Experimental evidence on post-program effects and spillovers from an agriculture-nutrition program.

ABSTRACT: Integrated agricultural-nutrition programs are often implemented under the premise that program effects are durable and spillover. This paper estimates one year post-program effects, three-year aggregate program effects and spillover effects using treated and untreated household cohorts. Two treatment interventions implemented agricultural interventions with behavior change communication strategies varying implementers using either village health committees or older female leaders. In the post-program period, program effects deteriorated relative to program period impacts documented in Olney et al. (2015), but the three-year agricultural, nutrition knowledge, health care practices and severe anemia impacts remained statistically significant. Despite the non-rival nature of nutrition education and promoted production techniques, there is little evidence of agricultural technology or health knowledge spillovers to non-treated households within treatment communities. Spillover effects measured for appropriate treatment of diarrhea (10 pp increase in giving rehydration salts rather than traditional medicine), wasting (20 pp lower probability of wasting) and children's anemia status (7 pp reduction in severe anemia) significantly improve in later cohorts. The aggregate program effects and spillovers are generally robust to multiple hypothesis testing.

Project description:The importance of women's roles for nutrition-sensitive agricultural projects is increasingly recognized, yet little is known about whether such projects improve women's empowerment and gender equality. We study the Agriculture, Nutrition, and Gender Linkages (ANGeL) pilot project, which was implemented as a cluster-randomized controlled trial by the Government of Bangladesh. The project's treatment arms included agricultural training, nutrition behavior change communication (BCC), and gender sensitization trainings delivered to husbands and wives together - with these components combined additively, such that the impact of gender sensitization could be distinguished from that of agriculture and nutrition trainings. Empowerment was measured using the internationally-validated project-level Women's Empowerment in Agriculture Index (pro-WEAI), and attitudes regarding gender roles were elicited from both men and women, to explore potentially gender-transformative impacts. Our study finds that ANGeL increased both women's and men's empowerment, raised the prevalence of households achieving gender parity, and led to small improvements in the gender attitudes of both women and men. We find significant increases in women's empowerment scores and empowerment status from all treatment arms but with no significant differences across these. We find no evidence of unintended impacts on workloads and inconclusive evidence around impacts on intimate partner violence. Our results also suggest some potential benefits of bundling nutrition and gender components with an agricultural development intervention; however, many of these benefits seem to be driven by bundling nutrition with agriculture. While we cannot assess the extent to which including men and women within the same treatment arms contributed to our results, it is plausible that the positive impacts of all treatment arms on women's empowerment outcomes may have arisen from implementation modalities that provided information to both husbands and wives when they were together. The role of engaging men and women jointly in interventions is a promising area for future research.

Project description:Background: Post-earthquake studies from around the world have reported that survivors relying on emergency food for prolonged periods of time experienced several dietary related health problems. The present study aimed to quantify the potential nutrient production of urban agricultural vegetables and the resulting nutritional self-sufficiency throughout the year for mitigating post-disaster situations. Methods: We estimated the vegetable production of urban agriculture throughout the year. Two methods were developed to capture the production from professional and hobby farms: Method I utilized secondary governmental data on agricultural production from professional farms, and Method II was based on a supplementary spatial analysis to estimate the production from hobby farms. Next, the weight of produced vegetables [t] was converted into nutrients [kg]. Furthermore, the self-sufficiency by nutrient and time of year was estimated by incorporating the reference consumption of vegetables [kg], recommended dietary allowance of nutrients per capita [mg], and population statistics. The research was conducted in Nerima, the second most populous ward of Tokyo's 23 special wards. Self-sufficiency rates were calculated with the registered residents. Results: The estimated total vegetable production of 5660 tons was equivalent to a weight-based self-sufficiency rate of 6.18%. The average nutritional self-sufficiencies of Methods I and II were 2.48% and 0.38%, respectively, resulting in an aggregated average of 2.86%. Fluctuations throughout the year were observed according to the harvest seasons of the available crops. Vitamin K (6.15%) had the highest self-sufficiency of selected nutrients, while calcium had the lowest (0.96%). Conclusions: This study suggests that depending on the time of year, urban agriculture has the potential to contribute nutrients to diets during post-disaster situations as disaster preparedness food. Emergency responses should be targeted according to the time of year the disaster takes place to meet nutrient requirements in periods of low self-sufficiency and prevent gastrointestinal symptoms and cardiovascular diseases among survivors.

Project description:The role of agriculture in reducing undernutrition is widely recognized, yet there is also consensus on the need to make the sector nutrition-sensitive. Evidence on the impact pathways from nutrition-sensitive agriculture (NSA) interventions, agricultural interventions with specific nutrition objectives, and actions detailing each temporal stage to reach nutrition outcomes is limited, however. We thus synthesized study results regarding impact of NSA interventions on nutrition outcomes relating to undernutrition, and constructed an impact pathway by mapping the evidence on each temporal stage from interventions to nutrition outcomes. We used Preferred Reporting Items for Systematic Reviews and Meta-Analyses to conduct and report our systematic review of studies on NSA interventions implemented in low- and lower-middle-income countries. Forty-three studies that met the inclusion criteria were extracted and synthesized across impact and pathways analyses. We carried out a thematic analysis of the effect of NSA interventions using evidence-based indicators and constructed the pathways by adopting a published framework on agriculture to nutrition pathways. Our findings reveal that NSA interventions can significantly improve dietary practices, and have the potential to enhance care practices and reduce occurrence of diseases, indicating their effectiveness in simultaneously addressing multiple determinants of undernutrition. However, NSA interventions have a lesser impact on nutritional status. NSA interventions lead to nutrition outcomes through 5 key pathways: food production, nutrition-related knowledge, agricultural income, women's empowerment, and strengthening of local institutions. We emphasize the need to carefully design, implement, and evaluate interventions with consideration for factors affecting impact pathways. Future research should focus on the effect of interventions combining multisector components, and pathways through non-food-production-related income, women's empowerment, strengthening of local institutions, food prices at intervention level, and expenditure on health care.
