"Crack, Reduce, and Implant": A Safe Phaco Technique in a Case with Hard Brown Cataract.

ABSTRACT: This report describes two maneuvers in different steps of phaco surgery in a case with hard cataract, which provide debulking of the central dense nucleus and prevents posterior capsule rupture (PCR). In the current case, clear corneal incisions were created, and anterior chamber was filled with ophthalmic viscosurgical device (OVD). Anterior capsule was punctured, and capsulorhexis was completed. Nucleus was cracked into two halves following vertical groove formation. Core nucleus was hollowed sideward bilaterally in the capsular bag. Nuclear halves were removed from capsular bag, and each one was pushed to one side on the iris plane. Capsular bag was inflated with OVD, and intraocular lens (IOL) was implanted. Nuclear halves were removed in confidence. The presented maneuvers initially reduce dense nucleus load in the safe zone and allow surgeon to use IOL as a barrier to protect floppy posterior capsule from early steps of the surgery.

Project description:Use of the phaco tip technique for lens cleavage and removal does not require manual hydrodissection using a syringe and cannula, or cortical removal using an irrigation/aspiration tip. The phaco tip is the only surgical instrument required for this technique. Its advantages include maintaining a stable intraocular pressure during cortical cleaving hydrodissection and lens removal, which includes the cortex.

Two-hole assisted phaco-chop technique: a more efficient method for safe nucleofractis vertical chopping.

Project description:PurposeTo evaluate the use of the two-hole technique in augmenting the efficiency of surgeons-in-training when performing the phaco-chop technique. We hypothesized that drilling two holes in opposite angles to each other adjacent to the capsulorhexis would mimic a new lens equator. The phaco-tip and the chopper can be inserted into these holes at appropriate depths and may sandwich and fracture the lens more easily than conventional methods.MethodsThe two-hole technique described above was performed by three first-year surgeons before they performed the standard phaco-chop technique. We collected data of their first 8 cases and analyzed a total of 64 cases: 16, divide-and-conquer; 24, two-hole method; 24, phaco-chop. The main outcome measures included the cumulative dissipated energy (CDE) and case ultrasound time (UST) with different phacoemulsification techniques.ResultsThe young practicing surgeons eventually performed the standard phaco-chop more safely after repeated practice using the two-hole method. The drilling of holes enabled deep and effortless impaling of the nucleus. Although there was no significant difference in the CDE among the techniques, there was a significant difference in the UST (P < 0.05).ConclusionThe two-hole method enabled surgeons-in-training to acquire standard phaco-chop skills more efficiently. However, further studies with higher statistical power will be needed to validate these findings. Additionally, a variation of this technique, the four-hole method, is applicable even for experienced surgeons in cases of a hardened nucleus.

Terminal chop: New technique for full thickness nuclear segmentation in mature hard cataract.

Project description:We describe an efficient technique, "Terminal Chop," for full thickness nuclear segmentation in mature hard cataracts. Terminal chop utilizes the principle of secondary rock breakage system with stress concentration to break these hard nuclei. In this technique consonant to drag picks, a specially designed chopper "Terminator" is used to initiate a unique dispersive mechanical force to create a full thickness nuclear crack (crack formation) at the weakest soft and thin equator, which automatically traverses through the center and to the equator on the other side. Lateral separation of both the instruments placed at the equator, propagates the initial full thickness nuclear crack (crack propagation), breaking the entire nucleus into two complete pieces including the posterior plate. The direction of splitting follows the cleavage plane in a more direct manner, thus requiring least fracture force, least manipulation and stress, much lower than compressive stress, causing minimal trauma, and highly satisfactory postoperative results.

A novel minimal fluid technique for effective and safe lens hydrodissection during cataract surgery.

Project description:Traditional hydrodissection may cause posterior capsule rupture (PCR) if excessive fluid accumulates. In this study, we describe the successful application of a novel minimal fluid hydrodissection technique in 100 consecutive cataract surgery cases. This technique separates the nucleus from the capsule utilizing low hydrostatic pressure and precise kinetic movement of a small volume (around 0.2 cc) of balanced salt solution. There were no instances of PCR. This technique is suitable for a range of cases, including femtosecond laser-assisted cataract surgery and posterior subcapsular cataract.

Chop and tumble nucleotomy - A technique for safe nuclear emulsification in posterior polar cataract.

Project description:A modified phaco chop technique for nuclear emulsification in posterior polar cataracts with nuclear sclerosis that avoids hydrodissection or nuclear rotation has been described. After performing a vertical chop maneuver to divide the nucleus, two pie-shaped nuclear fragments are removed from either side of the initial chop. The remaining nuclear fragments are then sequentially tumbled toward the center using the second instrument and emulsified while maintaining an intact epinuclear shell, which safeguards the fragile posterior capsule. The technique was successfully performed in 62 eyes of 54 patients with posterior polar cataract and grade II-IV nuclear sclerosis. The Chop and Tumble nucleotomy is a safe and effective technique for phacoemulsification in posterior polar cataracts with nuclear sclerosis, where hydrodissection and nuclear rotation are typically avoided.

Observations of the efficacy of the artificial lens cushion plate technique in hard-core cataract surgery.

Project description:ObjectiveTo evaluate the efficacy of intraocular lens (IOL) cushion plate technology in reducing corneal endothelial cell loss during hard-core cataract surgery compared with conventional ultrasonic emulsification.MethodsSeventy-six patients with hard-core cataracts who underwent surgery at our institution from April 2019 to June 2022 were included. The patients were divided into an observation group (IOL cushion plate technology, 38 patients) and a control group (conventional ultrasonic emulsification, 38 patients). Surgical outcomes, including the corneal endothelial cell loss rate, best corrected visual acuity (BCVA), and central corneal thickness (CCTc), were compared between the two groups.ResultsPreoperative patient characteristics were similar between the groups. Postoperatively, both groups demonstrated similar BCVA and CCTc values on days 7 and 30. However, compared with the observation group, the control group presented a significantly greater rate of corneal endothelial cell loss on postoperative days 7 and 30 (p < 0.05). Intraoperative complications and postoperative complications were notably greater in the control group (p < 0.05). The observation group had reduced ultramilk time and total energy consumption (p < 0.05).ConclusionIOL cushion plate technology offers advantages in preserving corneal endothelial cells during hard-core cataract surgery, potentially improving surgical safety and efficacy.

Zero Phaco: A new technique for dealing with soft cataracts.

Project description:Phacoemulsification in soft cataracts can be challenging due to the lack of rigid cleavage planes and the inability to crack. We describe a new phacoemulsification technique for dealing with soft cataracts using high vacuum and zero energy. Following capsulorhexis and hydrodissection, we introduced the phacoemulsification probe, keeping the torsional and longitudinal power at zero. A central groove was created in sculpting mode. We held the nucleus with adequate vacuum in chop mode and divided the nucleus. Then, we rotated and chopped the nucleus similarly into small pieces without using any power. For emulsification, we increased the vacuum to 600 mmHg and then shredded and stuffed the pieces into the phaco probe by the chopper. A newer generation phaco machine with active fluidic system and monitored pressurized infusion helps the surgeon control the intraocular pressure (IOP) and hold the nucleus with vacuum alone, allowing chopping and emulsifying of the pieces without any energy.

Novel tunnel staining technique to reduce premature entry in manual small-incision cataract surgery.

Project description:Mastering manual small-incision cataract surgery (MSICS) for beginner surgeons is difficult. In the initial days of residency or training, surgeons struggle to make a proper scleral tunnel and keratome entry. It commonly results in premature entry and iris prolapse. Most of the literature has shed light on premature entry during tunnel construction by a crescent blade, whereas a significant majority of iris prolapse happens due to improper keratome entry. This novel trypan blue dye-assisted tunnel staining (TBTS) technique helps in proper tunnel demarcation which can reduce the incidence of premature entry with a keratome.

'Phaco-ECP': combined endoscopic cyclophotocoagulation and cataract surgery to augment medical control of glaucoma.

Project description:Does phaco-ECP reduce intraocular pressure? Is phaco-ECP safe? Retrospective case note review of all patients undergoing phaco-ECP between June 2008 and June 2009. All glaucoma subtypes were included. Single District General Hospital Ophthalmology Department within the UK. 58 participants case notes reviewed. Mean age 79.0 years (SD ±9.8). All patients received combined cataract surgery and endoscopic cyclophotocoagulation. Follow-up was 1 day, 1 week, 1, 3, 6, 12, 18 and 24 months for intraocular pressure (IOP) measurement. Number of medications, visual acuity and presence of complications were also assessed. Of the 58 cases performed, 56 case notes (97%) were available for analysis. Mean age 79.0 years (SD ±9.8). Mean pre-procedural IOP was 21.54 mm Hg (95% CI 19.86 to 23.22, n=56). Mean IOP was 14.43 mm Hg (95% CI 13.65 to 15.21, n=53) at 18 months and 14.44 mm Hg (95% CI 13.63 to 15.25, n=41) at 24 months. The mean drop from baseline to 18 and 24 months was 7.1 mm Hg. Statistically significant decrease in IOP was demonstrated at all time points (p<0.001). Mean medication usage was 1.97 agents (95% CI 1.69 to 2.25) at baseline, 1.96 agents (95% CI 1.70 to 2.22) at 18 months and 2.07 agents (95% CI 1.76 to 2.38) at 24 months. No statistically significant change throughout. This study confirms the safety of phaco-ECP. In this case series, the IOP-lowering effect was significant at all time points; however, the effect of cataract surgery alone was not controlled. A randomised controlled trial is required to draw efficacy conclusions. The authors proposed endoscopic cyclophotocoagulation's main role is to optimise control of low-risk glaucoma of low-risk patients at the time of cataract surgery. However, the authors do not propose that phaco-ECP is a substitute for filtration surgery in high-risk eyes or when low target pressures (<14 mm Hg) are indicated.

The safety and efficacy of phaco-sleeve irrigation-assisted hydrodissection during femtosecond laser-assisted cataract surgery.

Project description:Femtosecond laser-assisted cataract surgery (FLACS) changes the intraoperative environment due to the generation of intracapsular gas that induces a high intracapsular volume. Manual hydrodissection (mH) may induce high intracapsular pressure (ICP) and additional intracapsular volume, thereby leading to capsular block syndrome (CBS). Since the phaco-sleeve irrigation-assisted hydrodissection (iH) technique is used to initially groove and split the lens and remove the intracapsular gas, this can reduce the intracapsular volume while bypassing the intracapsular lens prior to the hydrodissection. As iH uses the phaco tip to intentionally vacuum the intraocular fluid for use in inducing the irrigation jet from the sleeve side holes, the ICP cannot surpass the set irrigation pressure, thereby avoiding CBS. Using this technique, we performed FLACS without CBS in 310 cataract eyes. Our findings suggest that the iH technique may be beneficial for patients by preventing CBS during FLACS.
