ABSTRACT:

Objective

Objective of the study is to evaluate the on-admission day symptoms and signs, clinical, hematological parameters and liver transaminases of the dengue NS1 positive patients who got admitted on different clinical phases [Febrile phase (day 1-3) and Critical phase(day 4-5)] of dengue at medical wards of Jaffna Teaching Hospital.

Results

Blood samples were collected from 150 suspected dengue patients from day 1 to 5 of the illness. Seventy-eight patients were positive for dengue NS1, according to the WHO proposed dengue clinical phase framework 37 patients were from febrile phase and 41 patients from critical phase. Patients who admitted on critical phase framework suffered from leukopenia and thrombocytopenia. Nine patients had the evidence of leakage with fever and the leakers had significant rise in hemoglobin, hematocrit and liver transaminase levels which are considered as severe form of the disease.

Project description:BackgroundThe larval bionomics of Aedes across the Jaffna peninsula in northern Sri Lanka was investigated to obtain information needed for developing more effective larval source reduction measures to control endemic arboviral diseases.MethodsThe habitats of preimaginal stages of Aedes mosquitoes were surveyed, and ovitrap collections were carried out in densely populated areas of the Jaffna peninsula. Aedes larval productivities were analysed against habitat characteristics, rainfall and dengue incidence. Adults emerging from collected larvae were tested for dengue virus (DENV).ResultsOnly Aedes aegypti, Ae. albopictus and Ae. vittatus were identified in the field habitat collections and ovitraps. Aedes aegypti was the predominant species in both the field habitat and ovitrap collections, followed by Ae. albopictus and small numbers of Ae. vittatus. Tires and open drains were the preferred field habitats for Ae. aegypti, although larval productivity was higher in discarded plastic containers. The three Aedes species differed in field habitat preferences. Concomitant presence of the three Aedes species was observed in the field habitats and ovitraps. Larval productivities were inversely correlated with the salinity of the field habitat. Rainfall in the preceding month significantly correlated with larval productivity in the field habitats. DENV serotype 2 was detected in Ae. aegypti collected from ovitraps in the city of Jaffna. High Breteau, House and Container indices of 5.1, 5.1 and 7.9%, respectively, were observed in the field habitat surveys and ovitrap indices of up to 92% were found in Jaffna city.ConclusionsAedes larval indices in populated areas of the peninsula showed a high potential for dengue epidemics. Unacceptable littering practices, failure to implement existing dengue control guidelines, vertical transmission of DENV in vector mosquitoes and preimaginal development in brackish water and open surface drains, as well as in domestic wells that provide potable water, are serious constraints to the current Aedes larval source reduction methods used to control dengue in the Jaffna peninsula. Similar shortcomings in arboviral disease control are likely present in other resource-constrained tropical coastal zones worldwide.

Project description:Globally, the proportion of the elderly is increasing. In comparison to other Southeast Asian countries, Sri Lanka's population is rapidly aging. The elderly are a vulnerable age group that requires special attention to live a long and healthy life. As, there was a scarcity of data on the elderly's quality of life, studying the level of quality of life and the associated factors of the elderly in the Jaffna district will provide insight into how to plan interventions to improve the elderly's overall well-being in Jaffna District and Sri Lanka as well. The study aimed to determine the quality of life of the elderly in the Jaffna district of Sri Lanka and to study the association of socioeconomic factors with the quality of life. This cross-sectional study was conducted among 813 community-dwelling elderly in the Jaffna District of Sri Lanka. Socio-economic characteristics were recorded by way of a structured questionnaire. The WHOQOL-Bref questionnaire was used to assess quality of life in four domains: physical health, psychological, social participation and the environment. The statistical Package of Social Science Software (SPSS) version 21 was used to analyse the data. Univariate, bivariate, and multivariate analyses were applied, p-value less than 0.05 was considered statistically significant. Among the four QOL domains, the mean (SD) score for an environmental domain was (12.1±2.1), (12.0±2.8) for the psychological domain, (11.8±2.3) for the physical health domain, and (10.1±3.0) for the social relationship domain. Factors significantly associated with all domains of QOL included marital status, level of education, living arrangement, employment, level of income, income adequacy and ownership of the house. Furthermore, age, sex, religion, number of children, and presence of monthly income, were significantly associated with at least one domain of QOL of the elderly. According to these findings, the QOL of the elderly in the Jaffna district of Sri Lanka seems low. And it was associated with multiple socio-economic factors. Interventions to improve the QOL of the elderly are anticipated with a higher emphasis on social relationship for the elderly.
