Project description:This paper presents a novel method for fast and optimal determination of recruitment (input-output, IO) curve parameters in neural stimulation. A sequential parameter estimation (SPE) method was developed based on the Fisher information matrix (FIM), with a stopping rule based on successively satisfying a specified estimation tolerance. Simulated motor responses evoked by transcranial magnetic stimulation (TMS) were used as a test bed. Performance of FIM-SPE was characterized in 10 177 simulation runs for various IO parameter values corresponding to different virtual subjects, compared with uniform sampling. Unlike uniform sampling, FIM-SPE identifies and samples the areas of the IO curve that contain maximum information about the curve parameters. For the most relaxed stopping rule, the median number of samples required for convergence was only 17 for FIM-SPE versus 294 for uniform sampling. For the highest reliability stopping rule, more than 92% of the FIM-SPE runs converged, with a median of 88 samples, whereas all uniform sampling runs reached 1000 samples without converging. Compared to uniform sampling, FIM-SPE reduced estimation errors up to two-fold and required ten times fewer stimuli. FIM-SPE could improve the speed and accuracy of determination of IO curves for neural stimulation. A software implementation of the algorithm is provided online.

Project description:Transcranial magnetic stimulation (TMS) has emerged as a promising neuromodulation technique with both therapeutic and diagnostic applications. As accurate coil placement is known to be essential for focal stimulation, computational models have been established to help find the optimal coil positioning by maximizing electric fields at the cortical target. While these numerical simulations provide realistic and subject-specific field distributions, they are computationally demanding, precluding their use in real-time applications. In this paper, we developed a novel multi-task deep neural network which simultaneously predicts the optimal coil placement for a given cortical target as well as the associated TMS-induced electric field. Trained on large amounts of preceding numerical optimizations, the Attention U-Net-based neural surrogate provided accurate coil optimizations in only 35 ms, a fraction of time compared to the state-of-the-art numerical framework. The mean errors on the position estimates were below 2 mm, i.e., smaller than previously reported manual coil positioning errors. The predicted electric fields were also highly correlated (r> 0.97) with their numerical references. In addition to healthy subjects, we validated our approach also in glioblastoma patients. We first statistically underlined the importance of using realistic heterogeneous tumor conductivities instead of simply adopting values from the surrounding healthy tissue. Second, applying the trained neural surrogate to tumor patients yielded similar accurate positioning and electric field estimates as in healthy subjects. Our findings provide a promising framework for future real-time electric field-optimized TMS applications.

Project description:Navigated transcranial magnetic stimulation (nTMS) is a valuable tool for non-invasive brain stimulation. Currently, nTMS requires fixing of markers on the patient's head. Head marker displacements lead to changes in coil placement and brain stimulation inaccuracy. A markerless neuronavigation method is needed to increase the reliability of nTMS and simplify the nTMS protocol. In this study, we introduce and release MarLe, a Python markerless head tracker neuronavigation software for TMS. This novel software uses computer-vision techniques combined with low-cost cameras to estimate the head pose for neuronavigation. A coregistration algorithm, based on a closed-form solution, was designed to track the patient's head and the TMS coil referenced to the individual's brain image. We show that MarLe can estimate head pose based on real-time video processing. An intuitive pipeline was developed to connect the MarLe and nTMS neuronavigation software. MarLe achieved acceptable accuracy and stability in a mockup nTMS experiment. MarLe allows real-time tracking of the patient's head without any markers. The combination of face detection and a coregistration algorithm can overcome nTMS head marker displacement concerns. MarLe can improve reliability, simplify, and reduce the protocol time of brain intervention techniques such as nTMS.

Project description:OBJECTIVE:Identify the optimal number of pulses necessary to achieve reliable measures of motor evoked potentials (MEPs) in transcranial magnetic stimulation (TMS) studies. METHODS:Retrospective data was obtained from 54 healthy volunteers (30 men, mean age 61.7±13.1years) who as part of prior studies had completed three blocks of 30 consecutive TMS stimuli using neuronavigation. Data from four protocols were assessed: single-pulse TMS for measures of amplitude and latency of MEPs; paired-pulse TMS for short-interval intracortical inhibition (sICI) and intracortical facilitation (ICF); and single-pulse TMS to assess the effects of intermittent theta burst stimulation (iTBS). Two statistical methods were used: an internal consistency analysis and probability of inclusion in the 95% confidence interval (CI) around the mean MEPs amplitude. RESULTS:For single-pulse TMS, the minimum number of pulses needed to achieve reliable amplitude and latency MEPs measures was 21 and 23, respectively. For paired-pulse TMS, the minimum number of pulses needed to achieve reliable sICI and ICF measures was 20 and 25, respectively. Finally, the minimum number of pulses needed to achieve reliable amplitude and latency MEPs measures after iTBS was 22 and 23, respectively. CONCLUSIONS:This study provides guidelines regarding the minimum number of pulses needed to achieve reliable MEPs measurements in various study protocols using neuronavigated TMS. SIGNIFICANCE:Results from this study have the potential to increase the reliability and quality of future neuronavigated TMS studies.

Project description:BackgroundStimulus response curves (SR curves), measured using transcranial magnetic stimulation (TMS) induced motor evoked potentials (MEP), yield important information regarding corticomotor connectivity. Limited understanding of SR curve analyses techniques for leg muscles after stroke may limit the utility of TMS data for walking recovery.ObjectiveTo compare linear and non-linear curve fitting for MEP responses from the non-paretic and paretic tibialis anterior (TA) muscles.MethodsAccuracy of fit was measured using coefficient of determination (R2). Similarities of the fit were compared using slopes and area under the curve (AUC).ResultsThe non-linear function demonstrated higher R2 and slopes. The AUC was not significantly different between the two analyses approaches. The non-linear non-paretic SR slopes and paretic AUC had significant associations with walking speed.ConclusionOur results highlight the differences between non-linear and linear approaches to best fit the SR curves from bilateral TA muscles in stroke survivors. Although the linear function can appropriately fit the SR curve of the paretic and non-paretic TA, the non-linear function estimated a higher slope. We found the AUC to be a more robust measure that was not affected by the type of curve-fitting approach and only the AUC of the paretic TA showed significant association with walking speeds. A better understanding of SR curve fitting approaches for the TA muscles in individuals with chronic stroke allows for their optimal use in interpretation of TMS data and literature.

Project description:BackgroundTranscranial magnetic stimulation (TMS) can modulate neural activity by evoking action potentials in cortical neurons. TMS neural activation can be predicted by coupling subject-specific head models of the TMS-induced electric field (E-field) to populations of biophysically realistic neuron models; however, the significant computational cost associated with these models limits their utility and eventual translation to clinically relevant applications.ObjectiveTo develop computationally efficient estimators of the activation thresholds of multi-compartmental cortical neuron models in response to TMS-induced E-field distributions.MethodsMulti-scale models combining anatomically accurate finite element method (FEM) simulations of the TMS E-field with layer-specific representations of cortical neurons were used to generate a large dataset of activation thresholds. 3D convolutional neural networks (CNNs) were trained on these data to predict thresholds of model neurons given their local E-field distribution. The CNN estimator was compared to an approach using the uniform E-field approximation to estimate thresholds in the non-uniform TMS-induced E-field.ResultsThe 3D CNNs estimated thresholds with mean absolute percent error (MAPE) on the test dataset below 2.5% and strong correlation between the CNN predicted and actual thresholds for all cell types (R2 > 0.96). The CNNs estimated thresholds with a 2-4 orders of magnitude reduction in the computational cost of the multi-compartmental neuron models. The CNNs were also trained to predict the median threshold of populations of neurons, speeding up computation further.Conclusion3D CNNs can estimate rapidly and accurately the TMS activation thresholds of biophysically realistic neuron models using sparse samples of the local E-field, enabling simulating responses of large neuron populations or parameter space exploration on a personal computer.

Project description:Repetitive transcranial magnetic stimulation (rTMS) has been reported to improve naming in chronic stroke patients with nonfluent aphasia since 2005. In part 1, we review the rationale for applying slow, 1-Hz, rTMS to the undamaged right hemisphere in chronic nonfluent aphasia patients after a left hemisphere stroke; and we present a transcranial magnetic stimulation (TMS) protocol used with these patients that is associated with long-term, improved naming post-TMS. In part 2, we present results from a case study with chronic nonfluent aphasia where TMS treatments were followed immediately by speech therapy (constraint-induced language therapy). In part 3, some possible mechanisms associated with improvement after a series of TMS treatments in stroke patients with aphasia are discussed.

Project description:Background. Our goal was to perform a systematic review on the use of repetitive transcranial magnetic stimulation (rTMS) in the treatment of status epilepticus (SE) and refractory status epilepticus (RSE). Methods. MEDLINE, BIOSIS, EMBASE, Global Health, Healthstar, Scopus, Cochrane Library, the International Clinical Trials Registry Platform, clinicaltrials.gov (inception to August 2015), and gray literature were searched. The strength of evidence was adjudicated using Oxford and GRADE methodology. Results. We identified 11 original articles. Twenty-one patients were described, with 13 adult and 8 pediatric. All studies were retrospective. Seizure reduction/control with rTMS occurred in 15 of the 21 patients (71.4%), with 5 (23.8%) and 10 (47.6%) displaying partial and complete responses, respectively. Seizures recurred after rTMS in 73.3% of the patients who had initially responded. All studies were an Oxford level 4, GRADE D level of evidence. Conclusions. Oxford level 4, GRADE D evidence exists to suggest a potential impact on seizure control with the use of rTMS for FSE and FRSE, though durability of the therapy is short-lived. Routine use of rTMS in this context cannot be recommended at this time. Further prospective study of this intervention is warranted.

Project description:Electroencephalogram (EEG) phase synchronization analyses can reveal large-scale communication between distant brain areas. However, it is not possible to identify the directional information flow between distant areas using conventional phase synchronization analyses. In the present study, we applied transcranial magnetic stimulation (TMS) to the occipital area in subjects who were resting with their eyes closed, and analyzed the spatial propagation of transient TMS-induced phase resetting by using the transfer entropy (TE), to quantify the causal and directional flow of information. The time-frequency EEG analysis indicated that the theta (5 Hz) phase locking factor (PLF) reached its highest value at the distant area (the motor area in this study), with a time lag that followed the peak of the transient PLF enhancements of the TMS-targeted area at the TMS onset. Phase-preservation index (PPI) analyses demonstrated significant phase resetting at the TMS-targeted area and distant area. Moreover, the TE from the TMS-targeted area to the distant area increased clearly during the delay that followed TMS onset. Interestingly, the time lags were almost coincident between the PLF and TE results (152 vs. 165 ms), which provides strong evidence that the emergence of the delayed PLF reflects the causal information flow. Such tendencies were observed only in the higher-intensity TMS condition, and not in the lower-intensity or sham TMS conditions. Thus, TMS may manipulate large-scale causal relationships between brain areas in an intensity-dependent manner. We demonstrated that single-pulse TMS modulated global phase dynamics and directional information flow among synchronized brain networks. Therefore, our results suggest that single-pulse TMS can manipulate both incoming and outgoing information in the TMS-targeted area associated with functional changes.
