Project description:BackgroundOver the past three decades, allocation of foreign currency subsidies has been the primary strategy of various administrations in Iran to improve access to medicines. This strategy has resulted in several challenges, including stakeholder conflicts of interest.ObjectiveTo identify the power, interest, and role of the stakeholders in allocating foreign currency subsidies to medicines in the Iranian health system.MethodsIn this qualitative study, 39 semi-structured interviews were conducted. Key informants were recruited using a purposive sampling technique. The theoretical framework adopted by Varvasovszky and Brugha was employed. The data were analysed using directed content analysis.ResultsThe foreign currency subsidy for medicines included 21 stakeholders in five main categories: governmental organizations, Iranian Parliament, general population, nongovernmental organizations (NGOs), and the pharmaceutical industry. Stakeholders varied in their level of participation and support in the policy-making process. Among them, the Iranian Government, Planning and Budget Organization, the Ministry of Health and Medical Education (MoHME), and Iran Food and Drug Administration (IFDA) were the most important stakeholders, with highly supportive positions, while domestic drug manufacturers were the strongest opponents of this policy. The Government of Iran is the most powerful institution with regard to the ability to allocate foreign currency subsidies to medicines, followed by the MoHME and the IFDA.ConclusionThis study demonstrated that identifying and analysing the stakeholders involved in allocating foreign currency subsidies to medicines can provide valuable information for policy-makers to enable a more comprehensive understanding and better capacity to determine whether or not to eliminate these subsidies. Moreover, decision-making in this process is a long-term issue that requires consensus among all stakeholders. Because of the political and social consequences of eliminating foreign currency subsidies, the necessary political will is not institutionalized. We recommend a step-by-step approach in eliminating foreign currency subsidies if the requirements are met (i.e., those related to the consequences of such interventions). Therefore, revision of the current policy along with these requirements, in addition to financial transparency and enhanced efficiency, will facilitate progress towards achieving the Sustainable Development Goals by improving access to medicines.

Project description:Intensive cropping systems with diverse cropping patterns present a challenge for nutrient management on smallholder farms. Government-endorsed recommendations for crop fertilizer use are designed to assist farmers achieve profitable and balanced nutrient inputs, but it is unclear how closely farmers follow these recommendations. We identified farmers' current nutrient use gaps (overuse or underuse) relative to the Government-endorsed recommended nutrient doses in two cropping patterns in three representative Agro-ecological Zones of Bangladesh. A total of 330 farms were surveyed in 2019 from three farm size categories (referred to as large-, medium and small-scale) and their gaps in nutrient use were assessed relative to Government-endorsed Fertilizer Recommendation Guides (FRG) published in 2012 and in 2018: FRG-2012 and FRG-2018. The large- and medium-scale farms used 11-16%, 80-90% and 21-30%, respectively, over-doses of NPK in the cropping season under fully rice-based cropping pattern relative to FRG-2012 recommendations, while the over-dose levels were much lower for small-scale farms. Small-scale farms used much less than recommended S, Mg, Zn, B and organic manure (OM) rates relative to FRG-2012. The FRG-2018, which increased N and K recommendations but decreased the P recommendation for rice crops relative to FRG-2012, shows that all farms can decrease the dose of P (by 50%) while small-scale farms need to increase the dose of N (7%), K (16%), S (20%) and to apply Zn in the fully rice-based cropping pattern. On the other hand, the farms could greatly reduce NPK (19%, 86% and 44%, respectively) use while increasing S (14%), Mg, Zn, B and OM use relative to FRG-2018 in the pattern with the high-value potato crop. To increase crop profitability, enhance food security and save Government treasury in the Eastern Gangetic Plain enabling approaches are needed to effectively communicate the benefits of balanced nutrient use practices to farmers.

Project description:Protein stability is affected at different hierarchies - gene, RNA, amino acid sequence and structure. Gene is the first level which contributes via varying codon compositions. Codon selectivity of an organism differs with normal and extremophilic milieu. The present work attempts at detailing the codon usage pattern of six extremophilic classes and their harmony. Homologous gene datasets of thermophile-mesophile, psychrophile-mesophile, thermophile-psychrophile, acidophile-alkaliphile, halophile-nonhalophile and barophile-nonbarophile were analysed for filtering statistically significant attributes. Relative abundance analysis, 1-9 scale ranking, nucleotide compositions, attribute weighting and machine learning algorithms were employed to arrive at findings. AGG in thermophiles and barophiles, CAA in mesophiles and psychrophiles, TGG in acidophiles, GAG in alkaliphiles and GAC in halophiles had highest preference. Preference of GC-rich and G/C-ending codons were observed in halophiles and barophiles whereas, a decreasing trend was reflected in psychrophiles and alkaliphiles. GC-rich codons were found to decrease and G/C-ending codons increased in thermophiles whereas, acidophiles showed equal contents of GC-rich and G/C-ending codons. Codon usage patterns exhibited harmony among different extremophiles and has been detailed. However, the codon attribute preferences and their selectivity of extremophiles varied in comparison to non-extremophiles. The finding can be instrumental in codon optimization application for heterologous expression of extremophilic proteins.

Project description:Designing and implementing effective new energy vehicle (NEV) policy are policy priorities for policymakers and energy policy scholars. However, the formulation, adoption, and diffusion of the NEV policies have not been fully examined in the extant literature. This article explores the mechanisms driving the diffusion of local financial subsidy policy for NEVs in China. In this context, we aim at analyzing the factors affecting the diffusion of local financial subsidies for NEVs in cities, to explain why some cities have taken the lead in adopting local financial subsidy policies for NEVs, while other cities have lagged behind. Based on a data set of 286 cities in China from 2009 to 2016, and with event history analysis (EHA) to analyze the strategic behaviors of local governments, we found that the number of the city's neighbors that have adopted the NEV policy, the financial incentive policy of the provincial government, the administrative ranking of the city, the city's financial situation and innovation capacity have a direct impact on whether the city adopts a local financial subsidy policy for NEVs. This study has practical implications for policymakers in designing and promoting the spread of NEV policies.

Project description:BACKGROUND:Retaining doctors in rural areas is a challenge in Bangladesh. In this study, we analyzed three rural retention policies: career development programs, compulsory services, and schools outside major cities - in terms of context, contents, actors, and processes. METHODS:Series of group discussions between policy-makers and researchers prompted the selection of policy areas, which were analyzed using the policy triangle framework. We conducted document and literature reviews (1971-2013), key informant interviews (KIIs) with relevant policy elites (n=11), and stakeholder analysis/position-mapping. RESULTS:In policy-1, we found, applicants with relevant expertise were not leveraged in recruitment, promotions were often late and contingent on post-graduation. Career tracks were porous and unplanned: people without necessary expertise or experience were deployed to high positions by lateral migration from unrelated career tracks or ministries, as opposed to vertical promotion. Promotions were often politically motivated. In policy-2, females were not ensured to stay with their spouse in rural areas, health bureaucrats working at district and sub-district levels relaxed their monitoring for personal gain or political pressure. Impractical rural posts were allegedly created to graft money from applicants in exchange for recruitment assurance. Compulsory service was often waived for political affiliates. In policy-3, we found an absence of clear policy documents obligating establishment of medical colleges in rural areas. These were established based on political consideration (public sector) or profit motives (private sector). CONCLUSION:Four cross-cutting themes were identified: lack of proper systems or policies, vested interest or corruption, undue political influence, and imbalanced power and position of some stakeholders. Based on findings, we recommend, in policy-1, applicants with relevant expertise to be recruited; recruitment should be quick, customized, and transparent; career tracks (General Health Service, Medical Teaching, Health Administration) must be clearly defined, distinct, and respected. In policy-2, facilities must be ensured prior to postings, female doctors should be prioritized to stay with the spouse, field bureaucrats should receive non-practising allowance in exchange of strict monitoring, and no political interference in compulsory service is assured. In policy-3, specific policy guidelines should be developed to establish rural medical colleges. Political commitment is a key to rural retention of doctors.

Project description:BackgroundThere are few Australian studies showing how research evidence is used to inform the development of public health policy. International research has shown that compensation for injury rehabilitation can have negative impacts on health outcomes. This study examined transport injury compensation policy in the Australian state of Victoria to: determine type and purpose of reference to information sources; and to identify the extent of reference to academic research evidence in transport related injury rehabilitation compensation policy.MethodsQuantitative content analysis of injury rehabilitation compensation policies (N?=?128) from the Victorian state government transport accident compensation authority.ResultsThe most commonly referenced types of information were Internal Policy (median?=?6 references per policy), Clinical/Medical (2.5), and Internal Legislation (1). Academic Research Evidence was the least often referenced source of information. The main purpose of reference to information was to support injury treatment and rehabilitation compensation claims decision-making.ConclusionsTransport injury compensation policy development is complex; with multiple sources of information cited including legislation, internal policy, external policy and clinical/medical evidence. There is limited use of academic research evidence in Victorian state government injury treatment and rehabilitation compensation policies. Decisions regarding compensation for injury treatment and rehabilitation services could benefit from greater use of academic research evidence. This study is one of the first to examine the use of research evidence in existing Australian public health policy decision-making using rigorous quantitative methods. It provides a practical example of how use of research evidence in public health policy can be objectively measured.

Project description:BackgroundA national subsidy policy was introduced in 2007 in Burkina Faso to improve financial accessibility to facility-based delivery. In this article, we estimated the effects of reducing user fees on institutional delivery and neonatal mortality, immediately and three years after the introduction of the policy.MethodsThe study was based on a quasi-experimental design. We used data obtained from the 2010 Demographic and Health Survey, including survival information for 32,102 live-born infants born to 12,474 women. We used a multilevel Poisson regression model with robust variances to control for secular trends in outcomes between the period before the introduction of the policy (1 January, 2007) and the period after. In sensitivity analyses, we used two different models according to the different definitions of the period "before" and the period "after".ResultsImmediately following its introduction, the subsidy policy was associated with increases in institutional deliveries by 4% (RR = 1.04, 95% CI: 0.98-1.10) in urban areas and by 12% (RR = 1.12, 95% CI: 1.04-1.20) in rural areas. The results showed similar patterns in sensitivity analyses. This effect was particularly marked among rural clusters with low institutional delivery rates at baseline (RR = 1.44, 95% CI: 1.33-1.55). It was persistent for 42 months after the introduction of the policy but these increases were not statistically significant. At 42 months, the delivery rates had increased by 26% in rural areas (RR = 1.26; 95% CI: 0.86-1.86) and 13% (RR = 1.13; 95% CI: 0.88-1.46) in urban areas. There was no evidence of a significant decrease in neonatal mortality rates.ConclusionThe delivery subsidy implemented in Burkina Faso is associated with short-term increases in health facility deliveries. This policy has been particularly beneficial for rural households.

Project description:ObjectiveThis study determined the optimal ratio of whole plant corn silage (WPCS) to corn stover (stems+leaves) silage (CSS) (WPCS:CSS) to reach the greatest profit of dairy farmers and evaluated its consequences with corn available for other purposes, enteric methane production and milk nitrogen efficiency (MNE) at varying milk production levels.MethodsAn optimization model was developed. Chemical composition, rumen undegradable protein and metabolizable energy (ME) of WPCS and CSS from 4 cultivars were determined to provide data for the model.ResultsAt production levels of 0, 10, 20, and 30 kg milk/cow/d, the WPCS:CSS to maximize the profit of dairy farmers was 16:84, 22:78, 44:56, and 88:12, respectively, and the land area needed to grow corn plants was 4.5, 31.4, 33.4, and 30.3 ha, respectively. The amount of corn available (ton DM/ha/yr) for other purposes saved from this land area decreased with higher producing cows. However, compared with high producing cows (30 kg/d milk), more low producing cows (10 kg/d milk) and more land area to grow corn and soybeans was needed to produce the same total amount of milk. Extra land is available to grow corn for a higher milk production, leading to more corn available for other purposes. Increasing ME content of CSS decreased the land area needed, increased the profit of dairy farms and provided more corn available for other purposes. At the optimal WPCS:CSS, MNE and enteric methane production was greater, but methane production per kg milk was lower, for high producing cows.ConclusionThe WPCS:CSS to maximize the profit for dairy farms increases with decreased milk production levels. At a fixed total amount of milk being produced, high producing cows increase corn available for other purposes. At the optimal WPCS:CSS, methane emission intensity is smaller and MNE is greater for high producing cows.

Project description:Sustainable agriculture is an important global issue. The use of organic fertilizers can enhance crop yield and soil properties while restraining pests and diseases. The objective of this study was to assess the effects of long-term use of chemical and organic fertilizers on tea and rhizosphere soil properties in tea orchards. Inductively coupled plasma mass spectrometry (ICP-MS) and high-throughput sequencing technology analyses were used to investigate heavy metals content and bacterial composition in rhizosphere soils. Our results indicated that organic fertilizer treatment significantly decreased Cu, Pb and Cd contents in rhizosphere soil sample. The results also showed that treatment with organic fertilizer significantly decreased the contents of Cd, Pb and As in tea leaves. Furthermore, organic fertilizer significantly increased the amino acids content of tea and the pH of the soil. The use of organic fertilizer significantly increased in the relative abundance of Burkholderiales, Myxococcales, Streptomycetales, Nitrospirales, Ktedonobacterales, Acidobacteriales, Gemmatimonadales, and Solibacterales, and decreased the abundance of Pseudonocardiales, Frankiales, Rhizobiales, and Xanthomonadales. In conclusion, organic fertilizer can help to shape the microbial composition and recruit beneficial bacteria into the rhizosphere of tea, leading to improved tea quality and reduced heavy metals content in rhizosphere soil and tea leaves.
