Brucellosis seroprevalence in ovine and caprine flocks in China during 2000-2018: a systematic review and meta-analysis.

ABSTRACT:

Background

Brucellosis remains one of the most common zoonotic diseases globally, with more than half million human cases reported annually. Brucellosis is an emerging and re-emerging disease in China since the 1990s. An infectious reservoir constituted by domestic animals with brucellosis, especially ovine and caprine herds, poses a significant threat to public health. The seroprevalence of brucellosis in sheep and goat flocks in a national context is unavailable so far. Therefore, we conducted this systematic review and meta-analysis to assess the overall status of brucellosis in sheep and goats in China in almost two decades.

Results

The pooled prevalence of brucellosis in ovine and caprine flocks in China increased in 2000-2009 (1.00%; 95% CI, 0.70-1.30) to 2010-2018 (3.20%; 95% CI, 2.70-3.60). The seroprevalence of brucellosis in sheep and goat flocks was higher in Eastern China, with 7.00% of positive rate, than that in any other region, especially Shandong province (18.70%). Brucellosis is highly endemic in some local regions. The high prevalence of brucellosis in agricultural regions is suggestive of a shift of geographic distribution. The pooled prevalence of brucellosis is higher in goat flocks than in sheep flocks in China.

Conclusions

The overall data in this meta-analysis demands comprehensive intervention measures and further surveillance to facilitate the control of brucellosis in livestock. Further studies aimed at evaluating the risk factors associated with spreads of brucellosis in domestic animals unaddressed so far, and sufficient epidemiological data is important to the exploration and understanding of the prevalent status of brucellosis throughout the country and to disease control.

Project description:Simple Summary Fasciolia hepatica and Fasciola gigantica are widespread and found in the liver and bile ducts of ruminant animals and humans, which poses a major problem in animal husbandry. There are many studies on the prevalence of ovine and caprine fasciolosis in China, but the overall prevalence of fasciolosis and the risk factors are still unclear. This systematic review and meta-analysis was designed to investigate the prevalence and risk factors of ovine and caprine fasciolosis in China. The pooled prevalence of ovine and caprine fasciolosis was 26.00%. Out of all the subgroups, the type of season and sampling years showed the most significant (p < 0.05) differences. Abstract Fasciolosis is a significant zoonotic and common parasitic disease for animals and humans, creating public health concerns worldwide. This study retrieved articles related to the occurrence of Fasciola hepatica and Fasciola gigantica in sheep and goats in China by searching five databases: PubMed, ScienceDirect, the Chinese National Knowledge Infrastructure (CNKI), Wanfang Data, and the VIP Chinese Journal Database. A total of 60 valid articles were captured. The pooled prevalence of ovine and caprine fasciolosis was 26.00%. It was also found to be higher in the subgroups of Northwest China and Shaanxi Province, as well as in areas with a high altitude, rainfall of ≥800 mm, and temperature ranging between 10 °C and 20 °C. Analysis of the type of season and sampling years showed significant (p < 0.05) difference. In other subgroups, sheep (34.74%), hosts aged over 2 years (32.26%), females (48.33%) and free-range animals (26.83%) showed a higher disease prevalence. These results indicated that ovine and caprine fasciolosis was widely distributed, especially in Northwest China. The sampling years and the type of season are risk factors for the prevalence of ovine and caprine fasciolosis. Therefore, strategies for ovine and caprine fasciolosis control should be developed based on these epidemic risk factors, which will reduce the prevalence of fasciolosis in China.

Project description:Background: Brucellosis is an important zoonotic disease caused by Brucella spp. Brucellosis is widely distributed in more than 160 or 170 countries around the world, where it poses a huge threat to animal husbandry and human health. About 150 million head of water buffalo, distributed across more than 40 countries worldwide, are kept for the purposes of service, milk, and meat. High incidence of Brucella spp. in buffalo has negatively affected dairy products and meat products. Results: We searched all research related to seroprevalence of brucellosis in water buffalo anywhere in the world in PubMed, Science Direct, SpringerLink, China National Knowledge Infrastructure, Wanfang Database, and VIP Chinese Journal Databases. A total of 26 articles published from 1985 to 2020 met the final selection criteria. The overall seroprevalence of buffalo brucellosis worldwide was 9.7%. The seroprevalence before 2010 (20.8%) (95% CI: 5.6-42.2) was much higher than the seroprevalence rate from 2010 to 2020 (4.2%) (95% CI: 1.8-7.5). Subgroup analysis by feeding mode found that the point estimate of seroprevalence in stock buffalo (11.5%) (95% CI: 3.6-23.0) was higher than that in captive buffalo (10.6%) (95% CI: 4.9-18.1). Subgroup analysis by farming mode found that the seroprevalence was higher in captive-bred buffalo (10.7%) (95% CI: 6.6-15.7) than in intensively farmed buffalo (8.5) (95% CI: 0.9-22.2). The seroprevalence in buffalo living in dry lands (6.4%) (95% CI: 2.0-12.9) is greater than that in buffalo living in wetlands (5.1%) (95% CI: 1.8-10.4) (P < 0.05). The seroprevalence in female buffalo (10.1%) (95% CI: 3.4-19.7) was higher than that in male buffalo (4.4%) (95% CI: 2.0-7.4). The seroprevalence in lactating buffalo was higher than that in buffalo of other ages (26.9%) (95% CI: 1.8-66.5). Subgroup analysis by detection method found that the seroprevalence detected by the complement fixation test (27.3%) (95% CI: 0.7-70.8) was much higher than that detected by other methods. Conclusion: The results of this meta-analysis showed that buffalo brucellosis infection is very common in buffalo herds around the world. Although the seroprevalence of brucellosis in buffalo and humans is relatively low, serious effects upon animal husbandry and public health make it necessary to take effective control and preventive measures to control the spread of this disease.

Project description:BackgroundEthiopia is one of the tropical countries with a heavy burden of soil-transmitted helminths. As a result, the nation has been implementing mass drug administration, water, sanitation, and hygiene and health extension programs to control those parasites. Hence, updated data about the prevalence and trend of parasites over time has a pivotal role to assess the success of existing control programs.MethodsStudies conducted between 2000 and 2018 were searched from PubMed, Google Scholar, and local journals for systematic reviews and meta-analysis following the PRISMA guideline and checklists. Eligible studies were selected based on preset inclusion and exclusion criteria. The quality of the included studies was assessed using the Newcastle-Ottawa Scale in meta-analysis. Heterogeneity between studies was assessed using the Cochran Q test and I 2 test statistics based on the random effect model. Comprehensive meta-analysis (CMA 2.0) was used to calculate the pooled prevalence, and metaregression was run to assess the trend of parasite prevalence over time.ResultsThirty-eight studies recruiting 16,266 participants were included in the review. The pooled prevalence of intestinal parasites was 52.0% (95% CI: 44.4-59.5). Amhara region was with the highest prevalence (60.3%; 95% CI: 50.1-69.6). Among soil-transmitted helminths, Ascaris lumbricoides (11.2%; 95% CI: 8.4-14.8) was with the highest pooled prevalence followed by hookworms (10.4%; 95% CI: 7.9-13.7) and Trichuris trichiura (3.6%; 95% CI: 2.4-5.4). Metaregression analysis revealed that all soil-transmitted helminths did not show a significantly decreasing trend over time (p > 0.05).ConclusionDespite various control efforts having been made, soil-transmitted helminths are of high distribution, and their prevalence is not significantly decreasing in Ethiopia. Hence, other control approaches like community-led sanitation should be integrated with mass drug administration to achieve the national goal of soil-transmitted helminth elimination by 2025.

Project description:Toxoplasmosis ranks among the most prevalent parasitic diseases globally. It seems that chronic toxoplasmosis is associated with several neuropsychiatric and other harmful effects in infected people, therefore, there is a need to investigate the prevalence of toxoplasmosis across various world regions. In this study, we conducted a meticulous meta-analysis to estimate the seroprevalence of anti-Toxoplasma gondii IgG antibodies within the general population in Iran (GPI). International and national scientific databases for studies published between January 1, 2000, and September 30, 2023, were searched. Observational studies reporting anti-T. gondii IgG seroprevalence in the GPI was selected/included. The data were synthesized using a random-effects model to calculate with a 95% confidence interval (95% CI) the national and regional anti-T. gondii IgG seroprevalence rates in Iran. Additionally, subgroup analyses were conducted to investigate the frequency of exposition to T. gondii in different socio-demographic, climatic, and geographical scenarios. From 18661 identified studies, 327 were included in the present meta-analysis, encompassing 122,882 individuals across the 31 Iranian provinces. The pooled nationwide anti-T. gondii IgG seroprevalence among the GPI was determined to be 32.9% (95% CI: 30.9-35.1%). The highest anti-T. gondii IgG seroprevalence was observed in Mazandaran province (North of Iran) (61%), whereas the lowest was in Semnan province (12.5%).Anti-T. gondii IgG seroprevalence demonstrated a higher occurrence in provinces characterized by moderate temperatures of 16-21°C, high relative humidity, and annual precipitation. Additionally, a higher anti-T. gondii IgG seroprevalence was identified among individuals with a habit of consumption of undercooked meat, raw fruits or vegetables, and untreated water. Moreover, those reporting direct contact with cats, possessing a lower level of education, residing in rural areas, being engaged in farming occupations, or playing the role of housewives exhibited higher anti-T. gondii IgG seroprevalence figures.The anti-T. gondii IgG seroprevalence within GPI closely aligns with the estimated worldwide average exposition rates. This underscores the imperative for public health policymakers to prioritize educational efforts regarding toxoplasmosis transmission pathways and its link to harmful effects.
