Validity of a two-item screen for early psychosis.

ABSTRACT: Well-validated screening tools have been developed to identify people at high risk for psychosis, but these are rarely used outside of specialty clinics or research settings. The development of extremely brief and simple screening tools could increase dissemination, especially in settings with low buy-in such as those with low base rates of psychosis and/or time constraints. We sought to identify such a brief measure by modeling participant responses to three psychosis screening questionnaires (Prime Screen; Prodromal Questionnaire-Brief; Youth Psychosis At Risk Questionnaire) in a sample of 139 help-seeking individuals and 335 college students (age range: 12-25). Two screening questions with especially strong information characteristics were identified: "Do you see things that others can't or don't see?" and "Have you ever felt that someone was playing with your mind?" (Alternative two-item screens with similarly strong properties were also identified and validated using uncertainty quantified through Bayesian modeling.) The resulting measure was validated against clinician ratings of psychosis. The screen performed with a sensitivity of 53% and specificity 98% for clinically significant hallucinations or delusions, and sensitivity of 32% and specificity 99% for identifying people in an early phase of psychosis (clinical high risk or first episode psychosis).

Project description:BackgroundTo meet the surging demands for intubation and invasive ventilation as more COVID-19 patients begin their recovery, clinicians are challenged to find an ultra-brief and minimally invasive screen for postextubation dysphagia predicting feeding-tube dependence persisting for 72 h after extubation.MethodsThis study examined the predictive validity of a two-item swallowing screen on feeding-tube dependence over 72 h in patients following endotracheal extubation. Intensive-care-unit (ICU) patients (≥ 20 years) successfully extubated after ≥ 48 h endotracheal intubation were screened by trained nurses using the swallowing screen (comprising oral stereognosis and cough-reflex tests) 24 h postextubation. Feeding-tube dependence persisting for 72 h postextubation was abstracted from the medical record by an independent rater. To verify the results and cross-check whether the screen predicted penetration and/or aspiration during fiberoptic endoscopic evaluation of swallowing (FEES), participants agreeing to receive FEES were analyzed within 30 min of screening.ResultsThe results showed that 95/123 participants (77.2%) failed the screen, which predicted ICU patients' prolonged (> 72 h) feeding-tube dependence, yielding sensitivity of 0.83, specificity of 0.35, and accuracy of 0.68. Failed-screen participants had 2.96-fold higher odds of feeding-tube dependence (95% CI, 1.13-7.76). For the 38 participants receiving FEES, the swallowing screen had 0.89 sensitivity to detect feeding-tube dependence and 0.86 sensitivity to predict penetration/aspiration, although specificity had room for improvement (0.36 and 0.21, respectively).ConclusionThis ultra-brief swallowing screen is sufficiently sensitive to identify high-risk patients for feeding-tube dependence persisting over 72 h after extubation. Once identified, a further assessment and care are indicated to ensure the prompt return of patients' oral feeding.Trial registrationNCT03284892, registered on September 15, 2017.

Project description:The food frequency questionnaires (FFQs) are the most common tools used in dietary research. Each newly developed, modified, or adapted FFQ should be validated in the target population. The aim of this study was to assess the reproducibility and relative validity of the 72-item semi-quantitative food frequency questionnaire (72-item SQ-FFQ) for Polish adults. The 72-item SQ-FFQ was developed based on a non-quantitative FFQ covering 62 food items (62-item FFQ-6®). The study was conducted among 186 university students aged 19-26 years (47.8% of females). The FFQ was administered on two occasions (FFQcrude and FFQretest) to assess the test-retest reproducibility, and the FFQcrude was compared with the estimated food record (FRcrude) to evaluate the relative validity of the FFQ in assessing the intake of energy, 38 nutrients, and alcohol. The energy intake obtained with both methods was standardized to 2000 kcal/day (FFQstand, FRstand). The dietary intake obtained with FFQcrude was adjusted using linear regression analysis (FFQreg). The reproducibility and relative validity of the FFQ were assessed by comparing the mean values of energy and nutrient intake and using Spearman's correlation coefficient, the cross-classification analysis, and the Bland-Altman method. Spearman's correlation coefficient between both administrations of the FFQ ranged from 0.631 to 0.878 (the intraclass correlation coefficient: 0.583-0.935), for FFQcrude and FRcrude ranged from -0.025 to 0.390, for FFQstand and FRstand ranged from 0.021 to 0.546, and for FFQreg and FRcrude ranged from 0.028 to 0.391. The percentage of respondents classified into the same or adjacent quartiles of nutrient intake obtained from two administrations of the FFQ ranged from 84.9% to 97.8%, for FFQcrude and FRcrude ranged from 61.3% to 76.9%, for FFQstand and FRstand ranged from 63.4% to 83.9%, and for FFQreg and FRcrude ranged from 60.2% to 76.9%. In conclusion, our findings showed good reproducibility and acceptable relative validity of the 72-item SQ-FFQ. This tool can be recommended for assessing dietary intake among Polish young adults. However, the intake of certain nutrients should be interpreted with caution.

Project description:ObjectiveTo establish content validity of a single-item, migraine-specific symptom severity questionnaire for completion by migraine patients, key family members (KFMs) of migraine patients, and Healthcare Professionals (HCPs) who treat migraine patients.BackgroundMigraine is a common disabling primary headache disorder with high prevalence and significant socioeconomic burden and personal impacts. There is a need for a global assessment of migraine symptom severity to evaluate potential new therapies from multiple perspectives.MethodsThe migraine Global Impression Item (mGI-I) was drafted and tested in a non-interventional, qualitative study comprising telephone interviews with 15 migraine patients, 15 KFMs of migraine patients, and 15 migraine treating HCPs. The mGI-I was drafted with two different item stem options and two different response scale options to ask about the patient's migraine from the perspective of each respondent. Cognitive interviews were conducted to test comprehensiveness, clarity and ease of completion of the different versions of the mGI-I iteratively in three sequential waves of respondents.ResultsRevisions were made to the draft mGI-I after Wave 1 and Wave 2 of the interviews. Changes were made to simplify the item stem (removing unnecessary text), make language more patient-friendly (e.g. use of "migraine attack"), and add clarity to the item stem for consistent interpretation (include descriptive language of migraine attacks). Across both waves there was a preference for a 5-point response scale compared to a 7-point scale. In Wave 3, all respondents found the revised instructions, item stem, and 5-point response scale comprehensive, easy to understand and to answer. No further changes to the mGI-I were made after Wave 3.ConclusionsThis qualitative study of 45 total respondents across 3 subpopulations, established the content validity and appropriateness of the mGI-I in migraine patients, KFMs, and migraine-treating HCPs. The study specifically confirmed that the mGI-I is comprehensive, easily understood and answered for each respondent population.
