Project description:BackgroundWorkplace genetic and/or genomic testing (wGT) is one of many options that employers can offer within the scope of voluntary workplace wellness programs, though we know little about how many employers are offering this benefit, or what kinds of testing are included.MethodsOur landscaping review sought to discover the prevalence and distribution of wGT within voluntary wellness programs among U.S. companies using three approaches: (1) analysis of publicly available information; (2) national surveys; and (3) interviews with company representatives.ResultsIn total, 50/420 (11.9%) companies we investigated had publicly available data suggesting that they offer wGT to their employees. Survey data weighted to be representative of the type and distribution of U.S. companies suggest that ~1% of U.S. companies offer wGT to their employees.ConclusionOur research found little evidence of broad uptake of wGT among U.S. companies, though information gathering was challenging.

Project description:BackgroundThis systematic review aimed to uncover the evidence and benefits of employers' commitment to delivering workplace wellness programs for working mothers.MethodsThe articles published in PubMed, Embase, Scopus, and AgeLine-Medline databases between 2012 and 2021 were searched to evaluate the workplace wellness programs for working mothers with at least one resultant wellness or wellbeing (e.g., physical health, less stress, mental health, burnout, depression, smoking, bullying, alcohol consumption, overweight), work-life balance outcome, or job satisfaction.ResultsEight studies that met the criteria were retrieved from databases. They showed some effective workplace wellness programs that can reduce depression, stress, and burnout, improve mental health, healthy behaviors, work-family balance and work-life balance. Working mothers participating in a workplace wellness program generally gain some benefits; one of which is reduced stress typically related to childcare, economic, and personal health issues.ConclusionsThe implementation of workplace wellness programs for working mothers showed positive effects on their health problems and health costs. These eight studies revealed that workplace wellness programs specifically designed for working mothers can lead to time efficiency by holding the programs in or near the workplace and implementing them during the workdays. This greatly suits the conditions of many working mothers whose limited time and energy to balance the household, family and work tasks.

Project description:ObjectiveEvidence-based obesity treatments, such as bariatric surgery, are not considered essential health benefits under the Affordable Care Act. Employer-sponsored wellness programs with incentives based on biometric outcomes are allowed and often used despite mixed evidence regarding their effectiveness. This study examines consumers' perceptions of their coverage for obesity treatments and exposure to workplace wellness programs.MethodsA total of 7,378 participants completed an online survey during 2015-2016. Respondents answered questions regarding their health coverage for seven medical services and exposure to employer wellness programs that target weight or body mass index (BMI). Using ?2 tests, associations between perceptions of exposure to employer wellness programs and coverage for medical services were examined. Differences between survey years were also assessed.ResultsMost respondents reported they did not have health coverage for obesity treatments, but more of the respondents with employer wellness programs reported having coverage. Neither the perception of coverage for obesity treatments nor exposure to wellness programs increased between 2015 and 2016.ConclusionsEven when consumers have exposure to employer wellness programs that target BMI, their health insurance often excludes obesity treatments. Given the clinical and cost-effectiveness of such treatments, reducing that coverage gap may mitigate obesity's individual- and population-level effects.

Project description:Background and objectivesEvidence suggests participation in evidence-based programs by older adults is effective, yet most studies focus on participation in a single evidence-based program, leaving repeated participation insufficiently understood. We aimed to compare participation in multiple evidence-based programs (repeaters) versus a single evidence-based program (nonrepeaters).Research design and methodsSecondary data analysis was conducted on pre-post longitudinal data targeting older adults participating in evidence-based program(s) in Texas (2013-2016). Surveys included sociodemographic and health-related indicators (e.g., self-rated health, health behaviors, and falls-risks). Mixed-effects models examined pre-post changes in health-related indicators.ResultsOf the 734 study-eligible participants, 145 (20%) participated in two or more evidence-based programs. The participants' average age was 74 years, and the majority was female (80%), non-Hispanic White (79%), or lived in urban or large rural cities/towns (79%). At baseline, repeaters reported less depressive symptomology (p = .049), fewer chronic conditions (p = .048), and less concern of falling (p = .030) than nonrepeaters. Repeaters had better workshop attendance and completion rates (p < .001). Compared to nonrepeaters, repeaters showed significantly-better improvements in communication with physicians (p = .013).Discussion and implicationsStudy findings suggest potential benefits of participation in multiple evidence-based program workshops, but repeaters may have different health profiles than nonrepeaters in natural settings. Future evaluations should consider participants' past participations in evidence-based programs. Further research is needed to build more comprehensive evidence about the incremental benefits of participation in multiple evidence-based programs.

Project description:Given the burden of cardiovascular disease (CVD), increasing the prevalence of healthy lifestyle choices is a global imperative. Currently, cardiac rehabilitation programs are a primary way that modifiable risk factors are addressed in the secondary prevention setting after a cardiovascular (CV) event/diagnosis. Even so, there is wide consensus that primary prevention of CVD is an effective and worthwhile pursuit. Moreover, continual engagement with individuals who have already been diagnosed as having CVD would be beneficial. Implementing health and wellness programs in the workplace allows for the opportunity to continually engage a group of individuals with the intent of effecting a positive and sustainable change in lifestyle choices. Current evidence indicates that health and wellness programs in the workplace provide numerous benefits with respect to altering CV risk factor profiles in apparently healthy individuals and in those at high risk for or already diagnosed as having CVD. This review presents the current body of evidence demonstrating the efficacy of worksite health and wellness programs and discusses key considerations for the development and implementation of such programs, whose primary intent is to reduce the incidence and prevalence of CVD and to prevent subsequent CV events. Supporting evidence for this review was obtained from PubMed, with no date limitations, using the following search terms: worksite health and wellness, employee health and wellness, employee health risk assessments, and return on investment. The choice of references to include in this review was based on study quality and relevance.

Project description:Workplace wellness programs are commonly offered by employers with the expectation that they will reduce health care costs. We previously reported that Be Fit, a 10-week wellness program offered to employees of a large teaching hospital and geared towards improving nutrition and physical activity, led to improvements in weight, serum cholesterol, and blood pressure at program completion and 1-year follow-up. In the present study we assessed whether Be Fit participation was associated with reduced health expenditures by employing a difference-in-differences analysis of claims data for 289 employees who participated in Be Fit between 2010 and 2014 and 194 controls from the same parent company matched on sex, age, health risk score, and baseline health expenditures. We compared changes in expenditures from the year prior to Be Fit participation to the year after initiating participation. We did not observe any inflection in health care expenditures at the time of Be Fit participation for participants or matched controls. After adjusting for matching characteristics, changes in quarterly health expenditures were $236 lower for participants compared to controls (95% CI -$640 to $168). We similarly found no evidence of reduced health expenditures when expenditures were capped at the 99th percentile (excluding the impact of outliers) or limited to expenditures for cardiovascular disease or diabetes. Despite improvements in clinical risk factors, we find no evidence that Be Fit was associated with reduced health expenditures over 1-year follow-up. Reducing health expenditures may require a longer time horizon and programs targeting a broader set of risk factors.

Project description:This study examines the effect of Income Support Programs (ISPs) on job search effort, work- place mobility, COVID-19 cases, and mortality growth rates. To identify ISPs' causal effect, I use the variation in their introductions' timing across countries and implement a difference-in-difference and multi-event analysis method. I find that ISPs led to a 4.4-8.29 percentage points reduction in workplace mobility and a 6.6-11.6 percentage points reduction in job search effort levels. They also caused a 21.8-47.7 and 17.1-29.7 percentage points reduction in the COVID-19 case growth rate and COVID-19 mortality growth rates, respectively. Using the event analysis estimates, I simulated the counterfactual job search effort, workplace mobility, and the number of COVID-19 cases and mortality without income support programs. The average global job search effort and workplace mobility without ISPs would have been 11.12 and 9.26 percent higher than the observed mean job search effort and workplace mobility. However, these would have come at the cost of 3.69 million and 166, 690 additional COVID-19 cases and mortality than the cases and deaths registered by May 15th.

Project description:BackgroundInvestments in settings-based health interventions can include workplaces, however, engaging with businesses and convincing them to take a role can be difficult. Our research investigated the potential for trade or industry associations (IAs) to have a role in promoting workplace health initiatives to their members.MethodsSeventeen semi-structured interviews were undertaken with senior executives from IAs representing industries in the mining, transport, agriculture, manufacturing, farming, hospitality, and construction sectors. Analysis of interviews identified themes around attitudes to workplace health promotion programs and the perceived, actual and potential role/s of IAs in promoting workplace wellness.ResultsIA representatives believed workplaces had potential to be promoting the health and wellbeing of workers through their member organisations; however for some the extent of their role was unclear and for others there was confusion between government-mandated safety initiatives and non-mandated health and wellbeing initiatives. All reported that their IA could have a role in promoting worker health and wellbeing initiatives to member organisations. IAs with larger companies as members were more likely to recognise the importance of workplaces promoting workers' health; however, the degree of involvement considered appropriate varied. Most IAs had not discussed the topic with their member organisations although they identified resources and support that could assist them in encouraging members to undertake workplace health programs. Resources included industry-relevant business cases outlining the benefits of workplace health, and industry-appropriate worker health information.ConclusionsOur research suggests that across many industry sectors, larger IAs in particular are ready to take a more active role in workplace health initiatives and are well placed to promote these to member organisations.

Project description:Overweight and obesity has reach pandemic levels, with two-thirds (67%) of adult Australians classified as overweight or obese. As two of the most significant behavioral risk factors for obesity are modifiable (diet and exercise), there exists an opportunity for treatment through workplace health promotion initiatives. As one of Australia's largest industries with its own unique workplace factors, the mining industry has previously reported higher than population levels of overweight and obesity. This represented an opportune setting to test the RESHAPE workplace wellness program. RESHAPE is an eight-step framework (based on the WHO 'Health Workplace Framework and Model') which aims to provide a sustained approach to wellness in the workplace. This paper presents baseline findings from a pilot study that aimed to implement RESHAPE at three mine sites in NSW, Australia, and investigates the issue of overweight and obesity in the coal mining industry. Across three mine sites, 949 coal miners were examined cross-sectionally on a range of workplace, wellness, health, diet, and exercise factors using a paper-based survey. This was a predominantly male sample (90.4%) with the majority (59.2%) of participants aged 25-44 years. Self-reported height and weight measures indicated that less than 20 percent (18.9%) of participants were in a healthy BMI range, while there were effectively equal numbers of overweight (40.9%) and obese (39.1%) participants. Only 3.5% of participants met the daily recommendation for vegetables (5 serves) and shift-workers had greater association with elevated BMI compared to non-shift workers (B = 1.21, 95% CI: 0.23, 2.20, p = 0.016). Poor nutrition is likely to be a key component in elevated levels of overweight and obesity within this industry, with workplace factors compounding challenges workers face in implementing health behavior change. Future studies would benefit from assessing diet and physical activity knowledge in relation to recommendations and serving sizes.

Project description:ImportanceSun safety attitudes developed in early childhood can reduce lifetime UV radiation exposure and the risk of skin cancer.ObjectiveTo assess the current policies, practices, and attitudes among caregivers regarding sun protection in children aged 2 to 6 years.Design, setting, and participantsA survey of 202 administrators or managers and teachers of Illinois Head Start/Early Head Start (HS/EHS) and day care centers was conducted from July 3 through 21, 2017. Organizations were randomly selected from 4 lists of urban, suburban, town, and rural locations and stratified to ensure population-based proportional representation of the Illinois population vulnerable to UV exposure. Program administrators or managers participated in a 5- to 10-minute telephone interview that assessed importance of health and sun protection behaviors, program practices, and demographic characteristics. Data analysis was conducted August 2, 2017.Main outcomes and measuresOutcomes included attitudes toward the importance of health behaviors in comparison with sun protective behaviors, reported use of sun protection (seeking shade, scheduling outdoor activities, sun protective clothing, and sunscreen practices), and sunburn prevalence.ResultsRespondents (from 102 HS/EHS programs with 52% boys overall and a mean [SD] child age of 2.5 [0.5] y and 100 day care programs with 49% boys and age of 2.3 [0.4] y) stated that the 3 most important habits were good nutrition (66 [64.7%] HS/EHS, 71 [71.0%] day care), adequate exercise (41 [40.2%] HS/EHS, 55 [55.0%] day care), and brushing teeth (35 [34.3%] HS/EHS, 38 [38.0 %] day care). Scheduling outdoor activities to avoid peak sun intensity was performed less by HS/EHS programs (46 [45.1%]) in comparison with day care programs (71 [71.0%]; P < .001). Sunscreen was provided for students in 109 programs, but 84 (77.1%) did not allow children to apply sunscreen themselves. Half of the programs (100 of 202 [50.0%]) used spray sunscreen to avoid unnecessarily touching children. Most programs did not report any children having sunburns (129 [63.9%]) and followed heat index guidelines (114 [56.4%]).Conclusions and relevanceAdministrators and teachers did not identify sun safety as one of the most important health habits. While spray sunscreen was used frequently, avoiding spraying sunscreen directly on a child's face, which may get it into the child's eyes, was not done. The heat index was widely used to determine outdoor playtime. Adopting UV index policies could help prevent sunburns in early spring when the heat index is low but UV index is high. Dermatologists may assume responsibility for educating the administrators about the danger of spraying sunscreen into a child's face and the advantage of using the UV index to determine when sun protection is needed.
