vertical involves a weighted fusion of visual and vestibular tilt cues. Using a Bayesian approach and experimental psychophysics, we quantify how this fusion process changes with age. We show that, with age, the vestibular information is down-weighted whereas the visual weight is increased. This shift in sensory reweighting is primarily due to an age-related increase of the noise of vestibular signals.

Project description:BackgroundIn major depressive disorder (MDD), self-blame-related fMRI measures have shown the potential to be used as prognostic markers for recurrence risk. Like most potential fMRI markers, however, their reliability is unclear. Here, we probed the internal reliability of self-blame-related fMRI measures, as well as the impact of different modelling approaches on reliability metrics and validity.MethodsInternal consistency (i.e. split-half reliability) was calculated for blood oxygen level-dependent (BOLD) responses and psychophysiological interactions (PPI) related to self-blame-related biases in medication-free remitted MDD participants (n = 81) and healthy controls (n = 41). Trial-length was modelled using three durations (0, 2 and 5 s), which was convolved with the haemodynamic response function (HRF) with and without time and dispersion derivatives. Intraclass correlation coefficients (ICCs) were calculated for simple contrasts examining activation to self-blaming emotions and other-blaming emotions and the more complex contrast of the subtraction-based difference between self- and other-blaming emotions within the following a priori ROIs: right superior anterior temporal lobe seed region, anterior subgenual cingulate cortex, posterior subgenual cortex and right striatum / pallidum.ResultsAcross ROIs, we obtained fair reliability (ICC ≥ 0.40) for simple, but poor reliability (ICC < 0.40) for more complex fMRI measures related to self-blame. Despite this low internal consistency of complex measures at the individual level, we observed robust activation at the group-level, reproducing previously published results.ConclusionsWhile simple BOLD contrasts had fair reliability, previously employed PPI models had poor reliability and simple connectivity measures lacked predictive validity. This calls for the development of functional connectivity measures that strike a better balance between reliability and validity for future clinical applications, which require robust measures at the individual rather than group-level.
