Project description:BackgroundUnder-five year children are the most vulnerable group affected by malaria, they accounted for 61% of all malaria deaths worldwide. Sherkole refugee camp is stratified under high risk for malaria. Knowledge on malaria prevalence and associated factors among under-five children in Sherkole refugee camp is lacking.Methods and materialsInstitution-based cross-sectional survey was conducted among under-five children in Sherkole refugee camp from October to November 2019. Total sample size was 356. Stratified random sampling technique was employed to select the study participants. Standardized questionnaire was used to collect data. Care StartTM Malaria Rapid diagnostic test which detect histidine-rich protein 2 of P. falciparum and plasmodium lactate dehydrogenase of P. vivax was used to diagnose malaria. Bivariate and multivariable logistic regression analysis was done to identify factors associated with malaria.ResultsA total of 356 participants were included in this study with response rate of 97.5%. The prevalence malaria was 3.9% (95% CI = 2.0-6.2). Outdoor stay at night (AOR = 3.9, 95% CI = 1.14-13.8), stagnant water near to house (AOR = 4.0, 95% CI = 1.14-14.6), and the number of under-five children per household (AOR = 3.0, 95% CI = 1.03-13.0) were found to increase the odds of getting malaria. Whereas, insecticide treated net (ITN)utilization (AOR = 0.22, 95% CI = 0.06-0.61) and Health information about malaria (AOR = 0.29, 95% CI = 0.06-0.65) reduce the odds of getting malaria.Conclusions and recommendationsMalaria remains the major public health problem in Sherkole Refugee camp. Outdoor stay at night, stagnant water near to house, and number of under-five children per household were the risk factors for malaria. Health information dissemination that focuses on avoiding outdoor stay at night, eliminating stagnant water & using ITN, and considering number of under-five children per household during ITN distribution should be take into account.

Project description:BackgroundDiarrhea is second only to pneumonia as the cause of child mortality worldwide. Developing countries particularly in Sub Saharan Africa including Ethiopia have a high burden of this disease. Studies showed that different factors were associated with the occurrence of childhood diarrhea. Therefore, this study was aimed to identify determinant factors of diarrhea in underfive children in Benishangul Gumuz Regional State, western Ethiopia.MethodDemographic and Health Survey (DHS) data of 2011 was used for this study. The data was extracted from the National DHS data using data extraction tools. A total of 925 under five children were selected. The logistic regression model was employed to examine the determinants of childhood diarrhoea. Both bivariate and multivariate data analysis was performed using SPSS version 16.0.ResultThe results of this study indicated that low level of maternal education [AOR = 1.81, 95% CI (1.12,2.76)], absence of toilet facility [AOR = 3.5, 95% CI (2.4, 5.2)], improper child stool disposal methods [AOR = 2.05, 95%CI (1.36, 3.10)], having more than two under five children [AOR = 1.73, 95% CI (1.03, 2.93)], higher birth order [AOR = 6.1, 95% CI (3.1,12.2)] and the age of children [AOR = 1.9, 95% CI (1.2, 3.6)] were found to be the risk factors for childhood diarrhea after adjusting for other variables. When toilet facility was stratified by maternal education, it showed that children of mothers who had no education were the most vulnerable in the absence of toilet facilities [OR = 9.16, 95% CI (5.79, 14.48)].ConclusionUnder poor environmental conditions, mothers with primary education and above protected their children against diarrhea better than mothers with no education. Thus, implementing effective educational programs that emphasize environmental health and sanitation practices and encouraging female school enrolment would reduce childhood diarrheal morbidity in the region.

Project description:INTRODUCTION:Tuberculosis (TB) remains a major global public health problem and is the leading cause of death from a single bacterium, Mycobacterium tuberculosis (MTB) complex. The emergence and spread of drug-resistant strains aggravate the problem, especially in tuberculosis high burden countries such as Ethiopia. The supposedly high initial cost of laboratory diagnosis coupled with scarce financial resources has limited collection of information about drug resistance patterns and circulating strains in peripheral and emerging regions of Ethiopia. Here, we investigated drug susceptibility and genetic diversity of mycobacterial isolates among pulmonary tuberculosis patients in the Benishangul Gumuz region and its surroundings in northwest Ethiopia. METHODS AND MATERIAL:In a cross-sectional study, 107 consecutive sputum smear-positive pulmonary tuberculosis (PTB) patients diagnosed at two hospitals and seven health centers were enrolled between October 2013 and June 2014. Sputum samples were cultured at Armauer Hansen Research Institute (AHRI) TB laboratory, and drug susceptibility testing (DST) was performed against Isoniazid, Rifampicin, Ethambutol, and Streptomycin using the indirect proportion method. Isolates were characterized using polymerase chain reaction (PCR)based Region of Difference 9 (RD9) testing and spoligotyping. Statistical analysis was performed using Statistical Package for the Social Sciences (SPSS) for Windows version 24.0. RESULTS:Of 107 acid-fast-bacilli (AFB) smear-positive sputum samples collected, 81.3% (87/107) were culture positive. A PCR based RD9 testing revealed that all the 87 isolates were M. tuberculosis. Of these isolates, 16.1% (14/87) resistance to one or more drugs was observed. Isoniazid monoresistance occurred in 6.9% (6/87). Multidrug resistance (MDR) was observed in two isolates (2.3%), one of which was resistant to all the four drugs tested. Spoligotyping revealed that the majority, 61.3% (46/75) of strains could be grouped into ten spoligotype patterns containing two to 11 isolates each while the remaining 38.7% (29/75) were unique. SIT289 (11 isolates) and SIT53 (nine isolates) constituted 43.5% (20/46) among clustered isolates while 29.3% (22/75) were ''New" to the database. The dominant families were T, 37% (28/75), CAS, 16.0% (12/75), and H, 8% (6/75), adding up to 51.3% (46/75) of all isolates identified. CONCLUSION AND RECOMMENDATIONS:The current study indicates a moderate prevalence of MDR TB. However, the observed high monoresistance to Isoniazid, one of the two proxy drugs for MDR-TB, reveals the hidden potential threat fora sudden increase in MDR-TB if resistance to Rifampicin would increase. Clustered spoligotype patterns suggest ongoing active tuberculosis transmission in the area. The results underscore the need for enhanced monitoring of TB drug resistance and epidemiological studies in this and other peripheral regions of the country using robust molecular tools with high discriminatory power such as the Mycobacterial Interspersed Repetitive Units -Variable Number of Tandem Repeats (MIRU-VNTR) typing and whole-genome sequencing (WGS).

Project description:BackgroundIntegrated Community Case Management (iCCM) is a strategy for promoting access of under-served populations to lifesaving treatments through extending case management of common childhood illnesses to trained frontline health workers. In Ethiopia iCCM is provided by health extension workers (HEWs) deployed at health posts. We evaluated the association between the implementation of iCCM program in Assosa Zuria zone, Benishangul Gumuz region and changes in care-seeking for common childhood illnesses.MethodsWe conducted a pre-post study without control arm to evaluate the association of interest. The iCCM program that incorporated training, mentoring and supportive supervision of HEWs with community-based demand creation activities was implemented for two years (2017-18). Baseline, midline and endline surveys were completed approximately one year apart. Across the surveys, children aged 2-59 months (n = 1,848) who recently had cough, fever or diarrhea were included. Data were analysed using mixed-effects logistic regression model.ResultsOver the two-year period, care-seeking from any health facility and from health posts significantly increased by 10.7 and 17.4 percentage points (PP) from baseline levels of 64.5 and 34.1%, respectively (p<0.001). Care sought from health centres (p = 0.420) and public hospitals (p = 0.129) did not meaningfully change while proportion of caregivers who approached private (p = 0.003) and informal providers (p<0.001) declined. Caregivers who visited health posts for the treatment of diarrhea (19.2 PP, p<0.001), fever (15.5 PP, p<0.001), cough (17.8 PP, p<0.001) and cough with respiratory difficulty (17.3 PP, p = 0.038) significantly increased. After accounting for extraneous variables, we observed that care-seeking from iCCM providers was almost doubled (adjusted odds ratio = 2.32: 95% confidence interval; 1.88-2.86) over the period.ConclusioniCCM implementation was associated with a meaningful shift in care-seeking to health posts.

Project description:BACKGROUND:Diarrhea is responsible for 525,000 children under-five deaths and 1.7 billion cases globally and is the second leading cause of death among children under-five every year. It is a major public health problem in low income countries like Ethiopia. The main aim of this study was to assess the prevalence of diarrhea and associated risk factors among children under-five in Debre Berhan Town, Ethiopia. METHODS:A community-based cross-sectional study was conducted in 420 parent or caretaker/children pairs in Debre Berhan town between 13 and 18 April 2018. A multi-stage sampling strategy was used to select the study participants. Data were collected using pre-tested and structured questionnaires. Data were entered in Epi-info computer software version 3.5.1 and exported to SPSS Window Version-16 for analysis. Adjusted odds ratio with 95% confidence intervals were used to assess the level of significance. RESULTS:The two week prevalence of diarrhea among children under-five was 16.4% (69/351). Children aged 7-11 months (adjusted odds ratio (AOR): 4.2, 95% confidence interval (CI): 1.2-15.3), being the second-born child (AOR: 3.9, 95%CI: 1.8-8.5), not vaccinated against rotavirus (AOR: 10.3, 95%CI: 3.2-91.3) and feeding children by hand (AOR: 2.5, 95%CI: 1.1-6.1) were significant predictors of diarrhea. CONCLUSIONS:This study revealed that the two weeks period prevalence of diarrhea among children under-five years was 16.4%. Education programs on the importance of vaccination against rotavirus, increasing breast feeding frequency with complementary food after six months and the critical points of hand washing are recommended.

Project description:IntroductionFollowing global efforts to increase antiretroviral therapy (ART) access and coverage, Ethiopia has made significant achievement with a 6.3% annual decline in the HIV/AIDS incidence rate between 1990 and 2016. Such success depends not only on access to ART but also on attaining optimum treatment adherence. Emerging studies in Ethiopia has shown the increasing prevalence of poor adherence and lack of the desired viral suppression, but the extent and factors associated with non-adherence to ART are not well known, especially in the current study setup. In this study, we examined the magnitude and factors associated with treatment and non-adherence to ART among people living with HIV in Benishangul-Gumuz Regional State, northwest Ethiopia.MethodsAn institutional facility based cross-sectional descriptive study was carried out among adult people living with HIV/AIDS from mid-December 2016 to February 2017 with only 98.9% response rate. Sociodemographic factors (age, gender, marital status and residential area), economic factors (educational status, income, asset possession, employment status, dietary diversity, nutritional status and food security), and clinical characteristics (CD4 count, duration on ART and history of opportunistic infections) were explanatory variables. ART non-adherence was measured using a visual analogue scale (VAS). We used binary logistic regression and subsequent multivariate logistic regression analysis to determine the factors associated with ART non-adherence.ResultOverall, 39.7% of the participants were found non-adherent to ART. Strong association was found between non-adherence to ART and young age below 25 years (AOR: 4.30, 95% CI [1.39-3.35]; p = 0.011), urban residential area (AOR: 2.78, CI [1.23-7.09], p = 0.043), lack of employment (AOR: 1.75, 95% CI [1.05-2.91], p = 0.032), food insecurity (AOR: 2.67, 95% CI [7.59-8.97]; p < 0.0001), malnutrition (AOR: 1.55, 95% CI [1.94-2.56]; p = 0.027) and opportunistic infections (AOR: 1.81, 95% CI [1.11-2.97]; p = 0.018).ConclusionThe prevalence of non-adherence to ART in this study was high. Sociodemographic and economic factors such as young age of below 25 years, urban residential area, lack of employment, food insecurity, malnutrition and opportunistic infections were among the factors associated with non-adherence to ART.

Project description:BackgroundDespite remarkable progress in the reduction of under-five mortality, childhood diarrhea is still the leading cause of mortality and morbidity in this highly susceptible and vulnerable population. In Ethiopia, study findings regarding prevalence and determinants of diarrhea amongst under-five children have been inconsistent. Therefore, this systematic review and meta-analysis estimates the pooled prevalence of diarrhea and its determinants among under-five children in Ethiopia.MethodsInternational databases, including PubMed, Web of Science, EMBASE, CINAHL, Google Scholar, Science Direct, and the Cochrane Library, were systematically searched. All identified observational studies reporting the prevalence and determinants of diarrhea among under-five children in Ethiopia were included. Two authors independently extracted all necessary data using a standardized data extraction format. STATA Version 13 statistical software was used. The Cochrane Q test statistics and I2 test were used to assess the heterogeneity of the studies. A random effects model was computed to estimate the pooled prevalence of diarrhea. Moreover, the associations between determinant factors and childhood diarrhea were examined using the random effect model.ResultsAfter reviewing of 535 studies, 31studies fulfilled the inclusion criteria and were included in the meta-analysis. The findings from the 31 studies revealed that the pooled prevalence of diarrhea among under-five children in Ethiopia was 22% (95%CI: 19, 25%). Subgroup analysis of this study revealed that the highest prevalence was observed in Afar region (27%), followed by Somali and Dire Dawa regions (26%), then Addis Abeba (24%). Lack of maternal education (OR: 2.5, 95% CI: 1.3, 2.1), lack of availability of latrine (OR: 2.0, 95%CI: 1.3, 3.2), urban residence (OR: 1.9, 95%CI: 1.2, 3.0), and maternal hand washing (OR: 2.2, 95%CI: 2.0, 2.6) were significantly associated with childhood diarrhea.ConclusionIn this study, diarrhea among under-five children in Ethiopia was significantly high. Lack of maternal education, lack of availability of latrine, urban residence, and lack of maternal hand washing were significantly associated with childhood diarrhea.

Project description:BACKGROUND:Menge District has long been inhabited by people who have a long tradition of using wild mushrooms mainly as food, source of income, and medicine. Extensive utilization of wild edible mushrooms (WEM) coupled with an ever-increasing population growth, deforestation, and agricultural land expansion threatens fungal diversity and WEM in the area. Hence, this study is aimed at documenting and analyzing the ethnomycological knowledge of the people in order to preserve the dwindling WEM wealth and associated indigenous knowledge. METHODS:Ethnomycological data were collected using semi-structured interviews, focus group discussions, participant observations, and walk-in-the-woods methods. Statistical tests were used to compare the indigenous knowledge and practice of wild mushroom among different informant categories using One-way ANOVA and t tests. RESULTS:A total of 20 ethnomycologically important wild mushroom species belonging to ten genera and six families were identified, of which 15 were reported to be edible in the District. The family Lyophyllaceae was represented by the highest number of species (nine species, 45%) followed by Agaricaceae (seven species, 35%) and each of the remaining four families had single species representation. Significant difference (P < 0.05) was observed on the mean number of WEM reported among different group of respondents. Wild edible mushroom collection habit and practice was significantly (P < 0.05) influenced by gender, age, and literacy level. The output of preference ranking exercise indicated Termitomyces schimperi was ranked first followed by Termitomyces letestui, Termitomyces microcarpus, and Termitomyces eurhizusas as the most preferred edible mushrooms respectively. CONCLUSION:The present study shows that Menge District is rich in wild mushroom diversity and associated indigenous knowledge. However, anthropogenic factors together with loss of indigenous knowledge and very poor conservation efforts threaten the survival of economically and ecologically important mushrooms in the area. Thus, complementary in situ and ex situ mushroom conservation strategy is highly recommended.

Project description:Although diarrheal mortality is cheaply preventable with oral rehydration therapy (ORT), over 700,000 children die of diarrhea annually and many health providers fail to treat diarrheal cases with ORT. Provision of ORT may differ between for-profit and public providers. This study used Demographic and Health Survey data from 19,059 children across 29 countries in sub-Saharan Africa from 2003 to 2011 to measure differences in child diarrhea treatment between private for-profit and public health providers. Differences in treatment provision were estimated using probit regression models controlling for key confounders. For-profit providers were 15% points less likely to provide ORT (95% confidence interval [CI] 13-17) than public providers and 12% points more likely to provide other treatments (95% CI 10-15). These disparities in ORT provision were more pronounced for poorer children in rural areas. As private healthcare in sub-Saharan Africa continues to expand, interventions to increase private sector provision of ORT should be explored.

Project description:BackgroundDiarrheal disease is still one of the most common causes of mortality and morbidity in children under five in developing countries, including Ethiopia. Lack of specific data on the prevalence of acute diarrhea and associated factors among under-five children in the semi-urban areas of Gelsha, found in northeastern Ethiopia's South Wollo zone, remains a major gap. Therefore, this study was designed to provide data that is important for proper planning of intervention measures to reduce the problem in this area.MethodsA community-based cross-sectional study was conducted among 340 systematically selected children under five in semi-urban areas of Gelsha from January to March 2019. The data was collected using a structured questionnaire and an observational checklist. Bivariable (crude odds ratio [COR]) and multivariable analysis (adjusted odds ratio [AOR]) were employed using binary logistic regression model with 95% CI (confidence interval). Variables with a p-value < 0.05 from the multivariable analysis were declared as factors significantly associated with acute diarrhea.ResultThe prevalence of acute diarrhea among children under five in the study area was 11% (95%CI: 7.8-14.3%). About two-thirds (63.60%) of study participants used water from improved sources. About half (54.90%) of study participants practiced poor handwashing and 45.10% practiced good handwashing. We found that factors significantly associated with acute diarrhea were a child's age of 12-23 months (AOR = 4.68, 95% CI: 1.45-1.50), the presence of two or more under-five children in the house (AOR = 2.84, 95% CI: 1.19-6.81), unimproved water sources (AOR = 2.97, 95% CI: 1.28-6.87) and presence of feces around the pit hole/slab/floor of the latrine (AOR = 3.34, 95% CI: 1.34-8.31).ConclusionThe prevalence of acute diarrhea among children under five was relatively high. To reduce the problem, various prevention strategies are essential, such as the provision of health education to mothers/caregivers that focuses on keeping sanitation facilities clean and child care, and construction of improved water sources. Furthermore, implementing a strong health extension program, advocating an open defecation-free environment, and practicing a community-led total sanitation and hygiene approach might be helpful to sustainably reduce childhood diarrhea.
