Project description:BackgroundGirl child marriage is increasingly recognized as a critical barrier to global public health and gender discrimination. There are still more gaps in the global rate of child marriage and the underlying factors. Thus, the present systematic review aimed to explore the prevalence of child marriage and the underlying factors.MethodsA comprehensive search was conducted for all English-language studies that measured the prevalence of child marriage and its correlates from 2000 to March 2022, indexed in PubMed, Scopus, Web of Science, Poplin, and Google Scholar databases. Child marriage is defined as marriage under the age of 18. In the present study, Joanna Briggs' quality assessment checklist was used for data collection. Two independent reviewers reviewed all the articles.ResultsIn total, 34 eligible prevalence articles and 14 trend articles were included in the study with data from 127,945 participants. The prevalence of child marriage ranged between 1.8% to 90.85%. In most studies, the trend of child marriage was decreasing. The most important individual factors include the respondent's education and occupation, interpersonal factors such as the education and occupation of parents and husband, family size and type. Community factors include socioeconomic status, region, residence, ethnicity, and religion at the social level.ConclusionDespite a central focus of research and policies on interventions that decrease child marriage, this phenomenon is still prevalent in many places. Therefore, further specific interventions are required to improve education, reduce poverty and inequality. This may help achieve the 2030 Sustainable Development Goals.

Project description:BackgroundChild marriage, defined as marriage before 18 years of age, is a violation of human rights and a marker of gender inequality. Growing attention to this issue on the global development agenda also reflects concerns that it may negatively impact health. We conducted a systematic review to synthesize existing research on the consequences of child marriage on health and to assess the risk of bias in this body of literature.Methods and findingsWe searched databases focused on biomedicine and global health for studies that estimated the effect of marrying before the age of 18 on any physical or mental health outcome or health behaviour. We identified 58 eligible articles, nearly all of which relied on cross-sectional data sources from sub-Saharan Africa or South Asia. The most studied health outcomes were indicators of fertility and fertility control, maternal health care, and intimate partner violence. All studies were at serious to critical risk of bias. Research consistently found that women who marry before the age of 18 begin having children at earlier ages and give birth to a larger number of children when compared to those who marry at 18 or later, but whether these outcomes were desired was not considered. Across studies, women who married as children were also consistently less likely to give birth in health care facilities or with assistance from skilled providers. Studies also uniformly concluded that child marriage increases the likelihood of experiencing physical violence from an intimate partner. However, research in many other domains, including use of contraception, unwanted pregnancy, and sexual violence came to divergent conclusions and challenge some common narratives regarding child marriage.ConclusionsThere are many reasons to be concerned about child marriage. However, evidence that child marriage causes the health outcomes described in this review is severely limited. There is more heterogeneity in the results of these studies than is often recognized. For these reasons, greater caution is warranted when discussing the potential impact of child marriage on health. We provide suggestions for avoiding common biases and improving the strength of the evidence on this subject.Trial registrationThe protocol of this systematic review was registered with PROSPERO (CRD42020182652) in May 2020.

Project description:IntroductionThe global development agenda reflects greater attention to ending child marriage and supporting adolescent girls than ever before. Limited understandings of the evidence base on child marriage, however, make it challenging to assess gaps in the literature and inform policy and programming to respond to the needs of adolescent girls. The goal of this project is to systematically identify, evaluate and synthesise the global evidence on child marriage.Methods and analysisWe will include articles with a thematic focus on child marriage from all geographic settings, two decades of research (2000-2019) and in four languages (English, Spanish, French and Portuguese). We will search 18 electronic academic databases (7 in English and 4 each in French, Spanish and Portuguese, with 1 overlapping database) and for the grey literature, conduct targeted hand-searches of organisations engaged in work to prevent child marriage. The databases for studies in English are PubMed, PsychINFO, Embase, CINAHL Plus, Popline, Web of Science and Cochrane Library; for studies in French, the databases will be DialNet, Directory of Open Access Journals, Science Direct and Biblioteca CCG-IBT database; in Spanish, DialNet, La Biblioteca Científica Electrónica en Línea, Red Iberoamericana de Innovación y Conocimiento Científico and Jstor and in Portuguese, Coordenação de Aperfeiçoamento de Pessoal de Nível Superior, Biblioteca Virtual em Saúde, Biblioteca Científica Eletrônica On-line and Biblioteca Digital Brasileira de Teses e Dissertações. We will also review reference lists of select articles and seek input from key authors, field practitioners and participants in international convenings. We will collect and analyse data on publication characteristics, including type of document, institutional affiliation, publication year, language, focus country and region, study objective, specific focus, research method, key findings and recommendations of the material offered for future work. The database searches for publications in English were conducted in January 2020 and we plan to complete the searches in French, Spanish and Portuguese in early 2021.Ethics and disseminationAs a systematic review of already-published data, this study does not raise ethical or safety concerns. The authors plan to publish the results of the scoping review in a relevant international journal as well as present the results widely following publication. Building on this foundational work, the authors plan to conduct analyses that make use of the rich data.Registration detailsThe study design adheres to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews. Our protocol was registered with Open Science Framework on 14 January 2020 (https://osf.io/awh8v).

Project description:ObjectivesDespite the high prevalence of child marriage in Africa, little is known about the current state of the evidence on interventions to prevent and respond to child marriage in the region. The objectives of this systematic scoping review are to describe the breadth of existing evidence on child marriage prevention and response interventions, analyze where these interventions have been implemented, and identify research gaps and priorities for moving forward.MethodsThe inclusion criteria incorporated publications that: (1) focused on Africa, (2) described interventions to address child marriage, (3) were published 2000-2021 and (4) were published as peer-reviewed articles or reports in English. We searched seven databases (PubMed, PsychINFO, Embase, Cinahl Plus, Popline, Web of Science and Cochrane Library), hand-searched the websites of 15 organisations and used Google Scholar to identify research published in 2021. Two authors independently screened titles and abstracts followed by full-text reviews and data extraction for included studies.ResultsOur analysis of the 132 intervention studies identified highlights important disparities by intervention type, sub-region, and intervention activities, focus populations and impact. The greatest number of intervention studies focused on Eastern Africa. Health and empowerment approaches were most represented, followed by education and laws and policies. Norms or livelihoods approaches were least represented.ConclusionOur review finds few high-quality impact evaluations, most of which assess cash transfer programmes. There is a need to strengthen evaluative evidence on other intervention approaches including empowerment and norms change interventions, in particular. Given the linguistic and cultural diversity of the continent, more country-specific studies and research published in languages other than English are needed, particularly in high-prevalence Middle African countries.

Project description:IntroductionChild marriage and teen pregnancy have negative health, social and development consequences. Highest rates of child marriage occur in sub-Saharan Africa (SSA) and 40% of women in Western and Central Africa got married before the age of 18. This systematic review was aimed to fill a gap in evidence of effectiveness to reduce teen pregnancy and child marriage in SSA.MethodsWe considered studies conducted in sub-Saharan Africa that reported on the effect of interventions on child marriage and teen pregnancy among adolescent girls for inclusion. We searched major databses and grey literature sources.ResultsWe included 30 articles in this review. We categorized the interventions reported in the review into five general categories: (a) Interventions aimed to build educational assets, (b) Interventions aimed to build life skills and health assets, (c) Wealth building interventions, and (d) Community dialogue. Only few interventions were consistently effective across the studies included in the review. The provision of scholarship and systematically implemented community dialogues are consistently effective across settings.ConclusionProgram designers aiming to empower adolescent girls should address environmental factors, including financial barriers and community norms. Future researchers should consider designing rigorous effectiveness and cost effectiveness studies to ensure sustainability.Systematic review registrationhttps://www.crd.york.ac.uk/prospero/, identifier: CRD42022327397.

Project description:Global efforts to eradicate 'child marriage' (<18 years) increasingly target governments, the private sector and the general public as agents of change. However, understanding of child marriage may be subject to popular misconceptions, particularly because of ambiguity in the age threshold implied by the term 'child', and because awareness campaigns routinely emphasize extreme scenarios of very young girls forcibly married to much older men. Here, we ascertain public knowledge of child marriage via an online survey. Half of those surveyed mistakenly believed that the cut-off for child marriage is younger than the threshold of 18 years, and nearly three-quarters incorrectly believed that most child marriages occur at 15 years or below (it primarily occurs in later adolescence). Most participants also incorrectly believed that child marriage is illegal throughout the USA (it's illegal in only 4/50 states), substantially overestimated its global prevalence, and mistakenly believed that it primarily takes place among Muslim-majority world regions. Our results highlight important popular misconceptions of child marriage that may ultimately undermine global health goals and perpetuate harmful stereotypes. Organizations seeking to empower women by reducing child marriage should be cautious of these misunderstandings, and wary of the potential for their own activities to seed misinformation.

Project description:BACKGROUND:Despite an understanding of the circumstances of child marriage, including how it limits agency and erodes childhood support systems, not much is known about the relationship between child marriage and mental health of child brides, especially in the sub-Saharan African context. To address this gap, we use large-scale population-based data from ever-married women aged 18-45 in Niger (n?=?2764) and Ethiopia (n?=?4149) to examine the association of child marriage with overall psychological well-being and its sub-domains: depression, anxiety, positive well-being, vitality, self-control and general health. We complement this with qualitative data from Ethiopia to further contextualize the psychological well-being of child brides. METHODS:Multivariate linear regressions were conducted to estimate the association between child marriage and overall psychological well-being and its sub-domains. Thematic qualitative analysis was conducted to further understand the lives of child brides. RESULTS:Our regression analysis found significant negative associations between very early marriage (marriage at 15?years or earlier) and overall psychological well-being in both Niger and Ethiopia. With the exception of self-control, all sub-domains of psychological well-being - depression, anxiety, positive well-being, vitality and general health - were negatively associated with very early marriage. In addition, in the qualitative analysis, Ethiopian child brides reported suffering emotional distress and depression induced by the burden of handling marital responsibilities at an early age. CONCLUSION:The study highlights that even in settings where child marriage is normative, marrying very early is associated with negative outcomes. Further research is needed to understand the mechanisms that make those married during early adolescence particularly vulnerable to psychological distress, so that programs can address those vulnerabilities.

Project description:BackgroundPrior research documents that India has the greatest number of girls married as minors of any nation in the world, increasing social and health risks for both these young wives and their children. While the prevalence of child marriage has declined in the nation, more work is needed to accelerate this decline and the negative consequences of the practice. Expanded targets for intervention require greater identification of these targets. Machine learning can offer insight into identification of novel factors associated with child marriage that can serve as targets for intervention.MethodsWe applied machine learning methods to retrospective cross-sectional survey data from India on demographics and health, the nationally-representative National Family Health Survey, conducted in 2015-16. We analyzed data using a traditional regression model, with child marriage as the dependent variable, and 4000+ variables from the survey as the independent variables. We also used three commonly used machine learning algorithms- Least Absolute Shrinkage and Selection Operator (lasso) or L-1 regularized logistic regression models; L2 regularized logistic regression or ridge models; and neural network models. Finally, we developed and applied a novel and rigorous approach involving expert qualitative review and coding of variables generated from an iterative series of regularized models to assess thematically key variable groupings associated with child marriage.FindingsAnalyses revealed that regularized logistic and neural network applications demonstrated better accuracy and lower error rates than traditional logistic regression, with a greater number of features and variables generated. Regularized models highlight higher fertility and contraception, longer duration of marriage, geographic, and socioeconomic vulnerabilities as key correlates; findings shown in prior research. However, our novel method involving expert qualitative coding of variables generated from iterative regularized models and resultant thematic generation offered clarity on variables not focused upon in prior research, specifically non-utilization of health system benefits related to nutrition for mothers and infants.InterpretationMachine learning appears to be a valid means of identifying key correlates of child marriage in India and, via our innovative iterative thematic approach, can be useful to identify novel variables associated with this outcome. Findings related to low nutritional service uptake also demonstrate the need for more focus on public health outreach for nutritional programs tailored to this population.

Project description:BACKGROUND:Child marriage remains a challenge in Ghana. Over the years, government and development partners have made various commitments and efforts to curb the phenomenon of child marriage. However, there is little empirical evidence on the predictors, norms and practices surrounding the practice to support their efforts, a gap this study sought to fill. METHODS:The study employed a multiple-method approach to achieve the set objectives. Data from the women's file of the 2014 Ghana Demographic and Health Survey (GDHS) was used to examine the predictors of child marriage using frequencies and logistic regression methods. Data from Key Informant Interviews (KIIs) and Focus Group Discussions (FGDs) collected in Central and Northern regions of Ghana were used to examine norms and practices surrounding child marriage using thematic analysis. RESULTS:Two in ten (20.68%) girls in the quantitative sample married as children. The results revealed that girls who had never attended school compared to those who had ever attended school were more likely to marry as children (OR, 3.01). Compared with girls in the lowest wealth quintile, girls in the middle (OR, 0.59), fourth (OR, 0.37) and highest (OR, 0.32) wealth quintiles were less likely to marry as children. From the qualitative data, the study identified poverty, teenage pregnancy, and cultural norms such as betrothal marriage, exchange of girls for marriage and pressure from significant others as the drivers of child marriage. CONCLUSIONS:The findings show that various socio-economic and cultural factors such as education, teenage pregnancy and poverty influence child marriage. Hence, efforts to curb child marriage should be geared towards retention of girls in school, curbing teenage pregnancy, empowering girls economically, enforcing laws on child marriage in Ghana, as well as designing tailored advocacy programs to educate key stakeholders and adolescent girls on the consequences of child marriage. Additionally, there is the need to address socio-cultural norms/practices to help end child marriage.

Project description:The social burden of Bangladesh's already prevalent child marriage problem has further flared up amid the ongoing COVID-19 pandemic. We aim to briefly report on the current scenario of underage marriage in Bangladesh from available sources and highlight concerns. The prevalence of child marriage has increased by at least 13% due to pandemic-enforced long-term school closure throughout the country, while many cases remain unreported. Exacerbated financial crisis, the prolonged shutdown of educational institutions and social insecurities are the principal triggering factors for such a high pace of child marriages. We urge the government and local administrations to address the current situation through concerted and integrated efforts to combat the early marriage crisis in Bangladesh.
