Using data from the UN World Population Prospects, we document global trends in lifespan inequality from 1950 until 2015. Project description:Current measures of population health lack indicators capturing the variability in age-at-morbidity onset, an important marker to assess the timing patterns of individuals' health deterioration and evaluate the compression of morbidity. We provide global, regional, and national estimates of the variability in morbidity onset from 1990 to 2019 using indicators of healthy lifespan inequality (HLI). Using data from the Global Burden of Disease Study 2019, we reconstruct age-at-death distributions to calculate lifespan inequality (LI), and age-at-morbidity onset distributions to calculate HLI. We measure LI and HLI with the standard deviation. Between 1990 and 2019, global HLI decreased from 24.74 years to 21.92, and has been decreasing in all regions except in high-income countries, where it has remained stable. Countries with high HLI are more present in sub-Saharan Africa and south Asia, whereas low HLI values are predominant in high-income countries and central and eastern Europe. HLI tends to be higher for females than for males, and HLI tends to be higher than LI. Globally, between 1990 and 2019 HLI at age 65 increased from 6.83 years to 7.44 for females, and from 6.23 to 6.96 for males. Improvements in longevity are not necessarily accompanied by further reductions in HLI among longevity vanguard countries. Morbidity is compressing, except in high-income countries, where it stagnates. The variability in the ages at morbidity onset tends to be larger than the variability in lifespans, and such divergence broadens over time. As longevity increases worldwide, the locus of health inequality is moving from death-related inequalities to disease- and disability-centered ones.

Project description:Since the World War II, many economies have transitioned from an agrarian, biomass-based to an industrial, minerals-based metabolic regime. Since 1950, world population grew by factor 2.7 and global material consumption by factor 3.7-71 Gigatonnes per year in 2010. The expansion of the resource base required by human societies is associated with growing pressure on the environment and infringement on the habitats of other species. In order to achieve a sustainability transition, we require a better understanding of the currently ongoing metabolic transition and its potential inertia. In this article, we present a long-term global material flow dataset covering material extraction, trade, and consumption of 177 individual countries between 1950 and 2010. We trace patterns and trends in material flows for six major geographic and economic country groupings and world regions (Western Industrial, the (Former) Soviet Union and its allies, Asia, the Middle East and Northern Africa, Latin America and the Caribbean, and Sub-Saharan Africa) as well as their contribution to the emergence of a global metabolic profile during a period of rapid industrialization and globalization. Global average material use increased from 5.0 to 10.3 tons per capita and year (t/cap/a) between 1950 and 2010. Regional metabolic rates range from 4.5 t/cap/a in Sub-Saharan Africa to 14.8 t/cap/a in the Western Industrial grouping. While we can observe a stabilization of the industrial metabolic profile composed of relatively equal shares of biomass, fossil energy carriers, and construction minerals, we note differences in the degree to which other regions are gravitating toward a similar form of material use. Since 2000, Asia has overtaken the Western Industrial grouping in terms of its share in global resource use although not in terms of its per capita material consumption. We find that at a sub-global level, the roles of the world regions have changed. There are, however, no signs yet that this will lead to stabilization or even a reduction of global resource use.

Project description:BackgroundPhysical inactivity is a global pandemic associated with a high burden of disease and premature mortality. There is also a trend in growing economic inequalities which impacts population health. There is no global analysis of the relationship between income inequality and population levels of physical inactivity.MethodsTwo thousand sixteen World Health Organisation's country level data about compliance with the 2010 global physical activity guidelines were analysed against country level income interquantile ratio data obtained from the World Bank, OECD and World Income Inequality Database. The analysis was stratified by country income (Low, Middle and High) according to the World Bank classification and gender. Multiple regression was used to quantify the association between physical activity and income inequality. Models were adjusted for GDP and percentage of GDP spent on health care for each country and out of pocket health care spent.ResultsSignificantly higher levels of inactivity and a wider gap between the percentage of women and men meeting global physical activity guidelines were found in countries with higher income inequality in high and middle income countries irrespective of a country wealth and spend on health care. For example, in higher income countries, for each point increase in the interquantile ratio data, levels of inactivity in women were 3.73% (CI 0.89 6.57) higher, levels of inactivity in men were 2.04% (CI 0.08 4.15) higher and the gap in inactivity levels between women and men was 1.50% larger (CI 0.16 2.83). Similar relationships were found in middle income countries with lower effect sizes. These relationships were, however, not demonstrated in the low-income countries.ConclusionsEconomic inequalities, particularly in high- and middle- income countries might contribute to physical inactivity and might be an important factor to consider and address in order to combat the global inactivity pandemic and to achieve the World Health Organisation target for inactivity reduction.

Project description:OBJECTIVE:To advance the goal of "Grand Convergence" in global health by 2035, this study tested the convergence hypothesis in the progress of the health status of individuals from 193 countries, using both standard and cutting-edge convergence metrics. METHODS:The study used multiple data sources. The methods section is categorized into two parts. (1) Health inequality measures were used for estimating inter-country inequalities. Dispersion Measure of Mortality (DMM) is used for measuring absolute inequality and Gini Coefficient for relative inequality. (2) We tested the standard convergence hypothesis for the progress in Infant Mortality Rate (IMR) and Life Expectancy at Birth (LEB) during 1950 to 2015 using methods ranging from simple graphical tools (catching-up plots) to standard parametric (absolute β and σ-convergence) and nonparametric econometric models (kernel density estimates) to detect the presence of convergence (or divergence) and convergence clubs. FINDINGS:The findings lend support to the "rise and fall" of world health inequalities measured using Life Expectancy at Birth (LEB) and Infant Mortality Rate (IMR). The test of absolute β-convergence for the entire period and in the recent period supports the convergence hypothesis for LEB (β = -0.0210 [95% CI -0.0227 - -0.0194], p<0.000) and rejects it for IMR (β = 0.0063 [95% CI 0.0037-0.0089], p<0.000). However, results also suggest a setback in the speed of convergence in health status across the countries in recent times, 5.4% during 1950-55 to 1980-85 compared to 3% during 1985-90 to 2010-15. Although inequality based convergence metrics showed evidence of divergence replacing convergence during 1985-90 to 2000-05, from the late 2000s, divergence was replaced by re-convergence although with a slower speed of convergence. While the non-parametric test of convergence shows an emerging process of regional convergence rather than global convergence. CONCLUSION:We found that with a current rate of progress (2.2% per annum) the "Grand convergence" in global health can be achieved only by 2060 instead of 2035. We suggest that a roadmap to achieve "Grand Convergence" in global health should include more radical changes and work for increasing efficiency with equity to achieve a "Grand convergence" in health status across the countries by 2035.

Project description:BackgroundNational, regional and global scientific production and research capacity for physical activity - PA may contribute to improving public health PA policies and programs. There is an uneven distribution of research productivity by region and country income group, where countries with the highest burden of non-communicable diseases attributable to physical inactivity having low research productivity. A first step towards improving global research capacity is to objectively quantify patterns, trends, and gaps in PA research. This study describes national, regional and global trends and patterns of PA research from 1950 to 2019.MethodsA systematic review using searches in PubMed, SCOPUS and ISI Web of Knowledge databases was conducted in August 2017 and updated between January and May 2020. The review was registered at the PROSPERO database number CRD42017070153. PA publications per 100,000 inhabitants per country was the main variable of interest. Descriptive and time-trend analyses were conducted in STATA version 16.0.ResultsThe search retrieved 555,468 articles of which 75,756 were duplicates, leaving 479,712 eligible articles. After reviewing inclusion and exclusion criteria, 23,860 were eligible for data extraction. Eighty-one percent of countries (n = 176) had at least one PA publication. The overall worldwide publication rate in the PA field was 0.46 articles per 100,000 inhabitants. Europe had the highest rate (1.44 articles per 100,000 inhabitants) and South East Asia had the lowest (0.04 articles per 100,000 inhabitants). A more than a 50-fold difference in publications per 100,000 inhabitants was identified between high and low-income countries. The least productive and poorest regions have rates resembling previous decades of the most productive and the richest.ConclusionThis study showed an increasing number of publications over the last 60 years with a growing number of disciplines and research methods over time. However, striking inequities were revealed and the knowledge gap across geographic regions and by country income groups was substantial over time. The need for regular global surveillance of PA research, particularly in countries with the largest data gaps is clear. A focus on the public health impact and global equity of research will be an important contribution to making the world more active.

Project description:BackgroundCurrent measures to monitor population health include indicators of (i) average length-of-life (life expectancy), (ii) average length-of-life spent in good health (health expectancy), and (iii) variability in length-of-life (lifespan inequality). What is lacking is an indicator measuring the extent to which healthy lifespans are unequally distributed across individuals (the so-called 'healthy lifespan inequality' indicators).MethodsWe combine information on age-specific survival with the prevalence of functional limitation or disability in Spain (2014-2017) by sex and level of education to estimate age-at-disability onset distributions. Age-, sex- and education-specific prevalence rates of adult individuals' daily activities limitations were based on the GALI index derived from Spanish National Health Surveys held in 2014 and 2017. We measured inequality using the Gini index.ResultsIn contemporary Spain, education differences in health expectancy are substantial and greatly exceed differences in life expectancy. The female advantage in life expectancy disappears when considering health expectancy indicators, both overall and across education groups. The highly educated exhibit lower levels of lifespan inequality, and lifespan inequality is systematically higher among men. Our new healthy lifespan inequality indicators suggest that the variability in the ages at which physical daily activity limitations start are substantially larger than the variability in the ages at which individuals die. Healthy lifespan inequality tends to decrease with increasing educational attainment, both for women and for men. The variability in ages at which physical limitations start is slightly higher for women than for men.ConclusionsThe suggested indicators uncover new layers of health inequality that are not traceable with currently existing approaches. Low-educated individuals tend to not only die earlier and spend a shorter portion of their lives in good health than their highly educated counterparts, but also face greater variation in the eventual time of death and in the age at which they cease enjoying good health-a multiple burden of inequality that should be taken into consideration when evaluating the performance of public health systems and in the elaboration of realistic working-life extension plans and the design of equitable pension reforms.

Project description:Economic inequality is notoriously difficult to quantify as reliable data on household incomes are missing for most of the world. Here, we show that a proxy for inequality based on remotely sensed nighttime light data may help fill this gap. Individual households cannot be remotely sensed. However, as households tend to segregate into richer and poorer neighborhoods, the correlation between light emission and economic thriving shown in earlier studies suggests that spatial variance of remotely sensed light per person might carry a signal of economic inequality. To test this hypothesis, we quantified Gini coefficients of the spatial variation in average nighttime light emitted per person. We found a significant relationship between the resulting light-based inequality indicator and existing estimates of net income inequality. This correlation between light-based Gini coefficients and traditional estimates exists not only across countries, but also on a smaller spatial scale comparing the 50 states within the United States. The remotely sensed character makes it possible to produce high-resolution global maps of estimated inequality. The inequality proxy is entirely independent from traditional estimates as it is based on observed light emission rather than self-reported household incomes. Both are imperfect estimates of true inequality. However, their independent nature implies that the light-based proxy could be used to constrain uncertainty in traditional estimates. More importantly, the light-based Gini maps may provide an estimate of inequality where previously no data were available at all.

Project description:Understanding the causes of economic inequality is critical for achieving equitable economic development. To investigate whether global warming has affected the recent evolution of inequality, we combine counterfactual historical temperature trajectories from a suite of global climate models with extensively replicated empirical evidence of the relationship between historical temperature fluctuations and economic growth. Together, these allow us to generate probabilistic country-level estimates of the influence of anthropogenic climate forcing on historical economic output. We find very high likelihood that anthropogenic climate forcing has increased economic inequality between countries. For example, per capita gross domestic product (GDP) has been reduced 17-31% at the poorest four deciles of the population-weighted country-level per capita GDP distribution, yielding a ratio between the top and bottom deciles that is 25% larger than in a world without global warming. As a result, although between-country inequality has decreased over the past half century, there is ∼90% likelihood that global warming has slowed that decrease. The primary driver is the parabolic relationship between temperature and economic growth, with warming increasing growth in cool countries and decreasing growth in warm countries. Although there is uncertainty in whether historical warming has benefited some temperate, rich countries, for most poor countries there is >90% likelihood that per capita GDP is lower today than if global warming had not occurred. Thus, our results show that, in addition to not sharing equally in the direct benefits of fossil fuel use, many poor countries have been significantly harmed by the warming arising from wealthy countries' energy consumption.

Project description:Dietary trends and changing lifestyle patterns have been associated with the increasing occurrence of obesity in the Western world. These dietary trends are commonly studied using longitudinal food consumption surveys. An alternative to studying changes in eating behaviour may be found in recipe analysis of traditional cookbooks. Few such studies exist, however, and it is unclear whether dietary trends over time can be identified this way. The present paper analyses full-meal recipes from a traditional Dutch cookbook between 1950 and 2010. The selected recipes show an increase in energy density. Additionally, the protein weight per kcal increased. In general, the observed trends are similar to those identified by the Dutch National Food Consumption Survey. The analysis therefore suggests that traditional cookbooks can be used as an indicator to identify dietary trends over time, although further studies are necessary to support this hypothesis.

Project description:A new database on historical country-level fishing fleet capacity and effort is described, derived from a range of publicly available sources that were harmonized, converted to fishing effort, and mapped to 30-min spatial cells. The resulting data is comparable with widely used but more temporally-limited satellite-sourced Automatic Identification System (AIS) datasets for large vessels, while also documenting important smaller fleets and artisanal segments. It ranges from 1950 to 2017, and includes information on number of vessels, engine power, gross tonnage, and nominal effort, categorized by vessel length, gear type and targeted functional groups. The data can be aggregated to Large Marine Ecosystem, region and/or fishing country scales and provides a temporally and spatially explicit source for fishing effort and fleet capacity for studies aimed at understanding the implications of long-term changes in fishing activity in the global ocean.
