Project description:Most smallholder farmers in sub-Saharan Africa (SSA) are adversely affected by low soil fertility, land degradation and climate change-related shocks such as drought. These problems lead to low productivity and low household income. In addition, the adoption of soil carbon enhancing practices remains low in Western Kenya. This study analyses the factors that influence the probability and extent of adoption of soil carbon enhancing practices in Western Kenya utilizing plot-level information, socioeconomic characteristics and external supporting factors. Multivariate probit model and generalized ordered logit were utilized to assess the adoption of multiple soil carbon enhancing practices and the extent of adoption respectively. Results indicate that the adoption of soil carbon enhancing practices is correlated, suggesting interrelation in farmers' adoption decisions. Both the multivariate probit model and generalized ordered probit results indicate that the probability and extent of adoption of soil carbon enhancing practices are influenced by plot-level characteristics, literacy level, access to agricultural credit, agricultural group membership, participation in the market, and gender of the household.

Project description:The data described in this paper were collected in two watersheds - Azuga-suba watershed in Angacha woreda (district) of Southern Nations, Nationalities and Peoples (SNNP) region and Yesir watershed of Bure woreda (district) of the Amhara region - in Ethiopia. The data were collected from 379 households with the main objective being to assess the factors constraining the adoption of sustainable land management technologies (SLMT) that enhance soil organic carbon sequestration. The data were collected using a structured questionnaire that was designed in SurveyCTO. The data cleaning and analysis was done using STATA SE version 14. This data can be used by researchers to assess the extent of adoption of SLMTs in Ethiopia by, for example, comparing the North and the South regions of Ethiopia. It can also be used to assess the probability of adoption as well as the benefit and costs of adopting SLMTs in Ethiopia both at farm and plot level.

Project description:Declining soil fertility is one of the major causes of food insecurity and high levels of poverty, both of which tend to hamper economic development in sub-Saharan Africa (SSA). To improve soil fertility, the implementation of soil organic carbon (SOC) enhancement technologies has become crucial to slowing land degradation, through increasing SOC, which is the basis of soil fertility. Using data from 381 households from Azuga-Suba and Yesir watersheds in Ethiopia, this study explores the extent of the adoption of technologies that enhance SOC. Soil organic carbon enhancing technologies include the use of manure, fertilizer, and crop residue management. The Probit model was used to assess what constrains the adoption of these technologies. The results indicate that fertilizer is the most adopted technology having over 90% adoption in both watersheds. Manure at 28% and 56% adoption while crop residue management at 37% and 26% adoption in Azuga-Suba and Yesir respectively. Technology adoption is highly constrained by lack of education, access to extension services, and access to credit services. Institutions and local farmer groups influence these constraints through training, provision of information, offering incentives, and credit services. Large plots hinder the use of manure and fertilizer due to the bulky nature of manure and the high costs of fertilizers. Insecurity in land tenure limits the adoption of manure and residue management. Perception of soil erosion and soil fertility tends to constrain the adoption of SOC technologies, as farmers are afraid that all improvements through soil amendment will be diminished through soil erosion. At the same time, farmers do not perceive the importance of SOC enhancing technologies in plots that were fertile. These results imply that strengthening institutions that enhance farmers' knowledge and provide credit as well as strengthening social protection schemes and farmer groups is crucial in promoting the adoption of these technologies.

Project description:BackgroundInsecticide-treated nets (ITNs) and long-lasting insecticidal nets (LLINs) are effective for malaria prevention and are designed to provide nearly 5 years of mosquito protection. However, many ITNs and LLINs become damaged and ineffective for mosquito bite prevention within 1 to 2 years in field conditions. Non-adherence to recommended bed net care and repair practices may partially explain this shortened net longevity.MethodsUsing data from a cross-sectional study, a net care adherence score was developed and adherence to net care practices described from two regions of western Kenya. Relationships between attitudes and environmental factors that influence net longevity were measured with adherence to bed net care practices.ResultsWhile overall care practices are highly adherent particularly in the highlands, practices related to daily storage, washing frequency, and drying location need improvement in the lowlands. Seventy-seven percent of nets in the lowlands were washed < 3 months prior to the survey compared to 23% of nets in the highlands. More nets were dried in the sun in the lowlands (32% of nets) compared to the highlands (4% of nets). Different elements of care are influenced by various malaria attitudes and environmental factors, highlighting the complexity of factors associated with net care. For example, households that learned about net care from community events, that share a sleeping structure with animals, and that have nets used by adult males tend to adhere to washing frequency recommendations.ConclusionsIn western Kenya, many nets are cared for in accordance to recommended practices, particularly in the highlands sites. In the lowlands, demonstrating methods at community events to tie nets up during the day coupled with messaging to emphasize infrequent washing and drying nets in the shade may be an appropriate intervention. As illustrated by differences between the highlands and lowlands sites in the present study, should interventions to improve adherence to bed net care practices be necessary, they should be context-specific.

Project description:UnlabelledTaenia spp. infections, particularly cysticercosis, cause considerable health impacts in endemic countries. Despite previous evidence of spatial clustering in cysticercosis and the role of environmental factors (e.g. temperature and humidity) in the survival of eggs, little research has explored these aspects of Taenia spp.EpidemiologyIn addition, there are significant gaps in our understanding of risk factors for infection in humans and pigs. This study aimed to assess the influence of socio-economic, behavioural and environmental variables on human and porcine cysticercosis. A cross-sectional survey for human taeniasis (T. solium and T. saginata), human cysticercosis (T. solium) and pig cysticercosis (T. solium) in 416 households in western Kenya was carried out. These data were linked to questionnaire responses and environmental datasets. Multi-level regression was used to examine the relationships between covariates and human and porcine cysticercosis. The HP10 Ag-ELISA sero-prevalence (suggestive of cysticercosis) was 6.6% for humans (95% CI 5.6%-7.7%), and 17.2% for pigs (95% CI 10.2%-26.4%). Human taeniasis prevalence, based on direct microscopic observation of Taenia spp. eggs (i.e. via microscopy results only) was 0.2% (95% CI 0.05%-0.5%). Presence of Taenia spp. antigen in both humans and pigs was significantly associated with a range of factors, including positive correlations with land cover. The presence of HP10 antigen in humans was correlated (non-linearly) with the proportion of land within a 1 km buffer that was flooding agricultural land and grassland (odds ratio [OR] = 1.09 and 0.998; p = 0.03 and 0.03 for the linear and quadratic terms respectively), gender (OR = 0.58 for males compared to females, p = 0.02), level of education (OR = 0.62 for primary level education versus no formal education, p = 0.09), use of well water for drinking (OR = 2.76 for those who use well water versus those who do not, p = 0.02) and precipitation (OR = 0.998, p = 0.02). Presence of Taenia spp. antigen in pigs was significantly correlated with gender and breeding status of the pig (OR = 10.35 for breeding sows compared to boars, p = 0.01), and the proportion of land within a 1 km buffer that was flooding agricultural land and grassland (OR = 1.04, p = 0.004). These results highlight the role of multiple socio-economic, behavioural and environmental factors in Taenia spp. transmission patterns. Environmental contamination with Taenia spp. eggs is a key issue, with landscape factors influencing presence of Taenia spp. antigens in both pigs and humans.

Project description:In an effort to improve intervention strategies, community knowledge, attitudes, and practices on schistosomiasis were evaluated using focus group discussions involving 237 participants, in eight Schistosoma mansoni high prevalence districts in rural Nyanza Province, Kenya. The majority of participants reported having heard about schistosomiasis through schools, posters, radio announcements, and community gatherings. Participants had a variety of beliefs about contracting schistosomiasis, including associating it with dirty drinking water and uncooked or contaminated food. Avenues for seeking treatment included health centers, spiritual intervention, herbal treatments, and medicine shops, with health centers receiving the most mention. Barriers to schistosomiasis control included attitudes of community members toward the infection, especially misconceptions that lead to stigma and the perception that diagnosis and treatment are expensive. Schools were the most common avenue for receiving information, suggesting that the existing education infrastructure can be used for health education and improved sensitization about schistosomiasis control programs.

Project description:INTRODUCTION:Hantaviruses are a group of single-stranded RNA viruses carried by small rodent reservoirs, transmitted to humans through inhalation of aerosolized particles of rodent feces, urine, or saliva. In Panama, the Choclo orthohantavirus has been associated with Hantavirus Pulmonary Syndrome (n = 54) and Hantavirus Fever (n = 53). In 2018, there were 107 cases of hantavirus diseases, the majority in the Tonosí district, and 4 deaths. As there is no vaccine or treatment for hantavirus, proper prevention measures by community members is key to stopping outbreaks. METHODOLOGY AND PRINCIPAL FINDINGS:We investigated hantavirus knowledge, attitudes, and practices in one corregimiento of Tonosí, Panama to determine what factors influence uptake of prevention practices and high level of knowledge. We conducted a cross-sectional survey with 124 residents covering hantavirus knowledge, attitudes based in the Health Belief Model (perceived severity, perceived susceptibility, perceived obstacles, perceived benefits, and cues to action) and prevention practices. There was an overall high level of knowledge (median score: 4/6), though 20% did not know the route of transmission. The mean number of reported practices performed per person was 8.4 (range: 4-12). Most people had heard of hantavirus through other community members. In linear regression, lower perceived obstacles predicted higher preventive practice score. Reported obstacles to preventive practices included physical restrictions, such as age and health state. In ordinal logistic regression, higher education level and knowing more people who had previously been sick with hantavirus contributed to higher knowledge score. CONCLUSIONS:Future interventions should focus on removing barriers to performing preventive practices. As most people learned of hantavirus through community members, interventions should be community-based and involve those who have experienced the disease. Any future education materials should address confusions about route of transmission and be targeted at those with a lower education level.

Project description:Objective:Study objectives were to develop a questionnaire to assess factors influencing pharmacists' adoption of prescribing (i.e., continuing, adapting or initiating therapy), describe use of pre-incentive and mixed mode survey, and establish survey psychometric properties. Methods:Questions were developed based on prior qualitative research and Diffusion of Innovation theory. Expert review, cognitive testing, survey pilot, and main survey were used to test the questionnaire. Six content experts reviewed the questionnaire to establish face and content validity. Ten pharmacists from diverse practice settings were purposefully recruited for a cognitive interview to verify question readability. Content analysis was used to analyze the results. A pre-survey introduction letter with a monetary incentive was mailed via post to 100 (i.e. pilot) and 700 (i.e., main survey) randomly selected pharmacists. This was followed by an e-mail with a personalized link to the online questionnaire, e-mail reminders, and a telephone reminder if required. The psychometric properties of scales were evaluated with an exploratory factor analysis and Cronbach's alpha. Scale responses were described. Results:Engagement of six experts and ten pharmacists clarified definitions (e.g., prescribing), terminology, recall periods, and response options for the 34-item response scale. Fifty-six pharmacists completed the online pilot survey. Based on this data, ambiguous questions and routing issues were addressed. Three hundred and seventy-eight pharmacists completed the online main survey for a response rate of 54.6%. The factors analysis resulted in 27 questions in eight scales: (1) self-efficacy, (2) support from practice environment, (3) support from interprofessional relationship, (4) impact on professionalism, (5) impact on patient care), (6) prescribing beliefs, (7) technical use of electronic health record (EHR) and (8) patient care use of the EHR. Prescribing beliefs and technical use of the EHR scales had low reliability while the remaining six scales had strong evidence for reliability and validity. Conclusion:Through a multi-stage process, a survey instrument was developed to capture pharmacists' perceptions of prescribing influences. This questionnaire may support future research to develop interventions to enhance adoption of prescribing and enhance direct patient care by pharmacists.

Project description:This article provides a description of an agricultural household survey data of rice growers collected in Ecuador between October 2014 and March 2015. The household survey was implemented using a structured questionnaire administered among 1028 households in the main rice production areas of Ecuador (i.e. Guayas, Los Rios, Manabi, and El Oro provinces). Information collected was provided by household heads (male or female) and included household and plot level data. The survey information includes household socio-demographic characteristics (e.g. age, education, gender, main economic activity, etc.), farm characteristics (e.g. farm land size, assets ownership, other crops planted, etc.), rice management practices (e.g. variety and input use, production costs, etc.), and rice production and utilization (e.g. yields, prices, sales, etc.). Additional socio-economic context variables were also recorded such as government subsidies to rice production, participation in rural organizations, and food security related questions. The dataset contains a total of 6288 variables among numeric, categorical and string variables. The dataset is shared publicly on the Harvard dataverse site and provide access to questionnaires, the complete data and a brief report.

Project description:Western Kenya suffers a highly endemic and also very heterogeneous epidemic of human immunodeficiency virus (HIV). Although female sex workers (FSW) and their male clients are known to be at high risk for HIV, HIV prevalence across regions in Western Kenya is not strongly correlated with the fraction of women engaged in commercial sex. An agent-based network model of HIV transmission, geographically stratified at the county level, was fit to the HIV epidemic, scale-up of interventions, and populations of FSW in Western Kenya under two assumptions about the potential mobility of FSW clients. In the first, all clients were assumed to be resident in the same geographies as their interactions with FSW. In the second, some clients were considered non-resident and engaged only in interactions with FSW, but not in longer-term non-FSW partnerships in these geographies. Under both assumptions, the model successfully reconciled disparate geographic patterns of FSW and HIV prevalence. Transmission patterns in the model suggest a greater role for FSW in local transmission when clients were resident to the counties, with 30.0% of local HIV transmissions attributable to current and former FSW and clients, compared to 21.9% when mobility of clients was included. Nonetheless, the overall epidemic drivers remained similar, with risky behavior in the general population dominating transmission in high-prevalence counties. Our modeling suggests that co-location of high-risk populations and generalized epidemics can further amplify the spread of HIV, but that large numbers of formal FSW and clients are not required to observe or mechanistically explain high HIV prevalence in the general population.
