INTEGRATING PUBLIC HEALTH IN LAND REUSE AND REDEVELOPMENT: Part 2: Assessing Local Health Agency Capacity to Integrate Environmental Health and Land Reuse Work.

ABSTRACT: Many local health departments (LHDs) across the country coordinate with their service areas on environmental health or land reuse. The Brownfields & Reuse Opportunity Working Group (BROWN) is a multipartner land reuse stakeholder network that includes member representatives from state and local health agencies, federal agencies, environmental consultants, environmental health professionals, and academia. In 2015, BROWN provided input on five Environmental Health Resources Self Learning Modules (Epidemiology, Risk Assessment, Risk Communications, Land Reuse Sites, and Toxicology) that the Agency for Toxic Substances and Disease Registry (ATSDR) was developing. ATSDR created the educational modules as resources and self-study guides to increase LHD capacity to respond to environmental issues. Following input from BROWN members on the modules, the National Environmental Health Association independently developed a short survey to identify baseline capacity of environmental professionals, primarily LHD professionals, to address environmental health and land reuse issues. The survey results of 93 LHD personnel indicated variation in the level of education among LHD employees and how often specific environmental health and land reuse services were requested. A subset of three LHD respondents also provided input into the learning modules.

Project description:IntroductionThere has been a lot of attention on the role of human resource management interventions to improve delivery of health services in low- and middle-income countries. However, studies on this subject are few due to limited research on implementation of programmes and methodological difficulties in conducting experimental studies on human resource interventions. The authors present the protocol of an evaluation of a district-level capacity-building intervention to identify the determinants of performance of health workers in managerial positions and to understand how changes (if any) are brought about.Methods and analysisThe aim of this study is to understand how capacity building works. The authors will use realist evaluation to evaluate an intervention in Karnataka, India. The intervention is a capacity-building programme that seeks to improve management capacities of health managers at district and subdistrict levels through periodic classroom-based teaching and mentoring support at the workplace. The authors conducted interviews and reviewed literature on capacity building in health to draw out the programme theory of the intervention. Based on this, the authors formulated hypothetical pathways connecting the expected outcomes of the intervention (planning and supervision) to the inputs (contact classes and mentoring). The authors prepared a questionnaire to assess elements of the programme theory-organisational culture, self-efficacy and supervision. The authors shall conduct a survey among health managers as well as collect qualitative data through interviews with participants and non-participants selected purposively based on their planning and supervision performance. The authors will construct explanations in the form of context-mechanism-outcome configurations from the results. This will be iterative and the authors will use a realist evaluation framework to refine the explanatory theories that are based on the findings to explain and validate an improved theory on 'what works for whom and under what conditions'.DiscussionThe scope for applying realist evaluation to study human resource management interventions in health are discussed.

Project description:Transformations are fundamentally about agency: human intention, motivation, and power to influence and to resist. Most studies focus on deliberate system-level transformations, usually guided by a set of influential actors. However, system-level transformations may also occur as the result of the cascading effects of multiple individual transformations in response or in anticipation to various crises. Little is known about how crises foster these individual transformations, and how these may relate to different types of system-level change. This article fills this gap by looking at how crisis fosters two different types of agencies-internal and external-and how these link to individual transformations in the case of Greece's back-to-the-land movement whereby urbanites sought to reconnect with land-based livelihoods during the economic crisis (2008 onwards). The article draws on the qualitative analysis of 76 interviews of back-to-the-landers to further understand why people are going back-to-the-land (their motivations), how these relate to the concept of agency and individual transformation, and what implications might there be for system-level social-ecological transformations. This article makes three key points. First, crises create different opportunity contexts that may lead to rapid changes in what is valued in the broader social discourse. While social values and discourses are usually considered to be "deep levers" and slow to change, we found that they can rapidly shift in times of crises, challenging notions of the role of fast vs. slow variables in system transformations. Second, agency is needed to respond to crises but is also further catalyzed and enhanced through crisis; activating one's internal agency leads to personal transformations as well as collective transformations (linked to external agency), which are mutually co-constitutive. And third, systemic-level transformation emerges through multiple pathways including through the aggregation of multiple individual transformations that may lead to emergent system-level changes.Supplementary informationThe online version contains supplementary material available at 10.1007/s11625-021-01043-5.

Project description:BackgroundAlthough the COVID-19 pandemic has significantly boosted the implementation of digital services worldwide, it has become increasingly important to understand how these solutions are integrated into professionals' routine work. Professionals who are using the services are key influencers in the success of implementations. To ensure successful implementations, it is important to understand the multiprofessional perspective, especially because implementations are likely to increase even more.ObjectiveThe aim of this study is to examine health and social care professionals' experiences of digital service implementations and to identify factors that support successful implementations and should be considered in the future to ensure that the services are integrated into professionals' routine work.MethodsA qualitative approach was used, in which 8 focus group interviews were conducted with 30 health and social care professionals from 4 different health centers in Finland. Data were analyzed using qualitative content analysis. The resulting categories were organized under the components of normalization process theory.ResultsOur results suggested 14 practices that should be considered when implementing new digital services into routine work. To get professionals to understand and make sense of the new service, (1) the communication related to the implementation should be comprehensive and continuous and (2) the implementation process should be consistent. (3) A justification for the service being implemented should also be given. The best way to engage the professionals with the service is (4) to give them opportunities to influence and (5) to make sure that they have a positive attitude toward the service. To enact the new service into professionals' routine work, it is important that (6) the organization take a supportive approach by providing support from several easy and efficient sources. The professionals should also have (7) enough time to become familiar with the service, and they should have (8) enough know-how about the service. The training should be (9) targeted individually according to skills and work tasks, and (10) it should be diverse. The impact of the implementation on the professionals' work should be evaluated. The service (11) should be easy to use, and (12) usage monitoring should happen. An opportunity (13) to give feedback on the service should also be offered. Moreover, (14) the service should support professionals' work tasks.ConclusionsWe introduce 14 practices for organizations and service providers on how to ensure sustainable implementation of new digital services and the smooth integration into routine work. It is important to pay more attention to comprehensive and continuing communication. Organizations should conduct a competence assessment before training in order to ensure proper alignment. Follow-ups to the implementation process should be performed to guarantee sustainability of the service. Our findings from a forerunner country of digitalization can be useful for countries that are beginning their service digitalization or further developing their digital services.
