Pulmonary tuberculosis preventive practices among Anibessa Bus users at Addis Ababa, Ethiopia: a cross-sectional study.

ABSTRACT:

Objective

Tuberculosis (TB) is a chronic infectious disease caused by Mycobacterium tuberculosis. Smear positive tuberculosis patients are responsible for up to 90% of transmission occurring in the community. However, little is known about pulmonary tuberculosis preventive practices among bus users in Ethiopia. This study aimed to assess the level of Pulmonary Tuberculosis (PTB) preventive practices and associated factors among bus users at Addis Ababa.

Results

Community based cross-sectional study was conducted among bus users at Addis Ababa. Participants were selected using systematic sampling technique. Overall, 50.5% of bus users had good practices on prevention of PTB at Addis Ababa. The odds of practicing prevention of PTB among participants who were attended secondary school (AOR?=?4.63; 95% CI 2.62, 11.17) and higher education (AOR?=?2.86: 95% CI 1.13, 7.73), resided at Addis Ababa (AOR?=?2.51; 95% CI 1.61, 5.21), knowledgeable about PTB (AOR?=?4.12; 95% CI 3.14, 5.70), and using mass media (AOR?=?2.14; 95% CI 1.78, 4.27) as a source of information were higher than the odds of their respective counterparts. The overall practice of pulmonary tuberculosis prevention among city bus users in the study area was low. Therefore, enhancing educational opportunity and increase community awareness about the causes, risk factors and means of transmission using mass media might improve the practices of PTB prevention during bus transportation.

Project description:IntroductionHypertension is a major public health problem that is often unrecognized, and its detection and control should be prioritized. The level of undiagnosed hypertension and its associated factors among long-distance bus drivers in Ethiopia is unknown.ObjectiveThis study aimed to assess the magnitude of undiagnosed hypertension and its associated factors among long-distance bus drivers in Addis Ababa bus terminals.MethodsA facility-based cross-sectional study was conducted on 391 long-distance bus drivers from December 15, 2021, to January 15 2022 at five cross-country bus terminals in Addis Ababa. A standardized and structured questionnaire was adapted based on the WHO stepwise approach to a non-communicable disease study and translated into Amharic. Data were coded, cleaned, and entered using Epi-data version 4.6 and exported to SPSS version 26. Logistic regression analysis was performed. Variables with a P-value < 0.25 in the bivariable analysis were selected for multivariable logistic regression analysis. Independent variables with a P-value < 0.05 were considered statistically significant. The magnitude of association between independent and dependent variables was measured by odds ratio with a 95% confidence interval.ResultsIn this study, 391 study participants were involved with a response rate of 97.1%. The prevalence of undiagnosed hypertension was 22.5% (CI: 18.7%, 26.6%). Poor level of knowledge (AOR: 2.00, CI: 1.08, 3.70), long duration of driving per day (AOR: 2.50, 95% CI: 1.37-4.56), habit of chewing of chat (AOR: 2.61, 95% CI: 1.44, 4.73), regular alcohol consumption (AOR = 3.46; 95% CI: 1.70, 7.05), overweight (AOR:3.14, 95%CI: 1.54,6.42) obesity (AOR: 3.21, 95% CI 1.35, 7.61) and regular physical exercise (AOR: 0.16, 95% CI: 0.09, 0.29) were statistically significantly associated with undiagnosed hypertension.ConclusionThis study revealed that the prevalence of undiagnosed hypertension among long-distance bus drivers was 22.5%, which was associated with modifiable behavioral factors, lack of regular physical exercise, lack of adequate awareness and high body mass index.RecommendationStakeholders must implement the necessary preventive measures. These include increasing the level of awareness of hypertension among long-distance drivers and developing prevention of hypertension strategies and policies focusing on lifestyle and behavioral modifications.

Project description:Healthcare professionals are at higher risk of contracting the new coronavirus disease 2019 (COVID-19). Although appropriate preventive measures are the most important interventions to prevent coronavirus infection among healthcare workers, they are also highly concerned about the consequences of the pandemic. The aim of this cross-sectional study was to assess preventive practices, perceived risk and worry about COVID-19 crisis among healthcare professionals at six public hospitals in Addis Ababa, Ethiopia. A systematic random sampling technique was used to select 1,134 respondents (52.6% females). Data were collected between 9th and 20th June 2020 using self-administered questionnaires. Descriptive statistics were used to summarize the data. A multiple linear regression analysis was performed to identify factors associated with worry about COVID-19 crisis. The highest percentage of respondents were nurses (39.3%) and physicians (22.2%), followed by interns (10.8%) and midwives (10.3%). Wearing facemask (93%) and frequent hand washing (93%) were the commonly reported preventive practices. Perceived risk of becoming infected with coronavirus (88%) and the potential risk of infection to their family (91%) were very high. The mean (median) worry score about COVID-19 crisis was 2.37 (3.0), on 1 to 3 scale, with 1 (not worried) and 3 (highly worried). The majority worried a lot about the health system being overwhelmed by COVID-19 patients (92%), the health of their loved ones (90%) and losing someone due to COVID-19 (89%). Respondents who had previously provided clinical care to Ebola, SARS and cholera patients had significantly lower levels of worry about COVID-19 crisis than participants who had not (β = -1.38, P<0.001). Our findings reveal respondents' widespread practice of preventive measures, highest levels of perceived risk and worry about the COVID-19 crisis. Increased perceived risk and worry about COVID-19 might enable healthcare workers to adopt appropriate preventive measures more effectively against the disease.

Project description:BACKGROUND:Tuberculosis remains a major public-health problem in the world, despite several efforts to improve case identification and treatment compliance. It is well known cause of ill-health among millions of people each year and ranks as the second leading cause of death from infectious disease worldwide. Despite implementation of the World health organization recommended strategy, the reductions in the incidence of TB have been minimal in high burden countries. OBJECTIVES AND METHODS:A case control study was carried out to assess the risk factors of TB, where cases were newly registered bacteriologically confirmed pulmonary TB patients with age greater than 15 years who present at twenty health centres in Addis Ababa. Controls were age and sex matched attendees who presented in the same health centers for non-TB health problems. RESULTS:A total of 260 cases and 260 controls were enrolled in the study and 45.8% of cases and 46.2% of controls were in the 26-45 years age bracket. According to the multivariable logistic regression analysis, seven variables were found to be independent predictors for the occurrence of TB after controlling possible confounders. Patients who live in house with no window or one window were almost two times more likely to develop tuberculosis compared to people whose house has multiple windows (AOR = 1.81; 95% CI:1.06, 3.07). Previous history of hospital admission was found to pose risk almost more than three times (AOR = 3.39; 95% CI: 1.64-7.03). Having a household member who had TB was shown to increase risk of developing TB by three fold (AOR = 3.00; 95% CI: 1.60, 5.62). The study showed that illiterate TB patients were found to be more than twice more likely to develop TB compared to subjects who can atleast read and write (AOR, 95% CI = 2.15, 1.05, 4.40). Patients with household income of less than 1000 birrs per month were more than two times more likely to develop TB compared to those who had higher income (AOR = 2.2; 95% CI: 1.28, 3.78). Smoking has also been identified as important risk factor for developing TB by four times (AOR = 4.43; 95% CI: 2.10, 9.3). BCG was found to be protective against TB reducing the risk by one-third (AOR = 0.34; 95% CI: 0.22, 0.54). CONCLUSION:This study showed that TB is more common among the most agile and economically active age group, and number of windows, history of hospital admission, a household member who had TB, illiteracy, low household income and smoking and lack of BCG scar were identified as independent risk factors. Therefore it is imperative that the TB control effort need a strategy to address socio economic issues such as poverty, overcrowding, smoking, and infection control at health care facilities level is an important intervention to prevent transmission of TB within the facilities.

Project description:IntroductionAn increase in population and related increased demand for health services, expansion of industries, and increasing transportation demands have increased the emission of dioxin and furan persistent organic pollutants (POPs) in Addis Ababa, Ethiopia.ObjectiveThis study aimed to identify sources of dioxin and furan emissions, quantify their release into various environmental medias and assess related management practices.MethodsThe standard United Nations Environmental Programme (UNEP) (2005) toolkit guide and default emission factor were used to identify the main anthropogenic sources and to quantify the amount of released dioxin and furan. Stratified random sampling techniques were applied to assess current management practices.ResultsNine main groups of dioxin and furan emission sources were identified. The emission of each source group was calculated by the activity rate data multiplied by an emission factor. The results found that about 138.85 g toxic equivalent(TEQ)/a(TEQ/year)of dioxin and furan were released to air, water, residue (materials remaining as sludge after sewage treatment or in the form of ash after incineration activity) and soil. Waste disposal activities recorded the largest release of dioxin and furan, accounting for 68.30 g TEQ/a of dioxin and furan to water and residue, 34.00 g TEQ/a to air and 0.64 g TEQ/a emitted into soil.ConclusionsSeveral sources of dioxin and furan emission were identified and the present study found that their management is inadequate. Waste disposal services are especially inadequate and generate higher amounts of dioxin and furan gasses. In addition, the organizations that are responsible for the release of dioxin and furan have no awareness of their release and inadequate management practices. The present study points to the need for reformulation of the national legal management framework, adoption of best available technology for disposal services such as incinerators with flue gas management, increasing public and stakeholders' awareness and participation and capacitating the responsible government organizations.

Project description:BackgroundRisky sexual behaviors adversely affect the health of youth and young adults exposing them to sexually transmitted infections including HIV/AIDS and unwanted pregnancy to females that in turn lead to deleterious health, social and economic consequences. Youth centers inform their clients on sexually transmitted diseases, including HIV/AIDS, unwanted pregnancy, high risk abortion, and other reproductive health problems. Therefore, this study was designed to assess the prevalence of risky sexual behaviors among youth center reproductive health clinic users and non-users in Addis Ababa.MethodsA comparative cross-sectional study design was carried out among 524 youth in Addis Ababa from March to April, 2016. The data was entered in EPI-INFO 7 software; and cleaned and analyzed using SPSS version 16.0. The prevalence was computed. Binary and multivariable logistic regression was done to determine the strength, direction and significance of association between youth center reproductive health clinic utilization and risky sexual behavior and to control confounder variables respectively.ResultsA total of 524 youth with the response rate of 92% participated in the study. The overall prevalence of risky sexual behavior was 226 (43.1%) (With statistically significant difference in prevalence among users 101 (38.5%) and non-users 125 (47.7%) of youth center clinics, (p-value = 0.04). The odds of reporting risky sexual behavior was 60% higher among volunteers who did not use the reproductive health clinic, relative to those who did (AOR = 1.60; 95%CI = 1.08, 2.37). Teenagers aged 15-19 years were (AOR = 0.08; 95%CI = 0.05, 0.15) 92% less likely to practice risky sexual behavior compared to those aged 25-29 years old.ConclusionRisky sexual behavior was statistically significantly higher among non-users of the youth center reproductive health clinic compared with the users. In addition, a substantial proportion of the youth engaged in different risky sexual behaviors that are evidenced by the existence of multiple sexual partners, sexual practice without condom and early sexual debut that might predispose youth to STIs including HIV infection and unwanted pregnancy. The ministry of health and its partners should strengthen youth center reproductive health clinics in urban, semi-urban and rural parts of Ethiopia.
