Project description:Agrochemicals are essential but hazardous inputs being utilized at different stages in cocoa production. Safeguarding the health of workers handling these chemicals is therefore of utmost importance. Although Ghanaian government implemented mass spraying of cocoa with every essential occupational safety being followed, non-workability of the programme in many parts of the cocoa producing areas necessitates supplementary application of agrochemicals by many farmers. Therefore, a survey was conducted in Ahafo Ano North district of the Ashanti region in 2015 to understand the compliance of farmers to safety guidelines in handling agrochemicals. The survey was conducted with structured questionnaires that were written in English language and translated into the local language in the course of the interviews. A total of 246 cocoa farmers were interviewed using stratified sampling procedures. The questionnaire, which was divided into four sections solicited information on farmers' socioeconomic characteristics, safeguard measures being taken by the farmers in the course of handling agrochemicals, health complaints after handling agrochemicals and stress and occupational hazards. The dataset is herewith made available and it is considered of vital usefulness given some serious policy implications of occupational health hazards among cocoa farmers.

Project description:BackgroundThere is growing interest in the use of reliable evidence for health decision-making among low-and middle-income countries. Ghana has deployed DHIMS2 to replace the previously existing manual data harmonization processes.MethodsThis cross-sectional study was conducted in 12 districts comprising 12 district directorates, 10 district hospitals, 29 sub-district health centers, and 38 community health facilities in the Brong-Ahafo Region. Data collection tools were developed based on the Measure Evaluate assessment tools designed for evaluating the performance of routine information systems management tools. Utilization was assessed based on documented evidence and data was analyzed using STATA version 14.ResultsAlthough 93% of the health facilities studied submitted data unto the DHIMS2 platform, evidence suggested low use of this data in decision-making, particularly at the community level facilities where only 26% of the facilities used data from DHIMS2 to inform annual action plans and even less than 20% examined findings and issued directives for action. At the district level, 58% issued directives based on DHIMS2 information, 50% used DHIMS2 information for Advocacy purposes and 58% gave feedback reports based on DHIMS2 data for action. Functional computers were lacking across all facilities.ConclusionsActivities relating to the use of DHIMS2 information skew towards data quality checking with less focus on examining findings, making comparisons, and taking action-based decisions from findings and comparisons. Improving factors like internet access, availability of functional ICTs, frequency of supervisory visits, staff training and the provision of training manuals may facilitate the use of DHIMS2 in decision-making at all levels of the district health system.

Project description:A nationwide outbreak of Vibrio cholerae occurred in Ghana in 2014 with Accra, the nation's capital as the epi-center. The outbreak spread to the Brong Ahafo Region (BAR) which is geographically located in the middle of the country. In this region a review of data collected during the outbreak was carried out and analyzed descriptively to determine the hot spots and make recommendations for effective response to future outbreaks.A review of patient records and line lists of cases of cholera reported in all hospitals during the period of the outbreak (July-December 2014) was conducted. Hospitals used IDSR (Integrated Disease Surveillance and Response system) standard case definitions to detect and report cases for management. The GPS coordinates of all districts and health facilities were collected and utilized in the construction of spot maps. We also obtained populations (denominators) from the BAR Health surveillance unit of the Ghana Health Service. All the data thus collected was analyzed descriptively and expressed as frequencies and rates.A total of 1035 cases were reported, 550 (53.4%) were males and the rest females. Their ages ranged from 1 to 95 years; (mean age of 28.2 ± 19.6 years). The most affected (23.5%) was the 20-29 year old age group. On the 30th July, 2014, a 26 year old male (recorded as the index case of the cholera outbreak in the Brong Ahafo region) with a history of travel from Accra reported to the Nkoranza district hospital with a history of symptoms suggestive of cholera. The reporting of cholera cases reached their peak (17.3%) in week 15 of the outbreak (this lasted 25 weeks). An overall attack rate of 71/100,000 population, and a case fatality rate of 2.4% was recorded in the region. Asutifi South district however recorded a case fatality of 9.1%, the highest amongst all the districts which recorded outbreaks. The majority of the cases reported in the region were from Atebubu-Amanten, Sene West, Pru, and Asunafo North districts with 31.1, 26.0, 18.2 and 9.9% respectively. Vibrio cholerae serotype O1 was isolated from rectal swabs/stool samples tested.Vibrio cholerae serotype O1 caused the cholera-outbreak in the Brong Ahafo Region and mainly affected young adult-males. The most affected districts were Atebubu-Amanten, Sene west, Pru (located in the eastern part of the region), and Asunafo North districts (located in the south west of the region). Case Fatality Rate was higher (2.4%) than the WHO recommended rate (<1%). Active district level public health education is recommended on prevention and effective response for future outbreaks of cholera.

Project description:We present historical monthly spatial models of temperature and precipitation generated from the North American dataset version "j" from the National Oceanic and Atmospheric Administration's (NOAA's) National Centres for Environmental Information (NCEI). Monthly values of minimum/maximum temperature and precipitation for 1901-2016 were modelled for continental United States and Canada. Compared to similar spatial models published in 2006 by Natural Resources Canada (NRCAN), the current models show less error. The Root Generalized Cross Validation (RTGCV), a measure of the predictive error of the surfaces akin to a spatially averaged standard predictive error estimate, averaged 0.94?°C for maximum temperature models, 1.3?°C for minimum temperature and 25.2% for total precipitation. Mean prediction errors for the temperature variables were less than 0.01?°C, using all stations. In comparison, precipitation models showed a dry bias (compared to recorded values) of 0.5?mm or 0.7% of the surface mean. Mean absolute predictive errors for all stations were 0.7?°C for maximum temperature, 1.02?°C for minimum temperature, and 13.3?mm (19.3% of the surface mean) for monthly precipitation.

Project description:BackgroundUnmet need for contraception contributes to the burden of unwanted pregnancies, which are correlated with a host of adverse maternal and child outcomes. The aim of this study was to determine the prevalence and identify the determinants of unmet need for contraception in North Gonja District, Ghana.MethodsA cross-sectional survey involving 386 randomly selected women of childbearing age was conducted in North Gonja district, Ghana, with the use of a questionnaire in household interviews. Women were classified as having unmet need for contraception if they were fecund, sexually active and wished to postpone the next birth or halt childbearing completely but were not using any form of contraception. Chi-square/Fisher's exact test and logistic regression analysis were used to identify the determinants of unmet need.ResultsThe mean age of the study population was 26.1 (±8.4) years and awareness on contraception was almost universal in the district (95.9%). The overall prevalence of unmet need for contraception was 38.9%, with 27.5% having unmet need for limiting and 12.2% unmet need for spacing. In multivariate analysis, compared to women aged 25-29 years, those aged 20-24 years [Adjusted Odds Ratio (AOR) 0.26; 95% Confidence Interval (CI) 0.11-0.58] and 30 years and above (AOR 0.25; 95% CI 0.09-0.73) were less likely to have unmet need for contraception. However, uneducated women (AOR 5.06; 95% CI 1.07-24.01) compared with those educated to tertiary level; those unaware of family planning (AOR 3.93; 95% CI 1.12-13.80) compared to those aware; and those who had not previously practised contraception (AOR 1.81; 95% CI 1.09-3.00) compared to those who did were more likely to have unmet need.ConclusionsThe present study found high prevalence of both awareness on and unmet need for contraception among the study population. Unmet need for contraception is associated with age, educational status, awareness on family planning and previous contraception practice. Educational campaigns to promote contraception should prioritize women of middle age and low educational status. Further studies are needed to understand the low correlation between awareness on and unmet need for contraception.

Project description:Reduced climatic suitability due to climate change in cocoa growing regions of Ghana is expected in the coming decades. This threatens farmers' livelihood and the cocoa sector. Climate change adaptation requires an improved understanding of existing cocoa production systems and farmers' coping strategies. This study characterized current cocoa production, income diversification and shade tree management along a climate gradient within the cocoa belt of Ghana. The objectives were to 1) compare existing production and income diversification between dry, mid and wet climatic regions, and 2) identify shade trees in cocoa agroforestry systems and their distribution along the climatic gradient. Our results showed that current mean cocoa yield level of 288kg ha-1yr-1 in the dry region was significantly lower than in the mid and wet regions with mean yields of 712 and 849 kg ha-1 yr-1, respectively. In the dry region, farmers diversified their income sources with non-cocoa crops and off-farm activities while farmers at the mid and wet regions mainly depended on cocoa (over 80% of annual income). Two shade systems classified as medium and low shade cocoa agroforestry systems were identified across the studied regions. The medium shade system was more abundant in the dry region and associated to adaptation to marginal climatic conditions. The low shade system showed significantly higher yield in the wet region but no difference was observed between the mid and dry regions. This study highlights the need for optimum shade level recommendation to be climatic region specific.

Project description:Background:Anaemia in pregnancy is a major problem in both developed and developing countries. The commonest source of anaemia is nutritional deficiency of iron with evidence suggesting that up to 90% of maternal anaemia may be due to inadequate consumption of dietary iron; however, there are other causes which include worm infestation, HIV infection, and genetic disorders. There are some implemented approaches in Ghana including education and awareness creation, nutritional supplements, and control and prevention of parasitic infections among others to prevent and control anaemia in pregnancy. This study assessed pregnant women adherence to Ghana's anaemia prevention strategies being implemented in the Juaboso District. Method:A descriptive cross-sectional data on knowledge of and adherence to anaemia prevention strategies among pregnant women was collected. Pearson's chi-square and logistic regression models were used to assessed associations between predictor and outcome variables. A p value <0.05 was considered as statistically significant. Findings. About 13.5% of the pregnant women had high knowledge on anaemia, while 58.4% and 28.1% had moderate and low knowledge, respectively. Less than half (39.1%) of the women adhered to anaemia prevention strategies. There were significant associations between knowledge of anaemia and where pregnant woman resides in the district (AOR: 2.04, 95% CI: 2.16-9.83, p = 0.003), woman's educational (AOR: 10.43, 95% CI: 6.14-51.63, p = 0.002), and occupational status (AOR: 15.14, 95% CI: 13.57-18.43, p < 0.001). Again, there were significant associations between adherence to anaemia prevention strategies and the woman's ethnicity (AOR: 0.61, 95% CI: 0.04-0.92, p = 0.001) and her knowledge of anaemia (AOR: 3.88, 95% CI: 1.32-7.93, p = 0.001). Conclusions:Pregnant women's knowledge of anaemia and adherence to anaemia prevention strategies are not encouraging. However, anaemia in pregnancy and its consequences could be devastating to all stakeholders if actions are not taken to reduce the phenomenon. Therefore, we recommend that more education and sensitisation programs including good nutritional practices in the diet of pregnant women be promoted to increase awareness and adherence to anaemia prevention strategies among pregnant women in the Juaboso District.

Project description:Climate change threatens water, sanitation and hygiene (WaSH) facilities and services, as these are intimately linked to the water cycle and are vulnerable to changes in the quantity and quality of available water resources. Floods and droughts, which pollute and reduce water delivery respectively, have now become a perennial issue to deal with in the northern regions of Ghana. This study aimed to assess the degree to which climate change adaptation measures are mainstreamed into the water, sanitation and hygiene (WaSH) development planning process in Ghana. Stakeholders from government and non-government agencies were interviewed to gain perspectives on the threat of climate change, the inclusion of climate change in WaSH planning and the barriers preventing mainstreaming. Despite awareness of climate change, adaptation measures have not been considered, and the immediate WaSH needs remain the priority. Overall, stakeholders felt the adaptive capacity of the Municipality was low and that mainstreaming has not yet occurred. Despite the lack of progress, there are great opportunities for mainstreaming climate change adaptation into planning through increasing awareness and capacity, legislative and institutional changes and the development of participatory systems to provide early warning systems and disaster risk analyses that will inform future planning.

Project description:The dataset covers the determinants of adaptation innovation in the water sector in Africa over the period 1990-2016. The data is collected from secondary sources; namely the World Bank, Organization for Economic Co-operation and Development databases and the University of Notre Dame's Global Adaptation Initiative. The data is focused on susceptibility to water stress caused by climate change and the public response in the form of technology development. The analysis performed on the data focused on the degree to which exposure to the risk of water insecurity is a motivating factor in the public response. In the analysis, an econometric model was specified for a relationship between a measure of water stress induced by climate change and adaptation innovation, along with a series of socio-economic and socio-political indicators as controls. Sustainable development practitioners, environmental and social scientists with research and teaching interests on Africa will find the dataset very useful. Sustainable development practitioners can use the data to chart simple trends and for other summative purposes. The data can also be used to make regional or geopolitical comparisons on the same subject of our analysis. Furthermore, with similar technology innovation data on other sectors exposed to climate change risks, comparisons of public responses can be undertaken to understand relative effectiveness of climate change adaptation responses. Crucially, the simple format of the data makes it a very convenient teaching tool in a statistics or econometrics class.

Project description:ObjectiveTo describe healthcare worker (HCW)-identified system-based bottlenecks and the value of local engagement in designing strategies to improve referral processes related to emergency obstetric care in rural Ghana.DesignQualitative study using semistructured interviews of participants to obtain provider narratives.SettingReferral systems in obstetrics in Assin North Municipal Assembly, a rural district in Ghana. This included one district hospital, six health centres and four local health posts. This work was embedded in an ongoing quality improvement project in the district addressing barriers to existing referral protocols to lessen delays.Participants18 HCWs (8 midwives, 4 community health officers, 3 medical assistants, 2 emergency room nurses, 1 doctor) at different facility levels within the district.ResultsWe identified important gaps in referral processes in Assin North, with the most commonly noted including recognising danger signs, alerting receiving units, accompanying critically ill patients, documenting referral cases and giving and obtaining feedback on referred cases. Main root causes identified by providers were in four domains: (1) transportation, (2) communication, (3) clinical skills and management and (4) standards of care and monitoring, and suggested interventions that target these barriers. Mapping these challenges allowed for better understanding of next steps for developing comprehensive, evidence-based solutions to identified referral gaps within the district.ConclusionsProviders are an important source of information on local referral delays and in the development of approaches to improvement responsive to these gaps. Better engagement of HCWs can help to identify and evaluate high-impact holistic interventions to address faulty referral systems which result in poor maternal outcomes in resource-poor settings. These perspectives need to be integrated with patient and community perspectives.
