The Mediating Role of Psychological Capital in the Relationship Between Occupational Stress and Fatigue: A Cross-Sectional Study Among 1,104 Chinese Physicians.

ABSTRACT: Purpose: This study aimed to explore the association of occupational stress with fatigue and to examine the mediating role of psychological capital (PsyCap) among Chinese physicians. Materials and Methods: A cross-sectional study was conducted in Liaoning province, China, in 2018. Using a multistage stratified sampling method, a total of 1,500 physicians participated and 1,104 (73.6%) physicians responded effectively. The study used a self-administered questionnaire consisting of the 14-item Fatigue Scale (FS-14), the Effort-reward Imbalance questionnaire (ERI), the Psychological Capital Questionnaire (PCQ) and items about demographic and working characteristics. Hierarchical multiple regression analyses were performed to explore the association of occupational stress, PsyCap, and fatigue among physicians. Asymptotic and resampling strategies were used to examine the mediating effect. Results: The incidence of fatigue among Chinese physicians was 83.70%. The average level of fatigue was 7.96 ± 3.95 (mean ± SD). Occupational stress and PsyCap were significantly associated with fatigue. PsyCap significantly mediated the association of ERR (a × b = 0.106, bias-corrected and accelerated 95% confidence interval [BCa 95% CI]: 0.078, 0.138) and overcommitment (a × b = 0.068, BCa 95% CI: 0.044, 0.092) with fatigue. Two important components of PsyCap, self-efficacy and resilience, play more important roles in the mediating effect. Conclusions: The level of fatigue among Chinese physicians was high, which should be taken seriously by management. PsyCap could mediate the association between occupational stress and fatigue. The intervention strategies and measures to relieve fatigue could be focused on physicians' positive PsyCap improvement.

Project description:Burnout is recognized as an occupational hazard, and nursing has a high risk of burnout. This study aims to explore the relationship between psychological capital (PsyCap) and burnout among Chinese nurses and the mediating role of coping style in this relationship.A total of 1,496 nurses (effective response rate: 80.11%) from two large general hospitals in Daqing City of China were selected as participants. Data were collected via the Chinese Maslach Burnout Inventory (CMBI), the psychological capital questionnaire (PCQ-24), the Chinese Trait Coping Style Questionnaire (TCSQ) and demographic and caregiver-patient relationship. Hierarchical linear regression analyses were performed to explore the mediating role of positive coping and negative coping, and we used the Bootstrap method to confirm the mediating effect.Self-efficacy, hope, resilience and optimism of nurses were all negatively related with emotional exhaustion, depersonalization and reduced personal accomplishment among Chinese nurses. Positive coping partially mediated the relationship between hope/optimism and emotional exhaustion and between self-efficacy/optimism and reduced personal accomplishment. Negative coping fully mediated the relationship between self-efficacy and emotional exhaustion, and in the regression model self-efficacy was positively correlated with emotional exhaustion. And negative coping also partially mediated the relationship between hope/optimism and emotional exhaustion and between optimism and depersonalization.PsyCap had effects on burnout and coping style was a mediator in this relationship among Chinese nurses. Nurses who had a strong sense of self-efficacy adopted more negative coping style, which in turn would lead to higher levels of emotional exhaustion. These findings shed light on the influence of negative coping on burnout, and positive coping was a positive resource for fighting against nurses' burnout. Hence, in order to avoid negative coping style, improve skill of coping and enhance PsyCap of nurses, active interventions should be developed in the future.

Project description:BackgroundSocial capital has been related to various aspects of health. While literature suggests that men and women differently access and mobilize social capital, gender has received little attention within social capital research. This study examines whether the association between individual social capital and psychological distress is different for men and women.MethodsWe made use of data from a representative sample of 1025 adults within 50 neighbourhoods of Ghent (Belgium), collected in the context of the cross-sectional Social capital and Well-being In Neighbourhoods in Ghent (SWING) Survey 2011. Six components of social capital were discerned: generalized trust, social support, social influence, social engagement and attachment, the volume of social capital and the mean occupational prestige in one's network. Multilevel linear regression models were fitted to explore interactions between gender and these components of social capital.ResultsIn accordance with previous research, men report lower levels of psychological distress than women (t?=?4.40, p?<?0.001). Regarding the gender gap in social capital, the findings are mixed. Only for half of the social capital variables (social support, social influence and volume of social capital), a significant gender difference is found, favouring men (t?=?4.03, p?<?0.001; t?=?1.99, p?<?0.001 and t?=?4.50, p?<?0.001 respectively). None of the analysed interaction terms between gender and social capital is significantly related to psychological distress.ConclusionThe analyses indicate that the association between individual social capital and psychological distress is similar for men and women. The relatively low level of gender stratification in Belgium might have influenced this finding. Furthermore, it is possible that social capital is not of greater importance for women in general, but mainly for women who are in an especially vulnerable social situation that deprives their access to alternative resources (e.g. unemployed women, single mothers). Future studies should seek to identify subgroups for whom social capital might be particularly influential, by transcending 'simple' dyads such as 'men versus women'.
