Measurement Error in Discrete Health Facility Choice Models: an Example from Urban Senegal.

ABSTRACT: We use individual-level health facility choice data from urban Senegal to estimate consumer preferences for facility characteristics related to maternal health services. We find that consumers consider a large number of quality related facility characteristics, as well as travel costs, when making their health facility choice. In contrast to the typical assumption in the literature, our findings indicate that individuals frequently bypass the facility nearest their home. In light of this, we show that the mismeasured data used commonly in the literature produces biased preference estimates; most notably, the literature likely overestimates consumer distaste for travel.

The effects of health facility access and quality on family planning decisions in urban Senegal.

Project description:Research in developing countries is rarely focused on examining how supply side factors affect family planning decisions due to a lack of facility-level data. When these data exist, analyses tend to focus on rural environments. In this paper, we study the effects that health facility access and quality have on contraceptive use and desired number of children for women in urban Senegal. Unlike related studies focusing on rural environments, we find no evidence that greater access to health facilities and pharmacies increases contraceptive use among urban women. However, we do find that contraceptive use among urban women is higher with greater facility quality. For example, we find that increasing the proportion of pharmacies employing multiple pharmacists from 0% to 50% would increase contraceptive use by 6.0 percentage points, and increasing the proportion of facilities with family planning guidelines/protocols from 50% to 100% would increase use by 2.1 percentage points.

Multiscale measurement error models for aggregated small area health data.

Project description:Spatial data are often aggregated from a finer (smaller) to a coarser (larger) geographical level. The process of data aggregation induces a scaling effect which smoothes the variation in the data. To address the scaling problem, multiscale models that link the convolution models at different scale levels via the shared random effect have been proposed. One of the main goals in aggregated health data is to investigate the relationship between predictors and an outcome at different geographical levels. In this paper, we extend multiscale models to examine whether a predictor effect at a finer level hold true at a coarser level. To adjust for predictor uncertainty due to aggregation, we applied measurement error models in the framework of multiscale approach. To assess the benefit of using multiscale measurement error models, we compare the performance of multiscale models with and without measurement error in both real and simulated data. We found that ignoring the measurement error in multiscale models underestimates the regression coefficient, while it overestimates the variance of the spatially structured random effect. On the other hand, accounting for the measurement error in multiscale models provides a better model fit and unbiased parameter estimates.

Surrogate Residuals for Discrete Choice Models.

Project description:Discrete choice models (DCMs) are a class of models for modeling response variables that take values from a set of alternatives. Examples include logistic regression, probit regression, and multinomial logistic regression. These models are also referred together as generalized linear models. Although there exist methods for the goodness of fit of DCMs, defining intuitive residuals for such models has been difficult due to the fact that the responses are categorical values instead of continuous numbers. In this article, we propose the surrogate residual for DCMs based on the surrogate approach (Liu and Zhang 2018), which deals with an ordinal response. We consider categorical responses that may or may not be ordered. We shall show that our residual can be used to diagnose misspecification in the aspects of mean structure, individual-specific coefficients, and interaction effects. Measurement error models with interactions.

Project description:An important use of measurement error models is to correct regression models for bias due to covariate measurement error. Most measurement error models assume that the observed error-prone covariate (WW ) is a linear function of the unobserved true covariate (X) plus other covariates (Z) in the regression model. In this paper, we consider models for W that include interactions between X and Z. We derive the conditional distribution of X given W and Z and use it to extend the method of regression calibration to this class of measurement error models. We apply the model to dietary data and test whether self-reported dietary intake includes an interaction between true intake and body mass index. We also perform simulations to compare the model to simpler approximate calibration models.

Optimizing Differentiated HIV Treatment Models in Urban Zimbabwe: Assessing Patient Preferences Using a Discrete Choice Experiment.

Project description:Differentiated service delivery holds great promise for streamlining the delivery of health services for HIV. This study used a discrete choice experiment to assess preferences for differentiated HIV treatment delivery model characteristics among 500 virally suppressed adults on antiretroviral therapy in Harare, Zimbabwe. Treatment model characteristics included location, consultation type, healthcare worker cadre, operation times, visit frequency and duration, and cost. A mixed effects logit model was used for parameter estimates to identify potential preference heterogeneity among participants, and interaction effects were estimated for sex and age as potential sources of divergence in preferences. Results indicated that participants preferred health facility-based services, less frequent visits, individual consultations, shorter waiting times, lower cost and, delivered by respectful and understanding healthcare workers. Some preference heterogeneity was found, particularly for location of service delivery and group vs. individual models; however, this was not fully explained by sex and age characteristics of participants. In urban areas, facility-based models, such as the Fast Track model requiring less frequent clinic visits, are likely to better align with patient preferences than some of the other community-based or group models that have been implemented. As Zimbabwe scales up differentiated treatment models for stable patients, a clear understanding of patient preferences can help in designing services that will ensure optimal utilization and improve the efficiency of service delivery.

Clustering in General Measurement Error Models.

Project description:This paper is dedicated to the memory of Peter G. Hall. It concerns a deceptively simple question: if one observes variables corrupted with measurement error of possibly very complex form, can one recreate asymptotically the clusters that would have been found had there been no measurement error? We show that the answer is yes, and that the solution is surprisingly simple and general. The method itself is to simulate, by computer, realizations with the same distribution as that of the true variables, and then to apply clustering to these realizations. Technically, we show that if one uses K-means clustering or any other risk minimizing clustering, and a multivariate deconvolution device with certain smoothness and convergence properties, then, in the limit, the cluster means based on our method converge to the same cluster means as if there is no measurement error. Along with the method and its technical justification, we analyze two important nutrition data sets, finding patterns that make sense nutritionally.

Nonparametric Prediction in Measurement Error Models.

Project description:Predicting the value of a variable Y corresponding to a future value of an explanatory variable X, based on a sample of previously observed independent data pairs (X(1), Y(1)), …, (X(n), Y(n)) distributed like (X, Y), is very important in statistics. In the error-free case, where X is observed accurately, this problem is strongly related to that of standard regression estimation, since prediction of Y can be achieved via estimation of the regression curve E(Y|X). When the observed X(i)s and the future observation of X are measured with error, prediction is of a quite different nature. Here, if T denotes the future (contaminated) available version of X, prediction of Y can be achieved via estimation of E(Y|T). In practice, estimating E(Y|T) can be quite challenging, as data may be collected under different conditions, making the measurement errors on X(i) and X non-identically distributed. We take up this problem in the nonparametric setting and introduce estimators which allow a highly adaptive approach to smoothing. Reflecting the complexity of the problem, optimal rates of convergence of estimators can vary from the semiparametric n(-1/2) rate to much slower rates that are characteristic of nonparametric problems. Nevertheless, we are able to develop highly adaptive, data-driven methods that achieve very good performance in practice.

Random measurement error: Why worry? An example of cardiovascular risk factors.

Project description:With the increased use of data not originally recorded for research, such as routine care data (or 'big data'), measurement error is bound to become an increasingly relevant problem in medical research. A common view among medical researchers on the influence of random measurement error (i.e. classical measurement error) is that its presence leads to some degree of systematic underestimation of studied exposure-outcome relations (i.e. attenuation of the effect estimate). For the common situation where the analysis involves at least one exposure and one confounder, we demonstrate that the direction of effect of random measurement error on the estimated exposure-outcome relations can be difficult to anticipate. Using three example studies on cardiovascular risk factors, we illustrate that random measurement error in the exposure and/or confounder can lead to underestimation as well as overestimation of exposure-outcome relations. We therefore advise medical researchers to refrain from making claims about the direction of effect of measurement error in their manuscripts, unless the appropriate inferential tools are used to study or alleviate the impact of measurement error from the analysis.

Understanding what women want: eliciting preference for delivery health facility in a rural subcounty in Kenya, a discrete choice experiment.

Project description:ObjectiveTo identify what women want in a delivery health facility and how they rank the attributes that influence the choice of a place of delivery.DesignA discrete choice experiment (DCE) was conducted to elicit rural women's preferences for choice of delivery health facility. Data were analysed using a conditional logit model to evaluate the relative importance of the selected attributes. A mixed multinomial model evaluated how interactions with sociodemographic variables influence the choice of the selected attributes.SettingSix health facilities in a rural subcounty.ParticipantsWomen aged 18-49 years who had delivered within 6 weeks.Primary outcomeThe DCE required women to select from hypothetical health facility A or B or opt-out alternative.ResultsA total of 474 participants were sampled, 466 participants completed the survey (response rate 98%). The attribute with the strongest association with health facility preference was having a kind and supportive healthcare worker (β=1.184, p<0.001), second availability of medical equipment and drug supplies (β=1.073, p<0.001) and third quality of clinical services (β=0.826, p<0.001). Distance, availability of referral services and costs were ranked fourth, fifth and sixth, respectively (β=0.457, p<0.001; β=0.266, p<0.001; and β=0.000018, p<0.001). The opt-out alternative ranked last suggesting a disutility for home delivery (β=-0.849, p<0.001).ConclusionThe most highly valued attribute was a process indicator of quality of care followed by technical indicators. Policymakers need to consider women's preferences to inform strategies that are person centred and lead to improvements in quality of care during delivery.

Discrete choice experiment data for street-level urban greening in Berlin.

Project description:The data presented in this DiB article are the outcome of a survey implemented in a Berlin neighborhood from January to March 2018. The data consist of socio-demographic, attitudinal and perception questions, and, most importantly, a discrete choice experiment. This dataset is complementary to the full research article, "Economic valuation of street-level urban greening: A case study from an evolving mixed-use area in Berlin" [1]. The analysis of the discrete choice experiment provided in the full article could be used to guide policy- and project-level decision-making for green building practices and urban green initiatives, while the dataset available here can be used to provide insight about how our sample population responded to the remaining parts of the questionnaire and how the experiment could be replicated in context or elsewhere in Berlin.
