Project description:BackgroundUse of long-lasting insecticidal nets (LLINs), indoor residual spraying (IRS), community-based malaria education, prompt diagnosis and treatment are key programme components of malaria prevention and control in Ethiopia. However, the effectiveness of these interventions is often undermined by various challenges, including insecticide and drug resistance, the plasticity of malaria vectors feeding and biting behaviour, and certain household factors that lead to misuse and poor utilization of LLINs. The primary objective of this study was to document households' perceptions towards malaria and assess the prevalence of the disease and the constraints related to the ongoing interventions in Ethiopia (LLINs, IRS, community mobilization house screening).MethodsThe study was conducted in Jabi Tehnan district, Northwestern Ethiopia, from November 2019 to March 2020. A total of 3010 households from 38 villages were randomly selected for socio-economic and demographic survey. Focus group discussions (FGDs) were conducted in 11 different health clusters considering agro-ecological differences. A total of 1256 children under 10 years of age were screened for malaria parasites using microscopy to determine malaria prevalence. Furthermore, 5-year malaria trend analysis was undertaken based on data obtained from the district health office to understand the disease dynamics.ResultsMalaria knowledge in the area was high as all FGD participants correctly identified mosquito bites during the night as sources of malaria transmission. Delayed health-seeking behaviour remains a key behavioural challenge in malaria control as it took patients on average 4 days before reporting the case at the nearby health facility. On average, households lost 2.53 working days per person-per malaria episode and they spent US$ 18 per person per episode. Out of the 1256 randomly selected under 10 children tested for malaria parasites, 11 (0.89%) were found to be positive. Malaria disproportionately affected the adult segment of the population more, with 50% of the total cases reported from households being from among individuals who were 15 years or older. The second most affected group was the age group between 5 and 14 years followed by children aged under 5, with 31% and 14% burden, respectively.ConclusionDespite the achievement of universal coverage in terms of LLINs access, utilization of vector control interventions in the area remained low. Using bed nets for unintended purposes remained a major challenge. Therefore, continued community education and communication work should be prioritized in the study area to bring about the desired behavioural changes.

Project description:BackgroundFlood preparedness empowers the community to respond effectively to related hazards. However, there was no research done in the country concerning household flood preparedness. Therefore, the aim of this study was to assess household flood preparedness and associated factors in the flood-prone community of Dembia district, northwest Ethiopia.MethodsA community-based cross-sectional study was conducted from March to April 2014 in the Dembia district. A two-stage sampling technique was used. The study was conducted using 806 flood-prone participants. An interviewer-administered questionnaire was used to collect data. The collected data were entered using Epi info version 3.5.1 and transported into SPSS version 16 for further analysis. Descriptive and analytic statistics were computed. Variables having association with the outcome variable were reported using odds ratio with 95% confidence interval (CI). Model fitness was checked by Hosmer and Lemeshew chi-square test.ResultsHousehold flood preparedness was found to be 24.4%. The age group of ≥ 46 years (adjusted odds ratio [AOR]=2.62; 95% CI: 1.12, 6.00) above, monthly household income >893 Ethiopian Birr, (AOR=6.72; 95% CI: 2.2 7, 19.88) attending primary level education (AOR=22.08; 95% CI: 8.16, 59.74), warning system in household (AOR=5.41; 95% CI: 2.38, 12.32), knowledge of flood prevention, (AOR=2.52; 95% CI: 1.43, 5.57) were positively associated with household flood preparedness.Conclusion and recommendationThis study has demonstrated that household flood preparedness was found to be low in the study area. Household flood preparedness was significantly associated with the older age group, attending primary level education, having a higher monthly income, receive household level warning messages, having knowledge on preparedness, prior exposure to a flood, and length of flood >6 days. Strengthening household flood preparedness in advance is important in order to prevent flood and its related consequences.

Project description:BackgroundMaternal tetanus is defined as tetanus acquired during pregnancy or within 6 weeks after the end of conception. As tetanus is a vaccine-preventable disease, immunization of pregnant mothers with a TT (tetanus toxoid) dose is one of the most effective ways to protect against the disease. Some studies showed that 94% of neonatal mortality reduction could be achieved through immunization of pregnant and childbearing-age mothers with at least two doses of TT vaccination.ObjectiveTo assess the uptake of tetanus toxoid vaccine and associated factors among mothers who gave birth in the last 12 months in Errer district, Somali Regional State, Eastern Ethiopia, 2017.Methods and materialsA community-based cross-sectional study design was implemented to study 440 mothers who gave birth in the last 12 months. Participants were selected using the strata and systematic sampling technique after conducting a preliminary survey. Data were collected through a face-to-face interviewer-administered questionnaire. The collected data was entered into EpiData version 3.02 and then exported to Statistical Package for the Social Sciences (SPSS) version 20. Bivariate and multivariate logistic regressions were carried out to see the association between variables at p < 0.05 and 95% confidence interval. Finally, the information was presented by using frequencies, summary measures, and tables.ResultThe overall tetanus vaccination uptake (≥TT2) doses was found to be 51.8%, 95% CI (47.7%, 56.4%). The total number of mothers who complete the five TT doses was 31 (14.8%). Urban residence [AOR = 6.1, 95% CI: (2.33, 10.43)], multiparity [AOR = 2.3, 95% CI: (1.7, 6.4)], and traveling less than 30 minutes from the home to a health facility [AOR = 4.6, 95% CI: (1.34, 6.72)] were some the factors that were significantly associated with tetanus toxoid vaccination uptake. Conclusion and Recommendation. Although TT immunization is a scientifically proven mechanism to protect against maternal and neonatal tetanus, only half of the district mothers received ≥TT2 doses. Besides, our study revealed that the low vaccine uptake is attributed to long distance travel to reach a health facility, maternal illiteracy, and pastoralist lifestyle of mothers in the district. Thus, the regional stakeholders are required to scale up efforts on mother's awareness creation towards the importance of the vaccine through health education and to arrange outreach TT vaccination campaigns in distant pastoralist communities within the region.

Project description:Background:Home delivery is responsible to maternal mortality due to obstetric complication like hemorrhage, hypertensive disorders, and sepsis. The prevalence of home delivery is remained very high both nationally (73%) and regionally (SNNPR) with 74.5%. Efforts were made to increase institutional delivery through skilled birth attendance. But women still prefer home as a place of delivery. This study was done to determine whether home preference has association with home delivery or not and the reason why they prefer home delivery. Method:A community-based cross-sectional study was conducted in East Badawacho District from January 26 to February 25/2018. A total of 552 participants were selected by systematic sampling. Data were collected using both quantitative and qualitative methods. Bivariate and multivariable analyses were carried out to identify factors associated with home delivery. Qualitative data was analyzed thematically, and results were triangulated with the data. Associations were determined by using OR at 95% CI and p value at 0.05. Result:Home delivery is found to be 73.6% (95% CI, 69.9%-77.2%). Lack of written birth plan for birth preparedness and readiness (AOR = 14.965, 95% CI: 4.488-49.899), incomplete number of ANC visits (1-3)(AOR = 4.455, 95% CI: 1.942-10.221), and home preference as a place of delivery (AOR = 4.039, 95% CI: 1.545-10.558) were independent predictors of home delivery. Conclusion:Home delivery was high in the district. The independent factors significantly associated with home were lack of written birth plan for preparedness and readiness, incomplete number of ANC visits (1-3), and home preference as place of delivery. Actions targeting maternal education, encouraging number of ANC visits, and avoiding barriers for ID utilization were the crucial areas to tackle the problem.

Project description:BackgroundThe risk of death from complications relating to pregnancy and childbirth for women's lifetime is higher in developing countries. Improving maternal and child health through a well-organized institutional delivery service is central to achieving reduced maternal and child mortality. Despite the efforts that have been made to improve maternal health outcomes in Ethiopia, institutional delivery is still unacceptably low.ObjectiveThis study was conducted to assess institutional delivery service utilization and associated factors in the study area.MethodsA Community-based cross-sectional study was conducted. A multi-stage sampling technique was used to employ a total of 546 women. Data were collected using an interviewer-administered questionnaire and entered into EpiData version 3.1 and then exported to SPSS version 23.0. for analysis. Logistic regression models were used to determine factors associated with the outcome variable. Adjusted Odds ratios with 95% CI were computed to measure the strength of association and statistical significance was declared at p-value <0.05.ResultsThe Prevalence of institutional delivery in the study area was 38% (34%-42%). Factors significantly associated with institutional delivery were ANC visit 1.80 (1.12-2.91), knowledge of danger sign during pregnancy 3.60 (2.25-5.76), urban residency 2.09 (1.15-3.81), Parity 0.49 (0.25-0.95) accessibility of health facility 4.60 (2.01-10.89), husbands educational level: primary 2.50 (1.27-4.91), secondary and above 2.36 (1.24-4.48), mothers occupation: governmental employee 2.05 (1.00-4.18), and Private employee 2.42 (1.09-5.35).ConclusionsThe prevalence of institutional delivery in the District was low. Antenatal visits, residency, knowledge of pregnancy danger signs, parity, and accessibility of health facilities, maternal occupation, and husband education were factors significantly associated with institutional delivery.

Project description:BackgroundMaternal and child mortality remains a major public health problem in Ethiopia. Improving short birth interval practice is a main strategy to reduce neonatal mortality, maternal mortality, adverse pregnancy outcomes, high fertility rate, and enhance economic development efforts. However, there has been limited study done regarding short birth intervals in the study area. Therefore, this study aimed to assess short birth intervals and associated factors among women who gave birth in the last three years in Dembecha district, Northwest Ethiopia, 2019.MethodA community-based cross-sectional study was conducted among 880 reproductive-age multipara mothers using a stratified cluster sampling technique. The data were collected by face-to-face interviews through pretested and semi-structured questionnaires. Bivariable and multivariable logistic regression model was fitted. Variables with a p-value ≤0.05 were considered statistically significant factors. Adjusted odds ratio with a 95% confidence interval was used to report the association between predictors and the outcome variable.ResultsThe prevalence of short birth interval was found to be 43.4% (95% CI: 40.2, 46.9). Husband education (able to read and write) [AOR:2.81,(95% CI:1.04,7.85)], wealth index (lowest quartile) [AOR:3.75,(95% CI:2.35,5.97), residence (urban) [AOR:3.20,(95% CI:1.62,6.33)],age at first marriage (15-17 years old) [AOR: 1.65,(95% CI:1.15, 2.26),and non-use of contraceptive [AOR: 8.78, (95% CI: 6.18, 12.47) were statistically significant variables.ConclusionThe study revealed that the prevalence of short birth intervals among multipara women is found to be high. Husband education, wealth index, urban residence, age at first marriage, and non-use of contraceptives were variables significantly associated with short birth intervals. Hence, to overcome the problem the focus should be on increasing family planning utilization, avoiding early marriage, strengthening paternal education, and improving family income.

Project description:BackgroundMaternal mortality and adverse pregnancy outcomes are still challenges in developing countries. In Ethiopia, long distances and lack of transportation are the main geographic barriers for pregnant women to utilize a skilled birth attendant. To alleviate this problem, maternity waiting homes are a gateway for women to deliver at the health facilities, thereby helping towards the reduction of the alarming maternal mortality trend and negative pregnancy outcomes. However, there is a paucity of evidence regarding the utilization of maternity waiting homes in the study area. Therefore, this study aimed to assess utilization of maternity waiting home services and associated factors among mothers who gave birth in the last year in Dabat district, northwest Ethiopia.MethodsA community-based cross-sectional study was conducted from January 5 to February 30, 2019. A total of 402 eligible women were selected using a simple random sampling technique. Data were collected using a structured, pre-tested, and interviewer-administered questionnaire through face-to-face interviews. Data were entered into EPI info version 7.1.2 and exported to SPSS version 20 for analysis. Both bivariable and multivariable logistic regression models were fitted. Statistically significant associations between variables were determined based on the adjusted odds ratio (AOR) with its 95% confidence interval and p-value of ≤ 0.05.ResultsMaternity waiting home utilization by pregnant women was found to be 16.2% (95% CI: 13, 20). The mothers' age (26-30 years) (AOR = 0.24; 95% CI: 0.08,0.69), primary level of education (AOR = 9.05; 95% CI: 3.83, 21.43), accepted length of stay in maternity waiting homes (AOR = 3.15; 95% CI: 1.54, 6.43), adequate knowledge of pregnancy danger signs (AOR = 7.88; 95% CI: 3.72,16.69), jointly decision on the mother's health (AOR = 2.76; 95% CI: 1.08,7.05), and getting people for household activities (AOR = 2.59, 95% CI: 1.21, 5.52) had significant association with maternity waiting home utilization.ConclusionIn this study, maternity waiting home utilization was low. Thus, expanding a strategy to improve women's educational status, health education communication regarding danger signs of pregnancy, empowering women's decision-making power, and shortening the length of stay at maternity waiting homes may enhance maternity waiting home utilization.

Project description:Methods:A community-based cross-sectional study was employed among 686 mothers in Burie District from March 16 to March 25, 2017. A multistage sampling technique was used to select the study participants. Data were collected using face to face interviewer administered structured questionnaires. Then, the collected data was entered, coded, and cleaned into EPI Data version 3.1 and exported to SPSS version 20.0 for data analysis. Bivariate and multivariate logistic regression was done to assess the association of factors with postpartum modern contraceptive use. Adjusted odds ratios with 95% confidence intervals were calculated, and p values <0.05 were considered to indicate statistical significance. Result:This study revealed that postpartum modern contraceptive use was found to be 20.7%. Postpartum modern contraceptive use was significantly associated with women's level of education (AOR = 0.15, 95% CI (0.03-0.71)), discussing FP methods with partner (AOR = 0.60, 95% CI (0.40-0.90)), knowing menses return after birth (AOR = 0.39, 95% CI (0.25-0.59)), ever heard about modern FP methods (AOR = 0.06, 95% CI (0.01-0.43)), and contacting health professionals (AOR = 1.85, 95% CI (1.19-2.88)). Conclusion and Recommendations. Postpartum modern contraceptive use was found to be low. Therefore, health professionals should work on improvements in women's educational status, making awareness of the women and counseling of their husbands about the use of postpartum contraception, when fertility returned and risky timing for becoming pregnant.

Project description:BackgroundProviding preferred methods of contraceptive for HIV-positive women and avoiding unintended pregnancy is one of the primary means of preventing mother to child transmission of HIV. This study assessed the prevalence of contraceptive use and method preference among HIV-positive women in Amhara region, Ethiopia.MethodsA cross-sectional survey was conducted among HIV-positive women in three referral hospitals of Amhara region. Data were collected by interviewing HIV-positive women using a pre-tested and structured questionnaire. A binary logistic regression model was used to identify factors associated with contraceptive use, and odd ratio with 95% confidence interval (CI) was calculated to measure the strength of association.ResultsA total of 803 women living with HIV (with a response rate of 95.4%) were interviewed. The mean age of the study participants was 32.2 years (SD ± 6.2 years). The prevalence of current contraceptive use was 30.3% (95% CI: 27.0-33.7%). The preferred and most commonly used contraceptive methods were injectable (42.8%) and male condom (32.9%). Younger age group (15-24 years) (AOR = 9.67; 95%CI: 3.45, 27.10), one or more number of living children (AOR = 4.01; 95%CI: 2.07, 7.79), HIV diagnosis > 2-4 years (AOR = 2.37; 95%CI: 1.10, 5.08), and having high CD4 count > 500 cell/ul (AOR = 3.25; 95% CI: 1.42, 7.44) were significantly associated with contraceptive use.ConclusionThe prevalence of contraceptive use among HIV-positive women in Amhara region referral hospitals is low, which suggests a high risk of unintended pregnancy. Injectable and male condoms are the most preferred type of contraceptive methods. Thus, it is better to integrate these contraceptive methods with ART clinic.

Project description:BackgroundInternally displaced populations are susceptible to food deprivation. Specifically, children aged 6-23 are commonly vulnerable to poor complementary feeding. Proper complementary feeding is of paramount importance to the healthy growth and survival of a children. Therefore, this study aimed to assess the level of appropriate complementary feeding practices and associated factors among internally displaced children aged 6-23 months in northwest Ethiopia, 2020.MethodsA community-based cross-sectional study was conducted from June to July 2020 among 264 internally displaced mothers of children 6-23 months in northwest Ethiopia. A systematic random sampling technique was used to reach the study subjects and data were collected using a structured and pre-tested interviewer-administered questionnaire. Data were entered into the Epi Data version 4.1 and analyzed using SPSS version 23. Binary and multivariable analyses with a 95% confidence level were performed. In the final model, variables with P < 0.05 were considered statistically significant.ResultsThe overall level of appropriate complementary feeding practice was 26.8%. Only 14% of the mothers provided a diversified diet for their 6-23 months children. Child aged 6-11 months (AOR = 0.11, 95%; CI: 0.04-0.27), 12-17 months (AOR = 0.35, 95%; 95% CI: 0.17-0.70) and not having harmful culture on complementary feeding (AOR = 2.04; 95% CI: 1.06-3.96) were independent predictors of appropriate complementary feeding practices.ConclusionThe level of appropriate complementary feeding practice was found to be low, which would have negative implications on the health and nutritional status of infants and young children. Additional rations for breastfeeding mothers and children aged 6-23 months at refugee camps and nutritional counseling on child feeding practices are recommended.
