Project description:Deficits in facial emotion recognition have frequently been established in temporal lobe epilepsy (TLE). However, static, rather than dynamic emotion recognition paradigms have been applied. Affective prosody has been insufficiently studied in TLE, and there is a lack of studies investigating associations between auditory and visual emotion recognition. We wished to investigate potential deficits in a dynamic morph task of facial emotion recognition and in an affective prosody recognition task, as well as associations between both tasks. 25 patients with TLE and 24 healthy controls (CG) performed a morph task with faces continuously changing in their emotional intensity. They had to press a button, as soon as they were able to recognize the emotion expressed, and label it accordingly. In the auditory task, subjects listened to neutral sentences spoken in varying emotional tones, and labeled the emotions. Correlation analyses were conducted across both tasks. TLE patients showed significantly reduced prosody recognition compared to CG, and in the morph task, there was a statistical trend towards significantly reduced performance for TLE. Recognition rates in both tasks were significantly associated. TLE patients show deficits in affective prosody recognition, and they may also be impaired in a morph task with dynamically changing facial expressions. Impairments in basic social-cognitive tasks in TLE seem to be modality-independent.

Project description:Background and objectiveAlthough avatars are now widely used in advertisement, entertainment, and business today, no study has investigated whether brain lesions in neurological patients interfere with brain activation in response to dynamic avatar facial expressions. The aim of our event-related fMRI study was to compare brain activation differences in people with epilepsy and controls during the processing of fearful and neutral dynamic expressions displayed by human or avatar faces.MethodsUsing functional magnetic resonance imaging (fMRI), we examined brain responses to dynamic facial expressions of trained actors and their avatar look-alikes in 16 people with temporal lobe epilepsy (TLE) and 26 controls. The actors' fearful and neutral expressions were recorded on video and conveyed onto their avatar look-alikes by face tracking.ResultsOur fMRI results show that people with TLE exhibited reduced response differences between fearful and neutral expressions displayed by humans in the right amygdala and the left superior temporal sulcus (STS). Further, TLE was associated with reduced response differences between human and avatar fearful expressions in the dorsal pathway of the face perception network (STS and inferior frontal gyrus) as well as in the medial prefrontal cortex.ConclusionsTaken together, these findings suggest that brain responses to dynamic facial expressions are altered in people with TLE compared to neurologically healthy individuals-regardless of whether the face is human or computer-generated. In TLE, areas sensitive to dynamic facial features and associated with processes relating to the self and others are particularly affected when processing dynamic human and avatar expressions. Our findings highlight that the impact of TLE on facial emotion processing must be extended to artificial faces and should be considered when applying dynamic avatars in the context of neurological conditions.

Project description:Facial mimicry is commonly defined as the tendency to imitate-at a sub-threshold level-facial expressions of other individuals. Numerous studies support a role of facial mimicry in recognizing others' emotions. However, the underlying functional mechanism is unclear. A prominent hypothesis considers facial mimicry as based on an action-perception loop, leading to the prediction that facial mimicry should be observed only when processing others' facial expressions. Nevertheless, previous studies have also detected facial mimicry during observation of emotional bodily expressions. An emergent alternative hypothesis is that facial mimicry overtly reflects the simulation of an "emotion", rather than the reproduction of a specific observed motor pattern. In the present study, we tested whether blocking mimicry ("Bite") on the lower face disrupted recognition of happy expressions conveyed by either facial or body expressions. In Experiment 1, we tested participants' ability to identify happy, fearful and neutral expressions in the Bite condition and in two control conditions. In Experiment 2, to ensure that such a manipulation selectively affects emotion recognition, we tested participants' ability to recognize emotional expressions, as well as the actors' gender, under the Bite condition and a control condition. Finally, we investigated the relationship between dispositional empathy and emotion recognition under the condition of blocked mimicry. Our findings demonstrated that blocking mimicry on the lower face hindered recognition of happy facial and body expressions, while the recognition of neutral and fearful expressions was not affected by the mimicry manipulation. The mimicry manipulation did not affect the gender discrimination task. Furthermore, the impairment of happy expression recognition correlated with empathic traits. These results support the role of facial mimicry in emotion recognition and suggest that facial mimicry reflects a global sensorimotor simulation of others' emotions rather than a muscle-specific reproduction of an observed motor expression.

Project description:ImportanceImplementing multidisciplinary teams for treatment of complex brain tumors needing awake craniotomies is associated with significant costs. To date, there is a paucity of analysis on the cost utility of introducing advanced multidisciplinary standardized teams to enable awake craniotomies.ObjectiveTo assess the cost utility of introducing a standardized program of awake craniotomies.Design, setting, and participantsA retrospective economic evaluation was conducted at Mayo Clinic Florida. All patients with single, unilateral lesions who underwent elective awake craniotomies between January 2016 and December 2021 were considered eligible for inclusion. The economic perspective of the health care institution and a time horizon of 1 year were considered. Data were analyzed from October 2022 to May 2023.ExposureTreatment with an awake craniotomy before standardization (2016-2018) compared with treatment with awake craniotomy after standardization (2018-2021).Main outcomes and measuresPatient demographics, perioperative, and postoperative outcomes, including length of stay, intensive care (ICU) admission, extent of resection, readmission rates, and 1-year mortality were compared between patients undergoing surgery before and after standardization. Direct medical costs were estimated from Medicare reimbursement rates for all billed procedures. A cost-utility analysis was performed considering differences in direct medical costs and in 1-year mortality within the periods before and after standardization of procedures. Uncertainty was explored in probability sensitivity analysis.ResultsA total of 164 patients (mean [SD] age, 49.9 [15.7] years; 98 [60%] male patients) were included in the study. Of those, 56 underwent surgery before and 108 after implementation of procedure standardization. Procedure standardization was associated with reductions in length of stay from a mean (SD) of 3.34 (1.79) to 2.46 (1.61) days (difference, 0.88 days; 95% CI, 0.33-1.42 days; P = .002), length of stay in ICU from a mean (SD) of 1.32 (0.69) to 0.99 (0.90) nights (difference, 0.33 nights; 95% CI, 0.06-0.60 nights; P = .02), 30-day readmission rate from 14% (8 patients) in the prestandardization cohort to 5% (5 patients) (difference, 9%; 95% CI, 19.6%-0.3%; P = .03), while extent of resection and intraoperative complication rates were similar between both cohorts. The standardized protocol was associated with mean (SD) savings of $7088.80 ($12 389.50) and decreases in 1-year mortality (dominant intervention). This protocol was found to be cost saving in 75.5% of all simulations in probability sensitivity analysis.Conclusions and relevanceIn this economic evaluation of standardization of awake craniotomy, there was a generalized reduction in length of stay, ICU admission time, and direct medical costs with implementation of an optimized protocol. This was achieved without compromising patient outcomes and with similar extent of resection, complication rates, and reduced readmission rates.

Project description:Nonverbal emotion recognition accuracy (ERA) is a central feature of successful communication and interaction, and is of importance for many professions. We developed and evaluated two ERA training programs-one focusing on dynamic multimodal expressions (audio, video, audio-video) and one focusing on facial micro expressions. Sixty-seven subjects were randomized to one of two experimental groups (multimodal, micro expression) or an active control group (emotional working memory task). Participants trained once weekly with a brief computerized training program for three consecutive weeks. Pre-post outcome measures consisted of a multimodal ERA task, a micro expression recognition task, and a task about patients' emotional cues. Post measurement took place approximately a week after the last training session. Non-parametric mixed analyses of variance using the Aligned Rank Transform were used to evaluate the effectiveness of the training programs. Results showed that multimodal training was significantly more effective in improving multimodal ERA compared to micro expression training or the control training; and the micro expression training was significantly more effective in improving micro expression ERA compared to the other two training conditions. Both pre-post effects can be interpreted as large. No group differences were found for the outcome measure about recognizing patients' emotion cues. There were no transfer effects of the training programs, meaning that participants only improved significantly for the specific facet of ERA that they had trained on. Further, low baseline ERA was associated with larger ERA improvements. Results are discussed with regard to methodological and conceptual aspects, and practical implications and future directions are explored.

Project description:In the current study we examined whether timeline-reversals and emotional direction of dynamic facial expressions affect subjective experience of human observers. We recorded natural movies of faces that increased or decreased their expressions of fear, and played them either in the natural frame order or reversed from last to first frame (reversed timeline). This led to four conditions of increasing or decreasing fear, either following the natural or reversed temporal trajectory of facial dynamics. This 2-by-2 factorial design controlled for visual low-level properties, static visual content, and motion energy across the different factors. It allowed us to examine perceptual consequences that would occur if the timeline trajectory of facial muscle movements during the increase of an emotion are not the exact mirror of the timeline during the decrease. It additionally allowed us to study perceptual differences between increasing and decreasing emotional expressions. Perception of these time-dependent asymmetries have not yet been quantified. We found that three emotional measures, emotional intensity, artificialness of facial movement, and convincingness or plausibility of emotion portrayal, were affected by timeline-reversals as well as by the emotional direction of the facial expressions. Our results imply that natural dynamic facial expressions contain temporal asymmetries, and show that deviations from the natural timeline lead to a reduction of perceived emotional intensity and convincingness, and to an increase of perceived artificialness of the dynamic facial expression. In addition, they show that decreasing facial expressions are judged as less plausible than increasing facial expressions. Our findings are of relevance for both, behavioral as well as neuroimaging studies, as processing and perception are influenced by temporal asymmetries.

Project description:Deficits in social cognition have been proposed as a marker of schizophrenia. Growing evidence suggests especially hyperfunctioning of the right posterior superior temporal sulcus (pSTS) in response to neutral social stimuli reflecting the neural correlates of social-cognitive impairments in schizophrenia. We characterized healthy participants according to schizotypy (n = 74) and the single-nucleotide polymorphism rs1344706 in ZNF804A (n = 73), as they represent risk variants for schizophrenia from the perspectives of personality traits and genetics, respectively. A social-cognitive fMRI task was applied to investigate the association of right pSTS hyperfunctioning in response to neutral face stimuli with schizotypy and rs1344706. Higher right pSTS activation in response to neutral facial expressions was found in individuals with increased positive (trend) and disorganization symptoms, as well as in carriers of the risk allele of rs1344706. In addition, a positive association between right-left pSTS connectivity and disorganization symptoms during neutral face processing was revealed. Although these findings warrant replication, we suggest that right pSTS hyperfunctioning in response to neutral facial expressions presents an endophenotype of schizophrenia. We assume that right pSTS hyperfunctioning is a vulnerability to perceive neutral social stimuli as emotionally or intentionally salient, probably contributing to the emergence of symptoms of schizophrenia.

Project description:Recognizing and distinguishing the emotional states of those around us is crucial for adaptive social behavior. Previous work has shown that damage to the ventromedial frontal lobe (VMF) impairs recognition of subtle emotional facial expressions and affects fixation patterns to face stimuli. However, whether this relates to deficits in acquiring or interpreting facial expression information remains unclear. We tested 37 patients with frontal lobe damage, including 17 subjects with VMF lesions, in a series of emotion recognition tasks with different gaze manipulations. Subjects were asked to rate neutral, subtle and extreme emotional expressions while freely examining faces, while instructed to look only at the eyes, and in a gaze-contingent condition that required top-down direction of eye movements to reveal the stimulus. People with VMF damage were worse at detecting subtle disgust during free viewing and confused extreme emotional expressions more than healthy controls. However, fixation patterns did not differ systematically between groups during free or gaze-contingent viewing conditions. Moreover, instruction to fixate only the eyes did not improve the performance of VMF damaged subjects. These data argue that VMF is not necessary for normal fixations to emotional face stimuli, and that impairments in emotion recognition after VMF damage do not stem from impaired information gathering, as indexed by patterns of fixation.

Project description:ObjectiveImpaired memory is a common comorbidity of refractory temporal lobe epilepsy (TLE) and often perceived by patients as more problematic than the seizures themselves. The objective of this study is to understand what the relationship of these behavioral impairments is to the underlying pathophysiology, as there are currently no treatments for these deficits, and it remains unknown what circuits are affected.MethodsWe recorded single neurons in the medial temporal lobes (MTLs) of 62 patients (37 with refractory TLE) who performed a visual recognition memory task to characterize the relationship between behavior, tuning, and anatomical location of memory selective and visually selective neurons.ResultsSubjects with a seizure onset zone (SOZ) in the right but not left MTL demonstrated impaired ability to recollect as indicated by the degree of asymmetry of the receiver operating characteristic curve. Of the 1973 recorded neurons, 159 were memory selective (MS) and 366 were visually selective (VS) category cells. The responses of MS neurons located within right but not left MTL SOZs were impaired during high-confidence retrieval trials, mirroring the behavioral deficit seen both in our task and in standardized neuropsychological tests. In contrast, responses of VS neurons were unimpaired in both left and right MTL SOZs. Our findings show that neuronal dysfunction within SOZs in the MTL was specific to a functional cell type and behavior, whereas other cell types respond normally even within the SOZ. We show behavioral metrics that detect right MTL SOZ-related deficits and identify a neuronal correlate of this impairment.SignificanceTogether, these findings show that single-cell responses can be used to assess the causal effects of local circuit disruption by an SOZ in the MTL, and establish a neural correlate of cognitive impairment due to epilepsy that can be used as a biomarker to assess the efficacy of novel treatments.

Project description:Do our background beliefs, desires, and mental images influence our perceptual experience of the emotions of others? In this paper, we will address the possibility of cognitive penetration (CP) of perceptual experience in the domain of social cognition. In particular, we focus on emotion recognition based on the visual experience of facial expressions. After introducing the current debate on CP, we review examples of perceptual adaptation for facial expressions of emotion. This evidence supports the idea that facial expressions are perceptually processed as wholes. That is, the perceptual system integrates lower-level facial features, such as eyebrow orientation, mouth angle etc., into facial compounds. We then present additional experimental evidence showing that in some cases, emotion recognition on the basis of facial expression is sensitive to and modified by the background knowledge of the subject. We argue that such sensitivity is best explained as a difference in the visual experience of the facial expression, not just as a modification of the judgment based on this experience. The difference in experience is characterized as the result of the interference of background knowledge with the perceptual integration process for faces. Thus, according to the best explanation, we have to accept CP in some cases of emotion recognition. Finally, we discuss a recently proposed mechanism for CP in the face-based recognition of emotion.
