Mediastinal lipoma as a cause of dyspnea.

ABSTRACT: Mediastinal lipoma is a rare intrathoracic tumor which can present as shortness of breath. We describe a morbidly obese patient with progressive dyspnea who got diagnosed endoscopically and is scheduled for surgical resection for a large benign mediastinal lipoma.

Hematological dyspnea: A rare cause with gratifying recovery.

Project description:A 17-year-old female was referred for evaluation of fever of two-month duration with acute onset of dyspnea since 1 week. Clinical examination revealed tachycardia, tachypnea, elevated jugular venous pressure, bilateral basal crepitations with normal heart sounds, and no murmur. Chest X-ray showed infiltrates in bilateral lung fields. Echocardiography revealed obliteration of biventricular apices along with layered thrombus over the left ventricular endocardium. Doppler studies elicited restrictive physiology with mild mitral and tricuspid regurgitation. Complete hemogram revealed hypereosinophilia with eosinophilic count of 7.4 × 109/L. All secondary causes of elevated eosinophil count were excluded. The patient was started on steroids and anticoagulation. Serial echocardiograms showed clearing of the thrombus with marked symptomatic improvement. We highlight a case of idiopathic hypereosinophilic syndrome with classic cardiac (Loeffler endocarditis) and pulmonary manifestations and prompt recovery with steroids and anticoagulation. <Learning objective: Idiopathic hypereosinophilic syndrome is a rare entity leading to multi-organ involvement. Cardiac involvement is one of the most common causes of morbidity and mortality. On the basis of this case report, we discuss Loeffler endocarditis in idiopathic hypereosinophilic syndrome; demonstrate classic echocardiographic findings in the intermediate stage, and remarkable symptomatic and echocardiographic recovery after appropriate treatment.>.

Sinus of Valsalva Aneurysm: A Rare Cause of Dyspnea.

Project description:Sinus of Valsalva aneurysm (SOVA) is a rare clinical entity. Clinical manifestations can vary from an incidental finding on an imaging study to a life-threatening emergency. We report a case of a 51-year-old female with a large symptomatic left SOVA. Echocardiogram and computed tomography angiography (CTA) of the chest revealed marked dilatation of the left sinus of Valsalva, measuring 7.5 cm. This resulted in superior displacement of the left main coronary artery. Surgical repair of the aneurysm with reimplantation of the right and left coronary arteries was performed in addition to aortic valve replacement (Bentall procedure). The patient had an uneventful postoperative course and remains asymptomatic at the three-month follow-up visit.

Giant Adult Mesenteric Lipoma: A Rare Cause of Chronic Abdominal Distention and Discomfort.

Project description:Solitary or multiple lipomas are considered common tumors that can occur anywhere in the body; however, mesenteric lipoma is a rare entity that is well known to present with signs and symptoms of small bowel volvulus. Hereby, we present a case of a 54-year-old male patient with multiple comorbidities who was suffering from chronic abdominal discomfort and gradual increase of his abdominal distention over many years without seeking any medical attention. The patient was seen by a general practitioner after complaining of an inflated abdomen, as he described his condition. After several imaging studies, he was diagnosed with one of the largest mesenteric lipomas in the literature. Mesenteric lipoma should be present in the differential diagnosis of any abdominal tumor. Magnetic resonance imaging plays a major role in differentiating benign from malignant lipomas.

Pulmonary Artery Stenosis due to Lung Carcinoma: A Rare Cause of Dyspnea.

Project description:Acquired bilateral pulmonary artery stenosis in adults due to lung malignancy is infrequently reported. We describe an adult male who presented to us with chief complaints of dyspnea on exertion and one episode of hemoptysis. Two dimensional transthoracic echocardiography with color Doppler showed presence of an extra cardiac mass causing severe extrinsic compression of both the right and left pulmonary artery leading to high pressure severe tricuspid regurgitation and extension of the mass into the left atrium. Three dimensional transthoracic echocardiography clearly delineated the anatomy of the left atrial mass and its surrounding anatomical relationship. The diagnosis of non small cell lung carcinoma was confirmed by multidetector computed tomography (MDCT) and with MDCT guided biopsy with histopathology. Patient succumbed one month later due to an episode of massive hemoptysis.

An unusual cause of exertional dyspnea in a 55 years old man.

Project description:A 55-year-old former professional athlete reported out of proportion dyspnea on exertion. After a detailed cardiac investigation, a cardiopulmonary exercise test on an ergocycle demonstrated an abnormal and non-physiological ventilatory response characterized by a sharp rise in ventilation followed by a decrease while exercise workload was progressively increasing. This was accompanied by noisy breathing. A laryngoscopy with direct visualisation of larynx and vocal cord during voluntary eucapnic hyperventilation confirmed the diagnosis of exercise-induced laryngeal obstruction. The patient was treated with speech therapy and all the symptoms resolved. A second cardiopulmonary exercise test showed a normalisation of the ventilatory pattern during exercise. This case demonstrates the importance of recognizing the symptoms of an exercise-induced laryngeal obstruction regardless of age, and the effectiveness of the speech therapy on symptoms and on exercise testing.

Case Report: An Unusual Cause for Recurrent Hemopericardium in a Patient With Dyspnea.

Project description:The case concerns a female presenting with dyspnoea resulting from recurrent hemopericardium. Pericardiocentesis, coronary angiography, and extensive laboratory and imaging studies did not reveal the underlying etiology of the hemopericardium. Only after repeat and exploratory surgery, diffuse venous pericardial hemorrhages with localized thrombi typical of angiosarcoma were discovered.

A rare cause of right ventricle out flow tract obstruction: Anterior mediastinal teratoma.

Project description:A 36 years old female presented with six months history of shortness of breath, fatigue, and tiredness. Her chest X-Ray showed a left mediastinal mass. A computed tomographic scan (CT)of the chest revealed a left mediastinal mass, exhibiting typical teratoma features. An echocardiogram showed compression of the main pulmonary artery causing right ventricular out flow tract obstruction. The tumor was resected through a left thoracotomy, and the patient improved dramatically after surgery. She was discharged home for further follow-up.

Torsion of a giant antimesenteric lipoma of the sigmoid: a rare cause of acute abdomen.

Project description:Lipomas of the gastrointestinal tract are uncommon benign tumors of mature adipocytes and may occur in any portion along the gut. Depending on location they may have a variety of clinical presentations and even simulate malignant neoplasms. We report a case of a 58-year-old woman who presented with acute pelvic pain. An emergency sonogram detected a hyperechogenic mass in the left adnexal region, with no vascularization on Doppler. A computed tomography confirmed the hypothesis of a fat containing tumor with signals of torsion. The patient underwent laparoscopy which depicted a mass over the antimesenteric side of the sigmoid with signs of ischemia and twisted vascular pedicle. The lesion was resected, and the microscopy confirmed the diagnosis of lipoma. The multidisciplinary team in the emergency room must be aware of these possible complications in order to optimize patient care.

Mitral-aortic Intervalvular Fibrosa Aneurysm with Rupture into Left Atrium: An Uncommon Cause of Acute Dyspnea.

Project description:Aneurysm of the mitral-aortic intervalvular fibrosa (MAIF) is an exceptionally rare but a potentially catastrophic complication, commonly following aortic valve endocarditis. We present a 24-year-old male presenting with acute onset dyspnea secondary to MAIF aneurysm rupturing into a left atrium causing large shunt which was diagnosed on echocardiography. The MAIF aneurysm in the absence of infective endocarditis rupturing into left atrium is extremely rare.
