Project description:INTRODUCTION:Good quality of care is dependent on nurses' strong clinical skills and moral competencies, as well. While most nurses work with high moral standards, the moral performance of some nurses in some organizations shows a deterioration in their moral sensitivity and actions. The study reported in this paper aimed to explore the experiences of nurses regarding negative changes in their moral practice. MATERIALS AND METHODS:This was a qualitative study utilizing an inductive thematic analysis approach, which was conducted from February 2017 to September 2019. Twenty-five nurses participated in semi-structured interviews. RESULTS:The main theme that emerged from our analysis was one of moral neutralization in the context of an unethical moral climate. We found five sub-themes, including: (1) feeling discouraged; (2) normalization; (3) giving up; (4) becoming a justifier; and (5) moral indifference. CONCLUSIONS:Unethical moral climates in health organizations can result in deterioration of morality in nurses which can harm both patients and health systems. Some unethical behaviors in nurses can be explained by this process.

Project description:Indirect reciprocity is a major mechanism in the maintenance of cooperation among unrelated individuals. Indirect reciprocity leads to conditional cooperation according to social norms that discriminate the good (those who deserve to be rewarded with help) and the bad (those who should be punished by refusal of help). Despite intensive research, however, there is no definitive consensus on what social norms best promote cooperation through indirect reciprocity, and it remains unclear even how those who refuse to help the bad should be assessed. Here, we propose a new simple norm called "Staying" that prescribes abstaining from assessment. Under the Staying norm, the image of the person who makes the decision to give help stays the same as in the last assessment if the person on the receiving end has a bad image. In this case, the choice about whether or not to give help to the potential receiver does not affect the image of the potential giver. We analyze the Staying norm in terms of evolutionary game theory and demonstrate that Staying is most effective in establishing cooperation compared to the prevailing social norms, which rely on constant monitoring and unconditional assessment. The application of Staying suggests that the strict application of moral judgment is limited.

Project description:Experimental studies using fictional moral dilemmas indicate that both automatic emotional processes and controlled cognitive processes contribute to moral judgments. However, not much is known about how people process socio-normative violations that are more common to their everyday life nor the time-course of these processes. Thus, we recorded participants' electrical brain activity while they were reading vignettes that either contained morally acceptable vs unacceptable information or text materials that contained information which was either consistent or inconsistent with their general world knowledge. A first event-related brain potential (ERP) positivity peaking at ∼200 ms after critical word onset (P200) was larger when this word involved a socio-normative or knowledge-based violation. Subsequently, knowledge-inconsistent words triggered a larger centroparietal ERP negativity at ∼320 ms (N400), indicating an influence on meaning construction. In contrast, a larger ERP positivity (larger late positivity), which also started at ∼320 ms after critical word onset, was elicited by morally unacceptable compared with acceptable words. We take this ERP positivity to reflect an implicit evaluative (good-bad) categorization process that is engaged during the online processing of moral transgressions.

Project description:Empathy refers to the ability to perceive and share another person's affective state. Much neuroimaging evidence suggests that observing others' suffering and pain elicits activations of the anterior insular and the anterior cingulate cortices associated with subjective empathetic responses in the observer. However, these observations do not provide causal evidence for the respective roles of anterior insular and anterior cingulate cortices in empathetic pain. Therefore, whether these regions are 'necessary' for empathetic pain remains unknown. Herein, we examined the perception of others' pain in patients with anterior insular cortex or anterior cingulate cortex lesions whose locations matched with the anterior insular cortex or anterior cingulate cortex clusters identified by a meta-analysis on neuroimaging studies of empathetic pain perception. Patients with focal anterior insular cortex lesions displayed decreased discrimination accuracy and prolonged reaction time when processing others' pain explicitly and lacked a typical interference effect of empathetic pain on the performance of a pain-irrelevant task. In contrast, these deficits were not observed in patients with anterior cingulate cortex lesions. These findings reveal that only discrete anterior insular cortex lesions, but not anterior cingulate cortex lesions, result in deficits in explicit and implicit pain perception, supporting a critical role of anterior insular cortex in empathetic pain processing. Our findings have implications for a wide range of neuropsychiatric illnesses characterized by prominent deficits in higher-level social functioning.

Project description:In contrast to a wealth of human studies, little is known about the ontogeny and consistency of empathy-related capacities in other species. Consolation-post-conflict affiliation from uninvolved bystanders to distressed others-is a suggested marker of empathetic concern in non-human animals. Using longitudinal data comprising nearly a decade of observations on over 3000 conflict interactions in 44 chimpanzees (Pan troglodytes), we provide evidence for relatively stable individual differences in consolation behaviour. Across development, individuals consistently differ from one another in this trait, with higher consolatory tendencies predicting better social integration, a sign of social competence. Further, similar to recent results in other ape species, but in contrast to many human self-reported findings, older chimpanzees are less likely to console than are younger individuals. Overall, given the link between consolation and empathy, these findings help elucidate the development of individual socio-cognitive and -emotional abilities in one of our closest relatives.Non-human animals are known to exhibit behaviours suggestive of empathy, but the development and maintenance of these traits is unexplored. Here, Webb and colleagues quantify individual consolation tendencies over 10 years across two chimpanzee groups and show evidence of consistent 'empathetic personalities'.

Project description:Empathetic verbal feedback from others has been shown to alleviate the intensity of experimental pain. To investigate the brain changes associated with this effect, we conducted 3T-fMRI measurements in 30 healthy subjects who received painful thermal stimuli on their left hand while overhearing empathetic, neutral or unempathetic comments, supposedly made by experimenters, via headsets. Only the empathetic comments significantly reduced pain intensity ratings. A whole-brain BOLD analysis revealed that both Empathetic and Unempathetic conditions significantly increased the activation of the right anterior insular and posterior parietal cortices to pain stimuli, while activations in the posterior cingulate cortex and precuneus (PCC/Prec) were significantly stronger during Empathetic compared to Unempathetic condition. BOLD activity increased in the DLPFC in the Empathetic condition and decreased in the PCC/Prec and vmPFC in the Unempathetic condition. In the Empathetic condition only, functional connectivity increased significantly between the vmPFC and the insular cortex. These results suggest that modulation of pain perception by empathetic feedback involves a set of high-order brain regions associated with autobiographical memories and self-awareness, and relies on interactions between such supra-modal structures and key nodes of the pain system.

Project description:In many biological populations, such as human groups, individuals face a complex strategic setting, where they need to make strategic decisions over a diverse set of issues and their behavior in one strategic context can affect their decisions in another. This raises the question of how the interaction between different strategic contexts affects individuals' strategic choices and social norms? To address this question, I introduce a framework where individuals play two games with different structures and decide upon their strategy in a second game based on their knowledge of their opponent's strategy in the first game. I consider both multistage games, where the same opponents play the two games consecutively, and reputation-based model, where individuals play their two games with different opponents but receive information about their opponent's strategy. By considering a case where the first game is a social dilemma, I show that when the second game is a coordination or anti-coordination game, the Nash equilibrium of the coupled game can be decomposed into two classes, a defective equilibrium which is composed of two simple equilibrium of the two games, and a cooperative equilibrium, in which coupling between the two games emerge and sustain cooperation in the social dilemma. For the existence of the cooperative equilibrium, the cost of cooperation should be smaller than a value determined by the structure of the second game. Investigation of the evolutionary dynamics shows that a cooperative fixed point exists when the second game belongs to coordination or anti-coordination class in a mixed population. However, the basin of attraction of the cooperative fixed point is much smaller for the coordination class, and this fixed point disappears in a structured population. When the second game belongs to the anti-coordination class, the system possesses a spontaneous symmetry-breaking phase transition above which the symmetry between cooperation and defection breaks. A set of cooperation supporting moral norms emerges according to which cooperation stands out as a valuable trait. Notably, the moral system also brings a more efficient allocation of resources in the second game. This observation suggests a moral system has two different roles: Promotion of cooperation, which is against individuals' self-interest but beneficial for the population, and promotion of organization and order, which is at both the population's and the individual's self-interest. Interestingly, the latter acts like a Trojan horse: Once established out of individuals' self-interest, it brings the former with itself. Importantly, the fact that the evolution of moral norms depends only on the cost of cooperation and is independent of the benefit of cooperation implies that moral norms can be harmful and incur a pure collective cost, yet they are just as effective in promoting order and organization. Finally, the model predicts that recognition noise can have a surprisingly positive effect on the evolution of moral norms and facilitates cooperation in the Snow Drift game in structured populations.

Project description:Moral rules allow humans to cooperate by indirect reciprocity. Yet, it is not clear which moral rules best implement indirect reciprocity and are favoured by natural selection. Previous studies either considered only public assessment, where individuals are deemed good or bad by all others, or compared a subset of possible strategies. Here we fill this gap by identifying which rules are evolutionary stable strategies (ESS) among all possible moral rules while considering private assessment. We develop an analytical model describing the frequency of long-term cooperation, determining when a strategy can be invaded by another. We show that there are numerous ESSs in absence of errors, which however cease to exist when errors are present. We identify the underlying properties of cooperative ESSs. Overall, this paper provides a first exhaustive evolutionary invasion analysis of moral rules considering private assessment. Moreover, this model is extendable to incorporate higher-order rules and other processes.

Project description:ObjectiveEmpathy and empathy education have been reviewed a number of times through systematic reviews and meta-analyses; however, the topic of 'empathetic communication' remains poorly understood when considering engaging in hospital-based research. Therefore, this scoping review aimed to explore the existing literature concerning empathetic communication in hospital settings and to evaluate the definitions presented.DesignScoping review.Data sourcesSystematic searches of the PubMed, CINAHL, Cochrane, PsycINFO, and PsycArticles databases were conducted.Study selectionAll English studies in which empathetic communication in hospital settings were explored. The search terms used included empathy, communication, hospital settings, providers, and consumers.Data extractionData were assessed through the use of a pre-set analysis tool.ResultsAfter conducting the searches, 419 articles were identified, of which 26 were included in this review. No single article specifically defined the term 'empathetic communication'; however, 33 unique definitions of 'empathy' were identified, of which 23 considered communication to be a component of empathy. There was a considerable lack of consistency between the empathy definitions, with some classifying communication in empathy as an ability and others classifying it as a dynamic process.ConclusionFuture and contextually focused research is needed to develop a consistent and clear definition of empathetic communication and empathy within a hospital setting to better build positive healthcare cultures.Practice implicationsInconsistencies between definitions of empathy in empathetic communication research could reduce the efficacy of future research gains and impact the translation of research findings into clinical practice.

Project description:Facial electromyography research shows that corrugator supercilii ("frowning muscle") activity tracks the emotional valence of linguistic stimuli. Grounded or embodied accounts of language processing take such activity to reflect the simulation or "reenactment" of emotion, as part of the retrieval of word meaning (e.g., of "furious") and/or of building a situation model (e.g., for "Mark is furious"). However, the same muscle also expresses our primary emotional evaluation of things we encounter. Language-driven affective simulation can easily be at odds with the reader's affective evaluation of what language describes (e.g., when we like Mark being furious). To examine what happens in such cases, we independently manipulated simulation valence and moral evaluative valence in short narratives. Participants first read about characters behaving in a morally laudable or objectionable fashion: this immediately led to corrugator activity reflecting positive or negative affect. Next, and critically, a positive or negative event befell these same characters. Here, the corrugator response did not track the valence of the event, but reflected both simulation and moral evaluation. This highlights the importance of unpacking coarse notions of affective meaning in language processing research into components that reflect simulation and evaluation. Our results also call for a re-evaluation of the interpretation of corrugator EMG, as well as other affect-related facial muscles and other peripheral physiological measures, as unequivocal indicators of simulation. Research should explore how such measures behave in richer and more ecologically valid language processing, such as narrative; refining our understanding of simulation within a framework of grounded language comprehension.
