Anterior Loculated Pericardial Effusion Misinterpreted as Right Heart Dilation Suggesting Pulmonary Embolism.

ABSTRACT: We report a case of anterior loculated pericardial effusion misinterpreted on point-of-care ultrasound as a dilated right ventricle, and suggesting diagnosis of pulmonary embolism (PE), in a patient with renal failure. The compressed right ventricle from tamponade physiology appeared to be a thickened intraventricular septum. Heparin was given empirically for presumed PE. Later the same day, computed tomography of the chest showed the effusion, as did formal echocardiogram. The patient had drainage of 630 milliliters of fluid and recovered from tamponade. Loculated effusions comprise 15% of all pericardial effusions, and misdiagnosis of PE with heparin therapy could be fatal.

Project description:Pericardial effusions can occur as either circumferential or loculated when referencing their anatomic distribution in the pericardium. These effusions can result from multiple different etiologies, including malignancy, infection, trauma, connective tissue disease, acute pericarditis drug induced, or idiopathic. Loculated pericardial effusions can be difficult to manage. Even small loculated effusions can result in hemodynamic compromise. Oftentimes in the acute setting, point of care ultrasound can be used to evaluate pericardial effusions directly at the bedside. We present a case of a malignant loculated pericardial effusion and offer insight into management and clinical evaluation using point of care ultrasound.

Gene expression in rat right ventricles during chronic pulmonary embolism

Project description:Pulmonary vascular occlusions due to thromboemboli can result in pulmonary hypertension and right heart damage. Treatments to clear the vascular obstructions such as i.v. heparain or thrombolytics can resolve the hypertension but right ventricular damage often occurs first. Methods of protecting the right ventricle from hypertensive damage during the course of acute treatment to clear the thromboemboli are needed. Monocyte- and neutrophil-mediated inflammation and fibrosis are associated with chronic right ventricular damage but the pathways involved are not understood. A comprehesive survey of gene expression during chronic pulmonary embolism verses control rats has been conducted in this study.

Right ventricular variants and pulmonary embolism-association or coincidence?

Project description:It has been stated that the interior of the right ventricle is as unique to each individual as one's fingerprint. This statement is backed by numerous publications which demonstrates considerable variation in the number, shape and configuration of papillary muscles inside the normal right ventricle.It has also been shown that these variants may be the cause of cardiac rhythm disorders.In this case report another potential complication of such right ventricular papillary muscle variants is proposed-these muscles may be the source of pulmonary emboli.The pathogenesis may be that of local stasis around these aberrant muscular structures and/or emboli may form inside the right ventricle as a result of cardiac rhythm disorders, induced by these muscles.It is proposed that in future the role of the right ventricle as the source of pulmonary emboli will become more apparent and an important part of the diagnostic work up in cases of idiopathic pulmonary embolism.

Malignant Pericardial Effusion: A Systematic Review.

Project description:BackgroundMalignant pericardial effusion (Eff) is often asymptomatic and has an unknown prevalence, due to its occult presentation. The condition often is identified postmortem on autopsy, and it is associated with a poor prognosis. Given the late presentation of malignant pericardial Effs, a minimal volume of literature has examined the epidemiology, clinical characteristics, and outcomes of these complex patients. We conducted a systematic review to advance present understanding of this condition.MethodsA search of 4 databases resulted in 41 case reports meeting criteria. Inclusion criteria were being a patient aged > 18 years who presented with pericardial Eff in the setting of malignancy. Intervention was medical and/or surgical therapy, and the outcome was mortality.ResultsFor the 41 patients included, the median age was 54 years, and the majority were male patients (58%). Dyspnea was the leading symptom (90%), and cardiac tamponade was present in 78% of cases. Common cancers included lung, gastrointestinal, and renal neoplasms (59%). Pericardiocentesis occurred in 98% of cases, with a median fluid extraction volume of 1000 mL. Death occurred in 44%, primarily due to disease progression and/or metastasis.ConclusionsThis study presents the largest systematic review on malignancy-induced pericardial Effs to date. Notably, solid tumours, and specifically lung adenocarcinomas, are common culprits. Malignant pericardial Effs are often severe, with a majority of patients presenting with cardiac tamponade. Overall, treatment options are limited, and the associated mortality rate is high.

Hemodynamic intolerance and pericardial effusion associated with high-frequency jet ventilation during pulmonary vein isolation.

Project description:BackgroundHigh-frequency jet ventilation (HFJV) is used during pulmonary vein isolation (PVI) to increase catheter stability and improve outcomes. In prior studies, hemodynamic intolerance to HFJV was rare.ObjectivesTo evaluate the incidence of hemodynamic or respiratory intolerance of HFJV during PVI.MethodsRetrospective observational analysis of consecutive patients undergoing PVI performed by 2 operators (PT, JW) at our institution between February 2019 and June 2020 who developed persistent hypotension or abnormal ventilatory parameters in association with HFJV.ResultsAmong 194 PVIs, there were 8 cases (4%) of conversion from HFJV to conventional ventilation, 6 for refractory hypotension and 2 for persistently abnormal gas exchange. In 6 patients, including 5 of the 6 patients with refractory hypotension, a new, small pericardial effusion without tamponade was noted just after HFJV was initiated. In patients with persistent hypotension, a decrease in left ventricular filling and systolic function was frequently noted. Both the hemodynamic changes and effusion resolved almost immediately after discontinuation of HFJV. In 4 patients rechallenged with HFJV, the hypotension and/or effusion recurred quickly and again resolved immediately after return to conventional ventilation.ConclusionHFJV-associated hypotension and systolic dysfunction, often accompanied by a transient pericardial effusion, is present in a small proportion of patients undergoing PVI, and resolves with cessation of HFJV. The mechanism of these changes is unclear and warrants further study.
