ABSTRACT: BACKGROUND:Trachoma, caused by Chlamydia trachomatis is the leading infectious cause of blindness. It is transmitted via personal contact with infected ocular and nasal secretions by hands, fomites and eye- seeking flies. Active trachoma is more common among children aged 1 to 9?years. The objective of this study was determining the prevalence of active trachoma and associated factors among children aged 1 to 9?years in rural community of Lemo district. METHODS:Community-based cross-sectional study was conducted from March to April, 2018 in rural community of Lemo district. Multistage sampling technique was used to select 589 study participants. Data were collected by using structured pre-tested questionnaire, physical examination and observation. Binocular loupe was used to identify active trachoma cases. Project description:BackgroundTrachoma is the leading infectious disease that leads to blindness worldwide, especially in developing countries. Though Ethiopia had targeted a trachoma elimination program by 2020, the problem worsens, particularly in the Amhara Region. Even though sustained intervention measures are undertaken across the region, it is unclear why trachoma is still a significant public health problem. So, this study assessed the prevalence of active trachoma and associated factors among 1-9 years of age children from model and non-model kebeles in Dangila district Amhara Region, Northwest Ethiopia.MethodsA community-based comparative cross-sectional study was conducted from 20th September 2019 to 29th October 2019. A multistage stratified random sampling technique was used to reach 704 children from model and non-model kebeles. Samples were allocated proportionally to model and non-model kebeles. A structured and pretested data collection tool and observational checklist was used to manage the necessary data. Data were coded and entered in Epidata version 4.6, and further analysis was done using SPSS version 20 software. Bivariable and multivariable logistic regression analysis was employed to identify factors associated with active trachoma. Adjusted Odds Ratios (AOR), p-value, and respected Confidence Interval (CI) were used to report the findings.ResultsSeven hundred four children were included in this study, with a response rate of 97.8%. The overall prevalence of active trachoma was 6% (95% CI: 4.5, 8.1). The prevalence of active trachoma among non-model and model Kebele was not significantly different. Still, the prevalence of active trachoma among children from model Kebele were [4.5%, (95% CI: 2.4%, 7.1%)] relatively lower compared with non-model kebeles, [7.6%, 95% CI: (4.9%, 10.9%)]. Moreover, not using latrine (AOR = 4.29, 95% CI: 1.96, 9.34), fly-eye contact (AOR = 2.59, 95% CI: 1.11, 6.03), presence of sleep in eyes (AOR = 2.46, 95% CI: 1.10, 5.47), presence of ocular discharge (AOR = 2.79, 95% CI: 1.30, 6.00), presence of nasal discharges (AOR = 2.67, 95% CI: 1.21, 5.90) and washing faces with soap (AOR = 0.22, 95% CI: 0.07, 0.69) were found significantly associated with the prevalence of active trachoma among children 1-9 years old.ConclusionsThe prevalence of active trachoma in the model and non-model kebeles was high and did not show a statistical difference. Attention to be given to latrine utilization, washing face with soap, and other personal hygiene activities.

Project description:BackgroundIn developing countries particularly in sub-Saharan Africa trachoma is still a public health concern. Ethiopia is the most affected of all and bears the highest burden of active trachoma. In spite of this, the prevalence of active trachoma among the pastoralist population in Ethiopia not yet disclosed. The aim of this study was to determine the prevalence of active trachoma and associated risk factors among children in a pastoralist population in Madda Walabu rural district, Ethiopia.MethodsA community-based cross-sectional study was conducted among children in a pastoralist population in Madda Walabu rural district, from May 1 to 30, 2017. A systematic sampling technique was employed to select 409 children's. Simplified WHO classification scheme was used to assess trachoma. Descriptive and logistic regression analyses were performed.ResultsA total of 406 children aged 1-9 years have participated, 89 (22%) [95%CI: 18.0-25.6%] were positive for active trachoma. Of these cases, 75(84%) had TI alone in one or both eyes, 14(16%) had TF alone in one or both eyes, and none of the children had both TI and TF. The odds of having active trachoma among children from households using river/ponds, unprotected well/spring and rainwater as their source of drinking water were higher than those from households using water from piped or public tap water (AOR:13,95%CI: 2.9, 58.2), (AOR: 6.1, 95%CI:1.0,36.5) and (AOR: 4.8, 95%CI:1.3,17.8) respectively. Children's from households that lacked a latrine (AOR: 2.5, 95% CI: 1.8, 5.3), children who did not wash their face by using soap (AOR: 4.3, 95% CI: 1.8, 10.6) and children from households within 16-30 min of water source (AOR: 8.7, 95% CI: 2.20, 34.2) were higher odds of having active trachoma.ConclusionsThe findings of this study revealed that close to one-quarter of the total children screened for trachoma were positive for the disease. The finding implies that trachoma is still a major concern among children of the pastoralist community which demands further attention of the district health office. Again, intervention with the A, F and E components of SAFE strategy is strongly recommended.

Project description:BackgroundTrachoma is a public health issue in more than 50 nations worldwide, mainly in Sub-Saharan Africa, where hundreds of millions of people are considered blind. Ethiopia is projected to have 30% of the global active trachoma burden. The frequency of Trachoma Folliculitis in children aged 1 to 9 years old is 30% in the Oromia Region. Therefore, the aim of this study was to assess the prevalence of active trachoma and associated variables among children aged 1 to 9 years old in Arsi Negele Town, West Arsi Zone, Oromia Regional State, Southern Ethiopia, December 24-26, 2019.MethodsA community-based cross-sectional study was conducted in the Arsi Negele town community on December 24-26, 2019. A total of 178 study volunteers were recruited using a single population proportion formula and assigned to families in the town's three kebeles in proportion. A simple random selection procedure was used to choose study participants from the identified households. Madda Walabu University provided ethical approval, and different government structures provided letters of permission. Pre-tested structured questionnaires and binocular loupes X 2.5 were used to collect data from either mothers or fathers of eligible children for eye examination; torches with bottles of alcohol were used to gather data from either mothers or fathers of eligible children for eye examination. For analysis, data was entered into (IBM, SPSS) version 22. To assess factors associated with active trachoma, bivariate and multivariable logistic regressions were used. The crude and adjusted odds ratios with 95% confidence intervals were calculated to investigate the degree of association between the independent variables and active trachoma. Multivariate logistic regression was used to find connections between dependent and independent variables with a p≤ 0.05 confidence levels and a 95% confidence interval.ResultThe prevalence of active trachoma was determined to be 21.91% TF among 178 children aged 1 to 9 years. Flies on children's faces (AOR = 3.427; 95 percent CI: 1.432-8.171), unclean children's faces (AOR = 3.99; 95 percent CI: 1.427-11.158), face washing habits (AOR = 3.064; 95 percent CI: 1.273-7.373), and not using soap while face washing (AOR = 4.564; 95 percent CI 1.561-13.342) were found to be statistically significant associated factors with the prevalence of active trachoma.ConclusionThe prevalence of active trachoma was found to be relatively high. Face washing practices and the lack of soap use while washing faces were found as associated factors requiring optimal interventions to prevent trachoma infection among children aged 1-9 years in Arsi Negele town.

Project description:ObjectiveThe objective of this study is to determine the pooled prevalence of active trachoma among 1-9 years old children in Ethiopia.DesignA systematic review and meta-analysis were employed in accordance with the Preferred Reporting Items for Systematic Reviews.Data sourcesMedline/PubMed, Scopus, Web of Science, African Journal of Online and Google scholar databases were systematically explored to find studies published in English until July 2023.Eligibility criteriaThe following criteria apply: (1) condition (Co): studies examined the prevalence of trachoma among children (1-9) years old; (2) context (Co): studies conducted in Ethiopia; (3) population (Pop): studies that were done among children (1-9) years old; (4) study type: observational studies and (5) language: studies published in English.Data extraction and synthesisThe data were extracted using a Microsoft Excel spreadsheet. DerSimonian-Laird random effect model was used to estimate the pooled prevalence of active trachoma among 1-9 years old children. Cochrane Q-tests and I2 statistics were used across studies to assess heterogeneity. To identify possible publication bias, Egger's test was performed.Primary outcomePrevalence of active trachoma among children aged (1-9 years old)".ResultsOverall, a total of 42 articles with 235 005 study participants were included in the final analysis. The estimated pooled prevalence of active trachoma using random effect model was 24% (95% CI 20% to 27%). The subgroup analysis by region revealed that the highest prevalence of trachoma was 36% (95% CI 13% to 58%) in the Tigray region, and publication year revealed the prevalence of trachoma was decreasing from 32% to 19% after 2015.ConclusionIn this review, the pooled prevalence of active trachoma was found to be high in Ethiopia compared with WHO threshold level. This underscores the need for increased focus on high-risk age groups to decrease trachoma and to achieve the elimination of trachoma from the country by 2030.

Project description:BackgroundTo eliminate blinding trachoma, the World Health Organization emphasizes implementing the SAFE strategy, which includes annual mass drug administration (MDA) with azithromycin to the whole population of endemic districts. Prevalence surveys to assess impact at the district level are recommended after at least 3 years of intervention. The decision to stop MDA is based on a prevalence of trachomatous inflammation follicular (TF) among children aged 1-9 years below 5% at the sub-district level, as determined by an additional round of surveys limited within districts where TF prevalence is below 10%. We conducted impact surveys powered to estimate prevalence simultaneously at the sub-district and district in two zones of Amhara, Ethiopia to determine whether MDA could be stopped.MethodologySeventy-two separate population-based, sub-district surveys were conducted in 25 districts. In each survey all residents from 10 randomly selected clusters were screened for clinical signs of trachoma. Data were weighted according to selection probabilities and adjusted for correlation due to clustering.Principal findingsOverall, 89,735 residents were registered from 21,327 households of whom 72,452 people (80.7%) were examined. The prevalence of TF in children aged 1-9 years was below 5% in six sub-districts and two districts. Sub-district level prevalence of TF in children aged 1-9 years ranged from 0.9-76.9% and district-level from 0.9-67.0%. In only one district was the prevalence of trichiasis below 0.1%.Conclusions/significanceThe experience from these zones in Ethiopia demonstrates that impact assessments designed to give a prevalence estimate of TF at sub-district level are possible, although the scale of the work was challenging. Given the assessed district-level prevalence of TF, sub-district-level surveys would have been warranted in only five districts. Interpretation was not as simple as stopping MDA in sub-districts below 5% given programmatic challenges of exempting sub-districts from a highly regarded program and the proximity of hyper-endemic sub-districts.
