Reporting of Baseline Characteristics to Accompany Analysis by Instrumental Variables.

ABSTRACT: A property and criticism of instrumental variable (IV) estimators is that they estimate the exposure effect in the compliers, individuals whose exposure is influenced by the instrument. It is conceivable that the exposure effect is different in individuals whose exposure is not influenced by the instrument. For that reason, it is useful to know who the compliers are. We present methods for reporting averages and other statistics and comparing them between the compliers and noncompliers, which are applicable to scenarios in which the instrument and exposure are fixed in time and dichotomous. The methods are illustrated in a comparison of outcomes between teaching and nonteaching hospitals in patients undergoing brain or spinal surgery.

Project description:The use of genetic markers as instrumental variables (IV) is receiving increasing attention from economists, statisticians, epidemiologists and social scientists. Although IV is commonly used in economics, the appropriate conditions for the use of genetic variants as instruments have not been well defined. The increasing availability of biomedical data, however, makes understanding of these conditions crucial to the successful use of genotypes as instruments. We combine the econometric IV literature with that from genetic epidemiology, and discuss the biological conditions and IV assumptions within the statistical potential outcomes framework. We review this in the context of two illustrative applications.

Does breastfeeding duration decrease child obesity? An instrumental variables analysis.

Project description:BackgroundMany studies have documented that breastfeeding is associated with a significant reduction in child obesity risk. However, a persistent problem in this literature is that unobservable confounders may drive the correlations between breastfeeding behaviors and child weight outcomes.ObjectiveThis study examines the effect of breastfeeding practices on child weight outcomes at age 2.MethodsThis study relied on population-based data for all births in Oregon in 2009 followed for two years. We used instrumental variables methods to exploit variations in breastfeeding by mothers immediately after delivery and the degree to which hospitals encouraged mothers to breastfeed in order to isolate the effect of breastfeeding practices on child weight outcomes.ResultsWe found that for every extra week that the child was breastfed, the likelihood of the child being obese at age 2 declined by 0.82% [95% CI -1.8% to 0.1%]. Likewise, for every extra week that the child was exclusively breastfed, the likelihood of being obese declined by 0.66% [95% CI -1.4 to 0.06%]. While the magnitudes of effects were modest and marginally significant, the results were robust in a variety of specifications.ConclusionThe results suggest that hospital practices that support breastfeeding may influence childhood weight outcomes.

Robust best linear estimation for regression analysis using surrogate and instrumental variables.

Project description:We investigate methods for regression analysis when covariates are measured with errors. In a subset of the whole cohort, a surrogate variable is available for the true unobserved exposure variable. The surrogate variable satisfies the classical measurement error model, but it may not have repeated measurements. In addition to the surrogate variables that are available among the subjects in the calibration sample, we assume that there is an instrumental variable (IV) that is available for all study subjects. An IV is correlated with the unobserved true exposure variable and hence can be useful in the estimation of the regression coefficients. We propose a robust best linear estimator that uses all the available data, which is the most efficient among a class of consistent estimators. The proposed estimator is shown to be consistent and asymptotically normal under very weak distributional assumptions. For Poisson or linear regression, the proposed estimator is consistent even if the measurement error from the surrogate or IV is heteroscedastic. Finite-sample performance of the proposed estimator is examined and compared with other estimators via intensive simulation studies. The proposed method and other methods are applied to a bladder cancer case-control study.

Bayesian methods for meta-analysis of causal relationships estimated using genetic instrumental variables.

Project description:Genetic markers can be used as instrumental variables, in an analogous way to randomization in a clinical trial, to estimate the causal relationship between a phenotype and an outcome variable. Our purpose is to extend the existing methods for such Mendelian randomization studies to the context of multiple genetic markers measured in multiple studies, based on the analysis of individual participant data. First, for a single genetic marker in one study, we show that the usual ratio of coefficients approach can be reformulated as a regression with heterogeneous error in the explanatory variable. This can be implemented using a Bayesian approach, which is next extended to include multiple genetic markers. We then propose a hierarchical model for undertaking a meta-analysis of multiple studies, in which it is not necessary that the same genetic markers are measured in each study. This provides an overall estimate of the causal relationship between the phenotype and the outcome, and an assessment of its heterogeneity across studies. As an example, we estimate the causal relationship of blood concentrations of C-reactive protein on fibrinogen levels using data from 11 studies. These methods provide a flexible framework for efficient estimation of causal relationships derived from multiple studies. Issues discussed include weak instrument bias, analysis of binary outcome data such as disease risk, missing genetic data, and the use of haplotypes.

Analysis with respect to instrumental variables for the exploration of microarray data structures.

Project description:BackgroundEvaluating the importance of the different sources of variations is essential in microarray data experiments. Complex experimental designs generally include various factors structuring the data which should be taken into account. The objective of these experiments is the exploration of some given factors while controlling other factors.ResultsWe present here a family of methods, the analyses with respect to instrumental variables, which can be easily applied to the particular case of microarray data. An illustrative example of analysis with instrumental variables is given in the case of microarray data investigating the effect of beverage intake on peripheral blood gene expression. This approach is compared to an ANOVA-based gene-by-gene statistical method.ConclusionInstrumental variables analyses provide a simple way to control several sources of variation in a multivariate analysis of microarray data. Due to their flexibility, these methods can be associated with a large range of ordination techniques combined with one or several qualitative and/or quantitative descriptive variables.

Correcting Instrumental Variables Estimators for Systematic Measurement Error.

Project description:Instrumental variables (IV) estimators are well established to correct for measurement error on exposure in a broad range of fields. In a distinct prominent stream of research IV's are becoming increasingly popular for estimating causal effects of exposure on outcome since they allow for unmeasured confounders which are hard to avoid. Because many causal questions emerge from data which suffer severe measurement error problems, we combine both IV approaches in this article to correct IV-based causal effect estimators in linear (structural mean) models for possibly systematic measurement error on the exposure. The estimators rely on the presence of a baseline measurement which is associated with the observed exposure and known not to modify the target effect. Simulation studies and the analysis of a small blood pressure reduction trial (n = 105) with treatment noncompliance confirm the adequate performance of our estimators in finite samples. Our results also demonstrate that incorporating limited prior knowledge about a weakly identified parameter (such as the error mean) in a frequentist analysis can yield substantial improvements.

Causal Genetic Inference Using Haplotypes as Instrumental Variables.

Project description:In genomic studies with both genotypes and gene or protein expression profile available, causal effects of gene or protein on clinical outcomes can be inferred through using genetic variants as instrumental variables (IVs). The goal of introducing IV is to remove the effects of unobserved factors that may confound the relationship between the biomarkers and the outcome. A valid inference under the IV framework requires pairwise associations and pathway exclusivity. Among these assumptions, the IV expression association needs to be strong for the casual effect estimates to be unbiased. However, a small number of single nucleotide polymorphisms (SNPs) often provide limited explanation of the variability in the gene or protein expression and can only serve as weak IVs. In this study, we propose to replace SNPs with haplotypes as IVs to increase the variant-expression association and thus improve the casual effect inference of the expression. In the classical two-stage procedure, we developed a haplotype regression model combined with a model selection procedure to identify optimal instruments. The performance of the new method was evaluated through simulations and compared with the IV approaches using observed multiple SNPs. Our results showed the gain of power to detect a causal effect of gene or protein on the outcome using haplotypes compared with using only observed SNPs, under either complete or missing genotype scenarios. We applied our proposed method to a study of the effect of interleukin-1 beta (IL-1?) protein expression on the 90-day survival following sepsis and found that overly expressed IL-1? is likely to increase mortality.

Latent variable GIMME using model implied instrumental variables (MIIVs).

Project description:Researchers across many domains of psychology increasingly wish to arrive at personalized and generalizable dynamic models of individuals' processes. This is seen in psychophysiological, behavioral, and emotional research paradigms, across a range of data types. Errors of measurement are inherent in most data. For this reason, researchers typically gather multiple indicators of the same latent construct and use methods, such as factor analysis, to arrive at scores from these indices. In addition to accurately measuring individuals, researchers also need to find the model that best describes the relations among the latent constructs. Most currently available data-driven searches do not include latent variables. We present an approach that builds from the strong foundations of group iterative multiple model estimation (GIMME), the idiographic filter, and model implied instrumental variables with two-stage least squares estimation (MIIV-2SLS) to provide researchers with the option to include latent variables in their data-driven model searches. The resulting approach is called latent variable GIMME (LV-GIMME). GIMME is utilized for the data-driven search for relations that exist among latent variables. Unlike other approaches such as the idiographic filter, LV-GIMME does not require that the latent variable model to be constant across individuals. This requirement is loosened by utilizing MIIV-2SLS for estimation. Simulated data studies demonstrate that the method can reliably detect relations among latent constructs, and that latent constructs provide more power to detect effects than using observed variables directly. Latent class instrumental variables: a clinical and biostatistical perspective.

Project description:In some two-arm randomized trials, some participants receive the treatment assigned to the other arm as a result of technical problems, refusal of a treatment invitation, or a choice of treatment in an encouragement design. In some before-and-after studies, the availability of a new treatment changes from one time period to this next. Under assumptions that are often reasonable, the latent class instrumental variable (IV) method estimates the effect of treatment received in the aforementioned scenarios involving all-or-none compliance and all-or-none availability. Key aspects are four initial latent classes (sometimes called principal strata) based on treatment received if in each randomization group or time period, the exclusion restriction assumption (in which randomization group or time period is an instrumental variable), the monotonicity assumption (which drops an implausible latent class from the analysis), and the estimated effect of receiving treatment in one latent class (sometimes called efficacy, the local average treatment effect, or the complier average causal effect). Since its independent formulations in the biostatistics and econometrics literatures, the latent class IV method (which has no well-established name) has gained increasing popularity. We review the latent class IV method from a clinical and biostatistical perspective, focusing on underlying assumptions, methodological extensions, and applications in our fields of obstetrics and cancer research.

Falsification Testing of Instrumental Variables Methods for Comparative Effectiveness Research.

Project description:To demonstrate how falsification tests can be used to evaluate instrumental variables methods applicable to a wide variety of comparative effectiveness research questions.Brief conceptual review of instrumental variables and falsification testing principles and techniques accompanied by an empirical application. Sample STATA code related to the empirical application is provided in the Appendix.Comparative long-term risks of sulfonylureas and thiazolidinediones for management of type 2 diabetes. Outcomes include mortality and hospitalization for an ambulatory care-sensitive condition. Prescribing pattern variations are used as instrumental variables.Falsification testing is an easily computed and powerful way to evaluate the validity of the key assumption underlying instrumental variables analysis. If falsification tests are used, instrumental variables techniques can help answer a multitude of important clinical questions.
