Supplied Food Consistency and Oral Functions of Institutionalized Elderly.

ABSTRACT: Background:Maintaining good oral function is one of the goals of dental treatment. The Japanese national insurance system newly introduced the concept of management of oral function according to the life stage. For the application of management of oral functions of the elderly, seven kinds of examination is a must for the diagnosis: xerostomia, oral hygiene status, maximum occlusal pressure, tongue and labium function, tongue pressure, chewing ability, and swallowing function. We analyzed the relationship between oral functions and supplied food consistency. Methods:Oral functions and supplied food consistency of sixty-nine institutionalized elderly were investigated. There were 13 men and 56 women, and their mean age was 86.23?±?7.02. Oral functions were measured and evaluated according to the Japanese insurance system. Data were analyzed by item response theory analysis, ROC analysis, and decision analysis. Results:By the item response theory analysis, tongue pressure and swallowing functions had high discrimination ability. The subjects who had malfunction of the tongue and labium all had processed food. The subjects with difficulty in swallowing, even without malfunction of the tongue and labium, all had processed food. Conclusion:Supplied food consistency may depend on the oral functions. However, as oral function has some dimension, a systematic evaluation system is necessary to decide the supplied food consistency.

Project description:A fundamental question in multiclass classification concerns understanding the consistency properties of surrogate risk minimization algorithms, which minimize a (often convex) surrogate to the multiclass 0-1 loss. In particular, the framework of calibrated surrogates has played an important role in analyzing Bayes consistency of such algorithms, i.e. in studying convergence to a Bayes optimal classifier (Zhang, 2004; Tewari and Bartlett, 2007). However, follow-up work has suggested this framework can be of limited value when studying H -consistency; in particular, concerns have been raised that even when the data comes from an underlying linear model, minimizing certain convex calibrated surrogates over linear scoring functions fails to recover the true model (Long and Servedio, 2013). In this paper, we investigate this apparent conundrum. We find that while some calibrated surrogates can indeed fail to provide H -consistency when minimized over a naturallooking but naïvely chosen scoring function class F , the situation can potentially be remedied by minimizing them over a more carefully chosen class of scoring functions F . In particular, for the popular one-vs-all hinge and logistic surrogates, both of which are calibrated (and therefore provide Bayes consistency) under realizable models, but were previously shown to pose problems for realizable H -consistency, we derive a form of scoring function class F that enables Hconsistency. When H is the class of linear models, the class F consists of certain piecewise linear scoring functions that are characterized by the same number of parameters as in the linear case, and minimization over which can be performed using an adaptation of the min-pooling idea from neural network training. Our experiments confirm that the one-vs-all surrogates, when trained over this class of nonlinear scoring functions F , yield better linear multiclass classifiers than when trained over standard linear scoring functions.

Project description:The oral health of an ageing population, especially that of the institutionalized elderly population, constitutes a significant concern because it is closely linked to general health and the quality of life. Shared common risk factors drive the development and worsening of poor oral health and non-communicable diseases, which eventually lead to self-care inability. Several studies have reported on the poor oral health of the institutionalized elderly population. However, few comprehensive reports exist regarding the relationship between poor oral health, the oral health-related quality of life (OHRQoL) and the associated factors in this specific population. Objective: The objective is to describe recently reported oral health levels, the OHRQoL and the associated factors among older institutional residents. Methods: Studies published between July 2009 and June 2019 in MEDLINE, EMBASE and CINAHL were searched. The population, intervention, comparison and outcome (PICO) strategy was used as a guide. The reported factors related to poor oral health were identified (i.e., age, gender, educational level, acquired systemic conditions or dementia/cognitive impairment). Results: Twenty-five surveys (or study series) from 19 countries were included. The level of evidence reported by these studies was generally moderate to strong. The reported oral cleanliness and health of the surveyed institutionalized elderly were poor (>50% of residents had calculus; denture hygiene index > 80%). Gum (approximately 30% of dentate residents had moderate to severe periodontitis), teeth (decayed, missing or filled teeth >20), mucosa (>10% had mucosal lesions) and denture problems (up to 40%) were prevalent and were associated with a poor OHRQoL, especially in females, socially deprived residents or those with mild or above cognitive impairment. Those with a poor OHRQoL might show signs of poor nutrition. Conclusions: This report reviewed evidence-based knowledge on oral health, the OHRQoL and the associated factors among elderly institutional residents. Further research is needed to confirm these observations. For improved oral health, a better OHRQoL and the general well-being of older residents, clinical trials are needed, targeting modifiable factors, such as social inequality, oral healthcare accessibility, and/or nursing home service quality. The relationship between oral health, the OHRQoL and nutrition in this at-risk population also warrants exploration.
