Project description:ObjectiveCaregivers of children with Autism Spectrum Disorder (ASD) often experience emotional and psychological distress, as well as disruptions to family life and employment due to the challenges of caring for children with ASD. This study examines the relationship between problem behaviours and caregiver burden among children with ASD.MethodA cross-sectional study using convenience sampling recruited 230 caregivers of children with ASD aged 4 to 18 years from selected autism centres in Kuching, Sarawak. The caregivers completed the Aberrant Behaviour Checklist-2 and the Zarit Burden Interview.ResultsUnivariate analysis revealed a significant difference in caregiver burden for children with ASD receiving medications (p = 0.013), registered with the Social Welfare Department (p = 0.036), and having siblings with ASD (p = 0.046). About 40% of the children exhibited at least one domain of problem behaviour. More than half of the caregivers (53.9%) experienced burden, with the majority experiencing mild burden. Positive associations were seen between irritability (r = 0.458, p < 0.01), social withdrawal (r = 0.439, p < 0.01), stereotypic behaviour (r = 0.392, p < 0.01), hyperactivity/non-compliance (r = 0.467, p < 0.01), and caregiver burden. Child factors, including the duration of problem behaviour (r = 0.182, p = 0.007), medication use (eta = 0.187, p = 0.005), Social Welfare Department registration (eta = 0.138, p = 0.036), and the presence of siblings with ASD (eta = 0.130, p = 0.046) were associated with caregiver burden. Multiple linear regression showed that hyperactivity/noncompliance significantly predicted caregiver burden.ConclusionSpecific problem behaviours in children with ASD were associated with caregiver burden. These results highlight the need for interventions for the child with ASD and their caregivers.

Project description:Practical relevanceOne of the reasons why cats enjoy such a high level of popularity as domestic pets is the fact that they are clean. When there is a breakdown in this fastidious behaviour and elimination occurs outside of the litter box or tray, the strain on the cat-owner bond and on human relationships within the household can be considerable.Evidence baseIndoor elimination behaviour is one of the most common reasons for cat owners to seek professional advice and there is a wide range of articles, book chapters and research papers that reference it. In many cases the topic of urination and defecation in unacceptable indoor locations is considered in combination with the deposition of urine or faeces as a marker, but this review focuses purely on problematic elimination. An accompanying article in this special issue discusses urine spraying.Clinical challengesIndoor elimination problems necessitate a clinical approach combining knowledge from the fields of physical and emotional health and an understanding of normal feline behaviour. They also require comprehensive history-taking skills as well as effective communication skills and a degree of empathy for owners who are often finding their pet's behaviour very distressing.Global importanceEarly diagnosis of physical health disorders that are associated with indoor elimination is extremely important in terms of safeguarding feline welfare. As we become more aware of the interplay between physical and emotional health, the significance of identifying suboptimal social and physical environments in terms of optimising the welfare of domestic cats is also being recognised. The potential for indoor elimination problems to cause considerable human distress highlights the importance of the concepts of One Health and One Welfare.

Project description:Practical relevanceWhen a cat is presented for evaluation of a problem behaviour, it is likely that the cat's wellbeing is negatively affected by the condition. In addition, the owners and any other animals around the cat may also be experiencing negative consequences. When managing these cases, it is important to consider all options (including behaviour modification, environmental changes, medications) that can help to reach an optimal solution. Medication cannot teach the cat how to behave or change a particular behaviour; it can, however, reduce arousal, excitability, reactivity and anxiety.RationaleThe rationale for using psychoactive medications in behavioural medicine, or veterinary psychiatry, is to increase the wellbeing of the animal and to aid the owner and practitioner in managing problem behaviours. Medications should always be used as an adjunct to behavioural and environmental modification.Clinical challengesMany psychoactive medications cannot be used in the face of certain physical illnesses or concurrently with other medications. Some medications may also have side effects, not be effective at the recommended dose or have a paradoxical effect. Furthermore, success is reliant on the owner being able to administer the medication.AimsThis article aims to guide practitioners by discussing questions such as how to choose the appropriate medication, how to dose it and how long to use it. The psychoactive medications most commonly used in feline medicine are reviewed, as well as some that are newer or less common.Evidence baseData for the use of medications in cats is limited, with just a small number of clinical-, species- and problem-directed studies available, and a few more case series and case reports. Where feline-specific research is not available, the authors have drawn upon research published in other species, such as humans, dogs and rats, as well as anecdotal reports and expert opinions.

Project description:Despite many studies indicating an association between problem gambling and delinquent behaviours among adolescents, there has been no effort to systematically analyze the state of the literature on this relationship. To fill this gap, we conducted a scoping review of the literature published between 2000 and 2016 on problem gambling and delinquent behaviours among adolescents. We searched twelve databases and reviewed reference lists to identify eligible studies. Search terms included a combination of medical subject headings and keywords for gambling, youth, and delinquency, which were combined with the Boolean operator "AND". 1795 studies were identified through the literature search. Nine studies were eligible for inclusion. All of the studies were conducted in North America, with primarily male participants, and most of the data were cross-sectional. No qualitative studies met the inclusion criteria. Screening tools used to measure problem gambling were inconsistent, making comparisons across studies difficult. We found a consistent moderate to strong association between problem gambling and delinquent behaviour. Only one study presented associations by socio-economic status and none considered gender, sex or ethnic differences. Studies in the review showed that problem gambling is associated with both violent and non-violent behaviours among adolescents. These associations may suggest that problem gambling and delinquent behaviours have common risk factors and reflect a syndrome of risky behaviours best targeted through prevention and treatment that is holistic and considers the context in which the youth is situated. Further research is warranted to better understand the relationship between problem gambling and delinquent behaviours.

Hence gaining an understanding of how to engage clients to become involved in their pets' treatment, both medical and behavioural, is relevant to all who work in this field.Clinical challengesPoor communication skills can be responsible for owners' lack of compliance regarding both medical treatment and implementation of behavioural protocols.Evidence baseMuch of the evidence-based information on effective communication comes from the fields of human psychology and psychotherapy, human medicine and business, with only a limited amount coming from the veterinary field. Therefore, some opinions expressed in this article are based on the author's experience of treating cases in a cat-specific behaviour referral practice over the past 20 years and her work as a humanistic counsellor.AudienceAny veterinarians, veterinary nurses or technicians who are involved in the diagnosis or treatment of problem feline behaviour in practice would benefit from knowledge of what promotes and also hinders owner understanding and involvement.

Project description:Optimal sleep duration and quality is difficult to define. There are strong arguments for a relationship between sleep, in particular REM sleep, and emotional health and behaviour in a variety of species. This study aims to broaden the level of knowledge regarding canine sleep durations and characteristics and begin research into the relationship between behavioural responses and the duration and quality of sleep. A caregiver questionnaire was used to capture information regarding the duration and characteristics of canine sleep, how easily this cohort of dogs were disturbed from sleep, and caregiver perceptions of the severity of problem behaviours shown by this cohort (n = 1330). A quadratic relationship between canine sleep duration whilst a caregiver is in bed and severity of problem behaviour is shown, with less than 8 h sleep and more than 10 h sleep correlating with increased severity of problem behaviours in this cohort. Dogs which were more easily disturbed from sleep at times their caregiver was out of bed, showed increased reported severity of problem behaviours. Whilst it is not possible to determine an optimal canine sleep duration, sufficient evidence is presented to argue that problem behaviour should not be remedied by sleep deprivation.

As we have shown here, historically selected ‘household proteins’ may not be the most suitable for this task for technical or biological reasons. Project description:Obesity is reaching epidemic proportions in developed countries and represents a significant risk factor for hypertension, heart disease, diabetes, and dyslipidemia. Splicing mutations constitute at least 14% of disease-causing mutations, thus implicating polymorphisms that affect splicing as likely candidates for disease susceptibility. A recent study suggested that genes associated with obesity were significantly enriched for rare nucleotide variants. Here, we examined these variants and revealed that they are located near splice junctions and tend to affect exonic splicing regulatory sequences. We also show that the majority of the exons that harbor these SNPs are constitutively spliced, yet they exhibit weak splice sites, typical to alternatively spliced exons, and are hence suboptimal for recognition by the splicing machinery and prone to become alternatively spliced. Using ex vivo assays, we tested a few representative variants and show that they indeed affect splicing by causing a shift from a constitutive to an alternative pattern, suggesting a possible link between extreme body mass index and abnormal splicing patterns.

Project description:In children, necrotizing pneumonia (NP) is an uncommon, severe complication of pneumonia. It is characterized by destruction of the underlying lung parenchyma resulting in multiple small, thin-walled cavities and is often accompanied by empyema and bronchopleural fistulae.NP in children was first reported in children in 1994, and since then there has been a gradual increase in cases, which is partially explained by greater physician awareness and use of contrast computed tomography (CT) scans, and by temporal changes in circulating respiratory pathogens and antibiotic prescribing. The most common pathogens detected in children with NP are pneumococci and Staphylococcus aureus. The underlying disease mechanisms are poorly understood, but likely relate to multiple host susceptibility and bacterial virulence factors, with viral-bacterial interactions also possibly having a role. Most cases are in previously healthy young children who, despite adequate antibiotic therapy for bacterial pneumonia, remain febrile and unwell. Many also have evidence of pleural effusion, empyema, or pyopneumothorax, which has undergone drainage or surgical intervention without clinical improvement. The diagnosis is generally made by chest imaging, with CT scans being the most sensitive, showing loss of normal pulmonary architecture, decreased parenchymal enhancement and multiple thin-walled cavities. Blood culture and culture and molecular testing of pleural fluid provide a microbiologic diagnosis in as many as 50% of cases. Prolonged antibiotics, draining pleural fluid and gas that causes mass effects, and maintaining ventilation, circulation, nutrition, fluid, and electrolyte balance are critical components of therapy. Despite its serious nature, death is uncommon, with good clinical, radiographic and functional recovery achieved in the 5-6 months following diagnosis. Increased knowledge of NP's pathogenesis will assist more rapid diagnosis and improve treatment and, ultimately, prevention.It is important to consider that our understanding of NP is limited to individual case reports or small case series, and treatment data from randomized-controlled trials are lacking. Furthermore, case series are retrospective and usually confined to single centers. Consequently, these studies may not be representative of patients in other locations, especially when allowing for temporal changes in pathogen behaviour and differences in immunization schedules and antibiotic prescribing practices.
