Project description:BackgroundIt has been suggested that, during primary shoulder arthroplasty, surgeons should identify the axillary nerve through direct visualization, palpation, or the "tug test" to prevent iatrogenic nerve injury. Our goal was to document the rate of isolated axillary nerve injury (IANI) in patients who had undergone primary anatomic total shoulder arthroplasty (TSA) or reverse total shoulder arthroplasty (RTSA) without routine identification of the axillary nerve.MethodsData on 869 cases of primary shoulder arthroplasty (338 TSAs and 531 RTSAs) performed by 1 surgeon between 2003 and 2017 were reviewed. Neither the tug test nor identification of the axillary nerve through palpation or visualization was used in any case. The primary outcome was new IANI documented within 3 months after arthroplasty. The frequency of IANI was summarized using point estimates and 95% confidence intervals (CIs).ResultsSix cases met the criteria for IANI. The overall incidence of IANI was 0.7% (95% CI, 0.3%-1.4%). The incidence of IANI was 0.3% (95% CI, 0%-1.6%) after TSA and 0.9% (95% CI, 0.3%-2.1%) after RTSA. All IANIs were cases of neurapraxia, and all patients had experienced complete neurologic recovery at last follow-up.ConclusionComplete, permanent IANI resulting from direct surgical trauma during primary shoulder arthroplasty can be avoided without using the tug test or routine identification of the nerve. A low incidence of partial temporary IANI can be expected, which may be related to indirect traction injuries.

Project description:BACKGROUND:The relationship of training load to injury using wearable technology has not been investigated in professional American football players. The primary objective of this study was to determine the correlation between player workload and soft tissue injury over the course of a football season utilizing wearable global positioning system (GPS) technology. HYPOTHESIS:Increased training load is associated with a higher incidence of soft tissue injuries. STUDY DESIGN:Case-control study. LEVEL OF EVIDENCE:Level 3. METHODS:Player workloads were assessed during preseason and regular-season practice sessions using GPS tracking and triaxial accelerometry from 2014 to 2016. Soft tissue injuries were recorded during each season. Player workload during the week of injury (acute) and average weekly workload during the 4 weeks (chronic) prior to injury were determined for each injury and in uninjured position-matched controls during the same week. A matched-pairs t test was used to determine differences in player workload. Subgroup analysis was also conducted to determine whether observed effects were confounded by training period and type of injury. RESULTS:In total, 136 lower extremity injuries were recorded. Of the recorded injuries, 101 injuries with complete GPS and clinical data were included in the analysis. Injuries were associated with greater increases in workload during the week of injury over the prior month when compared with uninjured controls. Injured players saw a 111% (95% CI, 66%-156%) increase in workload whereas uninjured players saw a 73% (95% CI, 34%-112%) increase in workload during the week of injury (P = 0.032). Individuals who had an acute to chronic workload ratio higher than 1.6 were 1.5 times more likely to sustain an injury relative to time- and position-matched controls (64.6% vs 43.1%; P = 0.004). CONCLUSION:Soft tissue injuries in professional football players were associated with sudden increases in training load over the course of a month. This effect seems to be especially pronounced during the preseason when player workloads are generally higher. These results suggest that a gradual increase of training intensity is a potential method to reduce the risk of soft tissue injury. CLINICAL RELEVANCE:Preseason versus regular-season specific training programs monitored with wearable technology may assist team athletic training and medical staff in developing programs to optimize player performance.

Project description:Although a myriad of studies have been conducted on player behavior in football, in-depth studies with structured theory are rare due to the difficulty in quantifying individual player skills and team strategies. We propose a physics-based mathematical model that describes football players' movements during dribbling situations, parameterized by the attacker aggressiveness, the defender hesitance and the top speed of both players. These player- and situation-specific parameters are extracted by fitting the model to real player trajectories from Major League Soccer games, and enable the quantification of player dribbling attributes and decisions beyond classical statistics. We show that the model captures the essential dribbling dynamics, and analyze how differences between parameters in varying game situations provide valuable insights into players' behavior. Lastly, we quantitatively study how changes in the player's parameters impact dribbling performance, enabling the model to provide scientific guidance to player training, scouting and game strategy development.

Project description:BackgroundElite junior Australian football players experience high training loads across levels of competition and training. This, in conjunction with impaired wellness, can predispose athletes to injury.HypothesisElite junior Australian football players exposed to high loads with poor wellness are more likely to be at risk of injury than those with improved wellness.Study designLongitudinal prospective cohort study.Level of evidenceLevel 3.MethodsData were collected and analyzed from 280 players across the 2014 season. Internal load was measured via session rating of perceived exertion. Player wellness was reported according to ratings of sleep quality, fatigue, soreness, stress, and mood. Week- and month-based training load measures were calculated, representing a combination of absolute and relative load variables. Principal component analysis factor loadings, based on 17 load and wellness variables, were used to calculate summed variable covariates. Injury was defined as "any injury leading to a missed training session or competitive match." Associations between covariates and injury risk (yes/no) were determined via logistic generalized estimating equations.ResultsA significant interaction term between load and wellness on injury was found [odds ratio (OR) 0.76; 95% CI 0.62-0.92; P < 0.01), indicating that wellness acts as a "dimmer switch" of load on injury. Further, there was evidence of moderated mediation (OR 0.71; 95% CI 0.57-0.87; P < 0.01). When wellness was low, injury risk started to increase substantially at a 1-week load of 3250 au.ConclusionsSubjective measures of training load are associated with injury risk through a nonlinear relationship. This relationship is further influenced by player wellness, which can amplify the risk of injury. There is evidence that higher stress is linked with injury and that soreness and sleep mediate any stress-injury relationship.Clinical relevanceCoaching efforts to manage training load and player adaptive responses, including wellness, may reduce the risk of injury, with stress, soreness, and sleep particularly relevant at this level.

Project description:With 36 players on the field, congestion in Australian football is an important consideration in identifying passing capacity, assessing fan enjoyment, and evaluating the effect of rule changes. However, no current method of objectively measuring congestion has been reported. This study developed two methods to measure congestion in Australian football. The first continuously determined the number of players situated within various regions of density at successive time intervals during a match using density-based clustering to group players as 'primary', 'secondary', or 'outside'. The second method aimed to classify the level of congestion a player experiences (high, nearby, or low) when disposing of the ball using the Random Forest algorithm. Both approaches were developed using data from the 2019 and 2021 Australian Football League (AFL) regular seasons, considering contextual variables, such as field position and quarter. Player tracking data and match event data from professional male players were collected from 56 matches performed at a single stadium. The random forest model correctly classified disposals in high congestion (0.89 precision, 0.86 recall, 0.96 AUC) and low congestion (0.98 precision, 0.86 recall, 0.96 AUC) at a higher rate compared to disposals nearby congestion (0.72 precision, 0.88 recall, 0.88 AUC). Overall, both approaches enable a more efficient method to quantify the characteristics of congestion more effectively, thereby eliminating manual input from human coders and allowing for a future comparison between additional contextual variables, such as, seasons, rounds, and teams.

Project description:The description of current load monitoring practices may serve to highlight developmental needs for both the training ground, academia and related industries. While previous studies described these practices in elite men's football, no study has provided an overview of load monitoring practices in elite women's football. Given the clear organizational differences (i.e., professionalization and infrastructure) between men's and women's clubs, making inferences based on men's data is not appropriate. Therefore, this study aims to provide a first overview of the current load monitoring practices in elite women's football. Twenty-two elite European women's football clubs participated in a closed online survey (40% response rate). The survey consisted of 33 questions using multiple choice or Likert scales. The questions covered three topics; type of data collected and collection purpose, analysis methods, and staff member involvement. All 22 clubs collected data related to different load monitoring purposes, with 18 (82%), 21 (95%), and 22 (100%) clubs collecting external load, internal load, and training outcome data, respectively. Most respondents indicated that their club use training models and take into account multiple indicators to analyse and interpret the data. While sports-science staff members were most involved in the monitoring process, coaching, and sports-medicine staff members also contributed to the discussion of the data. Overall, the results of this study show that most elite women's clubs apply load monitoring practices extensively. Despite the organizational challenges compared to men's football, these observations indicate that women's clubs have a vested interest in load monitoring. We hope these findings encourage future developments within women's football.

Project description:BackgroundIsolated complete dislocation of the hamate bone is a rare wrist injury. Only 22 cases have been reported in the literature. Four of them were associated with nerve injury, but it is a pity that the causes of ulnar nerve injury had never been deeply analyzed. In the case report, it was stated that the ulnar nerve injury was caused by both displacement of uncinate bone and change of position of transverse carpal ligament. The complete dislocation of the hamate bone is mostly caused by direct force, often accompanied by tears of the surrounding ligaments.Case descriptionA 30-year-old woman was treated because her left hand was crushed by a machine roller. Plain film X-ray showed isolated dislocation of the volar hamate and rotation of the hamate bone to the volar side. Clinical manifestations and computed tomography (CT) signs suggest ulnar nerve injury. The patient was quickly transported to the operating room for open reduction and internal fixation of the hamate bone and repair of the dorsal carpal ligament. The patient fully recovered after 12 months.ConclusionsIsolated complete dislocation of the hamate bone is a rare injury. The most common cause is direct compression force to the wrist. Diagnosis can be made through review of the patient's medical history, physical examination and X-ray, however, CT can accurately evaluate the displacement and rotation of the hamate bone, hamate bone fracture and other concomitant wrist fractures, suggesting compression changes of the median nerve and ulnar nerve to provide a reliable basis for the formulation of a clinical care plan. The case report added the evidence-based practice, and discussed the anatomical mechanism and imaging manifestation of ulnar nerve injury associated with complete dislocation of hamate bone.

Project description:BackgroundThe incidence of sport-related concussion (SRC) in high school football is well documented. However, limited prospective data are available regarding how player characteristics and protective equipment affect the incidence of SRC.PurposeTo determine whether the type of protective equipment (helmet and mouth guard) and player characteristics affect the incidence of SRC in high school football players.DesignCohort study; Level of evidence, 2.MethodsCertified athletic trainers (ATs) at each high school recorded the type of helmet worn (brand, model, purchase year, and recondition status) by each player as well as information regarding players' demographics, type of mouth guard used, and history of SRC. The ATs also recorded the incidence and days lost from participation for each SRC. Incidence of SRC was compared for various helmets, type of mouth guard, history of SRC, and player demographics.ResultsA total of 2081 players (grades 9-12) enrolled during the 2012 and/or 2013 football seasons (2287 player-seasons) and participated in 134,437 football (practice or competition) exposures. Of these players, 206 (9%) sustained a total of 211 SRCs (1.56/1000 exposures). There was no difference in the incidence of SRC (number of helmets, % SRC [95% CI]) for players wearing Riddell (1171, 9.1% [7.6%-11.0%]), Schutt (680, 8.7% [6.7%-11.1%]), or Xenith (436, 9.2% [6.7%-12.4%]) helmets. Helmet age and recondition status did not affect the incidence of SRC. The rate of SRC (hazard ratio [HR]) was higher in players who wore a custom mouth guard (HR = 1.69 [95% CI, 1.20-2.37], P < .001) than in players who wore a generic mouth guard. The rate of SRC was also higher (HR = 1.96 [95% CI, 1.40-2.73], P < .001) in players who had sustained an SRC within the previous 12 months (15.1% of the 259 players [95% CI, 11.0%-20.1%]) than in players without a previous SRC (8.2% of the 2028 players [95% CI, 7.1%-9.5%]).ConclusionIncidence of SRC was similar regardless of the helmet brand (manufacturer) worn by high school football players. Players who had sustained an SRC within the previous 12 months were more likely to sustain an SRC than were players without a history of SRC. Sports medicine providers who work with high school football players need to realize that factors other than the type of protective equipment worn affect the risk of SRC in high school players.

Project description:Isolated third nerve palsy develops in numerous intracranial pathologies such as closed head trauma, tumor, and aneurysm. Isolated oculomotor nerve palsy caused by shrapnel injury is uncommon. After a penetrating intracranial shrapnel injury, our patient with oculomotor ophthalmoplegia underwent surgery. Microsurgery removed the shrapnel that was applying pressure on the third nerve, resulting in contusion. A partial recovery associated with regeneration was observed at month 9. Extraocular muscle surgery should be planned if palsy does not resolve over a prolonged period of time.

Project description:Although ankle injuries occur frequently in high school football players, no prospective studies have been performed to determine if wearing lace-up ankle braces will reduce the incidence and severity of ankle and other lower extremity injuries in these athletes.This study was conducted to determine if lace-up ankle braces reduce the incidence and severity of lower extremity injuries sustained by high school football players.Randomized controlled trial; Level of evidence, 1.A total of 2081 players from 50 high schools were randomly assigned to a braced or control group. Braced group players wore lace-up ankle braces during the 2010 football season. Athletic trainers recorded brace compliance, athlete-exposures, and injuries. Cox proportional hazards models were utilized to compare injury rates between groups. Injury severity (days lost) was tested with Wilcoxon rank sum.The rate of acute ankle injury (per 1000 exposures) was 0.48 in the braced group compared with 1.12 in the control group (Cox hazard ratio [HR] = 0.39; 95% confidence interval [CI], 0.24-0.65; P < .001). The severity (median days lost) of acute ankle injuries was the same (5 days) in both groups (P = .985). The rate of acute knee injury was 0.70 in the braced group compared with 0.69 in the control group (HR = 0.92; 95% CI, 0.57-1.47; P = .721). There was no difference (P = .242) in the severity of knee injuries between the groups (controls = 11.5 days, braced = 17 days). The rate of other lower extremity injuries was 0.95 in the braced group and 1.32 in the control group (HR = 0.72; 95% CI, 0.48-1.09; P = .117), while the severity was similar in both groups (6 days vs 7 days; P = .295).Players who used lace-up ankle braces had a lower incidence of acute ankle injuries but no difference in the incidence of acute knee or other lower extremity injuries. Braces did not reduce the severity of ankle, knee, or other lower extremity injuries.
