ABSTRACT: This prospective controlled observational cohort study assessed the performance of a novel panel of serum microRNA (miRNA) biomarkers on indicators of concussion, subconcussive impacts, and neurocognitive function in collegiate football players over the playing season. Male collegiate student football athletes participating in a Division I Football Bowl Subdivision of the National Collegiate Athletic Association (NCAA) were enrolled. There were a total of 53 participants included in the study, 30 non-athlete control subjects and 23 male collegiate student football athletes. Neurocognitive assessments and blood samples were taken within the week before the athletic season began and within the week after the last game of the season and measured for a panel of pre-selected miRNA biomarkers. Project description:The purpose of this study was to evaluate the effects of a competitive soccer season on biomarkers and performance metrics in order to determine the correlation between changes in biomarkers, body composition, and performance outcomes. Twenty-one Division 1 female collegiate soccer players were monitored throughout the 16-week season. Player workload was measured using heart rate and Global Position Satellite systems at all practices and games. Performance testing, including vertical jump, VO2max, and 3-repetition maximum testing for bench press, squat and deadlift, occurred prior to pre-season and immediately post-season. Blood draws occurred prior to preseason and every 4-weeks thereafter, following a game. Body composition was assessed prior to the start of season (week 0) and weeks 6, 10, 14, and 17 (post-season). Delta area under the curve was calculated for biomarkers and body composition variables to account for seasonal changes adjusted for baseline. Pearson-product moment correlations were used to assess relationships with significance set at p < 0.05. Trends were considered p ≤ 0.10. No significant time main effects were seen for anabolic biomarkers (p > 0.05). Significant time effects were seen for catabolic biomarkers throughout the season (p = 0.001). No changes in body weight, VO2max, vertical jump, and deadlift occurred. Squat and bench press improved (p = 0.01 and p = 0.02, respectively) with a decline in percent body fat (p = 0.03) and a trend for increased fat free mass (p = 0.09). Additionally, total cortisol (TCORT) negatively correlated with fat free mass (r = -0.48; p = 0.03) and positively correlated with VO2max (r = 0.47; p = 0.04). A trend was shown for a positive correlation between both TCORT and free cortisol (FCORT) and percent body fat (r = 0.39; r = 0.40; p = 0.08, respectively). IGF-1 and growth hormone positively correlated to deadlift (r = 0.57; P = 0.02 and r = 0.59; p = 0.03), whereas creatine kinase showed a trend for a positive correlation with deadlift (r = 0.49; p = 0.06). IL-6 negatively correlated with bench press (r = -0.53; p = 0.03). These findings support a relationship between biomarkers, performance outcomes, and body composition. Biomarker monitoring may be useful to detect individual player's physiological response to an athletic season and may help provide insights in efforts to optimize performance outcomes.

Project description:CONTEXT:? Sports officials can play an important role in concussion safety by calling injury timeouts so that athletic trainers can evaluate athletes with possible concussions. Understanding the determinants of whether officials call an injury timeout when they suspect a concussion has important implications for the design of interventions to better support officials in this role. OBJECTIVE:? To assess the knowledge of US collegiate football officials about concussion symptoms and to determine the associations between knowledge, perceived injunctive norms, and self-efficacy in calling injury timeouts for suspected concussions. DESIGN:? Cross-sectional study. SETTING:? Electronic survey. PATIENTS OR OTHER PARTICIPANTS:? Of the 3074 US collegiate football officials contacted, 1324 (43% response rate) participated. MAIN OUTCOME MEASURE(S):? Concussion knowledge, injunctive norms (belief about what others would want them to do), and behavioral self-efficacy (confidence in their ability to call injury timeouts for suspected concussions in athletes during challenging game-day conditions). RESULTS:? Officials reported calling approximately 1 injury timeout for a suspected concussion every 4 games during the 2015 season. Structural equation modeling indicated that officials with more concussion-symptom knowledge had greater self-efficacy. Independent of an official's symptom knowledge, injunctive norms that were more supportive of calling an injury timeout were associated with greater self-efficacy. CONCLUSIONS:? Concussion education for officials is important because when officials are aware of concussion symptoms, they are more confident in calling injury timeouts. Beyond increasing symptom knowledge, fostering sports environments that encourage concussion safety can support officials in calling injury timeouts. Athletic trainers can help by educating stakeholders, including officials, about the importance of concussion safety. When officials believe that other stakeholders support concussion safety, they are more likely to call injury timeouts if they suspect a concussion has occurred.

Project description:ImportanceRepetitive subconcussive head impacts in sports have emerged as a complex public health issue. Most of these head impacts remain asymptomatic yet have the potential to cause insidious neurological deficit if sustained repetitively. Near point of convergence (NPC) values have shown to reflect subclinical neuronal damage; however, the longitudinal pattern of NPC changes in association with subconcussive head impacts remains unclear.ObjectivesTo examine the NPC response to recurring subconcussive head impacts in a single high school football season through a series of repeated measurements.Design, setting, and participantsThis prospective case-series study of US varsity high school football players included baseline measurements of NPC, measurements at pregame and postgame points from 6 in-season games, and postseason follow-up measurements (a total of 14 points). An accelerometer-embedded mouthguard measured head impact frequency and magnitude from all practices and games. During the 6 games, players wore chest-strap heart rate monitors to record heart rate and estimate their excess postexercise oxygen consumption, accounting for possible physical exertion effects on NPC values.ExposuresPlayers participated in practices and games with no restriction.Main outcomes and measuresNear point of convergence.ResultsThe 12 included players were all boys, with a mean (SD) age of 16.4 (0.5) years. A total of 8009 head impacts, 177 907 g of peak linear acceleration, and 16 123 371 rad/s2 of peak rotational acceleration were recorded from the players in a single football season. There was a significant increase in NPC over time until the middle of the season (mean [SD] NPC: baseline, 5.25 [1.49] cm; pregame 3, 6.42 [1.93] cm; P = .01), which was significantly associated with subconcussive head impact frequency and magnitude (0.02 cm per 100 g of peak linear acceleration [SE, 0.0108; 95% CI, 0.0436-0.004]; P = .01; 0.023 cm per 10 000 rad/s2 of peak rotational acceleration [SE, 0.009; 95% CI, 0.041-0.0105]; P = .02). However, NPC values began to normalize toward baseline level from midseason (mean [SD] NPC: baseline, 5.25 [1.49] cm; pregame 6, 5.75 [2.23] cm; P = .32), as supported by a significant quadratic trend (β [SE], -0.002 [0.001] cm/d; P = .003), while participants continued to incur subconcussive head impacts.Conclusions and relevanceThis longitudinal case series study suggests that NPC can be perturbed over the long term by subconcussive head impacts but may normalize over time. The oculomotor system may have an adaptational capacity to subclinical head impacts, yet the mechanism for such remains an open question and warrants further investigation.
