Project description:ObjectivesThe aim of this study is to examine associations and interactions of education, and physical and mental health with unemployment in early, mid, and late work life.MethodsThis cross-sectional study uses data from 69,118 respondents from Lifelines. Health status was measured with the RAND-36, education was self-reported, and participants working <12 h per week or indicating to be unemployed were considered unemployed. The relative excess risk due to interaction (RERI) was calculated to measure interaction on the additive scale.ResultsInteractions of low education and poor mental health were found in early [RERI: 2.13; 95% confidence interval (CI): 0.61, 3.64], mid (1.41; 0.61, 2.20) and late (0.58; 0.04, 1.11) work life. Interaction between low education and poor physical health was only found in early (RERI: 1.66; 95% CI 0.26, 3.07) and mid-work life (1.25; 0.56, 1.93).ConclusionsLow education and poor physical and mental health exacerbate each other's impact on unemployment varying by work life stage. Policies addressing unemployment may become more effective if they better account for the physical and mental health status of adults in certain stages of their work life.

Project description:BACKGROUND:The Aotearoa/New Zealand population is ageing and numerous studies demonstrate with this phenomenon comes increases in non-communicable diseases, injuries and healthcare costs among other issues. Further, significant inequities exist between M?ori (Indigenous peoples of Aotearoa/New Zealand) and non-M?ori around poor ageing and health. Most research addressing these issues is deficit oriented; however, the current research project takes a strengths-based approach that highlights the potential of kaum?tua (elders) by asserting mana motuhake (autonomy, identity and self-actualisation). We believe that the esteem of elders in M?ori culture signals transformative potential. Specifically, this project utilises a 'tuakana-teina' (older sibling/younger sibling) peer-educator model, where kaum?tua work with other kaum?tua in relation to health and wellbeing. The objectives of the project are (a) to develop the capacity of kaum?tua as peer educators, whilst having positive impacts on their sense of purpose, health and wellbeing; and (b) to enhance the social and health outcomes for kaum?tua receiving the intervention. METHODS:The research is grounded in principles of Kaupapa M?ori and community-based participatory research, and brings together an Indigenous community of kaum?tua, community health researchers, and academic researchers working with two advisory boards. The project intervention involves an orientation programme for tuakana peer educators for other kaum?tua (teina). The research design is a pre- and post-test, clustered staggered design. All participants will complete a baseline assessment of health and wellbeing consistent with M?ori worldviews (i.e., holistic model). The tuakana and teina participants will be divided into two groups with the first group completing the intervention during the first half of the project and the second group during the second half of the project. All participants will complete post-test assessments following both interventions allowing comparison of the two groups along with repeated measures over time. DISCUSSION:The findings will provide an evidence base for the importance and relevancy of kaum?tua knowledge to create contextually based and culturally safe age-friendly environments that facilitate engagement and participation by kaum?tua for kaum?tua. If the model is effective, we will seek to facilitate the dissemination and scalability of the intervention. Project description:BackgroundResearch on the effect of body mass index (BMI) on ovarian cancer survival is inconsistent, but previous studies did not consider the possible impact of ascites, bowel obstruction, or cachexia, which commonly occur in late-stage disease.MethodsWe evaluated the association of BMI, before and around the time of diagnosis, with overall and disease-specific survival in a cohort study of primary invasive epithelial ovarian cancers diagnosed from 2000 to 2013 in Kaiser Permanente Northern California (KPNC) (n=1184). Deaths were identified through December 2014, with a median follow-up of 37 months. Proportional hazards regression was used to estimate overall and ovarian cancer-specific mortality, accounting for prognostic variables including age at diagnosis, race, stage, grade, histology, comorbidities, treatment, post-treatment CA125 levels, ascites, and bowel obstruction.ResultsThere was no evidence of an association between BMI and overall or ovarian cancer-specific survival. However, we found strong effect modification by stage (Pinteraction<0.01). Compared with normal prediagnosis BMI (18.5-24.9 kg m-2), for women who were obese before diagnosis (BMI⩾35 kg m-2) ovarian cancer-specific survival was lower among those diagnosed at stages I/II (hazard ratio (HR): 3.40; 95% confidence interval (CI): 1.16-9.99), but increased among those diagnosed with stage IV disease (HR: 0.58; 95% CI: 0.35-0.96). Associations were attenuated after excluding those diagnosed with cachexia (n=82) and further adjustment for ascites and bowel obstruction, with no evidence of effect modification by these factors.ConclusionsAssociations of obesity with ovarian cancer survival may differ by stage, with decreased survival among those with localised disease and increased survival among those with late-stage disease. Stage-specific effects of obesity on survival suggest a tailored approach to improve prognosis may be appropriate.

Project description:BackgroundThe impact of work-related asthma (WRA) on quality of life (QoL) and work productivity remains largely neglected/uncertain despite its high prevalence.ObjectiveTo investigate the association of WRA with QoL and work productivity as compared with subjects with non-WRA and those without asthma and rhinitis.MethodsA cross-sectional survey was carried out among workers during their periodic occupational health visit in Belgium. The Mini Asthma Quality of Life Questionnaire, the 8-item Medical Outcome Study Short Form instrument, and the Work Productivity and Activity Impairment-General Health questionnaire were administered. Survey participants were divided into 3 groups: (1) WRA (current asthma with ≥2 respiratory symptoms at work; n = 89); (2) non-WRA (current asthma without work-related respiratory symptoms; n = 119); and (3) the reference group (no asthma and no lower respiratory, nasal, or eye symptoms; n = 815). Associations of QoL and work productivity with WRA were evaluated by multivariable regression analyses.ResultsWRA and having poor asthma control were significantly associated with lower global Mini Asthma Quality of Life Questionnaire scores compared with non-WRA. Asthmatic subjects had significantly lower physical and mental health component scores of the 8-item Medical Outcome Study Short Form instrument and overall work productivity compared with the reference group, with greater impairment in workers with WRA than in those without WRA. Moreover, workers with WRA had higher percentages of doctor visits and income reduction because of respiratory symptoms than those with non-WRA. Work-related rhinitis and depression were associated with reduced QoL, independent of the effect of WRA.ConclusionsWRA should be managed comprehensively to reduce the worsening of QoL and work productivity of those affected.

Project description:The paradigm shifts of conventional office spaces for virtual workspaces which practiced Work from Home (WFH) due to Covid-19, created a serious change in the lifestyles of employees, due to the overlap of 'work' and 'life' domains in one's life. Since software engineers have a possibility of permanently adapting into WFH, the objective of this study is to unveil factors which would have a significant impact on the work-life balance of software engineers in Sri Lanka, while WFH. Only a very limited researches have shed light on this context, thereby this study would contribute to fill the empirical gap. The study undertook a quantitative approach by collecting primary data through a questionnaire from 384 participants, based on simple random sampling, and analyzing collected data based on Partial Least Squares Structural Equation Modelling (PLS-SEM), using Smart PLS 3.3.9 software. Study results revealed that 'supervisor's trust and support' and the 'individual workspace,' have a significant impact on work-life balance, while 'working conditions,' 'possibility to access the organization's networks' and 'number of children' have no such significant impact. Thereby the study infers that, sound support and trust extended by supervisors and a designated distraction-free workspace; as measures to demarcate the boundary of work and life. Distinctive findings of this study would primarily be fruitful for software engineers to dive into a balanced state of work and life not only during Covid-19 but in future too. Study findings will also contribute to software industry personnel and policymakers in Sri Lanka as well as other developing countries, to establish effective strategies in favor of software engineers who WFH. Further, considering IT industry's significant contribution towards Sri Lanka's economic growth amidst Covid-19, results of this study would be high-yielding to indirectly succor IT-services-supported economic growth amidst the pandemic-driven hardships in Sri Lanka.

It is generally acknowledged that the productivity of Nigerian HEIs is rather low, this survey examines the effect of the external environment on staff and student productivity in Nigerian HEIs, evaluates the effect of the internal environment on staff and student productivity in Nigerian HEIs, and determines the effect of psychosocial environment on staff and student productivity in Nigerian HEIs. Data were gathered based on conclusive research design. Stratified and convenience sampling techniques were adopted. The research instrument was confirmed to have all the necessary psychometric values considered appropriate for the research. Some descriptive statistical analyses were carried out to further clarify the data and provide the necessary platform for further analyses.

Project description:Extending working life beyond the state pension age is a key European Union policy. In the UK, women are more likely to extend paid work than men, indicating that factors other than the state pension age play a role in working longer. Women are less able to build pension income due to their role as carer within the family. It, therefore, follows that gender inequalities over the life course continue into older age to influence need, capacity and desire to undertake paid work after state pension age. This paper explores how work, marital and fertility history impact upon the likelihood of extending employment. It uses the British Household Panel Survey's retrospective data from the first 14 waves to summarise work-family histories, and logistic regression to understand the impact of work and family histories on extending paid work. Findings show that, on the one hand, women are extending paid work for financial reasons to make up for 'opportunity costs' as a result of their caring role within the family, with short breaks due to caring, lengthy marriages, divorcing and remaining single with children all being important. Yet, there is also evidence of 'status maintenance' from working life, with the women most likely to extend paid work, also those with the highest work orientation, prior to state pension age. But lengthy dis-attachment (due to caring) from the labour market makes extending working life more difficult. This has implications for policy strategies to entice women into paid work to make up for low independent financial resources.

Project description:BackgroundLabour-market marginalisation (LMM) and common mental disorders (CMDs) are serious societal problems. The aims were to describe trajectories of LMM (both work disability and unemployment) among young adults with and without CMDs, and to elucidate the characteristics associated with these trajectories.MethodsThe study was based on Swedish registers and consisted of all individuals 19-30 years with an incident diagnosis of a CMD in year 2007 (n = 7245), and a matched comparison group of individuals without mental disorders during the years 2004-07 (n = 7245). Group-based trajectory models were used to describe patterns of LMM both before, and after the incident diagnosis of a CMD. Multinomial logistic regressions investigated the associations between sociodemographic and medical covariates and the identified trajectories.ResultsTwenty-six percent (n = 1859) of young adults with CMDs followed trajectories of increasing or constant high levels of work disability, and 32 % (n = 2302) followed trajectories of increasing or constant high unemployment. In the comparison group, just 9 % (n = 665) followed increasing or constant high levels of work disability and 21 % (n = 1528) followed trajectories of increasing or constant high levels of unemployment. A lower share of young adults with CMDs followed trajectories of constant low levels of work disability (n = 4546, 63%) or unemployment (n = 2745, 38%), compared to the level of constant low work disability (n = 6158, 85%) and unemployment (n = 3385, 50%) in the comparison group. Remaining trajectories were fluctuating or decreasing. Around 50% of young adults with CMDs had persistent levels of LMM at the end of follow-up. The multinomial logistic regression revealed that educational level and comorbid mental disorders discriminated trajectories of work disability, while educational level, living area and age determined differences in trajectories of unemployment (R2difference = 0.02-0.05, p < 0.001).ConclusionsA large share, nearly 50%, of young adults with CMDs, substantially higher than in the comparison group of individuals without mental disorders, display increasing or high persistent levels of either work disability or unemployment throughout the follow-up period. Low educational level, comorbidity with other mental disorders and living in rural areas were factors that increased the probability for LMM.

Project description:BackgroundIn response to the COVID-19 pandemic, governments across the globe have imposed strict social distancing measures. Public compliance to such measures is essential for their success, yet the economic consequences of compliance are unknown. This is the first study to analyze the effects of good compliance compared with poor compliance to a COVID-19 suppression strategy (i.e. lockdown) on work productivity.MethodsWe estimate the differences in work productivity comparing a scenario of good compliance with one of poor compliance to the UK government COVID-19 suppression strategy. We use projections of the impact of the UK suppression strategy on mortality and morbidity from an individual-based epidemiological model combined with an economic model representative of the labour force in Wales and England.ResultsWe find that productivity effects of good compliance significantly exceed those of poor compliance and increase with the duration of the lockdown. After 3 months of the lockdown, work productivity in good compliance is £398.58 million higher compared with that of poor compliance; 75% of the differences is explained by productivity effects due to morbidity and non-health reasons and 25% attributed to avoided losses due to pre-mature mortality.ConclusionGood compliance to social distancing measures exceeds positive economic effects, in addition to health benefits. This is an important finding for current economic and health policy. It highlights the importance to set clear guidelines for the public, to build trust and support for the rules and if necessary, to enforce good compliance to social distancing measures.
