A suite of global accessibility indicators.

ABSTRACT: Good access to resources and opportunities is essential for sustainable development. Improving access, especially in rural areas, requires useful measures of current access to the locations where these resources and opportunities are found. Recent work has developed a global map of travel times to cities with more than 50,000 people in the year 2015. However, the provision of resources and opportunities will differ across the broad spectrum of settlements that range from small towns to megacities, and access to this spectrum of settlement sizes should also be measured. Here we present a suite of nine global travel-time accessibility indicators for the year 2015, at approximately one-kilometre spatial resolution, for a range of settlement size classes. We validated the travel-time estimates against journey times from a Google driving directions application across 1,511 2°?×?2° tiles representing 47,812 journeys. We observed very good agreement, though our estimates were more frequently shorter than those from the Google application with a median difference of -13.7?minutes and a median percentage difference of -16.9%.

Project description:AIM AND OBJECTIVES:To develop a suite of metrics and indicators to measure the quality of children's nursing care processes. The objectives were to identify available metrics and indicators and to develop consensus on the metrics and indicators to be measured. BACKGROUND:The Office of Nursing and Midwifery Services Director, Health Service Executive, in Ireland established seven workstreams aligned to the following care areas: acute, older persons, children's, mental health, intellectual disability, public health nursing and midwifery. DESIGN:A comprehensive design included stakeholder consultation and a survey with embedded open-ended questions. METHODS:A two-round online Delphi survey was conducted to identify metrics to be measured in practice, followed by a two-round online Delphi survey to identify the associated indicators for these metrics. A face-to-face consensus meeting was held with key stakeholders to review the findings and build consensus on the final metrics and indicators for use. A STROBE checklist was completed. RESULTS:A suite of eight nursing quality care process metrics and 67 associated process indicators was developed for children's nursing. CONCLUSIONS:By creating a national suite of metrics and indicators, more robust measurement and monitoring of nursing care processes can be achieved. This will enable the provision of evidence for any local and/or national level changes to policy and practice to enhance care delivery. RELEVANCE TO CLINICAL PRACTICE:The roll-out of the metrics and indicators in clinical practice has commenced. This national suite of metrics and indicators will ensure that a robust system of measurement for improvement is in place to provide assurance to Directors of Nursing of the quality of nursing care being provided to children and their families. It supports the value of nursing sensitive data to inform change and improvement in healthcare delivery and to demonstrate the contribution of the nursing workforce to safe patient care.

Project description:BackgroundFew low-burden indicators of diet quality exist to track trends over time at low cost and with low technical expertise requirements.ObjectiveThe aim was to develop and validate a suite of low-burden indicators to reflect adherence to global dietary recommendations.MethodsUsing nationally representative, cross-sectional, quantitative dietary intake datasets from Brazil and the United States, we tested the association of food-group scores with quantitative consumption aligned with 11 global dietary recommendations. We updated the Healthy Diet Indicator (HDI) to include current quantifiable recommendations of the WHO (HDI-2020). We developed 3 food-group-based scores-an overall Global Dietary Recommendations (GDR) score as an indicator of all 11 recommendations composed of 2 subcomponents: GDR-Healthy, an indicator of the recommendations on "healthy" foods, and GDR-Limit, an indicator of the recommendations on dietary components to limit. We tested associations between these scores and the HDI-2020 and its respective subcomponents. We developed 9 dichotomous food-group-based indicators to reflect adherence to the global recommendations for fruits and vegetables, dietary fiber, free sugars, saturated fat, total fat, legumes, nuts and seeds, whole grains, and processed meats. We conducted receiver operating characteristic and sensitivity-specificity analyses to determine whether the dichotomous indicators were valid to predict adherence to the recommendations in both countries.ResultsThe GDR score and its subcomponents were moderately to strongly associated with the HDI-2020 and its respective subcomponents (absolute values of rank correlation coefficients ranged from 0.55 to 0.66). Of the 9 dichotomous indicators, 8 largely met the criteria for predicting individual global dietary recommendations in both countries; 1 indicator (total fat) did not perform satisfactorily.ConclusionsFood-group consumption data can be used to indicate adherence to quantitative global dietary recommendations at population level. These indicators may be used to track progress of countries and populations toward meeting WHO guidance on healthy diets.
