Project description:Statement of problemTo separate the crown from the titanium base abutment, by using heat, caused oxidization of the titanium base abutment. The effect of this procedure on the retention of a crown is unclear.PurposeTo compare the resin bond strength and failure type between zirconia crowns and titanium base abutments utilizing four different surface treatments. Surface roughness and morphology of each surface treatment were also investigated.Material and methodsForty titanium base abutments (Variobase®) were divided into four groups, 1. Control, 2. Air abraded, 3. Oxidized, and 4. Oxidized-air abraded. Oxidized and oxidized-air abraded groups were debonded from zirconia crowns using constant dry heat at 500 ˚C. For air abraded and oxidized-air abraded groups (after oxidization), the titanium base abutments were air abraded with Al3O2. After cleaning, one specimen of each group was investigated under a non-contact profilometer (50x), then the same samples were investigated under SEM at 25,300,500,1000 magnification and EDS at 30kV of accelerated voltage. All specimens were then cemented (RelyX Ultimate). After aging, with thermocycling under 5C° to 55C°,120 seconds dwell time for 5,000 cycles, bond strength was tested and statistical differences were calculated with One-way ANOVA (p-value <0.05) follow by Tukey test. All separated crowns and titanium base abutments were investigated under a light microscope (20x), using fisher's exact test for correlation of the failure types.ResultsThere was a significant difference in the mean value of tensile bond strength among the control and test groups. Comparisons between control(237.6±46.3N) and oxidized(241.7±46.3N) showed statistically different values from air abraded(372.9±113.2N) when assembled using different surface treatments of the titanium-based abutments. (p-value<0.005) As for failure type, there were statistically significant differences between control versus air abraded, control versus oxidized-air abraded, oxidized versus air abraded, and oxidized versus oxidized-air abraded. (p-value<0.001) The titanium surface morphology shown from the profilometer and SEM was coordinated. Control (Ra 333.8nm) and oxidized (Ra 321.0nm) groups surfaces showed smooth, corrugated surfaces, meanwhile air abraded (Ra 476.0nm) and oxidized-air abraded (Ra 423.8nm) groups showed rough, rugged surfaces.ConclusionHeat oxidization of titanium-based abutments did not adversely affect tensile bond strength or the failure mode and surface roughness between titanium base abutments and zirconia crowns. However, air abrasion of the titanium surface increased surface roughness and retentive strength.Clinical implicationsThe titanium base abutments that were oxidized under heat treatment did not have an effect on crown retention. Thoroughly air abraded the titanium abutment prior to cementation can increase cement bond strength.

Project description:BACKGROUND:Removal of cement-retained implant fixed restorations when needed, can be challenging. Conventional methods of crown removal are time consuming and costly for patients and practitioners. This research explored the use of two different types of pulsed erbium lasers as a non-invasive tool to retrieve cemented zirconia crowns from zirconia implant abutments. MATERIALS AND METHODS:Twenty identical zirconia crowns were cemented onto 20 identical zirconia prefabricated abutments using self-adhesive resin cement. The specimens were divided into two groups for laser assisted crown removal; G1 for erbium-doped yttrium aluminum garnet laser (Er:YAG), and G2 for erbium, chromium-doped yttrium, scandium, gallium and garnet (Er,Cr:YSGG). For the G1, after the first crown removal, the specimens were re-cemented and removed again using the Er:YAG laser. Times needed to remove the crowns were recorded and analyzed using ANOVA (α = 0.05). The surfaces of the crown and the abutment were further examined using scanning electron microscopy (SEM) and energy-dispersive X-ray spectroscopy (EDS) analyses. RESULTS:The average times of zirconia crown removal from zirconia abutments were 5 min 20 sec and 5 min 15 sec for the Er:YAG laser of first and second experiments (G1), and 5 min 55 sec for the Er,Cr:YSGG laser experiment (G2). No statistical differences were observed among the groups. SEM and EDS examinations of the materials showed no visual surface damaging or material alteration from the two pulsed erbium lasers. CONCLUSIONS:Both types of pulsed erbium lasers can be viable alternatives for retrieving a zirconia crown from a zirconia implant abutment. Despite operating at different wavelengths, the Er:YAG and Er,Cr:YSGG lasers, perform similarly in removing a zirconia crown from a zirconia implant abutment with similar parameters. There are no visual and elemental composition damages as a result of irradiation with pulsed erbium lasers.

Project description:BACKGROUND:When implants are restored with cement-retained restorations, prosthetic retrievability can be difficult and often requires sectioning using rotary instruments. Sometimes repeated removals of a cement-retained implant crown are needed such as for treatment of peri-implantitis or immediate implant provisionalization. The purpose of this study was to evaluate the effect of erbium-doped yttrium aluminum garnet (Er:YAG) laser as a non-invasive treatment modality to remove lithium disilicate crowns from zirconia implant abutments following long-term cementation, repetitive debonding and re-cementation, and short-term retrieval. MATERIAL AND METHODS:Twenty identical lithium disilicate crowns were cemented onto zirconia prefabricated abutments using composite resin cement. Ten cemented crowns were removed at 48 hours after cementation as a short-term group (ST), while another 10 were removed 6 months after cementation as a long-term group (LT). To mimicking repetitive recementation and retrieval, the LT crowns were then recemented and removed after 48 hours as a long-term recemention (LTR) group. The LTR crowns were then again recemented and removed after 48 hours as a long-term repeated recemention (LTRR) group. Er:YAG laser was used to facilitate the retrieval of these crowns. recorded and analyzed using ANOVA and t-test. The surfaces of the crown and the abutment were further examined using light microscopy and scanning electron microscopy (SEM). Temperature changes of the abutment and crown upto 10 minutes were also measured and statistically analyzed (paired t-test). RESULTS:The average times of crown removal from zirconia abutments were 4 minutes (min) and 42 second (sec) in LT to 3 min 24 sec in LTR, and 3 min 12 sec in LTRR and ST groups. LTR took the longest time to remove, statistically (ANOVA and t-test, p < .001). No statistical differences were observed among the removal times of LTR, LTRR, and ST groups (t-test, p = .246, .246 and 1). SEM examination of the material surface showed no visual surface damaging from treatment with Er:YAG laser. The temperatures during irradiation ranged from 18.4°C to 20°C and 22.2°C to 24.5°C (Paired t-test, p < .0001) for the abutment and the crown during irradiation from 1 min to 10 mins. CONCLUSIONS:Long-term cementation can increase time in lithium disilicate crown removal from zirconia abutment using Er:YAG. Er:YAG laser is a non-invasive tool to remove cement-retained implant prostheses and should be considered as a viable alternative to rotary instruments.

Project description:AIM:This study aimed to evaluate the effect of lithium disilicate endocrowns compared to prefabricated zirconia crown used for restoring pulpotomized primary molars, on their Fracture Resistance and to compare the loads to failure these different ceramic restorations with previously reported posterior occlusal forces. METHODS:Twenty mandibular left second primary molars were randomly distributed into two groups (n = 10 in each group) the zirconia Crown (Nusmile zr.) Group (G1) and the lithium disilicate (IPS e.max Press) Endocrown Group (G2). In all groups pulpotomy procedure was done before preparation then each sample was prepared based on their allocated restoration, both zirconia crown (Nusmile zr.) and endocrown (IPS e.max Press) were cemented by dual-cure resin cement. All samples were loaded to failure using a universal testing machine (Instron, USA), and the compressive force was applied. The data were analysed using one-way (ANOVA) and Tukey's post hoc significance difference tests. Differences were considered significant at (p< 0.05). RESULTS:Group zirconia crown (G1) showed significantly higher fracture strength than Group (G2) lithium disilicate endocrown (p < 0.05). CONCLUSION:The zirconia crown showed higher fracture resistance than lithium disilicate endocrown. However, both tested zirconia crown and lithium disilicate endocrown withstood the application of axial occlusal forces greater than the reference values for posterior occlusal loads.

Project description:ObjectivesThis study aimed to evaluate the feasibility of monolithic zirconia laminate veneers (MZLV) compared to lithium disilicate laminate veneers (LDLV).Materials and methodsSixty resin replicas, each prepared with depths of 0.5 mm, 0.7 mm, and 1 mm, were produced using a 3D printer from acrylic teeth. Laminate veneers of these thicknesses were milled from pre-sintered monolithic zirconia (3rd generation) and lithium disilicate blocks. The intaglio surface of MZLV was treated with air abrasion using 110 μm diameter silica-modified aluminium oxide particles and ceramic primer, while LDLV was etched with etchant gel and treated with the ceramic primer before cementation with resin cement. Vertical marginal discrepancy (VMD) was assessed using a stereomicroscope, and a load-to-failure test was conducted using a universal testing machine. Failure modes were evaluated macroscopically on fractured surfaces. Data were analysed statistically using Two-way ANOVA and Bonferroni correction (α = 0.05).ResultsLDLV samples exhibited significantly larger VMD compared to MZLV samples across all thicknesses, especially in cervical, palatal, and mean data. Within the LDLV group, load-to-fracture values for 0.7 mm and 1.0 mm thicknesses were similar, whereas for 0.5 mm thickness, it was significantly lower. In the MZLV group, load-to-fracture values were lower for 0.7 mm and 1.0 mm thicknesses compared to LDLV, but higher for 0.5 mm thickness.ConclusionsMaterial choice and restoration thickness significantly influence laminate veneer restorations' success. MZLV generally exhibits superior vertical marginal fit compared to LDLV, with varying load-to-failure values across different thicknesses. Clinical management of debonding in MZLV is simpler compared to restoration fracture in LDLV.Clinical relevanceConsidering clinical factors, MZLV may be a preferable option to LDLV for this restoration with the thickness of 0.5 mm.

Project description:ObjectivesTo investigate survival rates, technical and biologic outcomes of buccally micro-veneered all-ceramic single implant crowns.Material and methodsSixty subjects randomly received immediately or early placed implants. Crowns out of lithium-disilicate (n = 30) and zirconia-ceramic (n = 30) were bonded to titanium-base-abutments. Restorations were inspected at baseline (BL) and during follow-up visits (6, 12 months). Technical and biologic parameters were recorded. Data were analysed descriptively. Differences between groups were tested with Student's t-test. Paired T-test was used when comparing data from the same implant or tooth over time. Linear model repeated measures were used to test differences between materials over time. Differences in counts were evaluated using Pearson Chi-square test. The level of significance was at p < .05.ResultsAfter a mean observation time of 13.2 ± 2.4 months, 54 restorations were re-examined. The implant survival rate was 98.3%, and the restoration survival rate was 100%. One early implant failure occurred. Two minor chippings occurred in lithium-disilicate restorations. No chippings or fractures occurred in any zirconia restorations at 1 year (0%). The technical complication rate was 3.7%, with 7.7% complications among the lithium-disilicate restorations and no differences between the two materials (p = .558). At 1-year follow-up, mean Bleeding on Probing (BOP) was higher at implants (0.27 ± 0.3) than adjacent teeth (0.17 ± 0.18) (p = .046) with no differences between materials (p = .36). Differences in pocket probing depth (PPD) between implants and adjacent teeth were significant (p < .01). Jemt Index improved significantly from BL to 1-y-follow-up (p < .001).ConclusionsPreliminary results were good, suggesting that the ceramic type does not impact technical and biologic parameters.

Project description:Two-piece abutments consisting of customized zirconia abutment copings and prefabricated titanium bases are popular due to their biological and esthetic advantages. Glass-ceramic solder (GS) is an alternative biocompatible connective agent. This in vitro study evaluated the retentive force of GS in comparison to classical resin composite cements (RC) after artificial aging and autoclaving. Ninety specimens consisting of prefabricated titanium bases and zirconia abutment copings were fabricated. The two parts of each specimen were fixed either by RC (n = 30) or GS with a luting space of either 30 µm (n = 30) or 100 µm (n = 30). Ten specimens of each group underwent autoclaving before artificial aging (water storage, thermocycling). Twenty specimens (including the 10 autoclaved specimens) of each group were exposed to a mechanical load. The retentive force between the zirconia and titanium in all specimens was determined. A fractographic analysis was performed to analyze the fracture surfaces of the GS specimens. The RC- and GS-connected two-piece abutments showed no relevant differences, independent of the luting space. RC appears to be more vulnerable to the thermal and mechanical loads than GS. Thus, GS may be an appropriate alternative to RC for two-piece abutments, especially for patients with enhanced biocompatibility requirements.

Project description:BackgroundGuidelines in pediatric restorative dentistry recommend the use of preformed pediatric stainless steel crowns (SSCs) in cases of severe tooth decay of at least two surfaces. This clinically effective and safe restorative option is frequently refused by parents for esthetic reasons; they prefer conventional restorations using esthetic filling materials (composites, glass ionomer) if lesion severity limited to two surfaces permits. Recently, manufacturers have proposed esthetic preformed pediatric zirconia crowns (ZCs) but these have been assessed in only two randomized clinical trials (RCT) with follow-ups of 6 and 12 months. Only one of these RCTs was carried out on primary molars to test ZCs (NuSmile ZR) without a groove in its inner surface. The primary objective of this proposed RCT is to assess the effectiveness of ZCs compared with SSCs. Our hypothesis is that the effectiveness of ZCs will be equivalent to that of SSCs.MethodsIn this split-mouth, 2-year RCT, pairs of primary molars in 101 child participants will be randomized and restored with SSCs (ESPE, 3M) and ZCs (EZCrowns, Sprig Oral Health Technologies) characterized by grooves on their inner surface. Primary molars will first be allocated to SSCs, and 1 to 2 weeks later the other primary molar of the same pair will be restored by ZC. The primary outcome is the success defined by the "absence of major clinical and radiographic failure" (e.g., pain, pulp infection, dental abscess or periradicular pathology visible on radiographs). The secondary outcomes include the retention and fracture rates, the gingival condition, the wear of the antagonist of the treated teeth, as well as both parental and child satisfaction.DiscussionThis study will investigate two types of preformed pediatric crowns for the management of severe decay on primary molars. The results may help practitioners choose the better therapeutic option and to explain to parents the advantages and disadvantages of these two therapies.Trial registrationNCT03296709 . Project description:The aim of the study was to evaluate the stability, reactivity, and bond strength with a lithium disilicate ceramic of a self-etch silane primer (Monobond Etch and Prime/MEP). The stability was evaluated by 1H-,31P-NMR spectroscopy (before/after aging), and the reactivity by micro MIR-FTIR spectroscopy on Ge surfaces (0, 1, 24 h) using a prehydrolyzed silane primer (Calibra Silane Coupling Agent/CLB), as a control. The effect of MEP vs. 5% HF-etching on ceramic roughness was assessed by optical profilometry. The shear bond strength (SBS) of a resin composite bonded to polished ceramic surfaces treated with MEP, HF without silane (HF+NS), HF+CLB, and HF+MEP (n = 20) was evaluated after storage in water (A: 37 °C/1 week, B: 5000×/5-55 °C and C: 100 °C/24 h). Aging did not affect the silanol groups of MEP, but only the phosphate co-monomer. Silanols were reactive forming siloxanes, but exhibited lower consumption rate than CLB. HF-etching induced significantly higher values than MEP, in all the roughness parameters tested (Sa, Sz, Sdr, Sc, Sv), with the greatest differences found in Sdr and Sv. For SBS, MEP was inferior to all treatments/storage conditions, except of HF+NS in A, where the values were similar. However, on a HF-etched substrate, MEP provided highest strength and reliability.

Project description:There has been little research conducted on how ultrasonic cavitation may affect glass dissolution. The focus of this study was to examine how the mechanisms and kinetics of glass dissolution may change in a system that included ultrasonication. Experiments were conducted on lithium disilicate glass in deionized water at 50 °C between 1 and 7.5 h. Results showed that the erosion from ultrasonication affected the kinetics of glass dissolution. Samples with erosion had 2-3 × more dissolution compared to samples without erosion. The change in dissolution was thought to be partly caused by an increase in the surface area of the sample to volume of solution (SA/V) ratio due to the roughening of the surface and release of particulates and a reduction in the size of the depleted layer due to erosion. Stereoscopic 3D reconstruction of eroded samples was used to calculate the increase in surface area due to erosion. Type 2 surface areas (exfoliation mixed with normal leaching) were roughly 3-6% greater while Type 3 surface areas (heavy roughening of surface) were roughly 29-35% greater than the surfaces areas from Type 1 surfaces (normal leaching).
