Sexual victimization histories and substance use trajectories during the transition out of college.

ABSTRACT: The college years are a period of peak vulnerability for sexual victimization (SV) and substance misuse. During college, students with SV histories report riskier substance use patterns, yet little is known about the influence of SV on substance use behaviors as students begin to transition away from the college environment. This was the purpose of the present study. College seniors (N = 480; 61% female) reported on their alcohol and drug use behaviors across 5 time points spanning 1 calendar year. For many, this year included the transition out of college. Latent growth curve analysis was used to determine whether trajectories for alcohol and drug use as well as alcohol and drug consequences differed based on SV histories (no SV, precollege SV, college SV, precollege + college SV). Results revealed that at the start of senior year, young adults with SV histories reported greater substance use and consequences relative to nonvictimized peers. Over the year, SV histories were associated with steeper declines in substance use and consequences. Despite the declines, those who were revictimized across developmental time periods (precollege + college SV) consistently reported higher alcohol use and consequences across the transitional year, although this did not replicate for other drugs. In sum, although alcohol and other drug involvement among those with SV histories decreased over time, precollege + college SV histories continued to be a persistent risk factor for heavier alcohol use behaviors. Project description:IntroductionExposure to violence victimization may help explain disparities of substance use among gender-nonconforming youth (i.e., those whose gender expression differs from societal expectations).MethodsIn 2015, three large urban school districts (2 in California and 1 in Florida) conducted a Youth Risk Behavior Survey that included the assessment of gender expression among a racially/ethnically diverse population-based sample of 6,082 high school students. Five categories of violence victimization were assessed (felt unsafe at school, threatened or injured with a weapon at school, bullied at school, electronically bullied, and forced sexual intercourse). In 2019, the effect of violence victimization on substance use disparities was examined by calculating sex-stratified prevalence ratios by gender nonconformity, adjusted for sexual identity, race/ethnicity, and grade (adjusted prevalence ratio 1 [APR1]), and comparing these with prevalence ratios adjusted for those variables plus violence victimization (adjusted prevalence ratio 2 [APR2]).ResultsAmong female students, only being threatened or injured with a weapon was significantly (p<0.05) associated with gender nonconformity and there were no substance use disparities by gender nonconformity. Among male students, every category of violence victimization was more prevalent among gender-nonconforming than among gender-conforming students and most substance use categories demonstrated significant gender nonconformity disparities. After controlling for violence victimization, these disparities decreased but remained statistically significant for the use of cocaine (APR1=2.84 vs APR2=1.99), methamphetamine (APR1=4.47 vs APR2=2.86), heroin (APR1=4.55 vs APR2=2.96), and injection drug use (APR1=7.90 vs APR2=4.72).ConclusionsSchool-based substance use prevention programs may benefit from strategies that support gender diversity and reduce violence victimizations experienced by gender-nonconforming students, by providing a safe and supportive school environment.

Project description:Evidence on the prevalence of sexual aggression among college students is primarily based on studies from Western countries. In Chile, a South American country strongly influenced by the Catholic Church, little research on sexual aggression among college students is available. Therefore, the purpose of the present study was to examine the prevalence of sexual aggression victimization and perpetration since the age of 14 (the legal age of consent) in a sample of male and female students aged between 18 and 29 years from five Chilean universities (N = 1135), to consider possible gender differences, and to study the extent to which alcohol was involved in the reported incidents of perpetration and victimization. Sexual aggression victimization and perpetration was measured with a Chilean Spanish version of the Sexual Aggression and Victimization Scale (SAV-S), which includes three coercive strategies (use or threat of physical force, exploitation of an incapacitated state, and verbal pressure), three victim-perpetrator constellations (current or former partners, friends/acquaintances, and strangers), and four sexual acts (sexual touch, attempted sexual intercourse, completed sexual intercourse, and other sexual acts, such as oral sex). Overall, 51.9% of women and 48.0% of men reported at least one incident of sexual victimization, and 26.8% of men and 16.5% of women reported at least one incident of sexual aggression perpetration since the age of 14. For victimization, only few gender differences were found, but significantly more men than women reported sexual aggression perpetration. A large proportion of perpetrators also reported victimization experiences. Regarding victim-perpetrator relationship, sexual aggression victimization and perpetration were more common between persons who knew each other than between strangers. Alcohol use by the perpetrator, victim, or both was involved in many incidents of sexual aggression victimization and perpetration, particularly among strangers. The present data are the first to provide a systematic and detailed picture of sexual aggression among college students in Chile, including victimization and perpetration reports by both men and women and confirming the critical role of alcohol established in past research from Western countries.

Project description:ObjectiveDistress tolerance (DT), the ability to withstand aversive internal states, represents an important risk factor for substance use relapse and a potential treatment target. Neurobiological research in substance using populations suggests that continued substance use could erode DT, whereas abstinence could bolster it. The current study characterized trajectories of behavioral and self-reported indices of DT and examined the prospective effect of substance use on DT trajectories among those seeking treatment for substance use.MethodIndividuals (N = 263, Mage = 42.68, SD = 11.8, 70.7% male, 94.7% African American) in residential substance use treatment completed subjective (Distress Tolerance Scale) and behavioral (Mirror Tracing Persistence Task-computerized version) DT measures, as well as report of daily substance use (timeline follow-back) over 5 assessment time-points from pretreatment to 12 months posttreatment. Latent curve modeling estimated DT trajectories and their associations with substance use behavior, including abstinence duration (days until first use) and substance use frequency (percentage of substance use days between assessments).ResultsSelf-reported and behavioral DT indicators both exhibited positive, nonlinear change over time (standardized slope parameter estimates: Distress Tolerance Scale β = 0.61, p < .01; Mirror Tracing Persistence Task β = 0.34, p < .01). Abstinence duration was associated with greater improvement in behavioral (β = .20, p = .03) DT specifically. Frequency of use was statistically significantly associated with attenuated behavioral DT at 6-month (β = -.12, p = .03) and 12-month follow-ups (β = -.08, p = .045).ConclusionsDT appears to improve appreciably posttreatment, and return to substance use may shape the degree of this improvement. Collectively, these findings support the conceptualization of DT as a malleable treatment target and emphasize the benefit of abstinence on improvement in DT.
