Project description:BackgroundOne of the most common pollutants in residences due to gas appliances, NO2 has been shown to increase the risk of asthma attacks after small increases in short term exposure. However, standard environmental sampling methods taken at the regional level overlook chronic intermittent exposure due to lack of temporal and spatial granularity. Further, the EPA and WHO do not currently provide exposure recommendations to at-risk populations.AimsA pilot study with pediatric asthma patients was conducted to investigate potential deployment challenges as well as benefits of home-based NO2 sensors and, when combined with a subject's hospital records and self-reported symptoms, the richness of data available for larger-scale epidemiological studies.MethodsWe developed a compact personal NO2 sensor with one minute temporal resolution and sensitivity down to 15 ppb to monitor exposure levels in the home. Patient hospital records were collected along with self-reported symptom diaries, and two example hypotheses were created to further demonstrate how data of this detail may enable study of the impact of NO2 in this sensitive population.Results17 patients (55%) had at least 1 h each day with average NO2 exposure >21 ppb. Frequency of acute NO2 exposure >21 ppb was higher in the group with gas stoves (U = 27, p ≤ 0.001), and showed a positive correlation (rs = 0.662, p = 0.037, 95% CI 0.36-0.84) with hospital admissions.SignificanceSimilar studies are needed to evaluate the true impact of NO2 in the home environment on at-risk populations, and to provide further data to regulatory bodies when developing updated recommendations.

Project description:Non-invasive continuous alcohol (ethanol) monitoring has potential applications in both population research and in clinical management of acute alcohol intoxication or chronic alcoholism. Current wearable monitors based on transdermal alcohol content (TAC) sensing have limited accessibility and blood alcohol content (BAC) quantification accuracy. Here we describe the development of a self-contained discreet wearable transdermal alcohol (TAC) sensor in the form of a wristband or armband. This sensor can detect vapor-phase alcohol in perspiration from 0.09 ppm (equivalent to 0.09 mg/dL sweat alcohol concentration at 25 °C under Henry's Law equilibrium) to over 500 ppm at one-minute time resolution. Additionally, a digital sensor was employed to monitor the temperature and humidity levels inside the sensing chamber. Two male human subjects were recruited to conduct studies with alcohol consumption using calibrated prototype TAC sensors to validate the performance. Our preliminary data demonstrated that, under well-controlled conditions, this sensor can acquire TAC curves at low doses (1-2 standard drinks). Moreover, TAC data for different doses can be easily distinguished. However, substantial interpersonal and intrapersonal variabilities in measurement data were also observed in experiments with less controlled conditions. Our observations suggest that perspiration rate might be an important contributing factor to these variabilities. Further studies with sufficient sample sizes are required to validate and characterize the impact of different perspiration rates on TAC sensors, which may inform more reproducible and accurate sensor designs in the future.

Project description:IntroductionPediatric asthma exacerbations account for >1.8 million emergency department (ED) visits annually. Asthma guidelines are intended to guide time-dependent treatment decisions that improve clinical outcomes; however, guideline adherence is inadequate. We examined whether an automatic disease detection system increases clinicians' use of paper-based guidelines and decreases time to a disposition decision.MethodsWe evaluated a computerized asthma detection system that triggered NHLBI-adopted, evidence-based practice to improve care in an urban, tertiary care pediatric ED in a 3-month (7/09-9/09) prospective, randomized controlled trial. A probabilistic system screened all ED patients for acute asthma. For intervention patients, the system generated the asthma protocol at triage for intervention patients to guide early treatment initiation, while clinicians followed standard processes for control patients. The primary outcome measures included time to patient disposition.ResultsThe system identified 1100 patients with asthma exacerbations, of which 704 had a final asthma diagnosis determined by a physician-established reference standard. The positive predictive value for the probabilistic system was 65%. The median time to disposition decision did not differ among the intervention (289 min; IQR = (184, 375)) and control group (288 min; IQR = (185, 375)) (p=0.21). The hospital admission rate was unchanged between intervention (37%) and control groups (35%) (p = 0.545). ED length of stay did not differ among the intervention (331 min; IQR = (226, 581)) and control group (331 min; IQR = (222, 516)) (p = 0.568).ConclusionDespite a high level of support from the ED leadership and staff, a focused education effort, and implementation of an automated disease detection, the use of the paper-based asthma protocol remained low and time to patient disposition did not change.

Project description:Community-level approaches for pediatric asthma management rely on locally collected information derived primarily from two sources: claims records and school-based surveys. We combined claims and school-based surveillance data, and examined the asthma-related risk patterns among adolescent students. Symptom data collected from school-based asthma surveys conducted in Oakland, CA were used for case identification and determination of severity levels for students (high and low). Survey data were matched to Medicaid claims data for all asthma-related health care encounters for the year prior to the survey. We then employed recursive partitioning to develop classification trees that identified patterns of demographics and healthcare utilization associated with severity. A total of 561 students had complete matched data; 86.1% were classified as high-severity, and 13.9% as low-severity asthma. The classification tree consisted of eight subsets: three indicating high severity and five indicating low severity. The risk subsets highlighted varying combinations of non-specific demographic and socioeconomic predictors of asthma prevalence, morbidity and severity. For example, the subset with the highest class-prior probability (92.1%) predicted high-severity asthma and consisted of students without prescribed rescue medication, but with at least one in-clinic nebulizer treatment. The predictive accuracy of the tree-based model was approximately 66.7%, with an estimated 91.1% of high-severity cases and 42.3% of low-severity cases correctly predicted. Our analysis draws on the strengths of two complementary datasets to provide community-level information on children with asthma, and demonstrates the utility of recursive partitioning methods to explore a combination of features that convey asthma severity.

Project description:A wearable C-shaped antenna based on a fabric material operating at 2.4 GHz frequency is proposed for use in flexible/wearable IoT medical systems. The wearable IoT device plays a key role in medical applications, and the antenna is a key part of it. Loading the presented antenna on the body models showed a frequency detuned with the gain and efficiency reduced from 1. 28 to −9 dB and 90% to 10%. In addition, the SAR did not meet the safety health requirement defined by the FCC or ICNIRP standards. Therefore, an “Artificial Magnetic Conductor” structure (AMC) is added to the C-shaped antenna to overcome these problems. The AMC acts as shielding material between the human skin and the presented antenna because of its 0° reflection phase, which mimics the action of the Perfect Magnetic Conductor (PMC). The overall size of the proposed design was 54 × 54 × 3.9 mm3. Numerical and experimental findings indicated that integrating the AMC structures with a C-shaped antenna was robust for body deformation and load. The C-shaped antenna worked equally well with the AMC, whether positioned in free space or on the chest or the arm of the human body. The integrated antenna with AMC structures has excellent performances. The gain and efficiency without loading on the chest were 6.49 dB and 84%, respectively. While for loaded on the chest were 6.21 dB and 81%, respectively. It also decreased the back radiation and raised the Front to Back Ration (FBR) by 13.8 dB. SAR levels have been reduced by more than 90% between the FCC and ICNIRP standards compared to the C-shaped antenna alone, which does not comply with the standards. As a result, the C-shaped integration with AMC structures is highly suitable for assembly in any wearable system. Project description:ObjectiveTo evaluate the frequency of EMS protocol non-adherence during pediatric asthma encounters and its association with emergency department (ED) length of stay (LOS) and hospital admission.MethodsThis is a retrospective review of asthma encounters aged 2-17 years transported by EMS to a pediatric ED from 2012 to 2017. Our primary outcome was hospital admission based on prehospital protocol adherence defined as: (1) bronchodilator administration, (2) treatment of hypoxia with oxygen, or (3) administration of intramuscular (IM) epinephrine in encounters with high severity of distress. Multivariable logistic regression estimated the association between protocol non-adherence and hospital admission.ResultsDuring the study period, 290 EMS encounters met inclusion criteria. Median age was 9 years (IQR 5-12), 63% were male, 40% had moderate to severe exacerbations, and 24% were admitted. Protocol non-adherence occurred in 32% of encounters with failure to administer bronchodilators in 27% and failure to administer IM epinephrine when indicated in 83%. Prehospital steroids were administered in 8% of encounters. After adjusting for covariates, protocol non-adherence was not statistically associated with likelihood of inpatient admission (OR 1.3; 95% CI: 0.6-2.6).ConclusionsAmong prehospital pediatric asthma encounters, EMS protocol non-adherence is common but not associated with a higher frequency of hospital admission. Hospital admission was associated with acute exacerbation severity suggesting further research is needed to develop a valid prehospital asthma severity assessment scoring tool.Supplemental data for this article can be accessed at publisher's website.

Project description:The Internet of Things (IoT) has been implemented to provide solutions for certain event detection because of ease of installation, computing and communication capability, and cost-effectiveness. Seismic event detection, however, is still a challenge due to a lack of high-fidelity sensing and classification efficiency. This study proposes BLESeis, an IoT sensor for smart earthquake detection. BLESeis comprises three main parts: (1) high-fidelity vibration sensing using a MEMS accelerometer and digital filtering; (2) an embedded earthquake detection algorithm; (3) BLE (Bluetooth low energy) beacon for earthquake notification. For high-fidelity vibration sensing, a triggering algorithm and embedded finite impulse response (FIR) low-pass filter are developed. The acquired vibration is then classified by the earthquake detection algorithm developed to identify the earthquake signal from other vibration sources using time and frequency domain analysis. Upon detection of an earthquake, the BLE beacon broadcasts using the proposed data packet for efficient notification and visualization. The performance of the proposed system is evaluated through numerical simulations and a set of experiments using shaking table tests. The experiments show the feasibility of the low-cost earthquake detection and notification system.

Project description:Internet of Things (IoT) projects are increasing in size over time, and some of them are growing to reach the whole world. Sensor arrays are deployed world-wide and their data is sent to the cloud, making use of the Internet. These huge networks can be used to improve the quality of life of the humanity by continuously monitoring many useful indicators, like the health of the users, the air quality or the population movements. Nevertheless, in this scalable context, a percentage of the sensor data readings can fail due to several reasons like sensor reliabilities, network quality of service or extreme weather conditions, among others. Moreover, sensors are not homogeneously replaced and readings from some areas can be more precise than others. In order to address this problem, in this paper we propose to use collaborative filtering techniques to predict missing readings, by making use of the whole set of collected data from the IoT network. State of the art recommender systems methods have been chosen to accomplish this task, and two real sensor array datasets and a synthetic dataset have been used to test this idea. Experiments have been carried out varying the percentage of failed sensors. Results show a good level of prediction accuracy which, as expected, decreases as the failure rate increases. Results also point out a failure rate threshold below which is better to make use of memory-based approaches, and above which is better to choose model-based methods.

Project description:A wristwatch-based wireless sensor platform for IoT wearable health monitoring applications is presented. The paper describes the platform in detail, with a particular focus given to the design of a novel and compact wireless sub-system for 868 MHz wristwatch applications. An example application using the developed platform is discussed for arterial oxygen saturation (SpO2) and heart rate measurement using optical photoplethysmography (PPG). A comparison of the wireless performance in the 868 MHz and the 2.45 GHz bands is performed. Another contribution of this work is the development of a highly integrated 868 MHz antenna. The antenna structure is printed on the surface of a wristwatch enclosure using laser direct structuring (LDS) technology. At 868 MHz, a low specific absorption rate (SAR) of less than 0.1% of the maximum permissible limit in the simulation is demonstrated. The measured on-body prototype antenna exhibits a -10 dB impedance bandwidth of 36 MHz, a peak realized gain of -4.86 dBi and a radiation efficiency of 14.53% at 868 MHz. To evaluate the performance of the developed 868 MHz sensor platform, the wireless communication range measurements are performed in an indoor environment and compared with a commercial Bluetooth wristwatch device.

Project description:Health care is one of the least funded sectors in Bangladesh and many other similar developing countries. People living in rural and remote areas do not have access to proper health care, and when they do, it is too expensive. This research aimed to develop a real-time health monitoring system that is cheap, easy to use, and accessible by doctors and patients. The system consists of several Internet of Things (IoT)-based sensors connected to an Arduino microprocessor, which thus measures the vital body signs of the patients. The measured readings are then transmitted to an Android application on a smartphone via a Bluetooth module. The sensors are connected to analog inputs. These sensors measure analog data, which is amplified by the microprocessor after being sorted. Doctors can also carry out the diagnosis of ailments using the data collected remotely from the patient. An Android-based mobile application that interfaces with a web-based application is implemented for efficient patients-doctors dual real-time communication. The Android application, which is connected to a MySQL database, updates the said database, which updates and displays the readings on a website accessible by both doctors and patients. Initially, the health monitor was tested using an Arduino Integrated Development Environment (IDE) monitor and a single user. Once initial simulations were successful, the proposed system was tested on five different real-human test subjects. The testing of the wireless health monitor produced successful results that measured patient vitals with a high level of accuracy. The proposed IoT-based system monitors vital signs such as the patient's body temperature, heart rate, ECG, SpO2 levels, blood pressure, and glucose levels. This system also includes a medical treatment plan by the doctors. The proposed system is novel as it integrates the IoT-based patient monitoring system with telemedicine. This proposed system has different sensors for real-time measuring the vital signs of the human body. A mobile and web application have also been integrated with this system for real-time remote patient monitoring and treatment plan. There are now systems available that only offer a telemedicine facility, where patients and doctors can have discussions, but do not have an IoT-based patient vital sign monitoring system integrated with telemedicine. The proposed system in this paper has the facility of IoT-based patient vital signs monitoring integrated with telemedicine, which makes this research work novel. The proposed system will increase the life expectancy of people throughout the world.
